Avaliou-se o efeito da solução concentrada de albumina eqüina diluída a 5% em solução fisiológica (SF) durante fluidoterapia em eqüinos, após indução de desidratação leve a moderada, utilizando-se cinco eqüinos adultos, sem alterações clínicas. Cada animal passou por dois protocolos de fluidoterapia: apenas com SF (metade sob pressão e metade em fluxo contínuo - grupo-controle); com solução de albumina eqüina e SF (apenas em fluxo contínuo - grupo experimental). Avaliaram-se peso, exame físico geral, hematócrito, osmolalidade plasmática, gasometria, proteína total, albumina, uréia, creatinina, Na, K, débito cardíaco e pressão arterial, e calcularam-se pressão oncótica e volume plasmático. Após a aplicação de metade da SF sob pressão nos animais do grupo-controle, as alterações no hematócrito, na proteína total, na albumina, na pressão arterial e na pressão oncótica foram semelhantes às encontradas nos do grupo experimental após aplicação apenas da solução de albumina. Objective: We investigated the effect of advanced glycated albumin (AGE-albumin) on macrophage sensitivity to inflammation elicited by S100B calgranulin and lipopolysaccharide (LPS) and the mechanism by which HDL modulates this response. We also measured the influence of the culture medium, isolated from macrophages treated with AGE-albumin, on reverse cholesterol transport (RCT). Methods and results: Macrophages were incubated with control (C) or AGE-albumin in the presence or absence of HDL, followed by incubations with S100B or LPS. Also, culture medium obtained from cells treated with C- or AGE-albumin, following S100B or LPS stimulation was utilized to treat naive macrophages in order to evaluate cholesterol efflux and the expression of HDL receptors. In comparison with C-albumin, AGE-albumin, promoted a greater secretion of cytokines after stimulation with S100B or LPS. A utilização de colóides é indicada em várias situações, mas nem sempre aplicável na clínica de eqüinos. O objetivo desse trabalho foi avaliar o uso de solução concentrada de albumina eqüina (diluída a 5%) durante fluidoterapia em eqüinos com desidratação leve a moderada, comparando-a com fluidoterapia apenas com solução fisiológica. Foram utilizados dois grupos de cinco eqüinos adultos, sem alterações clínicas. Cada animal passou pelo protocolo dos dois grupos experimentais (fluidoterapia apenas com solução fisiológica; fluidoterapia com solução de albumina eqüina e solução fisiológica). A desidratação foi induzida com duas aplicações de furosemida e jejum. Durante o experimento foram realizadas várias avaliações: pesagem; exame físico geral; hematócrito; osmolalidade plasmática; gasometria; proteína total; albumina; Na; K; débito cardíaco; pressão arterial; uréia e creatinina, e cálculo da pressão oncótica e volume plasmático. Produtos de glicação avançada (AGE) são prevalentes no diabete melito e alteram o metabolismo de lípides e lipoproteínas. Neste estudo, avaliou-se a influência da albumina, isolada do soro de indivíduos controles (C, n =12) e de portadores de diabete melito tipo 1 (DM 1, n=13) e tipo 2 (DM 2, n=11), com controle glicêmico inadequado, sobre a remoção de colesterol de macrófagos, o acúmulo intracelular de lípides, o conteúdo do receptor de HDL, ABCA-1 e a captação seletiva de colesterol esterificado de HDL. Além disso, foi determinada a expressão diferencial de genes em macrófagos tratados com albumina C, DM 1 ou DM 2. A concentração plasmática de albumina glicada foi superior no grupo DM 1 e DM 2 em relação ao C e correlacionou-se positivamente com glicemia, hemoglobina glicada e frutosamina. Albumina sérica foi isolada por cromatografia para separação rápida de proteínas e purificada por extração alcoólica. Produtos de glicação avançada (AGE) prejudicam o metabolismo de lipoproteínas e o transporte reverso de colesterol, o que contribui para a aterosclerose no diabete melito (DM). Em particular, a albumina modificada por AGE (albumina-AGE) reduz a remoção de colesterol por diminuir o conteúdo do receptor ABCA-1 em macrófagos. Isto se vincula ao insulto oxidativo e inflamatório, os quais são indutores do estresse do retículo endoplasmático (RE). O objetivo do presente estudo foi avaliar, em macrófagos, os efeitos do tratamento com albumina-AGE sobre o estresse do RE e suas vias adaptativas (UPR), relacionando-os com o prejuízo na expressão do ABCA-1 e efluxo de colesterol celular. Albumina-AGE foi produzida pela incubação de albumina isenta em ácidos graxos com glicolaldeído 10 mM e, albumina controle (albumina-C) com PBS apenas. Nesta dissertação foi estudado o comportamento de partição da albumina, proteína presente em elevada concentração no plasma sangüíneo e que interfere na determinação em diversas técnicas analíticas. No estudo efetuado foram avaliados diferentes sistemas aquosos de duas fases, explorando um fenômeno denominado ponto nuvem. Tais sistemas empregam surfactantes, sob condições experimentais específicas, obtendo-se a formação de duas fases imiscíveis: uma rica e outra pobre em tensoativo. Observou-se que devido às características hidrofílicas da albumina, bem como à dimensão dos agregados formados, sua extração para fase rica em tensoativo apresenta valores de coeficiente de partição que não ultrapassam 0,66 o que representa uma eficiência de extração de ca.40%. A extração da proteína apresentou-se viável mediante a utilização de uma mistura composta por um tensoativo não-iônico (Triton® X-114), que possibilita a separação de fases em temperatura biocompatível, na presença de outro tensoativo iônico (dodecil sulfato de sódio - SDS), que atua favorecendo as interações eletrostáticas entre os agregados e as moléculas protéicas, desde que se trabalhe em meio com pH abaixo de 5,0. A origem da albumina nos órgãos do trato reprodutor masculino tem sido questionada ao longo dos anos sendo sua presença tecidual constantemente atribuída a um mecanismo de extravasamento do plasma sanguíneo para o espaço extravascular intersticial, atribuindo-se ao fígado o papel de sítio de síntese desta proteína no período pós-natal. O padrão de distribuição da proteína albumina nos tecidos epiteliais de órgãos do trato reprodutor masculino reserva questionamentos quanto à veracidade de seu alcance luminal pelo rompimento de suas barreiras hematoteciduais. CONTEXT: Treatment of hepatorenal syndrome type 1 (HRS-1) with splancnic vasoconstrictors and high-dose albumin has been associated with reversal of renal failure in approximately 60% to 80% of the cases in pilot or uncontrolled studies. OBJECTIVE: To evaluate the results of treatment of HRS-1 with terlipressin and high-dose albumin. METHODS: All patients with HRS-1 that underwent treatment with terlipressin and high-dose albumin at our unit were retrospectively reviewed. Outcomes including reversal of renal failure and death were recorded and compared to baseline clinical and laboratory parameters. RESULTS: Seven subjects (median age 64 [47-69] years, 5 males) with median Child-Pugh and MELD scores of 12 [10-15] and 22 [17-38], respectively, hospitalized with decompensated chronic liver disease secondary to tense ascitis and infections, who exhibited criteria for HRS-1, were submitted to therapy with terlipressin and high-dose albumin according to a predefined standard protocol. Baseline creatinine levels were 2.9 [2.3-4.0] mg/mL. None of the patients achieved reversal of HRS-1 and five subjects died on-treatment due to sudden-death (n = 1), multiple organ dysfunction associated with end-stage liver failure (n = 2) and sepsis (n = 2). Schistosomiasis is an endemic parasite disease and praziquantel is the only drug currently in use to control this disease. Experimental and epidemiological evidence strongly suggests that Microtus fortis ( Mf ) is a naturally resistant vertebrate host of Schistosoma japonicum . In the present study, we found that Mf serum albumin ( Mf -albumin) and the conditioned medium of pcDNA3.1- Mf -albumin caused 46.2% and 38.7% schistosomula death rates in 96 h, respectively, which were significantly higher than that of the negative control (p < 0.05). We also found that mice injected with Mf -albumin had a 43.5% reduction in worm burden and a 48.1% reduction in liver eggs per gram (p < 0.05) in comparison to the control animals. To characterise the mechanisms involved in clearance, schistosomula were incubated with fluorescein isothiocyanate-labelled Mf -albumin and fluorescent enrichment effects were found in the gut lumen of schistosomula after 48 h of incubation. Next, digestive tract excretions from schistosomula were collected and the sensitivity of Mf -albumin to digestive tract excretions was evaluated. The results indicated that schistosomula digestive tract excretions showed indigestibility of Mf -albumin. The aim of the method described here is to remove hemoglobin, the major contaminant in the bovine plasma obtained from slaughterhouses, by adding a mixture of 19% cold ethanol and 0.6% chloroform, followed by fibrinogen and globulin precipitation by the Cohn method and nonspecific hemagglutinin by thermocoagulation. The experimental volume of bovine plasma was 2,000 ml per batch. Final purification was performed by liquid chromatography using the ion-exchange gel DEAE-Sepharose FF. The bovine albumin thus obtained presented > or = 99% purity, a yield of 25.0 ± 1.2 g/l plasma and >71.5% recovery. N-acetyl-DL-tryptophan (0.04 mmol/g protein) and sodium caprylate (0.04 mmol/g protein) were used as stabilizers and the final concentration of albumin was adjusted to 22.0% (w/v), pH 7.2 to 7.3. Viral inactivation was performed by pasteurization for 10 h at 60°C. The bovine albumin for the hemagglutination tests used in immunohematology was submitted to chemical treatment with 0.06% (w/v) glutaraldehyde and 0.1% (w/v) formaldehyde at 37°C for 12 h to obtain polymerization. A change in molecular distribution was observed after this treatment, with average contents of 56.0% monomers, 23.6% dimers, 12.2% trimers and 8.2% polymers. The binding of chlorpromazine (CPZ) and hemin to bovine serum albumin was studied by the fluorescence quenching technique. CPZ is a widely used anti-psychotic drug that interacts with blood components, influences bioavailability, and affects function of several biomolecules. Hemin is an important ferric residue of hemoglobin that binds within the hydrophobic region of albumin with high specificity. Quenching of the intrinsic fluorescence of bovine serum albumin (BSA) was observed by selectively exciting tryptophan residues at 290 nm. Emission spectra were recorded in the range from 300 to 450 nm for each quencher addition. Stern-Volmer graphs were plotted, and the quenching constant estimated for BSA solution titrated with hemin at 25ºC was 1.44 (± 0.05) x 10(5) M-1. Results showed that bovine albumin tryptophans are not equally accessible to CPZ, in agreement with the idea that polar or charged quenchers have more affinity for amino acid residues on the outer wall of the protein. Hemin added to albumin solution at a molar ratio of 1:1 quenched about 25% of their fluorescence. The quenching effect of CPZ on albumin-hemin solution was stronger than on pure BSA. This increase can be the result of combined conformational changes in the structure of albumin caused firstly by hemin and then by CPZ. OBJECTIVE: To determine whether there is an association between serum ischemia-modified albumin and the risk factor profile in type 2 diabetic patients with peripheral arterial disease and to identify the risk markers for peripheral arterial disease. METHODS: Participants included 290 patients (35.2% women) with type 2 diabetes. The ankle-brachial pressure index was measured using a standard protocol, and peripheral arterial disease was defined as an ankle-brachial index <0.90 or >1.3. The basal ischemia-modified albumin levels and clinical parameters were measured and analyzed. The risk factors for peripheral arterial disease were examined by multiple logistic analyses. RESULTS: Age, systolic blood pressure, total cholesterol, low-density lipoprotein cholesterol, urine albumin, homocysteine, and ischemia-modified albumin were significantly higher in patients with peripheral arterial disease than in disease-free patients (p<0.05), while ankle-brachial index was lower in the former group (p<0.05). Objective: Patients with stage I non-small cell lung cancer who have undergone complete surgical resection harbor a 30% risk for tumor recurrence. Thus, the identification of factors that are predictive for tumor recurrence is urgently needed. The aim of this study was to test the prognostic value of serum albumin levels on tumor recurrence in patients with stage I non-small cell lung cancer. Methods: Stage I non-small cell lung cancer patients who underwent complete surgical resection of the primary tumor at Zhejiang Hospital were analyzed in this study. Serum albumin levels were measured before surgery and once again after surgery in 101 histologically diagnosed non-small cell lung cancer patients. Correlations between the pre- and post-operative serum albumin levels and various clinical demographics and recurrence-free survival rates were analyzed. Results: Patients with pre-operative hypoalbuminemia (<3.5 g/dl) had a significantly worse survival rate than patients with normal pre-operative serum albumin levels (≥3.5 g/dl) (p=0.008). OBJECTIVE: Hypoalbuminemia is a common clinical deficiency in burn patients and is associated with complications related to increased extravascular fluid, including edema, abnormal healing, and susceptibility to sepsis. Some prognostic scales do not include biochemical parameters, whereas others consider them together with comorbidities. The purpose of this study was to determine whether serum albumin can predict mortality in burn patients. METHODS: We studied burn patients ≥16 years of age who had complete clinical documentation, including the Abbreviated Burn Severity Index, serum albumin, globulin, and lipids. Sensitivity and specificity analyses were performed to determine the cut-off level of albumin that predicts mortality. RESULTS: In our analysis of 486 patients, we found that mortality was higher for burns caused by flame (p = 0.000), full-thickness burns (p = 0.004), inhalation injuries (p = 0.000), burns affecting >30% of the body surface area (p = 0.001), and burns associated with infection (p = 0.008). OBJECTIVE: To investigate the effect of obstructive sleep apnea and continuous positive airway pressure treatment on serum butyrylcholinesterase activity and ischemia-modified albumin levels. METHODS: Thirty-two patients with obstructive sleep apnea and 30 age- and sex-matched controls were enrolled and underwent a diagnostic polysomnogram. The serum butyrylcholinesterase activity, ischemia-modified albumin levels, metabolic parameters, and polysomnography scores were detected and evaluated. Nine patients were studied before and after treatment with continuous positive airway pressure. RESULTS: The serum ischemia-modified albumin levels were significantly higher and the butyrylcholinesterase activity was significantly lower in patients with obstructive sleep apnea than in the controls (p<0.001). The continuous positive airway pressure treatment decreased the modified albumin levels and elevated the buthrylcholinesterase activity (p = 0.019 and p = 0.023, respectively). The modified albumin levels were positively correlated with the apnea-hypopnea index (r = 0.462, p = 0.008) at baseline. Elevated ischemia-modified albumin levels can be more accurate than butyrylcholinesterase activity at reflecting the presence of obstructive sleep apnea. Background: The anatomy of PFO suggests that it can allow thrombi and potentially harmful circulatory factors to travel directly from the venous to the arterial circulation – altering circulatory phenotype. Our previous publication using high-resolution LC-MS/MS to profile protein and peptide expression patterns in plasma showed that albumin was relatively increased in donor samples from PFO-related than other types of ischemic strokes. Since albumin binds a host of molecules and acts as a carrier for lipoproteins, small molecules and drugs, we decided to investigate the albumin-bound proteins (in a similar sample cohort) in an effort to unravel biological changes and potentially discover biomarkers related to PFO-related stroke and PFO endovascular closure. Methods: The method used in this study combined albumin immuno-enrichment with high resolution LC-MS in order to specifically capture and quantify the albumin-bound proteins. Background: Albumin is a critical component in the standard therapeutic approach to acute renal failure (ARF) and spontaneous bacterial peritonitis (SBP) in the setting of ascites. However, data regarding the safety and minimum effective dose are limited. Methods: We conducted a retrospective review of patients with decompensated cirrhosis who received albumin within the first 48 hours of hospitalization at Beth Israel Deaconess Medical Center between 2010 and 2013. Outcomes included 90-day risk of death or transplantation (primary) and (secondary) complications of albumin infusion (length of stay (LOS) and need for critical care)), all adjusted for comorbidity and severity of illness. Results: We included 169 patients with ARF and 88 patients with SBP. The optimal doses of albumin for a survival benefit were found to be 87.5 g and 100 g in the ARF and SBP cohorts, respectively. The odds ratio (OR) for the 90-day risk of death or liver transplantation associated with the optimal loading dose was 0.36 (95% CI: 0.17–0.76, P = 0.008) and 0.28 (95% CI: 0.07–0.97, P = 0.04) for the ARF and SBP cohorts, respectively. Einleitung: Die aktuelle Arteriosklerosetheorie sieht in Störungen der endothelialen NO-Produktion einen Triggermechanismus morphologischer Veränderungen im Prozess zum stenosierenden Plaque. Entsprechend der vorliegenden Studienlage kommt der Diagnostik einer gestörten endothelialen NO-Bioverfügbarkeit somit eine relevante prognostische Bedeutung in der Primärprävention kardiovaskulärer Erkrankungen zu. Insbesondere in der Diabetologie hat sich als weiterer kardiovaskulärer Risikoprädiktor die quantitative Bestimmung der renalen Albuminausscheidung etabliert. Unklar ist jedoch, ob im Rahmen einer optimalen Frühdiagnostik kardiovaskulärer Hochrisikogruppen die Bestimmung der renalen Albuminausscheidung ausreicht, oder ob Störungen in der endothelialen NO-Bioverfügbarkeit bzw. pathologisch erhöhte vWF-Werte bereits vor Auftreten einer Mikroalbuminurie nachweisbar sind. OBJECTIVES: To develop, optimize, and validate a generalized mass action, equilibrium solution that incorporates measured concentrations of albumin as well as cortisol binding globulin (CBG) to estimate free cortisol. DESIGN AND METHODS: Free cortisol was estimated by Coolens method or by cubic equilibrium equation and compared to measured free cortisol, determined by ultrafiltration method, in subjects with septic shock (n=45), sepsis (n=19), and healthy controls (n=10) at 0, 30, and 60 min following administration of cosyntropin (250 mcg). The data set also included repeat testing in 30 subjects following recovery from sepsis/septic shock. The equilibrium dissociation constant for cortisol binding to albumin (K(A)) was optimized by non-linear regression. The cubic equilibrium solution was also used to model the influence of cortisol, CBG, and albumin concentration on free cortisol. RESULTS: Compared to measured free cortisol, the cubic solution, using an optimized K(A) of 137,800 nM, was less biased than Coolens solution, with mean percent error of -23.0% vs. -41.1% (paired t test, P<0.001). INTRODUÇÃO: Produtos de glicação avançada (AGE) encontram-se aumentados no diabete melito e contribuem independentemente para o risco cardiovascular. O reconhecimento dos AGE pelo receptor para produtos de glicação avançada (RAGE) potencializa vias de sinalização pró-aterogênicas. Antagonistas do receptor de angiotensina II (ANGII) do tipo 1 (AT-1) diminuem a expressão de RAGE em área de lesão aterosclerótica. No presente projeto, avaliou-se, na aorta de camundongos dislipidêmicos (knockout para apoE) tratados ou não com inibidor do receptor AT-1 (losartana, LOS), o efeito do tratamento crônico com albumina-AGE sobre o infiltrado de lípides, a expressão de mRNA e proteínas envolvidas no eixo AGE/RAGE, ANGII/AT-1, modulação da resposta inflamatória e fluxo de lípides, além da secreção de citocinas inflamatórias por macrófagos tratados com albumina controle ou AGE, concomitantemente ou não a losartana, isolados da cavidade peritoneal de camundongos apoE knockout.
