Estudaram-se as propriedades ácido base e de complexação com íons Cu(II) dos ácidos húmico (AH) e fúlvico (AF) extraídos de vermicomposto. O isolamento e a purificação foram realizados de acordo com o método descrito pela Sociedade Internacional de Substâncias Húmicas (IHSS). Estudaram-se as propriedades ácido-base por titulação potenciométrica em meio de KNO3 0,10 mol L-1 utilizando-se funções de Gran modificadas. Caracterizaram-se cinco classes de grupos ionizáveis, constatando-se predominância de grupos carboxílicos sobre grupos fenólicos e amínicos, especialmente para o AF, cuja acidez total foi maior do que a do AH. Estudaram-se as propriedades complexantes com íons Cu(II) nos pH 4, 5 e 6 por titulação potenciométrica com um eletrodo íon-seletivo para Cu(II), adotando-se os métodos de Scatchard e de regressão não linear com a equação de Langmuir para tratamento de dados. Em pH 5 e 6 caracterizaram-se duas classes de sítios de complexação, enquanto em pH 4, apenas uma classe foi caracterizada. De maneira geral, a estabilidade e a capacidade de complexação aumentaram com o pH, indicando a competição de prótons pelos sítios de complexação. Os resultados indicam maior estabilidade e capacidade para a complexação com AH em relação ao AF. O principal interesse desta pesquisa foi descrever e analisar o resultado do aprendizado de conceitos relacionados ao tema equilíbrio químico buscando as concepções dos alunos, sobre aspectos gerais dos sistemas em equilíbrio químico e os aspectos gerais de sistemas ácido base em equilíbrio químico. Para isso acompanhamos e gravamos o áudio das aulas referentes ao ensino do conceito de equilíbrio químico ao longo das disciplinas de Química Geral II e Química Analítica I, no IQUSP. Ao longo das disciplinas aplicamos avaliações iniciais, progressivas e finais. Os resultados foram apresentados em três itens, no primeiro apresentamos a análise da avaliação inicial (AI) e avaliação final (AF), aplicadas na disciplina de Química Geral II (QGII), onde discutimos as concepções sobre aspectos gerais dos sistemas em equilíbrio químico. Metabolic acid-base status in critically ill patients: is standard base excess correlated with serum lactate level?

BACKGROUND AND OBJECTIVES: To correlate standard base excess (SBE) with serum lactate level and demonstrate the independent prognostic significance of each one. METHODS: In a retrospective study, we retrieved data from 333 patients of our prospectively collected database of 7-bed medical intensive care unit of a 1800-bed university hospital. RESULTS: The results have shown a poor correlation between SBE and lactate, r = - 0.358, p < 0.001, and an independent prognostic significance of each one when analyzed concomitantly, odds ratio (95% Confidence interval) = 0.996 (0.992 - 0.999) to standard base excess and 1.000 (1.000 - 1.002) to lactate at entrance; and odds ratio (95% Confidence interval ) = 0.990 (0.985 - 0.994) to standard base excess and 1.003 (1.001 - 1.005) to lactate after 24 hours. The accuracy of standard base excess was close to lactate to determine in-intensive care unit death. CONCLUSIONS: The lactic component of the metabolic acidosis is not the major determinant of standard base excess. Serum lactate and SBE are independent outcome predictors in critically ill patients.

Previous studies in metabolic alkalosis have demonstrated that two factors are the prime determinants of acid excretion and bicarbonate reabsorption; first, the diversion to distal exchange sites of sodium previously reabsorbed in the proximal tubule and loop of Henle; and, second, a stimulus to sodium-cation exchange greater than that produced by a low-salt diet alone. In the present study we have examined the hypothesis that these two factors are also the prime determinants of acid excretion during the administration of mineral acid loads. To test this hypothesis, we have administered to dogs ingesting a low NaCl diet a daily dose of 7 meq/kg of H+ with anions (chloride, sulfate, or nitrate) whose differing degrees of reabsorbability influence the speed and completeness with which each is delivered to the distal nephron with its accompanying Na+. After 2-3 wk of acid administration, and after an initial urinary loss of Na+ and K+, the steady-state value for plasma [HCO3-] was 8.6 meq/liter below control in the HCl group, 3.7 meq/liter below control in the H2SO4 group, and unchanged from control in the HNO3 group; all of these values were significantly different from each other. Recent studies have shown that chronic hypotonic volume expansion (HVE) induced by administration of vasopressin and water stimulates distal hydrogen ion secretion and thereby (a) permits dogs with HCl-acidosis to restore acid-base equilibrium to normal despite continued acid feeding and (b) permits normal dogs to conserve filtered bicarbonate quantitatively despite the natriuresis induced by water retention. To examine whether these effects of chronic HVE are mediated by augmented mineralocorticoid secretion, urinary and plasma aldosterone levels were monitored during prolonged administration of vasopressin. In HCl-fed animals, the HVE-induced rise in plasma [HCO3] (from 13.8 to 21.3 meq/liter) was associated with a rise in aldosterone excretion from 0.45 to 0.88 mug/day (P less than 0.02). In normal animals, in which plasma [HCO3] remained stable during HVE (21.9 vs. 20.0 meq/liter), aldosterone excretion rose from 0.51 to 2.28 mug/day (P less than 0.02) and plasma aldosterone concentration rose from 8.1 to 39.8 ng/100 ml (P less than 0.01). In purple membrane added with general anesthetics, there exists an acid-base equilibrium between two spectral forms of the pigment: bR570 and bR480 (apparent pKa = 7.3). As the purple 570 nm bacteriorhodopsin is reversibly transformed into its red 480 nm form, the proton pumping capability of the pigment reversibly decreases, as indicated by transient proton release measurements and proton translocation action spectra of mixture of both spectral forms. It happens in spite of a complete photochemical activity in bR480 that is mostly characterized by fast deprotonation and slow reprotonation steps and which, under continuous illumination, bleaches with a yield comparable to that of bR570. This modified photochemical activity has a correlated specific photoelectrical counterpart: a faster proton extrusion current and a slower reprotonation current. The relative areas of all photocurrent phases are reduced in bR480, most likely because its photochemistry is accompanied by charge movements for shorter distances than in the native pigment, reflecting a reversible inhibition of the pumping activity.

A 0.9 per cent solution of sodium chloride when given intravenously to anesthetized naturally nephropathic animals is not effective in preventing the development of an acid intoxication and the associated kidney injury. 2. A solution of sodium carbonate equimolecular with a 0.9 per cent solution of sodium chloride when given intravenously to anesthetized naturally nephropathic animals confers a variable degree of protection to the kidney. 3. The degree of protection conferred by the alkaline solution is associated with the ability of the solution to maintain a normal acid-base equilibrium of the blood of the anesthetized animal.

1. The acid-base equilibrium of the blood as indicated by determinations of the reserve alkali of the blood remains constant in non-pregnant animals at different age periods. This statement does not imply that the acid-base balance of such animals at different age periods is a stable balance. 2. In pregnant animals of the same age periods as the control animals there may develop an instability of this equilibrium which is either associated with the occurrence of a renal injury and which may be looked upon as a retention phenomenon, or arises independently of such an injury. In this latter group of animals the disturbance in the equilibrium increases in frequency and is earlier in its appearance in the gestation period as the age of the organism increases. 3. In old and in senile pregnant animals some physiological mechanism other than that of the kidney becomes unable to stabilize the acid-base equilibrium of the blood with the result that as the pregnancy advances this physiological state of the organism (gestation) becomes pathological.

The rate at which displacement and recovery of the acid-base equilibrium of the blood occur in young adult males subjected to short periods of maximal exertion has been determined. Displacement of acid-base equilibrium produced by severe exercise is along the fixed acid path, similar to the path of displacement produced by ingestion of acidifying agents such as ammonium chloride. Maximum displacement of the acid-base equilibrium is not reached until 7 to 10 minutes after the cessation of exercise. By this time over 50 per cent of the displacement in oxygen consumption, respiratory volume, and blood pressure have disappeared. A much greater metabolic acidosis was produced by exercise than could be induced by the oral administration of ammonium chloride. Recovery from the metabolic acidosis produced by exercise was much more rapid (10 times) than was recovery from the acidosis produced by ammonium chloride. After exercise the pH, returned to normal values more rapidly than did the bicarbonate content of the serum.

Acid–base equilibrium constants of triethanolamine (TEA) have been determined by potentiometric titrations with a glass electrode, at 25 °C. Ionic strength was kept constant with only one electrolyte (using one of these salts: NaCl, KCl, MgCl2 or CaCl2), with binary mixtures of MgCl2 and CaCl2, and finally, in a solution with a composition approximately similar to that of natural seawater without sulfate. Equilibrium constants have been expressed in function of ionic strength by means of Pitzer equations and interaction parameters proposed in this theory have been obtained. It has been found that acid–base behaviour of TEA depends greatly on the salt used: basicity of TEA is decreased by CaCl2, while it is increased by the other electrolytes used in this work.

OBJECTIVES:To study the effect of two intravenous maintenance fluids on plasma sodium (Na), and acid-base balance in pediatric intensive care patients during the first 24h of hospitalization. METHODS: A prospective randomized controlled study was performed, which allocated 233 patients to groups: (A) NaCl 0.9% or (B) NaCl 0.45%. Patients were aged 1 day to 18 years, had normal electrolyte concentrations, and suffered an acute insult (medical/surgical). MAIN OUTCOME MEASURED: change in plasma sodium. Parametric tests: t-tests, ANOVA, X2 statistical significance level was set at α=0.05. RESULTS: Group A (n=130): serum Na increased by 2.91 (±3.9)mmol/L at 24h (p<0.01); 2% patients had Na higher than 150mmol/L. Mean urinary Na: 106.6 (±56.8)mmol/L. No change in pH at 0 and 24h. Group B (n=103): serum Na did not display statistically significant changes. Fifteen percent of the patients had Na<135mmol/L at 24h. The two fluids had different effects on respiratory and post-operative situations. CONCLUSIONS: The use of saline 0.9% was associated with a lower incidence of electrolyte disturbances.

Introducción: La hemorragia digestiva (HVDA) es la principal causa de descompensación en pacientes con cirrosis. Caracterizar el estado ácido-base de estos pacientes sería útil para reflejar la severidad del sangrado e identificar pacientes con alto riesgo de complicación. Objetivo: Describir el estado ácido-base de los pacientes que consultaron a urgencias con cirrosis descompensada por HVDA y posteriormente fueron manejados en la unidad de cuidado intensivo (UCI) o fallecieron. Métodos: Se realizó el análisis del estado ácido-base a 10 pacientes con estas características, utilizando tres métodos distintos. Resultados: El perfil ácido-base encontrado fue: acidosis metabólica por iones no medidos, acidosis láctica, alcalosis por hipoalbuminemia y anión gap elevado en la mayoría de pacientes. Conclusiones: La teoría de Henderson-Hasselbach no fue suficiente para identificar pacientes con alto riesgo, debería implementarse concomitantemente el análisis anión gap, base déficit y el método físico–químico, para entender los fenómenos acido base de estos pacientes.; Introduction: Upper gastrointestinal bleeding (HVDA) is the leading cause of acute complications among patients with cirrhosis. Introducción: Los pacientes con lesiones térmicas presentan alteraciones fisiológicas complejas que hacen difícil la caracterización del estado ácido-base y así mismo alteraciones electrolíticas e hipoalbuminemia que pudieran estar relacionados con un peor pronóstico. Se ha estudiado la base déficit (BD) y el lactato, encontrando una gran divergencia en los resultados. Por lo anterior, el análisis físico-químico del estado ácido-base podría tener un rendimiento superior a los métodos tradicionales. Metodología: Se realizó el análisis de una serie de casos de 15 pacientes mayores de 15 años, con superficie corporal quemada mayor al 20% que ingresaron a una unidad de cuidado intensivo (UCI) de quemados, dentro de las siguientes 48 horas del trauma. Para el análisis se utilizaron tres métodos distintos: 1) método convencional basado en la teoría de Henderson-Hasselbalch, 2) anión-gap (AG) y anión-gap corregido por albúmina, 3) análisis físico-químico del estado ácido-base según la teoría de Stewart modificado por Fencl y Figge. Resultados: Por el método de Henderson-Hasselbalch, 8 pacientes cursaron con acidosis metabólica, 4 pacientes con una BD leve, 5 pacientes con una BD moderada y 5 pacientes con una BD severa. 1. The naturally acquired chronic glomerulonephropathies of the dog are not due to an acid intoxication. 2. Such an injury renders the acid-base equilibrium of the animal unstable and susceptible to an agent such as an anesthetic which tends to induce an acid intoxication. 3. When naturally nephropathic animals are anesthetized by Grehant's anesthetic, the principal anesthetic ingredient of which is chloroform, the animals develop an acid intoxication, and become anuric and non-responsive to diuretic substances. 4. The development of the anuria has been constantly associated with swelling, vacuolation, and necrosis of the convoluted tubule epithelium. 5. In the kidneys of these animals there occurs an accumulation of fat which is largely confined to the ascending limbs of Henle's loops and which shows a quantitative relation with the degree of acid intoxication.

O equilíbrio hidroeletrolítico e o equilíbrio acidobásico são assuntos de importância para todas as especialidades. Porém, seu entendimento prático é, por vezes, revestido de variados graus de dificuldades. Para investigar o efeito da idade sobre o equilíbrio ácido-básico de bovinos neonatos, colheram-se amostras de sangue venoso de 300 bezerras sadias da raça Holandesa distribuídas por 15 grupos etários: do nascimento até 8 h; de 8 a 16 h; de 16 a 24 h; 2 d; 3 d; 4 d; 5 d; 6 e 7 d; 8 e 9 d; 10 e 11 d; 12 e 13 d; 14 e 15 d; 16 a 20 d; 21 a 25 d; e 26 a 30 d. Determinaram-se os valores de pH, pCO2, pO2, HCO3-, TCO2, BE, StB e SatO2, testando-se o efeito da idade por meio da Análise de Variâncias. Nas bezerras em seu primeiro dia de idade, estudaram-se as correlações das variáveis com o tempo de vida em horas; e a influência da condição de parto (espontâneos contra auxiliados ou distócicos). O pH e a pCO2 foram menor e maior, respectivamente, no primeiro grupo; e o BE e o StB mais reduzidos até 16 horas de vida, com uma tendência a aumentarem até o quinto dia. Ao longo do primeiro dia de vida o pH, o BE e o StB se elevaram e a pCO2 diminuiu. A condição do parto não atuou, em geral, como um fator adicional de variação. Tais resultados reforçam as evidências de que o estado de acidose mista exibido ao nascimento tende a se reverter nos primeiros dias de vida; e comprovam que os valores das variáveis hemogasométricas mantêm-se estabilizados ao longo de todo o período neonatal restante.; Venous blood samples of 300 healthy female Holstein calves were used to study the effect of the age on the acid-base balance in the neonatal period. The acid-base equilibrium is essential for the maintenance of human life. It ensures optimal conditions for cell function, regulating the right amount of hydrogen ions in the blood. In an intensive care unit there are many diseases that alter blood pH, for example, the sepsis. During its evolution, disseminated intravascular coagulation occurs which impairs the body perfusion, resulting in a distributive shock and a potential ischemia of the organs. The arrival of oxygen deficiency in cells stimulates anaerobic metabolism, causing excessive lactate and severely decreasing the blood pH. Given the enormity of the problem, this article aims to answer the clinical question: in the critical patient, which is the effective management of acid-base equilibrium in the treatment of septic shock? This is a literature review through computerized search over the months between April and November 2010. As the most important activities of nursing were shown the control of diuresis, water balance and blood gases analysis.
