Intravenous zanamivir is a neuraminidase inhibitor suitable for treatment of hospitalized patients with severe influenza. Patients were treated with intravenous zanamivir 600 mg twice daily, adjusted for renal impairment, for up to 10 days. Primary outcomes included adverse events (AEs), and clinical/laboratory parameters. Pharmacokinetics, viral load, and disease course were also assessed.

Los criterios del RIFLE y del AKIN han ayudado definitivamente a llamar la atención sobre la relación entre un deterioro de la función renal que produce un pequeño incremente de la concentración sérica de creatinina y un peor pronóstico. Sin embargo, la utilidad clínica concreta del uso de estos criterios permanece por definir. Se cree que la principal utilidad de estos criterios reside en su uso en estudios epidemiológicos y en ensayos clínicos, para definir criterios de inclusión y objetivos de una intervención.

A syndrome encompasses a number of nosologic entities that do not necessarily share the same cause, pathogenesis, structural abnormalities, and treatment. The identification of a syndrome has different diagnostic and therapeutic implications because it prompts further testing to achieve the diagnosis of a specific disease. For instance, the diagnosis of acute coronary syndrome in a patient presenting with chest pain requires a more-specific diagnosis (ie, acute myocardial infarction) to direct therapy. The discovery of biomarkers has been a key advance to define one of the underlying causative entities (eg, acute myocardial infarction) and, therefore, allows appropriate identification of patients with a specific condition and treatment.

La reanimación con fluidos es esencial para la supervivencia del paciente crítico en shock, independientemente de la causa que lo origine. Hoy en día disponemos de diversos cristaloides y coloides (sintéticos y naturales), existiendo una viva controversia sobre qué tipo de fluidos debemos emplear y los posibles efectos adversos asociados a su uso, especialmente el desarrollo de fracaso renal con necesidad de técnicas de reemplazo renal. Por ello, hemos realizado una revisión narrativa de la literatura con el fin de elaborar unas recomendaciones prácticas sobre el empleo de fluidos en la fase de reanimación del paciente crítico adulto y que se presentan en este documento.

La epidemia causada por el virus del Ébola (VE) no es nueva en África. El VE se identificó por primera vez en1976. El actual es el 25.° brote conocido de infección por el VE1, 2. A 28 de noviembre, se habían registrado un total de 16.933 casos (10.585 confirmados en laboratorio), con 6.002 fallecimientos3. Los casos registrados corresponden a países con una transmisión extendida (16.899 casos en Guinea, Liberia y Sierra Leona), países con un caso inicial o transmisión limitada (4 casos en los EE.UU. y 8 casos en Mali) y países afectados previamente (uno en Senegal, 20 en Nigeria y uno en España, donde los brotes se declararon erradicados el 17 de octubre, el 19 de octubre y el 2 de diciembre de 2014, respectivamente).

La cistectomía KeyPort® umbilical constituye en nuestro entorno la alternativa menos invasiva posible para tratar eficientemente el cáncer de vejiga músculo-invasivo, con excelente resultado estético, mínimo dolor postoperatorio y corta estancia hospitalaria. La cicatriz umbilical disminuye los requerimientos analgésicos y acaba siendo prácticamente invisible, por lo que supone un paso importante en el desarrollo de la cirugía mínimamente invasiva para el cáncer vesical.

Laparoendoscopic single-site (LESS) radical prostatectomy (RP) has been performed through different approaches. A new DuoRotate manual system developed by Richard Wolf (KeyPort; Richard Wolf GmbH, Knittlingen, Germany) can be applied to RP. Our aim was to describe the surgical technique and report early outcomes of KeyPort LESS-RP to determine if this procedure is feasible and safe. Prospective study performed between October 2011 and January 2012 to standardize LESS-RP. A total of 31 procedures were performed (10 with and 21 without neurovascular preservation, 8 with and 23 without pelvic lymph node dissection).

The utility of a virtual reality simulator for training of the photoselective vaporization of the prostate with diode laser was studied. Two experiments were performed with a simulator (VirtaMed AG, Zürich, Switzerland) with software for specific training in prostate vaporization in contact mode with Twister fiber (Biolitec AG, Jena, German). Eighteen surgeons performed ablation of the prostate (55 cc) twice and compared the score obtained (190 points efficacy and 80 safety) in the second one of them by experience groups (medical students, residents, specialists). They also performed a spatial orientation test with scores of 0–6. After, six of these surgeons repeated 15 ablations of the prostate (55 and 70 ml). Improvement of the parameters obtained was evaluated to define the learning curve and how experience, spatial orientation skills and type of sequences performed affects them.

The objective of this study is to define the natural history of T1G3 bladder tumor not receiving intravesical Bacillus Calmette-Guerin (BCG) and to assess the diagnostic and therapeutic value of a second transurethral resection (Re-TUR) in these patients. Retrospective study on the natural history of 210 patients treated at two institutions for T1G3 bladder carcinoma without associated CIS. In no case was BCG administered; 79 (37.6%) received TUR alone, and 131 (62.4%) Re-TUR 4–6 weeks later; 23 (12.4%) underwent cystectomy for tumor progression.

DNA hypermethylation has emerged as a novel molecular biomarker for the evaluation of prostate cancer diagnosis and prognosis. Defining the specific gene hypermethylation profile for prostate cancer could involve groups of genes that specifically discriminate patients with indolent and aggressive tumors. Genome-wide methylation analysis was performed on 83 tumor and 10 normal prostate samples using the GoldenGate Methylation Cancer Panel I (Illumina, Inc.). All clinical stages of disease were considered.
