Perspectives on sexual and reproductive health among women in an ancient mining area in Brazil

The purpose of this study was to describe the reproductive profile and frequency of genital infections among women living in the Serra Pelada, a former mining village in the Para state, Brazil. A descriptive study of women living in the mining area of Serra Pelada was performed in 2004 through interviews that gathered demographics and clinical data, and assessed risk behaviors of 209 randomly-selected women. Blood samples were collected for rapid assay for HIV; specimens were taken for Pap smears and Gram stains. Standard descriptive statistical analyses were performed and prevalence was calculated to reflect the relative frequency of each disease. Of the 209 participants, the median age was 38 years, with almost 70% having less than four years of education and 77% having no income or under 1.9 times the minimum wage of Brazil. About 30% did not have access to health care services during the preceding year. Risk behaviors included: alcohol abuse, 24.4%; illicit drug abuse, 4.3%; being a sex worker, 15.8%; and domestic violence, 17.7%. Abnormal Pap smear was found in 8.6%. Este estudo, de referencial materialista histórico dialético (MHD), inserido numa perspectiva de gênero, teve dentre seus objetivos caraterizar as mulheres de acordo com a inserção social, idade, vínculo com a instituição, antecedentes ginecológicos e obstétricos; identificar as contradições acerca da percepção quanto ao primeiro exame ginecológico de prevenção de câncer de colo uterino e de mama e identificar caminhos para a transformação da atenção ginecológica. A análise dos dados foi realizada através da depreensão de temas contidos nas gravações, nas quais se identificaram como categorias empíricas à atenção à saúde, o exame ginecológico (razão, conhecimento e sentimentos) e a mulher, sua vida e saúde. Verificou-se que a atenção continua circunscrita ao “corpo biológico”, uma atitude alienante assumida pela usuária, reforçada pela mídia, pelo pessoal de saúde e pela própria mulher. Foram identificados também sentimentos ligados ao medo, à vergonha e ao sentimento de culpa. Introdução: o aumento da expectativa de vida no Brasil e no mundo, com destaque para o gênero feminino, reflete a necessidade de estudos que se dediquem a investigar o cuidado à mulher idosa pelo enfermeiro, considerando a perspectiva dos sujeitos envolvidos nessa relação de cuidado. Objetivo: compreender o cuidado à saúde na perspectiva das mulheres idosas e dos enfermeiros. Metodologia: estudo fundamentado na Fenomenologia Social de Alfred Schütz realizado no município de Cascavel, Paraná-Brasil, em nove Unidades de Saúde, sendo sete Unidades Básicas e duas pertencentes à Estratégia Saúde da Família. Participaram dez enfermeiros que atuam no cuidado à saúde das mulheres idosas e oito idosas cuidadas por enfermeiros. Os depoimentos foram obtidos no período de setembro de 2010 a janeiro de 2011 por meio de entrevista aberta, com as seguintes questões norteadoras para as mulheres idosas: conte para mim sobre as suas necessidades de cuidado à saúde neste momento de sua vida. A chegada do primeiro filho traduz um cenário de intensas mudanças na rotina das mulheres, o qual é pautado por diferentes percepções a respeito da sexualidade e da maternidade. As formas como as novas mães percebem tais mudanças e, identificam suas necessidades em relação ao exercício da sexualidade exercem influência sobre o cotidiano e a saúde das mulheres. Este estudo teve por objetivo compreender como as mulheres que se tornaram mães pela primeira vez percebem as mudanças e identificam as suas necessidades em relação ao exercício da sexualidade durante o primeiro ano do período pós-parto. Justificativa: O câncer de mama e do colo de útero são neoplasias com altas taxas de incidência e mortalidade no Brasil e que dispõem de métodos eficazes de rastreamento para detecção precoce, oferecidos pelo Sistema Único de Saúde. OBJECTIVE To analyze the content of policies and action plans within the public healthcare system that addresses the issue of violence against women. METHODS A descriptive and comparative study was conducted on the health policies and plans in Catalonia and Costa Rica from 2005 to 2011. It uses a qualitative methodology with documentary analysis. It is classified by topics that describe and interpret the contents. We considered dimensions, such as principles, strategies, concepts concerning violence against women, health trends, and evaluations. RESULTS Thirteen public policy documents were analyzed. In both countries’ contexts, we have provided an overview of violence against women as a problem whose roots are in gender inequality. The strategies of gender policies that address violence against women are cultural exchange and institutional action within the public healthcare system. The actions of the healthcare sector are expanded into specific plans. The priorities and specificity of actions in healthcare plans were the distinguishing features between the two countries. This dissertation has as general objective discuss the meanings and practices related to reproduction, with special interest in the way women interpret and experience these discourses. Adopting the Sociology and Health Anthropology perspective, this research started from the understanding that reproduction is a social phenomenon whose meanings are constructed throughout history and from values and cultural beliefs that change continuously. The research data were obtained with ethnography support and started at a public health unit that integrates the basic care level in the city of Maceió-AL. The informants were women seeking reproductive health services and professionals working with this demand. As a result, a context permeated by inadequacies of public health assistance was registered, highlighting the SUS representation as a health plan focused on the poor, whose impasses reproduce inequalities. The relationship between professionals and users is marked by tensions and conflicts arising largely from dissatisfactions on the structure provided by the public health system. This context of limited rights further accentuates the difficulties women experience in their reproductive lives. This work discusses the evaluation of the satisfaction of the users on the women health care focusing on the quality of the primary care in the State of Rio Grande do Norte-BR. The main objective of this research is evaluate the satisfaction of the users about the actions applied to women health in the primary health care in Rio Grande do Norte, observing the information available through the Programa de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB). The specific objectives are: the evaluation of aspects related to women health; the evaluation of the specific actions related to welcoming the pregnant and; the evaluation of the information related to the postpartum. This dissertation is characterized as an evaluative research made through a multicentric transversal study, using a quantitative approach, which is part of the External Evaluation of the PMAQ-AB in the State of Rio Grande do Norte, made by the Federal University of Rio Grande do Norte. Some secondary data of the interviews with the users who were in the Basic Health Units were used during the External Evaluation of the PMAQ-AB in Rio Grande do Norte. The sample was collected following these criteria: the users that were in the Basic Health Units to attend to any procedure; they must had used the services for at least one year; and they must had agreed to participate the research. The ones that were attending to the services for the first time and the ones that did not use the services for at least 12 months were excluded from the sample. With incidence rates of osteoporosis increasing (Osteoporosis Canada, 2007), preventative efforts to minimize costs associated with condition diagnosis are a public health priority. Cues to action are specific internal (e.g., physical symptoms, family member with a condition) or external stimuli (e.g., public service announcements, health education campaigns) that are necessary to trigger appropriate health behaviours and serve to create an awareness of the health threat (Mattson, 1999). To date, limited understanding of the scope of influence cues to action have on health beliefs and behaviour associated with osteoporosis is known. The present investigation was designed to address this gap in the literature. More specifically, the influence of cues to action, a public service announcement (PSA) developed by Osteoporosis Canada and a bone screening by way of Quantitative Ultrasound, on health beliefs and health-enhancing physical activity (HEPA) across a four week period was investigated. Achieving access to basic health services for the entire population without risk of financial hardship or impoverishment from out-of-pocket expenditures (‘universal health coverage’ or UHC) is a challenge that continues to confront most low- and middle-income countries. As coverage expands in these countries, issues of financial sustainability, efficiency, and quality of care quickly rise to the surface. Strategic health purchasing is an important lever to efficiently manage funds for UHC through the definition of what is purchased (which services and benefits the covered population is entitled to receive), from whom services are purchase (which providers are contracted to deliver the covered services), and how and how much the providers are paid. The assessment was conducted to help inform the design and implementation of Mongolia’s provider payment systems going forward. This paper examines how different patterns of kinship and inheritance affect intergenerational relationships and the ramifications of gender inequality. Peasant societies of pre-industrial Northern Europe are contrasted with those of contemporary South Asia to illuminate some of these relationships. While Northern European kinship and inheritance systems made for high status in youth and a loss of power and status as people aged, South Asian systems make for lower power and status in youth and a rise as people age. From this follow more conflict-ridden relationships between the generations and a stronger conjugal bond in Northern Europe, while in South Asia intergenerational ties are strong and the conjugal bond is weak. This in turn leads to a greater potential for marginalizing women in South Asia, although gender inequality exists in both settings. The convergence of low autonomy due to youth as well as sex amongst young married women in South Asia means that women are at the lowest point in their life cycle in terms of autonomy during their peak childbearing years. This paper addresses two main topics: the effects of conflict on the psychological; and well-being of women methodological issues related to the measurement of psychological well-being. The literature on these topics is reviewed, and data from a qualitative study conducted in northern Uganda is used to explore them in more depth. Finally, the paper considers the ways in which conflict has contributed to women's psychological distress in northern Uganda, and the implications of this study for future programming and research.

A number of development partners, including the World Bank, have been actively supporting the health sector in Afghanistan since 2003-04 (1382 AC). Collectively, they invested more than $820 million between 2003 (1382 AC) and 2008-09 (1387 AC) and played key roles in supporting the government in reshaping the country's health sector. This support continues, with all partners starting new projects aimed at further strengthening the sector and building on the successes that have been achieved. The book is organized as follows. Chapters one-four tell a coherent story about the achievements of the sector between 2002 and 2008 (1381-87AC), the financial resources used to achieve the results, and the contribution the private sector has made to the achievements. Chapters five-eight) look forward. They identify the challenges the sector is facing in meeting human resource needs, expanding the coverage of the basic package of health services (BPHS), and increasing the institutional capacity of the Ministry of Public Health (MoPH). Chapter eight summarizes the lessons learned and provides options for moving forward.

Female Genital Mutilation or Cutting (FGM/C) is a customary practice indigenous to 28 African countries and is also reported among African immigrants in countries in Europe, North America, Australia, and New Zealand. FGM/C is also found in some Muslim communities in the highlands of India. The report documents the extent to which FGM/C is perceived as a public health issue. The social and political situation in Somalia in light of the civil unrest is likely to be a challenge in expanding the reach and effectiveness of FGC interventions particularly in rural communities. The experience of Kouroussa in Haute Guinea through the Population and Reproductive Health Project presented at the development marketplace 2000 competition with the project ending female genital cutting also gave the opportunity to the Bank to address FGM/C in a more systematic way, using all available resources. ABSTRACT Introduction The purpose of this study was to assess specific osteoporosis-related health behaviours and physiological outcomes including daily calcium intake, physical activity levels, bone strength, as assessed by quantitative ultrasound, and bone turnover among women between the ages of 18 and 25. Respective differences on relevant study variables, based on dietary restraint and oral contraceptive use were also examined. Methods One hundred women (20.6 ± 0.2 years of age) volunteered to participate in the study. Informed written consent was obtained by all subjects prior to participation. The study and all related procedures were approved by the Brock University Research Ethics Board. Body mass, height, relative body fat, as well as chest, waist and hip circumferences were measured using standard procedures. The 10-item restrained eating subscale of the Dutch Eating Behaviour Questionnaire (DEBQ) was used to assess dietary restraint (van Strien et al., 1986). Daily calcium intake was assessed by the Rapid Assessment Method (RAM) (Hertzler & Frary 1994). Weekly physical activity was documented by the 4-item Godin Leisure-Time Exercise Questionnaire (Godin & Shephard 1985). Bone strength was determined from the speed of sound (SOS) as measured by QUS (Sunlight 7000S). SOS measurements (m/s) were taken of the dominant and non-dominant sides of the distal one third of the radius and the mid-shaft of the tibia. Objetivos: avaliar o conteúdo e qualidade científica de textos informativos sobre saúde da mulher apresentados em revistas semanais brasileiras. Métodos: pesquisa de corte transversal, a partir da coletânea de textos sobre saúde da mulher, difundidos pelas três revistas semanais do segmento atualidades (Veja, Época e IstoÉ), pelo período de um ano, entre agosto de 2005 e julho de 2006. Inicialmente, foi identificado o tema em saúde da mulher presente no texto jornalístico, como saúde reprodutiva, prevenção, riscos e cuidados, beleza e estética, sexualidade, saúde mental, violência, menopausa e saúde geral. Em seguida, outras variáveis foram identificadas, como o gênero informativo do texto, que poderia ser nota, notícia, reportagem ou entrevista; a presença de ilustração; o tipo de fonte que deu informações ao repórter, seja ela científica, empresarial, governamental e a região geográfica de origem da informação. Para avaliar a qualidade científica dos textos, 80 unidades foram analisadas de forma independente por dois médicos e dois jornalistas, através de questionário Index Of Scientific Quality, adaptado para o português e submetido à prova piloto e reteste. OBJETIVO: Analizar las propuestas sanitarias de las políticas públicas y los planes de salud que abordan la violencia contra las mujeres. MÉTODOS: Estudio descriptivo y comparativo de las políticas y planes de salud en Cataluña y Costa Rica del 2005 al 2011. Se utilizó una metodología cualitativa con análisis documental. Se clasificó por temas comunes y se describieron e interpretaron los contenidos. Se consideraron dimensiones como: principios, estrategias, concepción de violencia contra las mujeres, tendencias en salud y evaluación. RESULTADOS: Se analizaron 13 documentos de políticas públicas de género para abordar la violencia contra las mujeres. La desigualdad de género fue concebida como origen de la violencia contra las mujeres en ambos contextos. Las estrategias comunes fueron el cambio cultural y la acción interinstitucional. Las actuaciones sanitarias se ampliaron en planes específicos y en los protocolos y normas de actuación. La prioridad del tema y la especificidad de las actuaciones en los planes de salud fue un rasgo diferenciador entre Cataluña y Costa Rica. Uptake of health care services and health status of HIV-infected women diagnosed through antenatal HIV screening in Barbados, 1996-2004

OBJECTIVES: To study utilization of HIV-related health care services and to describe the health status of HIV-infected women diagnosed through antenatal voluntary counseling and testing (VCT) for HIV infection in Barbados. METHODS: This is a descriptive study. The study population includes all HIV-infected women in Barbados diagnosed as HIV-infected through VCT for HIV infection during 1996-2004. RESULTS: The median duration of HIV infection from time of diagnosis to the time of this report for the 163 women diagnosed during the study period was 72 months (low range, 9 months; high range, 117 months). Of the 163 women, 102 (62.6%) had attended the centralized HIV/AIDS clinic for follow-up (care, treatment, and monitoring), whereas 61 (37.4%) had never attended the clinic. The median time lag between diagnosis of HIV infection and first presentation to the HIV/AIDS clinic was 36 months (low range, 1 month; high range, 114 months). Of the HIV-infected women who attended the HIV/AIDS clinic, more than one-fourth had severe immunodeficiency at the time of their first follow-up visit. Of the 53 women undergoing highly active anti-retroviral therapy (HAART) at the time of the study, 23 (43.4%) began the therapy within three months of their first follow-up visit. CONCLUSIONS: Early HIV diagnosis through antenatal VCT is not enough to ensure that women with HIV will get adequate and timely HIV-related health care. OBJECTIVE To analyze the content of policies and action plans within the public healthcare system that addresses the issue of violence against women. METHODS A descriptive and comparative study was conducted on the health policies and plans in Catalonia and Costa Rica from 2005 to 2011. It uses a qualitative methodology with documentary analysis. It is classified by topics that describe and interpret the contents. We considered dimensions, such as principles, strategies, concepts concerning violence against women, health trends, and evaluations. RESULTS Thirteen public policy documents were analyzed. In both countries’ contexts, we have provided an overview of violence against women as a problem whose roots are in gender inequality. The strategies of gender policies that address violence against women are cultural exchange and institutional action within the public healthcare system. The actions of the healthcare sector are expanded into specific plans. The priorities and specificity of actions in healthcare plans were the distinguishing features between the two countries.
