A pesquisa que fundamenta este trabalho foi realizada na galeria de seguro da Penitenciária Feminina Madre Pelletier, mediante grupo de discussão e escrita, junto a mulheres em cumprimento de pena privativa de liberdade. É abordado o dia-a-dia da vida aprisionada, apresentando peculiaridades do trabalho grupal numa galeria de seguro. Também são exploradas as relações entre viver, conhecer e poder. Neste contexto, o estudo do limite enquanto barreira e contato comporta aspectos não racionais e não conscientes da cognição, que é inerente à vida. A partir das noções de autopoiese, de processos de subjetivação e de invenção de si e do mundo, consideramos que tomar o ser vivo como indivíduo - e o ser humano como um caso do vivo – é apenas uma possibilidade dentre outras. Desse modo, mediante vivências do coletivo dentro de uma instituição onde prima o modo indivíduo de subjetivação, é possível questionar o indivíduo como fundamento para o Direito na criminalização e, conseqüentemente, a aplicação individual da pena. A prática gerencial hegemônica costuma privilegiar a padronização de condutas e a fragmentação do trabalho, em lugar do apoio aos profissionais de saúde no desafio de fazer uma clínica ampliada, que reconheça a singularidade dos sujeitos e grupos. Esta prática gerencial reforça um tipo de clínica que costuma ter dificuldade para lidar com pessoas reais, uma vez que enxerga na complexidade do adoecimento e do sofrimento, apenas os aspectos diagnósticos e recortes disciplinares, com graves conseqüências para a qualidade da atenção. É necessário, portanto, que os profissionais desenvolvam tanto alguma crítica a este tipo de conhecimento universalizante (e ao hábito quase inconsciente de tomar a "parte" pelo "todo") quanto alguma capacidade de lidar com a subjetividade inerente ao trabalho em saúde (do usuário, dos grupos e dos próprios profissionais). Na educação médica há uma experiência longa de ensino e aprendizagem em pequenos grupos (essencialmente para resolução de problemas). Essa metodologia, além de requerer número considerável de docentes-tutores, parece mais adequada à introdução coerente de elementos clínicos na aprendizagem das ciências básicas do que ao ciclo clínico da formação pré-graduada. A sua aplicação às disciplinas clínicas impõe adaptações específicas. Para os alunos da disciplina de Obstetrícia e Ginecologia da Faculdade de Medicina de Lisboa no ano lectivo de 2006-2007foi estabelecido um novo modelo de aulas teórico-práticas de Ginecologia: discussão de uma história clínica breve (com elaboração de hipóteses diagnósticas, subsequente investigação e propostas terapêuticas) em grupos de 8 a 10 alunos seguida de discussão entre os grupos e integração final pelo docente. O total dos alunos do curso foi distribuído por três blocos aproximadamente iguais ao longo do ano. A avaliação da adesão dos alunos à metodologia e a comparação com o modelo clássico de aulas teórico-práticas usado nas aulas de Obstetrícia foram efectuadas através de questionário essencialmente com perguntas fechadas (tipo escala de Likert). Introduction Vibratory exercises are increasingly used in sport training and physical rehabilitation. This study aims to determine the comparative effects of two vibratory frequencies on the neuromuscular leg function in healthy young active females. Methods Twenty-four women (aged 21.9 ± 2.6 years; weight 59.9 ± 7.1 kg) were randomly assigned into 3 groups of 8 subjects: group 25 Hz (G25), group 30Hz (G30), and control group (CG). All intervention programs consisted of 30 training sessions within a 10-week period.The peak torque at 60º/s in concentric and eccentric actions were measured by an isokinetic dynamometer (Biodex System-3, Biodex Corp., Shirley, NY, USA). Fitness tests such Squat Jump, Counter Movement Jump, Stair-Climbing 10-stairs time, 10-m walking time, were also performed. The effects of the interventions were analysed by an adaptation of Analysis of Covariance adjusted by weight. Results Vibratory training at 25Hz induced a significant reduction of peak torque in concentric contraction at the velocity of 60°.sec-l and improved the stair-climbing capacity. All other variables remained unchanged in the three groups. Adrenal metastasis is an unusual site of disease involvement in the natural course of differentiated thyroid carcinoma (DTC). This paper discusses the clinical and imaging features of DTC with adrenal metastasis. An unusual case of unilateral solitary asymptomatic adrenal metastasis in the setting of DTC is described in this report with the imaging features including 131I scintigraphy and Fluorodeoxyglucose- Positron emission tomography/computed tomography. The adrenal metastasis was associated with other sites of metastatic disease involvement and was unidentified on initial pre-treatment evaluation studies. All such suspicious lesions should be further evaluated with clinicoradiological correlation by other imaging modalities. A post-radioiodine therapy scan revealed radioiodine uptake in the thyroid bed, sternum and a focus of intense radioiodine concentration in the left suprarenal region. Spot oblique images and single photon emission computed tomography of the upper abdomen was undertaken to ascertain the position and better characterization of the lesion. A subsequent whole body PET-CT (non-contrast) was done which revealed a well defined 6.5 cm × 5.0 cm left adrenal lesion with a SUVmax (standardized uptake value-maximum) of 9.5 in addition to a fluorodeoxyglucose avid osteolytic sternal lesion. The serum thyroglobulin level was significantly raised (more than 250 ng/mL) with thyroid stimulating hormone being 4.9 μΙU/mL (even following an adequate period of levothyroxine withdrawal)...

The purpose of this study was to evaluate the effects of ovariectomy on intramuscular energy metabolism in adult rats. Based on the results, we discussed the skeletal muscle metabolism in female athlete with sports related amenorrhea. Twenty-five adult (20-week-old) Sprague-Dawley female rats were used. Fifteen rats underwent ovariectomy (OVX group), and the other ten rats were sham-operated (Sham group). One and four weeks after surgery, muscular oxidative capacity was measured using 31P-MR spectra of the gastrocnemius-plantaris-soleus (GPS) muscles group at rest and during electric stimulation. Wet weight and maximum tension of the whole GPS muscles group were also measured. From the MRS measurements, the muscle oxidative capacity in the OVX group was significantly lower than that in the Sham group (p < 0.05) at both one and four weeks after surgery. The muscle’s wet weight one week after surgery in the OVX group was the same as the Sham group, while four weeks after surgery it was significantly greater than that in the Sham group (p < 0.05). There were no significant differences in maximum tension among the groups. In conclusion, in adult rats the oxidative capacity decreased due to ovariectomy despite the increase in muscle weight. It is suggested that the muscular endurance capacity in female adult athletes with sports related amenorrhea may deteriorate.

Recent developments in whole genome sequencing have made a substantial contribution to understanding the genomes, neurotoxins and biology of Clostridium botulinum Group I (proteolytic C. botulinum) and C. botulinum Group II (non-proteolytic C. botulinum). Two different approaches are used to study genomics in these bacteria; comparative whole genome microarrays and direct comparison of complete genome DNA sequences. The properties of the different types of neurotoxin formed, and different neurotoxin gene clusters found in C. botulinum Groups I and II are explored. Specific examples of botulinum neurotoxin genes are chosen for an in-depth discussion of neurotoxin gene evolution. The most recent cases of foodborne botulism are summarised.

Poor informational reading and writing skills in early grades and the need to provide students more experience with informational text have been identified by research as areas of concern. Wilkinson and Son (2011) support future research in dialogic approaches to investigate the impact dialogic teaching has on comprehension. This study (N = 39) examined the gains in reading comprehension, science achievement, and metacognitive functioning of individual second grade students interacting with instructors using dialogue journals alongside their textbook. The 38 week study consisted of two instructional phases, and three assessment points. After a period of oral metacognitive strategies, one class formed the treatment group (n=17), consisting of two teachers following the co-teaching method, and two classes formed the comparison group (n=22). The dialogue journal intervention for the treatment group embraced the transactional theory of instruction through the use of dialogic interaction between teachers and students. Students took notes on the assigned lesson after an oral discussion. Culturally responsive instruction refers to the identification of relevant cultural aspects of students’ lives and infusion of these into the curriculum. This instructional approach assumes that a culturally appropriate curriculum can potentially motivate, engage, and lead students to higher rates of achievement. This quasi-experimental study (N=44) investigated the relationship of culturally responsive instruction and the reading comprehension and attitude of struggling urban adolescent readers. The study incorporated the use of culturally responsive instruction using culturally relevant literature (CRL), the Bluford Series Novels, as authentic texts of instruction. Participants were seventh grade reading students at a Title I middle school in South Florida. After a baseline period, two different classes were taught for 8 weeks using different methods. Hintergrund: Sprunggelenksnahe Frakturen gehören zu den häufigsten Verletzungen nach Berufsunfällen und führen nicht selten zu hohen Behandlungskosten und länger dauernder Arbeitsunfähigkeit. Es gibt bisher wenige Studien zu Therapieeffekten bei gesetzlich unfallversicherten Patienten mit o. g. Traumata nach Arbeitsunfällen. Methodik: Die vorliegende prospektiv-randomisierte Studie evaluiert die Effekte einer 3-4 wöchigen stationären Rehabilitationsbehandlung an 76 Patienten mit arbeitsunfallbedingten sprunggelenksnahen Frakturen und verzögertem Heilungsverlauf. Neben einem standardi-sierten Behandlungsprogramm (Standardgruppe – SG, n = 25) erfolgte in Ergänzung dazu ein Ausdauertraining auf dem Fahrradergometer (Ausdauergruppe – AG, n = 27) bzw. ein Ganzkörper-Vibrationstraining mit einem Galileo Fitness-Gerät (Vibrationsgruppe – VG, n = 24). Zu Beginn und am Ende der stationären Rehabilitationsbehandlung, sowie 4 Wochen und 6 Monate danach wurden die Funktionsparameter Beweglichkeit, Kraft, Ausdauer, Gleichgewicht und der OMA-Funktionsscore nach Olerud/Molander (1984) erfasst. This report is the second in a program of evaluations that the independent evaluation group (IEG) is conducting on the learning that takes place through World Bank projects. Estudo qualitativo, exploratório-descritivo, desenvolvido em uma unidade de internação de pacientes adultos do Hospital de Clínicas de Porto Alegre (HCPA). O objetivo consistiu em analisar o processo grupal da equipe de enfermagem, adotando-se como referencial norteador a Teoria de Grupo Operativo de Pichon- Rivière. Como desdobramento, buscou-se identificar os aspectos dinamizadores e obstaculizadores do processo grupal e os respectivos nós críticos, oportunizando discussões acerca de estratégias que favorecem a articulação do trabalho coletivo. Trata-se de uma abordagem norteada pelos pressupostos da pesquisa-ação em que todos os envolvidos - pesquisadores e sujeitos da pesquisa - interagem em busca de soluções de problemas previamente identificados através de um diagnóstico da situação. A coleta de dados ocorreu no período entre junho e outubro de 2008, em duas etapas. A primeira, de caráter exploratório, consistiu na coleta de informações por meio de questionário semiestruturado junto a enfermeiros e auxiliares de enfermagem. Offering a prognosis in lung cancer: when is a team of experts an expert team?

The outlook for patients with lung cancer is poor, so an accurate estimation of prognosis will underpin treatment decisions and allow patients to make personal plans for the future. However, evidence suggests that there is a variation between doctors in their predictions of outcomes and also they tend to be overoptimistic. Two main questions are addressed in this study: whether multidisciplinary team discussion changes prognostic accuracy of individual clinicians; and whether team discussion improves the accuracy of the team's aggregated prediction. A real‐time study of 50 newly diagnosed patients discussed by a regional lung cancer team was undertaken. A case proforma informed the completion of a pre‐discussion questionnaire by each team member, seeking prognostic predictions at specific time points. This was repeated after team discussion. Medical notes were reviewed at 6 months to establish actual survival status. Group discussion did not significantly change the accuracy of survival predictions for any one clinician, but the team as a whole performed better after case discussion. Predictions which the clinicians were more confident about were found to be no more accurate than those where they were less confident. BACKGROUND—Polymeric feeds have shown variable efficacy in active Crohn's disease (CD) with remission rates from 36% to 82%. Meta-analyses of elemental, peptide, and whole protein feeds have shown a strong negative correlation between remission rate in CD and the long chain triglyceride (LCT) content of the feed. We performed a randomised controlled double blind trial in patients with active CD comparing two single whole protein feeds with LCT supplying 5% or 30% of the total energy.
METHODS—Fifty four patients with active CD (Crohn's disease activity index (CDAI) >200, serum C reactive protein (CRP) 10 mg/l) were randomised to a high or low LCT feed for three weeks. The total amount of energy supplied by fat was identical in the two feeds. Remission was defined as a CDAI ⩽150 and response as a fall in CDAI of ⩾70 or a CRP <10 mg/l.
RESULTS—Overall remission rate by intention to treat was 26% for the low LCT feed and 33% for the high LCT feed (p=0.38). Response was achieved in 33% with the low LCT and in 52% with the high LCT feed (p=0.27). CRP <10 mg/l was achieved in 30% in the low LCT and 33% in the high LCT group (p=0.99). Se evidencian diferencias relevantes en la forma en la que cada profesor ha preparado las discusiones en gran grupo y se presenta una caracterización de sus episodios según dos dimensiones: instrumental y discursiva. Un análisis detallado de los episodios, mediante el estudio de las acciones que se producen en ellos, posibilita la determinación de oportunidades de aprendizaje, hecho que permite constatar una relación directa entre la preparación de la discusión en gran grupo y la generación de oportunidades de aprendizaje.; This study aims to determine how teaching activity affects the generation of mathematical learning opportunities. Este trabajo de tesis doctoral contribuye al estudio de la enseñanza y aprendizaje de las isometrías mediante discusiones en gran grupo con tecnología. Para dar respuesta a cómo potenciar las relaciones entre enseñanza y aprendizaje de las isometrías en dicho entorno, se plantean dos objetivos: 1) Analizar una sistemática de planificación, implementación y evaluación de una secuencia didáctica con discusiones en gran grupo bajo el soporte de un software de geometría dinámica; 2) Detectar oportunidades de aprendizaje de las isometrías y su aprovechamiento en el contexto de la secuencia didáctica creada a partir de la sistemática anterior. Para el análisis de los procesos de enseñanza y aprendizaje en el aula de matemáticas de secundaria se consideran las dimensiones discursiva e instrumental. Se elabora una secuencia didáctica de resolución de problemas de isometrías, que se ajusta a las orientaciones dadas por la sistemática y se diseña un instrumento de análisis inédito a fin de detectar oportunidades de aprendizaje que se hayan generado durante la implementación de la secuencia. Luego se comprueba si algunas de estas oportunidades han sido aprovechadas por los alumnos desde la perspectiva de su progreso matemático. Tanto el diseño de instrumentos inéditos como su aplicación y validación son parte sustancial del trabajo. The different ways of action and reflection facing group phenomenon are well known as controversial. They run against in the dichotomy between the subject and the society, between the parts and the whole. According to the view of the observer either the subject or the group disappears. This fact alone opens a mental abyss impossible to cross over in terms of social and individual phenomena. Conceptions of subject and society, subject and group that might be seen as antagonistic and concurrent, at first sight, might be taken as supplementary under the perspective of complex thinking. This theoretical article aims to bring into discussion the possibility of articulating complexity paradigm by studying small groups. The main theoretical reference is The Complexity Theory in Edgar Morin. In complex thinking, subject, small group and larger context constitute themselves at the same time. No período de 2001 a 2004, foi feita uma pesquisa entre escoteiros da Região Metropolitana de Belo Horizonte – RMBH, a fim de analisar suas práticas informacionais, em particular aquelas feitas utilizando como canal de comunicação a Internet. O presente artigo tem por finalidade apresentar a metodologia de pesquisa adotada na época, em particular discutir o uso da ferramenta de coleta de dados de grupos focais. O artigo coloca em pauta a possibilidade de se realizar grupos focais online, discutindo as vantagens e desvantagens do uso dessa técnica via Internet. Não se pretende apresentar, aqui, a pesquisa completa, incluindo sua fundamentação teórica, justificativa, motivação, coleta e análise de dados e, finalmente, resultados.
