As Úlceras Venosas (UV) desenvolvem-se devido à Insuficiência Venosa Crônica, caracterizada como uma condição crônica que afeta o estado de saúde das pessoas acometidas. Têm maior incidência em mulheres, 80% dos casos, com índice de prevalência de 70% em pessoas acima de sessenta anos. Seu tratamento gera elevados custos. A pessoa com UV necessita de acompanhamento e monitoramento, com terapia tópica envolvendo uso de produtos e recomendações baseadas em estudos com evidência científica. Na Atenção Básica, particularmente na Estratégia de Saúde da Família, há possibilidade de se ter acesso e aproximar-se destes usuários, como também de se estabelecer vínculo, atributos essenciais no cuidado ao usuário. Adolescentes do sexo feminino são a população mais frequentemente acometida pela anorexia nervosa (AN), com prevalência média de 2,5%, quando considerados critérios diagnósticos adaptados para esta faixa etária. A apresentação da AN em adolescentes é semelhante a de adultos, no entanto existem particularidades nos sintomas relacionadas ao nível de desenvolvimento cognitivo e emocional. A AN manifesta-se por: perda de peso, perturbações na forma de vivenciar a forma corporal, medo de engordar, restrição alimentar, comportamentos compensatórios e alterações menstruais. No Brasil, existem poucos recursos especializados para o tratamento da AN na adolescência e não foram realizados previamente estudos sistematizados sobre o tema. Estudos realizados em países de língua inglesa demonstram que o tratamento familiar (TF) apresenta bons resultados no tratamento da AN em adolescentes. Este trabalho objetivou conhecer o processo de busca e a clientela atendida em terapia de casal e família. Para tanto, realizaram-se dois estudos. Inicialmente, através de uma revisão sistemática da literatura, foram identificados seis fatores associados à busca pelas terapias de casal e família: fatores pessoais, relacionais, culturais, de gênero, estratégias anteriores de ajuda e tipo de problema. Posteriormente, através de um estudo empírico, a clientela atendida em terapia familiar em uma clínica-escola de Porto Alegre (RS) foi caracterizada. Foram realizadas entrevistas estruturadas com 41 adultos encaminhados para terapia familiar, nas quais foram investigadas variáveis sociodemográficas e familiares, o processo de busca de ajuda, o problema apresentado, tentativas anteriores de ajuda utilizadas e expectativas sobre o tratamento. Objective: To explore the effect of cognitive behavioral therapy (CBT) in combination with systemic family therapy (SFT) on mild to moderate postpartum depression and sleep quality. Methods: 249 primiparous women with mild to moderate postpartum depression were recruited and randomly assigned to a control group (n=128), which received conventional postpartum care, or to a psychological intervention group (n=121), which received conventional postpartum care combined with psychological intervention. The Edinburgh Postnatal Depression Scale (EPDS) and Pittsburgh Sleep Quality Index (PSQI) were employed to evaluate depression and sleep quality, respectively. Results: 104 patients in the intervention group and 109 in the control group completed the study. After intervention, the EPDS score, PSQI score, sleep quality score, sleep latency score, sleep duration score, habitual sleep efficiency score, sleep disturbance score, and daytime dysfunction score were significantly lower in the intervention group than in the control group. The EPDS and PSQI scores of each group at different time points after intervention were markedly decreased compared with those before intervention, and the reduction in the intervention group was more evident than that in the control group. Conclusion: CBT in combination with SFT can improve depression and sleep quality in patients with mild to moderate postpartum depression.

Gonadotropin therapy plays an integral role in ovarian stimulation for infertility treatments. Efforts have been made over the last century to improve gonadotropin preparations. Undoubtedly, current gonadotropins have better quality and safety profiles as well as clinical efficacy than earlier ones. A major achievement has been introducing recombinant technology in the manufacturing processes for follicle-stimulating hormone, luteinizing hormone, and human chorionic gonadotropin. Recombinant gonadotropins are purer than urine-derived gonadotropins, and incorporating vial filling by mass virtually eliminated batch-to-batch variations and enabled accurate dosing. Recombinant and fill-by-mass technologies have been the driving forces for launching of prefilled pen devices for more patient-friendly ovarian stimulation. The most recent developments include the fixed combination of follitropin alfa + lutropin alfa, long-acting FSH gonadotropin, and a new family of prefilled pen injector devices for administration of recombinant gonadotropins. The next step would be the production of orally bioactive molecules with selective follicle-stimulating hormone and luteinizing hormone activity.

This article describes a systematic program of research that focuses on Brief Strategic Family Therapy (BSFT) and the adaptations that were developed based on BSFT principles. The culture-specific origins of BSFT are reviewed, as well as its broader applications to the field of family therapy. Research is reviewed demonstrating that BSFT is a promising family-based approach to treating Hispanic youth behavior problems and drug abuse. Treatment innovations are described that address the combination of intergenerational and cultural differences that occur among youths and their Hispanic parents. Programmatic work is described that challenges basic principles of family therapy by expanding BSFT to a One Person modality and a strategic engagement procedure. Both of these novel approaches are intended to add tools to therapists’ repertoire in working with difficult-to-engage families. A preview discussion of results is presented from a randomized clinical trial that is an application of an ecosystemic prevention version of BSFT. The implications of the work of the Center for Family Studies are discussed in the context of the broader service system. OBJECTIVE: To describe evidence-based management of patients presenting to family physicians with typical signs and symptoms of recent onset of rheumatoid arthritis (RA). STUDY SELECTION: Articles for critical review were included if relevant to primary care management of early RA (less than 1 year duration). Sources included MEDLINE from 1966 to December 1995, the reference library of the Arthritis Community Research and Evaluation Unit, and conference abstracts. FINDINGS: Evidence from randomized, controlled trials supports the short-term benefit of nonsteroidal anti-inflammatory drugs, disease-modifying agents for rheumatic diseases, intravenous pulse corticosteroid therapy, intra-articular therapy, aerobic exercise, patient education, psychologic intervention, home physiotherapy, home occupational therapy, and rehabilitation programs. Some evidence favours acetaminophen for analgesia, low-dose oral corticosteroids for symptom control, and referral to a rheumatologist. Children from substance-affected families show an elevated risk for developing own substance-related or other mental disorders. Therefore, they are an important target group for preventive efforts. So far, such programs for children of substance-involved parents have not been reviewed together. We conducted a comprehensive systematic review to identify and summarize evaluations of selective preventive interventions in childhood and adolescence targeted at this specific group. From the overall search result of 375 articles, 339 were excluded, 36 full texts were reviewed. From these, nine eligible programs documented in 13 studies were identified comprising four school-based interventions (study 1–6), one community-based intervention (study 7–8), and four family-based interventions (study 9–13). Studies’ levels of evidence were rated in accordance with the Scottish Intercollegiate Guidelines Network (SIGN) methodology, and their quality was ranked according to a score adapted from the area of meta-analytic family therapy research and consisting of 15 study design quality criteria. We conducted a systematic review of studies employing telehealth interventions which focused on family caregivers’ outcomes. The Embase, CINHAL, Cochrane and PubMed databases were searched using combinations of keywords including “telehealth,” “telemedicine,” “telecare,” “telemonitoring,” “caregiver” and “family.” The initial search produced 4205 articles, of which 65 articles met the inclusion criteria. The articles included 52 experimental studies, 11 evaluation studies, one case study and one secondary analysis. Thirty-three articles focused on family caregivers of adult and older patients, while 32 articles focused on parental caregivers of paediatric patients. The technologies included video, web-based, telephone-based and telemetry/remote monitoring. Six main categories of interventions were delivered via technology: education, consultation (including decision support), psychosocial/cognitive behavioural therapy (including problem solving training), social support, data collection and monitoring, and clinical care delivery. Descriptive research aimed at evaluating the assistance offered to patients with venous ulcers, on lower limbs, attended by the Family Health Program (FHP) team, from the municipality of Natal/RN. The target population was composed of 74 patients with venous ulcers (VU), attended by the FHP teams in the 31 FHUs. The study was approved by the Ethics Committee of the Federal University of Rio Grande do Norte (protocol n.55/05). The data collection was performed in patients homes and in the FHUs, through structured interviews and physical examinations of patients with VU and non-participant observation during the changing of wound dressings in these Units and in users homes. The data was organized into an Excel electronic table and transported into the SPSS 14.0 program, for descriptive analysis on 2x2 contingency tables and inferential (Qui-Square χ2, Spearman Correlation, Binomial Proportion Test and p-value <0.05). The prevalence of VU (0.36/1000) in the target population (over 20 years of age) was greater than in the population registered in FHP (0.25/1000). We detected a greater prevalence in the age area of over 60 years (2.22/1000), with 2.98/1000 for females and 1.3/1000 for males (p-value=0.008). BACKGROUND: Antihypertensive drug therapy is considered lifelong but in the family practice environment drug cessation may provide an opportunity to attempt nonpharmacologic strategies for blood pressure (BP) control with a clear outcome, maintaining drug-free status. The identification of simple predictors would assist the family physician to select who may or may not have their medication ceased. METHODS: To monitor a drug cessation program in currently treated hypertensive patients in Australian family practice, 25,826 patients aged 65 to 84 years currently receiving antihypertensive medication, were offered drug withdrawal as part of the run-in phase of a large clinical trial. Outcomes investigated were the proportion of patients completing drug withdrawal and maintaining short-term BP control and factors that predicted these patients. RESULTS: A total of 18,993 patients did not enter the withdrawal program; 6291 (92% of those who entered) completed drug withdrawal. In comparison to patients who did not complete drug withdrawal, they were younger and more likely male. A total of 1,228 (18% of those who entered) ceased medication and maintained adequate BP control for 0 to 76 weeks (median, 4 weeks). RESUMEN: Objetivo: El objetivo de esta revisión sistemática fue estudiar que tratamientos y técnicas son las que tienen más evidencia científica en la actualidad para modular el tono en niños con Parálisis Cerebral infantil (PCI). Del mismo modo, como objetivo secundario, se quiso abordar y hacer una propuesta de futuras investigaciones hacia la terapia familiar y adaptación del entorno Fuentes de datos y selección de los estudios: Los estudios fueron buscados en la literatura en cuatro bases de datos, publicadas entre el 2009 y 2014. Se tuvieron en cuenta los siguientes criterios de inclusión: los estudios debían ser ensayos clínicos aleatorizados o estudios controlados no aleatorizados que abordasen un tipo de intervención médica en combinación con fisioterapia o bien solo fisioterapia. Los pacientes debían tener una edad comprendida entre 0 y 18 años y cursar con PCI y espasticidad. Una vez seleccionados todos los estudios se procedió a una lectura crítica utilizando la CASPe. Tras este cribado finalmente 15 artículos fueron seleccionados para esta revisión. Resultados: No hay evidencia significativa que respalde que el uso de una terapia tenga más efectos en disminuir la espasticidad que otra, pero se encontraron más estudios que avalan el uso de la toxina botulínica tipo A (TXB-A) con un posterior tratamiento de fisioterapia o terapia ocupacional. The authors used cellular cultures obtained from endocervix samples in order to determine the predominant factors of infection caused by Chlamydia trachomatis in a population of 391 women who attended a family planning consultation. The authors studied the relation between the infection and the following variables: age, geographical origin, social class, marital status, history of sexually transmitted diseases (STD), risk behaviours, contraceptive methods and attitudes towards both the disease and therapy. Chlamydia trachomatis was isolated in 7 women (1,8%). The highest infection rates were found in the following categories: women between the ages of 35-40 (4,9%), women who resort to hormonal contraception (3,4%), women between the ages of 20-25 (3,2%) and single women and/or women who do not have a regular partner (2,6%). No statistically relevant relation between the above variables and the infection was found, although a relatively high risk was revealed for women who resort to hormonal contraception (O.R = 7,4). The authors have concluded the following: 1. There is no need to proceed with the systematic universal screening of infection caused by Chlamydia trachomatis in the yearly family planning consultations for the low risk STD populations; 2. Hormonal contraception is a factor to be considered when selecting the women to be screened; 3. Background: Children with chronic kidney disease and their families have to deal, on a daily basis, with various medical procedures and a life of uncertainty. Renal replacement therapy with dialysis and kidney transplantation made survival possible, but caring for these children is complex, with a great impact on family's well-being and life quality. Aim: Identifying the difficulties felt by the parents/care providers, while taking care of the child on peritoneal dialysis, and the strategies adopted by nurses to minimize these difficulties. Method/design: Systematic review of literature without meta-analysis study carried through an on-line literature research from different search engines and databases. 7 articles were selected to build up the research sample: 5 from MEDLINE with Full Text and 2 from CINHAL Plus with Full Text. Results: The family dynamics is negatively affected by the changes imposed from the disease. Purpose of review: Over a million children and their families have now experienced the stress of the deployment of a family member during the recent wars in Iraq and Afghanistan. Whereas there is an extensive clinical literature about the developmental challenges facing children and issues of family adjustment, there is a lack of systematic research. This review summarizes the findings of recent publications. Recent findings: Some veterans develop posttraumatic stress disorder as a consequence of their experiences. This condition drives many of the adverse changes in the families of returning veterans through the effects on intimacy and nurturance in their families of withdrawal, numbing and irritability that are components of posttraumatic stress disorder. There is the more general challenge that all families and children face when a partner/parent deploys of role ambiguity consequent on anxiety that is provoked by the threat that deployed family members experience. A study of Kuwaiti military showed that mothers' anxiety had the greatest impact on the children of deployed fathers, although absence of posttraumatic stress disorder in mothers could mitigate the effects of their fathers' posttraumatic stress disorder. CONTEXTO: O transtorno de estresse pós-traumático (TEPT), uma das possíveis consequências de abuso sexual de crianças e adolescentes, pode ser encontrado em aproximadamente 40% a 50% dos casos. OBJETIVO: Conduzir uma revisão sistemática de estudos investigando o uso de terapia cognitivo-comportamental (TCC) para o tratamento de crianças e adolescentes abusadas sexualmente com TEPT. MÉTODOS: Uma pesquisa por ensaios clínicos randomizados que avaliaram TEPT em crianças e adolescentes de 1980 a 1º de fevereiro de 2006 foi conduzida nas seguintes bases de dados: MedLine, EMBASE, LILACS, PsycLIT, PsycINFO, Cochrane Depression, Anxiety and Neurosis Group Database of Trials, Cochrane Controlled Trials Register, Science Citation Index (SciSearch) e PILOTS. As referências em todos os ensaios clínicos foram localizadas manualmente. RESULTADOS: Dos 43 estudos inicialmente selecionados, apenas três preencheram critérios de inclusão. As seguintes comparações foram encontradas nos estudos: TCC para tratar a criança e membros da família versus não tratamento (lista de espera); TCC para tratar apenas a criança, apenas os pais ou ambos versus Community Care; e TCC focada no trauma versus terapia centrada na criança (CCT). This article presents a systematic literature review based on scientific data achieved through critical analysis of articles from 2006 to 2010, and has as objective to describe the support needs of families with HIV / AIDS feel face of living with the disease and to identify nurses' performance against that family / patient to a greater family integration and better acceptance of drug therapy.
