In 2008 the International Society of Physical and Rehabilitation Medicine (ISPRM) started an initiative to systematically develop its capacity and its internal and external policy agenda. This paper sums up achievements that have been made with this ISPRM initiative as well as pending issues and strategies to address them. The paper treats the following: ISPRM`s policy agenda in collaboration with the World Health Organization (WHO), research capacity in functioning and rehabilitation, ISPRM world conferences, relationships with regional societies of Physical and Rehabilitation Medicine (PRM), and ISPRM`s membership and governance structure.

Compared with other specialties, the field of physical and rehabilitation medicine has not received the deserved recognition from clinicians and researchers in the scientific community. One of the reasons is the lack of sound evidence to support the traditional physical and rehabilitation medicine treatments. The best way to change this disadvantage is through a well conducted clinical research, such as standard placebo- or sham-controlled randomized clinical trials. Therefore, having placebo groups in clinical trials is essential to improve the level of evidence-based practice in physical and rehabilitation medicine that ultimately translates to better clinical care. To address the challenges for the use of placebo in physical and rehabilitation medicine and randomized clinical trials and to create useful recommendations, we convened a working group during the inaugural International Symposium in Placebo (February 2009, in Sao Paulo, Brazil) in which the following topics were discussed: (1) current status of randomized clinical trials in physical and rehabilitation medicine, (2) challenges for the use of placebo in physical and rehabilitation medicine, (3) bioethics, (4) use of placebo in acupuncture trials and for the treatment of low-back pain...

Background: To optimize patient functioning, rehabilitation professionals often rely on measurements of functioning as well as on classifications. Although the International Classification of Diseases (ICD) and the International Classification of Functioning, Disability and Health (ICF) are used, their joint use has yet to become an established practice. To encourage their joint use in daily practice, the World Health Organization (WHO) has invited all rehabilitation practitioners worldwide to support the ICD-11 revision process by identifying the ICF categories that correspond to specific rehabilitation-relevant health conditions. The first step in completing this task, generating the list of these health conditions, was taken at a February 2012 workshop in Sao Paulo, Brazil. Objectives: The objectives of this paper are to present the results of the Sao Paulo workshop, and to invite practitioners to participate in the ICD-ICF joint use initiative. Discussion: Alternating plenary and small working group sessions were held and 103 rehabilitation-relevant health conditions were identified. With this list available, WHO together with the International Society of Physical and Rehabilitation Medicine (ISPRM), is reaching out to clinicians of all rehabilitation disciplines to take on the challenge of identifying the ICF categories for at least one of the health conditions listed.

A reabilitação de edifícios tem sido tema recorrente junto aos setores interessados no desenvolvimento urbano das grandes metrópoles, tanto no Brasil como em países da Europa e América do Norte. Particularmente, no Brasil, o setor da construção civil vem aumentando seu interesse pela área de reabilitação, seja pelo aumento do volume de recursos aplicados pelos governos federal, estadual e municipal, como pela potencialidade econômica que se vislumbra em função do expressivo parque edificado vazio nas áreas centrais urbanas do país. Apesar deste contexto, as particularidades relativas à produção de obras de reabilitação de edifícios não têm sido objeto de estudo sistêmico. Há expressivo desconhecimento sobre a dinâmica de produção deste tipo de empreendimento, o que tem resultado, sobretudo, em obras cujos custos usualmente extrapolam as estimativas iniciais. Diante do expressivo número de Hospitais Psiquiátricos no Estado de São Paulo, que continuam presentes no atendimento das pessoas com transtornos mentais mesmo frente à política de desinstitucionalização e a priorização do atendimento nos serviços substitutivos, surge a questão se os profissionais que trabalham neles conhecem e discutem o conceito de reabilitação psicossocial e como trabalham no contexto da Reforma Psiquiátrica. Este estudo objetivou analisar as possibilidades e impossibilidades de se implantar estratégias de reabilitação psicossocial em hospitais psiquiátricos, foi escolhido como objeto as representações que os trabalhadores têm sobre a reabilitação psicossocial e como categorias analíticas, as representações sociais e o processo de trabalho em saúde. Dissertação de mestrado integrado em Engenharia Civil; A minha dissertação consistiu na reabilitação sustentável de estruturas de betão armado. Estudou-se um caso de estudo, um reservatório de abastecimento de água que se encontrava bastante degradado. As técnicas de reabilitação estudadas foram reabilitação com argamassa de reparação e laminados e carbono colados com uma cola possuindo resinas epóxi e reabilitação com betão projectado e laminados de carbono colados também com uma cola possuindo resinas epóxi. Dentro destas técnicas havia duas hipóteses, ou se reabilitava todo o reservatório ou restituía-se a cobertura e reabilitava-se a estrutura. Para saber-se qual a técnica de reabilitação mais sustentável foi necessário estimar os custos de reabilitação através de um software que foi o CYPE. Feita a orçamentação concluiu-se que a técnica economicamente mais sustentável era a reabilitação com argamassa de reparação e laminados de carbono que custa cerca de 19.653,32 euros para reabilitar toda a estrutura, enquanto que a reabilitação com betão projectado e laminados de carbono custa cerca de 47.122,09 euros. Rehabilitation is seen as a key strategy for minimising biodiversity losses. Although most rehabilitation strategies aim to provide habitat for fauna, they usually focus entirely on establishing vegetation. Successful vegetation establishment, however, does not necessarily provide habitat to the same species that are threatened by habitat loss. Improved understanding of faunal response to rehabilitation is required if rehabilitation techniques are to be refined and deliver the hoped for biodiversity outcomes. This study aimed to assess to what extent post-mining rehabilitation on the Weipa bauxite plateau has restored the bird habitat values of the pre-mining native forest. Bird assemblages, vegetation, and landscape functionality were compared between: (1) Eucalyptus tetrodonta open forest reference sites representative of the pre-mining native forest; (2) two reference land units of Eucalyptus tetrodonta tall woodland that have previously been nominated as ecologically appropriate analogues for the post-mining landscapes; and (3) a chronosequence of post-mining rehabilitation sites up to 23 years old. Objective: to assess the effect of home versus day rehabilitation on patient outcomes. Design: randomised controlled trial. Setting: post-hospital rehabilitation. Participants: two hundred and twenty-nine hospitalised patients referred for ambulatory rehabilitation. Interventions: hospital-based day rehabilitation programme versus home-based rehabilitation programme. Main Outcome Measures: at 3 months, information was collected on hospital readmission, transfer to residential care, functional level, quality of life, carer stress and carer quality of life. At 6 months, place of residence, hospital re-admissions and mortality status were collected. Results: there were significant improvements in the functional outcomes from baseline to 3 months for all participants. At discharge, carers of patients in day hospital reported higher Caregiver Strain Index (CSI) scores in comparison to home rehabilitation carers (4.95 versus 3.56, P = 0.047). Patients in day hospital had double the risk of readmission compared to those in home rehabilitation (RR = 2.1; 95% CI 1.2–3.9). This effect persisted at 6 months. Conclusions: day hospital patients are more likely to be readmitted to hospital possibly due to increased access to admitting medical staff. Study Design. Cost-effectiveness analysis alongside a factorial randomized controlled trial. Objective. To assess the cost-effectiveness of a rehabilitation program and/or an education booklet each compared with usual care for the postoperative management of patients undergoing discectomy or lateral nerve root decompression surgery. Summary of Background Data. There is little knowledge about the cost-effectiveness of postoperative management of patients after spinal surgery. Methods. A total of 338 patients were recruited into the study between June 2005 and March 2009. Patients were randomized to rehabilitation only, booklet only, rehabilitation plus booklet, or usual care only. Interactions between booklet and rehabilitation were nonsignificant; hence, we compare booklet versus no booklet and rehabilitation versus no rehabilitation. We adopt an English National Health Service and personal social services perspective. Data on outcomes and costs are based on patient level data from the trial. A 1-year time horizon was used. Outcomes were measured in terms of quality-adjusted life years. Health-related quality of life was reported by patients using the EuroQol-5D (EQ-5D). A comprehensive range of health service contacts were included in the cost analysis. Results. There were no significant differences in costs or outcomes associated with either intervention. Introduction: A one-level hypothetical model was formulated to explore factors that influenced the self-reported readiness of workplace managers to engage in workplace (vocational) rehabilitation of past-injured workers attending their workplaces. Methods: Seven supervisor latent variables were considered, using 270 supervisors self-rated self-efficacy measures, estimating their ability to pursue four related roles associated with vocational rehabilitation in the workplace. Models identifying supervisor self-efficacy pathways leading to their readiness to engage in vocational rehabilitation in their workplaces were tested using Partial Least Square Analysis (PLSPATH). Results: The study’s outcomes suggest that supervisor readiness to engage in workplace vocational rehabilitation with rehabilitating employees can be directly associated with four variables. Together, these variables accounted for 41 per cent of the variance of the supervisors’ self-efficacy scores, defining their readiness to engage in workplace vocational rehabilitation. Significant predictors which had a direct influence on supervisor readiness to engage in vocational rehabilitation were: their perceived financial and liaison roles associated with vocational rehabilitation and their concern with meeting legal aspects of this process. Since antiquity, the trauma of a severe burn has beset humankind with poor outcomes and survival rates. However, over the last two decades there has been substantial progress in the management of acute burns that has resulted in life expectancy similar to the general population. Despite these advancements in the management of severe burns, the physical, psychosocial and economic implications following a severe burn injury are variable with a lack of substantial progress in the area of burn rehabilitation. Consequently, the rehabilitation of those with severe burns remains a lengthy process with significant associated physical and psychosocial problems. A crucial point raised by burn professionals internationally, is that burn rehabilitation needs to be recognised as a continuum of active care rather than a separate isolated phase, with rehabilitation commencing on the day of injury. The immediate commencement of therapy is a key factor in the management of burns with the initial focus directed towards life support, wound closure, infection control and aggressive metabolic support. Objective. Examine demographics, clinical characteristics and rehabilitation outcomes of lower-limb amputees, using the Australasian Rehabilitation Outcomes Centre (AROC) database. Methods. Lower-limb amputee rehabilitation separations between 2004 and 2010 were identified using AROC impairment codes 5.3–5.7.1 Analysis was conducted by year, impairment code, Australian National Sub-acute and Non-Acute Patient (AN-SNAP) classification (S2–224, Functional Independence Measure (FIM) motor(Mot) score 72–91; S2–225, FIM (Mot) score 14–71) and states of Australia. Results. Mean length of stay (LOS) for all lower-limb amputee episodes was 36.1 days (95% confidence interval (CI): 35.4–36.9). Majority of episodes were unilateral below knee (63.6%), males (71.8%) with a mean age of 67.9 years (95% CI: 67.6–68.3). Year-on-year analysis revealed a trend for increasing LOS and decreasing age. Analysis by impairment code demonstrated no significant difference in rehabilitation outcomes. Analysis by AN-SNAP found that LOS was 16.2 days longer for S2–225 than for S2–224 (95% CI: 14.7–17.8, P < 0.001), and FIM (Mot) change was 12.0 points higher for S2–225 than for S2–224 (95% CI: 11.5–12.6, P < 0.001). Neben anderen stehen Aufmerksamkeits- und Gedächtnisfaktoren im Zusammenhang mit funktioneller Unabhängigkeit und der Entlassung ins prämorbide Umfeld. Obwohl von der Effektivität einer kognitiven Rehabilitation ausgegangen werden kann, sind bisher keine speziell auf die Anforderungen einer geriatrischen Rehabilitationseinrichtung ausgerichteten, prospektiven Kontrollgruppenerhebungen bekannt. Zudem besteht ein Mangel an Studien, die eine Wirkung neuropsychologischer Interventionsstrategien auf das funktionelle Outcome von Patienten erhebt. The restoration of motor function to chronic stroke patients with a severe hand impairment is challenging, because no effective rehabilitation option exists. In recent years, brain-computer interfaces (BCIs) have shown potential to remedy this problem by decoding the patient’s movement intention from the brain signal and translating it into movements of an orthosis. In addition, brain stimulation has been investigated for the treatment of a variety of neurological conditions, including stroke. Combining both approaches into a closed-loop stimulation system has been proposed to increase the effectiveness of this treatment approach. However, this has not been put into practice before, therefore it is unknown, if closed-loop paradigms can be realized for stroke patients. In this thesis, closed-loop stimulation for stroke rehabilitation is studied for the first time. Two approaches for a closed-loop system are investigated: (i) Stimulation coupled to the movement intention of the patient and (ii) adaptation of stimulation parameters to control the shape of the evoked activity. Abstract The focus of this thesis is stress in rehabilitation professionals. Within the thesis, burnout encompasses compassion fatigue and moral stress. Therefore, burnout is the emotional and ethical fatigue which is produced through organizational and clinical expectations present when working with individuals who live with disabilities. This thesis argues that current rehabilitation service delivery models exacerbate burnout through their neglect of emotional and ethical needs in professionals. The goal of this thesis is to develop an alternative model of service delivery which addresses burnout in rehabilitation professionals. The thesis answers the following question. How does Jean Vanier's thinking about relationships between individuals, living with and without disabilities, contribute to the field of rehabilitation therapy and, more specifically, to reducing stress currently experienced by rehabilitation professionals? To answer this question and meet the thesis goal, the research is situated within a constructivist paradigm and uses a single, interpretive case study design. BACKGROUND: Many people who have suffered a stroke require rehabilitation to help them resume their previous activities and roles in their own environment, but only some of them receive inpatient or even outpatient rehabilitation services. Partial and unmet rehabilitation needs may ultimately lead to a loss of functional autonomy, which increases utilization of health services, number of hospitalizations and early institutionalization, leading to a significant psychological and financial burden on the patients, their families and the health care system. The aim of this study was to explore partially met and unmet rehabilitation needs of older adults who had suffered a stroke and who live in the community. The emphasis was put on needs that act as obstacles to social participation in terms of personal factors, environmental factors and life habits, from the point of view of four target populations. METHODS: Using the focus group technique, we met four types of experts living in three geographic areas of the province of Québec (Canada): older people with stroke, caregivers, health professionals and health care managers, for a total of 12 groups and 72 participants. OBJETIVO: o objetivo deste estudo clínico foi avaliar a qualidade de vida (QV) e a recuperação físico-funcional das mulheres com câncer de mama inicial submetidas à biópsia do linfonodo sentinela (BLS) ou à linfadenectomia axilar (LA) e investigar os efeitos de um programa de reabilitação. SUJEITOS E MÉTODOS: um estudo clínico longitudinal foi conduzido usando três grupos de pacientes. Os participantes de estudo foram mulheres tratadas por câncer de mama primário estádios I e II no Centro de Atenção Integral à Saúde da Mulher da Universidade Estadual de Campinas (UNICAMP). Recrutamos mulheres que realizaram quadrantectomia, sem evidências de metástases a distância e nenhuma malignidade prévia. Essas mulheres assinaram um termo de consentimento informado, anteriormente aprovado pelo Comitê de Ética e Pesquisa, estabelecido na Declaração de Helsinque. As mulheres recrutadas iniciaram a pesquisa em maio de 2006 e terminaram em dezembro de 2007. Um dia antes da cirurgia, as 89 mulheres incluídas no estudo foram examinadas e responderam a uma entrevista sobre características demográficas, clínicas e sociais. European Master in Social Work with Families and Children; This study explores expressions of hope and aspiration among young people receiving rehabilitation services in Kampala, Uganda. The study objectives were (i) to examine expressions of hope and aspiration among young people receiving rehabilitation services (ii) to document the social determinants of hope and aspiration among young people receiving rehabilitation services and (iii) to assess barriers to the realization of hope and aspiration among young people receiving rehabilitation services. The study was carried out in Masooli Rehabilitation Centre of Ugandan Youth Development Link (UYDEL). UYDEL is a non- profit making organization (NGO) that was founded by a group of social welfare professionals in 1993. UYDEL currently runs four main programs concerning (i) child rights protection (child sexual abuse, child trafficking, commercial sexual exploitation, and child labour), (ii) HIV prevention among high risk groups of children and youth, (iii) alcohol and substance abuse, and (iv) adolescent sexual and reproductive health. In addition to the four major programs, UYDEL provides rehabilitation and livelihood skills training to young people and also conducts social research.
