OBJETIVO: Analisar o consumo de medicamentos e os principais grupos terapêuticos consumidos por pessoas com deficiências físicas, auditivas ou visuais. MÉTODOS: Estudo transversal em que foram analisados dados do Inquérito Multicêntrico de Saúde no Estado de São Paulo (ISA-SP) em 2002 e do Inquérito de Saúde no Município de São Paulo (ISA-Capital), realizado em 2003. Os entrevistados que referiram deficiências foram estudados segundo as variáveis que compõem o banco de dados: área, sexo, renda, faixa etária, raça, consumo de medicamentos e tipos de medicamentos consumidos. RESULTADOS: A percentagem de consumo entre as pessoas com deficiência foi de: 62,8 por cento entre os visuais; 60,2 por cento entre os auditivos e 70,1 por cento entre os físicos. As pessoas com deficiência física consumiram 20 por cento mais medicamentos que os não-deficientes. Entre as pessoas com deficiência visual, os medicamentos mais consumidos foram os diuréticos, agentes do sistema renina-angiotensina e analgésicos. Pessoas com deficiência auditiva utilizaram mais analgésicos e agentes do sistema renina-angiotensina. Tomaram-se as incapacidades físicas da hanseníase (IFH) como objeto central de estudo, no sentido de deslindar se, na atualidade, constituem coisa do passado ou problema do futuro. Partindo-se do amplo patamar de textos acadêmicos e de documentos de organismos nacionais e internacionais voltados à epidemiologia e controle da hanseníase, procurou-se estudar, no seu interior, destacadamente, as incapacidades físicas. A reconstrução metodológica adotada foi de natureza qualitativa, fulcrada nas técnicas de revisão bibliográfica e análise de conteúdo. Esta aqui foi empregada na tipificação documental categorial frequencial contigencial, de acordo com devida fundamentação pertinente. Uma das principais tendências das IFH que repetidamente se constatou foi o reconhecimento oficial da importância que vêm merecendo, em flagrante contraponto com a prioridade que lhes é conferida, em termos de decisões, investimentos e práticas, na medida em que estes elementos têm centralizado seus saberes e agires na ação bacilar direta dentro e fora do organismo humano, abrindo, em conseqüência, espaço expressivo para atuação de organizações não governamentais e associações congêneres quantos aos aspectos reabilitacionais da moléstia. OBJECTIVE To analyze evidence of the validity and reliability of a Brazilian Portuguese version of the Quality of Care Scale from the perspective of people with physical and intellectual disabilities. METHODS There were 162 people with physical disabilities and 156 with intellectual disabilities from Porto Alegre and metropolitan region, who participated in the study in 2008. Classical psychometrics was used to independently analyze the two samples. Hypotheses for evidence of criterion validity (concurrent type) were tested with the Mann-Whitney test for non-normal distributions. Principal components analysis was used to explore factorial models. Evidence of reliability was calculated with Cronbach alpha for the scales and subscales. Test-retest reliability was analyzed for individuals with intellectual disabilities through intra-class correlation coefficient and the Willcoxon test. RESULTS The principal components in the group with physical disabilities replicated the original model presented as a solution to the international project data. Evidence of discriminant validity and test-retest reliability was found. In many countries, governments pursue a policy of offering persons with disabilities greater opportunities for participation in society, based on the assumption that this will also improve their subjective well-being. Currently, however, it is not known whether this assumption is valid. In this study we relate the objective participation and the subjective well-being aspects of both disabled and non-disabled persons to an array of social and health-related determinants. Linear structural equation modelling of data of a sample selection of the Dutch population is analysed. The sample size is 5,826, including 642 respondents with physical disabilities. In terms of objective participation, the persons with disabilities are at a greater disadvantage as regards labour participation than is the case for social and cultural participation. When it comes to subjective well-being, we find that the persons with disabilities are more likely to lag behind in perceived physical health than in mental health and happiness. In a multivariate model relating objective participation to subjective health and happiness, correlations are much weaker than expected. It is striking to find that participation, perceived health and happiness are much less closely related than is often assumed. Their determinants differ widely in nature and strength. The empirical model leads to rejection of the hypothesis that higher participation by the persons with disabilities is associated with higher subjective well-being.

The purpose of this study was to determine sexual functioning of Polish athletes with disabilities (including paralympians). The study encompassed 218 people with physical disabilities, aged between 18 and 45 (149 men and 69 women). The entire research population was divided into three groups: Polish paralympians (n = 45), athletes with disabilities (n = 126) and non-athletes with disabilities (n = 47). The quality of sexual life of Polish paralympians was measured by using the Polish version of Female Sexual Function Index and International Index of Erectile Function. Clinically significant erectile dysfunctions were most often diagnosed in non-athletes (83.33%) with 50% result of severe erectile dysfunctions, followed by athletes and paralympians with comparable results of 56.98% and 54.17% respectively (p = 0.00388). Statistically significant clinical sexual dysfunctions concerned lubrication, orgasm as well as pain domains, and prevailed among female non-athletes (68.42%, 68.42% and 57.89%). Practising sports at the highest level has a favourable effect on the sexuality of men and women with physical disabilities. Men with physical disabilities manifest more sexual disorders than women, an aspect which should be considered by health-care professionals working with people with disabilities.

Occupational Therapy as a dynamic, client-centred process is well positioned to address stigma. Whist the process of stigmatisation in intellectual disability has been thoroughly examined in literature, there is a lack of research examining stigma associated with acquired physical disabilities, and the role it plays in the trajectory and function of friendship. The UN Convention on the Rights of Persons with Disabilities aims to minimise the attitudinal barriers to engagement and promote inclusion. However, this is yet to be ratified in Ireland. Objectives The purpose of this study is to explore the experiences of stigma in adults with acquired physical disabilities specifically investigating the impact of stigma on the role of friendship. Advances in medical technologies and changing philosophies of health care have led to a rapid increase in home-based care for children with disabilities. While there are cost savings for health services if children are cared for at home there are extensive additional demands on the time and resources of parents, particularly primary caregivers, who are usually mothers. Previous studies have shown that parents caring for children with disabilities experience considerable stress and increased rates of mental health problems. The present dissertation investigated the impact of caring on the daily lives of parents and in particular, a model proposing factors contributing to parental psychological, social and health outcomes. A preliminary qualitative study found time demands to be a core theme when discussing the consequences of caring, and when describing tasks of caring. A second, larger scale quantitative study focused on assessing the time constraints facing parents of children with developmental disabilities. non-peer-reviewed; Background: Older caregivers of patients with mental heath difficulties report poor physical health. This study examined whether younger parental caregivers of children with developmental disabilities would also report poor physical health. Methods: A cross-sectional between subjects design was used. Thirty-eight parents of children with developmental disabilities and 34 parents of typically developing children completed standard measures of perceived stress, social support, mastery and child problem behaviours. Results: ANOVA revealed that caregivers reported more physical health problems than controls (Mean = 52.5 + 12.24 vs 43.1 + 13.35; F (1, 70) = 9.62, p < .05). ANCOVA indicated that this appeared to be partly mediated by mastery and child problem behaviours. Conclusion: The negative impact of caregiving on self-report physical health is also evident in younger parents of children with developmental disabilities. The coping style of parents and the behavioural characteristics of the care recipients may be a key consideration in whether physical health is compromised in this context.

Individuals with non-apparent, physical disabilities face unique social challenges from those that are encountered by the more visibly disabled. The absence of visible cues indicating physical impairment causes ambiguity in social situations, leaving the sufferer vulnerable to moral judgments and social sanctions when they are unable to embody and perform to cultural norms. This dynamic generates a closeted status that the individual must learn to navigate. Using Eve Sedgwick's "The Epistemology of the Closet," this paper deploys auto-ethnography, traditional ethnographic techniques, and literature reviews to illuminate a third space of functioning between the outwardly 'healthy' and the visibly disabled.

This report argues that the current paradigm of criminal justice is not conducive to the rehabilitation and treatment of people with non-physical disabilities, a general term referring to people with disabilities associated with mental and intellectual impairments. In order to prevent people with non-physical disabilities from entering the criminal justice system, or divert those who enter it, the criminal justice system and human services providers (namely, the health and social services) must develop an integrated approach for the early identification and appropriate treatment for them. Not only do the level of integration between human service providers and the criminal justice system need to improve, resources should be directed to build the overall capacity within these institutions and the wider community to effectively deal with them. Research has shown that persons with non-physical disabilities are over-represented in the criminal justice system. Despite the physical health and psychosocial benefits, few individuals with acquired physical disabilities participate in sport. This manuscript-based thesis is composed of four studies that aim to provide a more nuanced understanding of the theoretical and contextual factors that influence sport participation among this population. Study 1 investigated the predictive value of the Health Action Process Approach (HAPA). The HAPA model has been used among a variety of populations; however, it has not been used to predict sport participation among adults with acquired physical disabilities nor has athletic identity been considered as a supplement to the model. Results of the structural equation model demonstrated that the modified HAPA model explained 19% of the variance in sport participation. Study 2 explored how athletic identity was lost or (re)developed after acquiring a physical disability. Eleven adults with an acquired physical disability participated in semi-structured interviews. Participants’ stories of sport pre- and post-injury fit into three distinct narratives. Contexte : Les enfants atteints de maladies chroniques utilisent souvent des médecines complémentaires. Plusieurs études traitent de l’utilisation de ces traitements et des facteurs qui y sont associés chez les enfants atteints d’arthrite juvénile mais aucune étude n’est longitudinale. De plus, aucune n’a documenté l’utilisation de ces traitements chez les enfants ayant des incapacités physiques en attente de services publics de réadaptation. Objectifs : Les objectifs de cette étude étaient de déterminer la fréquence d’utilisation des médecines complémentaires chez les enfants atteints d’arthrite juvénile et d’incapacités physiques, d’évaluer leur efficacité telle que perçue par les parents et d’explorer les facteurs associés à leur utilisation. Méthodes : Une cohorte d’enfants atteints d’arthrite juvénile idiopathique (n=182, âge moyen : 10,2 ans) qui fréquentent des cliniques d’arthrite et une cohorte d’enfants ayant des incapacités physiques en attente de services de réadaptation publics (n=224, âge moyen : 2,6 ans) ont été suivis durant une période d’un an. L’utilisation des médecines complémentaires et la perception de leur efficacité d’après les parents ont été évaluées à l’aide de statistiques descriptives à chaque trois mois pour la cohorte d’enfants atteints d’arthrite et au début de l’étude pour la cohorte d’enfants ayant des incapacités physiques. Actualmente se reconoce la actividad física (AF) como estrategia para el mejoramiento de la calidad de vida, en virtud del problema de salud pública que representa el sedentarismo en el ámbito mundial y que se acentúa en el colectivo de personas en situación de discapacidad. Este proyecto se articula desde referentes teóricos relacionados con la AF, la discapacidad y los imaginarios sociales. Tiene como objetivo avanzar en la comprensión de los imaginarios que tienen las personas con discapacidad en torno a la AF, como un elemento fundamental en el diseño de políticas y programas para su fomento y efectiva articulación en la vida cotidiana de estos sujetos. OBJETIVO: Analisar o consumo de medicamentos e os principais grupos terapêuticos consumidos por pessoas com deficiências físicas, auditivas ou visuais. MÉTODOS: Estudo transversal em que foram analisados dados do Inquérito Multicêntrico de Saúde no Estado de São Paulo (ISA-SP) em 2002 e do Inquérito de Saúde no Município de São Paulo (ISA-Capital), realizado em 2003. Os entrevistados que referiram deficiências foram estudados segundo as variáveis que compõem o banco de dados: área, sexo, renda, faixa etária, raça, consumo de medicamentos e tipos de medicamentos consumidos. RESULTADOS: A percentagem de consumo entre as pessoas com deficiência foi de: 62,8% entre os visuais; 60,2% entre os auditivos e 70,1% entre os físicos. As pessoas com deficiência física consumiram 20% mais medicamentos que os não-deficientes. Entre as pessoas com deficiência visual, os medicamentos mais consumidos foram os diuréticos, agentes do sistema renina-angiotensina e analgésicos. Pessoas com deficiência auditiva utilizaram mais analgésicos e agentes do sistema renina-angiotensina. Entre indivíduos com deficiência física, analgésicos, antitrombóticos e agentes do sistema renina-angiotensina foram os medicamentos mais consumidos. Review question: We reviewed the evidence about the impact of community-based rehabilitation on the lives of people with disabilities and their carers in low- and middle-income countries. Background: People with disabilities include those who have long-term physical, mental, intellectual or sensory impairments, which in interaction with various barriers may hinder their full and effective participation in society on an equal basis with others. There are estimated to be over one billion people with disabilities globally and 80% of them live in low- and middle-income countries. They are often excluded from education, health, and employment and other aspects of society leading to an increased risk of poverty. Community-based rehabilitation interventions are the strategy endorsed by the World Health Organization and other international organisations (e.g. ILO, IDDC) for addressing the needs of this group of people in low- and middle-income countries. These interventions aim to enhance the quality of life of people with disabilities and their carers, by trying to meet their basic needs and ensuring inclusion and participation using predominantly local resources. OBJECTIVE To analyze evidence of the validity and reliability of a Brazilian Portuguese version of the Quality of Care Scale from the perspective of people with physical and intellectual disabilities. METHODS There were 162 people with physical disabilities and 156 with intellectual disabilities from Porto Alegre and metropolitan region, who participated in the study in 2008. Classical psychometrics was used to independently analyze the two samples. Hypotheses for evidence of criterion validity (concurrent type) were tested with the Mann-Whitney test for non-normal distributions. Principal components analysis was used to explore factorial models. Evidence of reliability was calculated with Cronbach alpha for the scales and subscales. Test-retest reliability was analyzed for individuals with intellectual disabilities through intra-class correlation coefficient and the Willcoxon test. RESULTS The principal components in the group with physical disabilities replicated the original model presented as a solution to the international project data. Evidence of discriminant validity and test-retest reliability was found.
