CCW, the assess of FC on two scores (1-5) and (1-15) and the assess of KKCF amount on two scores (1-3) and (1-9) were used as predictors of leg and rib tissue composition of 31 Blanca Celtibérica kids, with an average CCW of 6,9± 2,1 Kg. Proportionately 97% of the variation in muscle weight was accounted for by variation in CCW and the inclusion of KKCF amount scored from 1 to 9 improve the precision of the prediction 1%, reducing the RSD in 11,8%. The inclusion of KKCF amount (scored 1-3) did not improve the precision of the rib muscle composition but reduced the RSD in 6,2%. In relation to bone composition of both joints, 95% and 73% of the variation of the leg and rib bone weight, respectively were accounted for by variation on CCW. The 87 and 85% of the leg and rib fat weight variation were accounted for by variation in CCW associated with fat score (1-15) and KKCF amount (1-3) with a RSD of 17 and 32,4 g, respectively. To examine the effect of long lasting practice on pedal behavior in sport, we compared experienced adult soccer players and nonsoccer players on leg preference in motor tasks requiring general mobilization, soccer related mobilization, and body balance stabilization. We also evaluated performance asymmetry between the right and left legs in static and dynamic unipedal body balance based on center of pressure displacement, and correlated that with kg preference in balance stabilization tasks. Results revealed (a) a distinct leg preference between mobilization and stabilization tasks, which were significantly different between Mayers and nonplayers, (b) similar balance stability between the right and left legs, (c) greater stability of experienced players compared with nonplayers in static and dynamic balance, and (d) absence of a significant kg preference correlation with interlateral balance asymmetry. Objective: This is a clinical study of our experience using pedicle perforator flaps to cover skin defects in the middle and distal segment of the leg. Design: Prospective study. Setting: University hospital. Patients/Intervention: Twenty-four patients underwent treatment of a skin defect in the middle or distal segment of the leg by means of pedicled flaps based on perforating arteries. The perforating arteries were located before the operation by means of echo-Doppler examination. The flaps were planned in propeller fashion (21 cases) and as advancement (three cases). Main Outcome Measurements: The results were evaluated according the origin of perforator flap, size of the flap, and donor area and viability of the flap. The success rate of the echo-Doppler to identify the location of perforator vessel was also evaluated. Results: In nine cases, the perforating vessels originated from the fibular artery, in 10 the posterior tibial artery, and in five the anterior tibial artery. The mean size of the flaps was 5 cm in width by 12 cm in length. The success rate using an echo-Doppler was 87%. The flaps were fully viable in 20 cases and partially viable in four cases. Úlcera de perna é uma doença muito freqüente na população idosa. Inúmeros são os tipos de curativos atualmente usados para tratamento das úlceras de perna, com diferentes indicações, vantagens e desvantagens, cuja eficácia não está bem esclarecida devido à descontinuidade dos tratamentos e os custos envolvidos em algumas situações. Para avaliar a ação da biomembrana de látex (BML) no tratamento de úlceras de perna, que se comportou como eficiente indutora do tecido de cicatrização. Foram realizadas avaliações clínicas e imunohistopatológicas em 21 doentes, casualmente selecionados, submetidos à aplicação da biomembrana (14 pacientes), comparando com o tratamento clássico (07 pacientes) constituído de pomada com cloranfenicol e enzimas proteolíticas (Fibrase) com o objetivo de analisar e comparar as alterações histopatológicas e imunohistoquímicas relacionadas aos diferentes tratamentos. Foram coletadas biópsias da lesão antes e 30 dias após os tratamentos. O treinamento físico resistido tem sido enfaticamente incentivado, proporcionando efeitos favoráveis na força e endurance muscular, na função cardiovascular, metabolismo e na redução do risco cardiovascular. No entanto, é escassa a literatura sobre os ajustes cardiovasculares, ventilatórios e metabólicos no exercício resistido na população idosa. Além disso, pouco é sabido sobre tais ajustes na carga crítica (CC), a qual demarcaria a transição do exercício moderado para o exercício intenso nesta modalidade de exercício. A úlcera de perna representa demanda crescente nos serviços de saúde. Por tratar-se de uma doença crônica, recorrente e de difícil manejo, a úlcera de perna é um problema que vem desafiando profissionais, pacientes e famílias. A necessidade de entender melhor a problemática existente no cuidado oferecido pelo enfermeiro ao paciente com úlcera e sua família, motivou a pergunta do estudo, a qual buscou responder como a família influencia e é influenciada pela úlcera de perna. O estudo teve como objetivo conhecer as percepções e crenças do paciente e de sua família em relação à úlcera de perna, bem como compreender como essas crenças influenciam o enfrentamento da família na experiência. O estudo de caso foi o método adotado, e as técnicas da história oral foram utilizadas para a coleta de dados. O estudo foi realizado com um casal idoso, o qual convive com a úlcera de perna da mulher, focando o impacto desse evento na história da família. As narrativas do casal permitiram a construção da memória coletiva organizada em três temas distintos os quais se inter-relacionam: Envelhecimento e doença, Jornada do tratamento, A ferida e o tempo. Os resultados permitiram compreender que a família influencia e é influenciada pela úlcera de perna e esta dinâmica determina um grande impacto na vida do casal que se encontra na fase tardia do ciclo de vida da família. O autor realizou um estudo anatômico da região lateral e proximal da perna por meio de dissecção anatômica em cadáver, de exame arteriográfico e de mapeamento com Doppler em pacientes, visando a descrição da artéria denominada fibular superior, que se origina no tronco tíbio-fibular em 70% dos casos, da artéria fibular, em 20% e da artéria tibial anterior, em 10% e participa na irrigação do músculo sóleo e gastrocnêmio. Este vaso possui características adequadas para a realização de micro-anastomose. The rectus femoris was analysed in 10 volunteers during knee flexion and extension with the feet in normal, plantar flexion and dorsal flexion positions. Hewlett-Packard surface electrodes, an electromyographic signal amplifier, a computer equipped with an A/D conversion plaque (Model CAD 10/26), software specially designed to record and analyse the signals, Horizontal Leg Press, and electrogoniometers were used. The rectus femoris muscle showed strong potentials at the beginning of knee extension. In the simultaneous bendings of the knee and hip the activity was strong toward the end of the movement. The rectus femoris showed a similar activity both in the upper and lower platforms. As for foot positions, the rectus femoris showed the smallest potentials with the foot in plantar flexion and the largest ones with the foot in dorsal flexion.

A 35-year-old African Brazilian patient had sickle cell anemia complicated with recurrent vasoocclusive (VOC) crises and refractory painful leg ulcers for 16 years. The ulcers started over both medial malleoli and expanded gradually. The ulcer on the left leg spread from the foot to the knee circumferentially and was refractory to all forms of therapy within the frame work of multi-disciplinary care. The patient agreed to a below the knee amputation of the left leg. He felt much better after the amputation but developed severe neuropathic phantom pain that was well controlled medically. He could differentiate the sickle cell anemia and ulcer pain from the neuropathic pain. About 6 months after the amputation he had dengue fever with fatal outcome. This is the first report of treatment of refractory sickle cell anemia leg ulcer with amputation and probably the first report of a Brazilian patient with sickle cell anemia and dengue fever.

Forty-four New Zealand White x Californian rabbits were used to evaluate the real time ultrasonography (RTU) in assessing hind leg muscle weight and hind leg muscularity. For in vivo muscularity determination animals were scanned over the hind leg using a RTU machine equipped with a 5 MHz probe. The RTU images were taken on hind leg at three positions perpendicular to femur bone. At each position a section of hind leg muscle was analysed and the area of muscles was determined. The muscle volume of the hind leg was calculated by combining the muscle area measurements with the femur length. The weight of hind leg muscle was estimated from this volume of the hind leg and knowing the rabbit muscle density. Information related to hind leg muscle weight and femur length was used for hind leg muscularity. After scanning, animals were weighed, slaughtered and the reference carcass was cut. The right hind leg was dissected and the muscle was weighed and femur length was measured. The hind leg muscularity in carcass was calculated as previously described for RTU measurements. Correlations between the RTU measurements and their corresponding on carcass were determined. The LW varied from 1200 to 3410 g (average 2202 g) and reference carcass weight ranged between 561 and 1940 g (average 1199 g). O objetivo desse trabalho é apresentar o desenvolvimento de uma perna robótica, modelada com base em um mecanismo espacial de quatro barras do tipo 4R, denominado mecanismo de Bennett. Sabe-se que com uma configuração adequada desse mecanismo é possível descrever uma trajetória espacial, com perfil similar ao perfil de um passo, utilizando apenas um grau de liberdade. Em função disso, a perna modelada é bem mais simples do que a grande maioria das pernas que são utilizadas atualmente nos robôs móveis. Em função da necessidade de se transpor obstáculos, foram adicionados à perna dois graus de liberdade, dando a ela uma maior mobilidade. Esses graus de liberdade adicionais não comprometem o desempenho da estrutura, pois são utilizados apenas durante a transposição de obstáculos, que é uma situação na qual o robô normalmente se movimenta a velocidades mais baixas. A perna modelada, apesar de não ter tido sua configuração otimizada, apresentou resultados satisfatórios, sendo capaz de transpor obstáculos de dimensões comparativas às dimensões da perna. Leg lengthening by external fixation is associated with various difficulties. We evaluated eight adolescent patients who underwent leg lengthening with a motorized intramedullary lengthening device. We asked whether this method could reduce the time of hospitalization and rehabilitation and whether the incidence of complications commonly associated with external fixators could be reduced. We compared our preliminary results with those from other reports, with a focus on leg length achieved, time of rehabilitation, and rate of complications. The average leg-length discrepancy was 3.8 cm (range, 3–5 cm). The average lengthening distance was 3.8 cm (range, 2.9–4.7 cm). In six patients, leg lengthening was combined with successful correction of the mechanical axis alignment. The consolidation index averaged 26 days/cm (range, 19–41 days/cm). The average hospital stay was 9.6 days. No bone or soft tissue infections were observed. In comparison to other studies (1.0–2.8 complications/patient), our results suggest that the difficulties commonly associated with external fixators can be reduced with this method. Fatty acid metabolism is influenced by training and diet with exercise training mediating this through activation of nuclear hormone receptor peroxisome proliferator-activated receptor α (PPARα) in skeletal muscle. This study investigated the effect of training and high fat or normal diet on PPARα expression in human skeletal muscle. Thirteen men trained one leg (T) four weeks (31.5 h in total), while the other leg (UT) served as control. During the 4 weeks six subjects consumed high fat (FAT) diet and seven subjects maintained a normal (CHO) diet. Biopsies were obtained from vastus lateralis muscle in both legs before and after training. After the biopsy, one-leg extension exercise was performed in random order with both legs 30 min at 95% of workload max. A training effect was evident as citrate synthase activity increased (P < 0.05) by 15% in the trained, but not the control leg in both groups. During exercise respiratory exchange ratio was lower in FAT (0.86 ± 0.01, 0.83 ± 0.01, mean ± SEM) than CHO (0.96 ± 0.02, 0.94 ± 0.03) and in UT than T legs, respectively. STUDY OBJECTIVES: To describe the time structure of leg movements (LM) in obstructive sleep apnea (OSA) syndrome, in order to advance understanding of their clinical significance. LOCATION: Sleep Research Centre, Oasi Institute (IRCCS), Troina, Italy. SETTING: Sleep laboratory. PATIENTS: Eighty-four patients (16 females, 68 males, mean age 55.1 y, range 29-74 y). METHODS: Respiratory-related leg movements (RRLM) and those unrelated to respiratory events (NRLM) were examined within diagnostic polysomnograms alone and together for their distributions within the sleep period and for their periodicity. MEASUREMENTS AND RESULTS: Patients with OSA and RRLM exhibited more periodic leg movements in sleep (PLMS), particularly in NREM sleep. A gradual decrease in number of NRLM across the sleep period was observed in patients with RRLM. This pattern was less clear for RRLM. Frequency histograms of intermovement intervals of all LMs in patients with RRLM showed a prominent first peak at 4 sec, and a second peak at approximately 24 sec coincident with that of PLMS occurring in the absence of OSA. Leg ulceration negatively affects quality of life (QOL). The relationship between important clinical attributes such as ulcer severity, pain and health related quality of life (HRQL) is poorly understood. The Wilson and Cleary model of Health Related Quality of Life (WC model) is a guiding framework that links clinical measures to QOL. Thesis Objectives: I describe the relationship between ulcer severity, comorbidity, etiology and pain and HRQL. Methods: An integrative mixed-methods approach was used: a systematic review of studies that examined the relationship between clinical variables and HRQL or QOL as outlined in the WC model (n=22); a review of studies (n=11) that compared the HRQL of persons with leg ulcers to population normative scores; and, an analysis of the relationship between clinical factors and HRQL in a cohort of 564 persons with venous leg ulcers drawn from two randomized control trials. Results: There is empirical support for the causal linkages between disease severity, symptoms, function, health perception and QOL that comprise the causal pathway as outlined in the WC model. BACKGROUND Changing from a supine to an orthostatic posture is associated with substantial increments in leg blood pressure (BP) levels, which could ultimately influence the hemodynamic burden imposed on the heart. This study investigated the relationship between brachial and leg BP measurements and the left cardiac chamber's structure and assessed the role of body posture changes in this regard. METHODS One hundred and thirty normotensive, nondiabetic, nonsmoking, normolipemic subjects were evaluated by a clinical history, anthropometry, the analysis of metabolic parameters, echocardiography, and the measurement of BP in the arm and the calf in both supine and orthostatic positions. RESULTS Significant correlation coefficients between the leg BP measurements and the cardiac structure were detected, especially between the orthostatic pulse pressure (PP) and the left ventricular (LV) wall thickness (r = 0.38; P < 0.001), the orthostatic PP and the LV mass (r = 0.37; P < 0.001), and the orthostatic systolic BP (SBP) and the left atrial size (r = 0.35; P < 0.001). The objective of this study is to identify the nursing interventions for people with venous, arterial or mixed leg ulcers. This study was performed using the EBSCO search engine: CINAHL and MEDLINE yielded results, based on full-text articles published between 2000 and 2010, using the following descriptors: Leg* Ulcer* AND Nurs* AND Intervention*, filtered using a starting question using PICO. At the same time, a search was performed on the National Guideline Clearinghouse, using the same search guidelines. A person-centered intervention increased positive health outcomes, with a range of direct wound care in agreement with the etiology. BACKGROUND: Neurogenic and referred leg pain are some of the most challenging clinical problems in spinal surgery due to the diversity of conditions that can act as aetiological factors. METHODS: The literature was reviewed and combined with case studies to demonstrate different aetiological factors. RESULTS: At least 300 conditions were identified that might cause neurogenic and referred leg pain. An aetiological classification of neurogenic and referred leg pain is presented. The classification includes systemic conditions, conditions from the brain, spinal cord, cervical and thoracic spinal canal, lumbar spinal canal, lumbar nerve root canal, lumbar extraforaminal area, the pelvis and the lower extremity. Each one of these conditions can mimic a lumbar disc herniation accurately. CONCLUSION: The aetiological classification can be used as a checklist when evaluating neurogenic and referred leg pain. Each condition deserves careful consideration and when overlooked might result in a missed diagnosis.

Investigation of effects of three candidate genes on leg action and fat deposition traits in pigs

Data from 188 sows were used in the current study to examine the effects of high mobility group AT-hook1 (HMGA1), transcription factor 7-like-2 (TCF7L2) and insulin-like growth factor binding protein 3 (IGFBP3) genes on leg action and fat deposition traits, and further to explore the possible relationships between these genes on both traits. The candidate genes used in the study are known for their roles in fat deposition and growth. Overall leg action was scored on a scale of 1 (good movement) to 9 (leg weakness). Fatness traits included 10th rib backfat (BF10), adjusted 10th rib backfat to 125 kg (adjBF10) and last rib backfat (last BF), measured by ultrasonic imaging approach. The association analyses between single nucleotide polymorphisms (SNPs) and traits were performed using PROC MIXED procedures of SAS. The results showed that the associations between HMGA1, TCF7L2 and IGFBP3 genotypes with fat deposition traits were mostly suggestive in this limited data set. Leg action was also suggestively associated with IGFBP3 gene effects but was not associated with HMGA1 and TCF7L2 genes. Thus, IGFBP3 AA homozygote individuals tended to have had better movements (5.40), and were fatter when compared to GG homozygotes (5.84). OBJECTIVE: To describe the characteristic presentation of exertional leg pain in athletes and to discuss the diagnostic options and surgical management of exertional anterior compartment syndrome of the leg in this group of patients. METHODS: Data from a series of athletes presenting with exertional leg pain were analysed and categorized according to aetiology. RESULTS: Sixty-six athletes presenting with exertional leg pain in 102 limbs were analysed. Sixteen patients in a first group of 20 patients with a provisional diagnosis of exertional anterior compartment syndrome of the leg underwent a closed fasciotomy with complete resolution of symptoms. A second group of 42 patients were diagnosed as medial tibial stress syndrome and a third group of four patients had confirmed stress fracture of the tibia. CONCLUSION: Exertional leg pain is a common presenting complaint of athletes to sports physicians and physiotherapists. Careful analysis can lead to an accurate diagnosis and commencement of effective treatment. Exertional anterior compartment syndrome can be successfully treated utilizing a closed fasciotomy with a rapid return to sport.
