Nursing development depends on clarification of terms currently used, but apparently with different meanings. That is the case of therapeutic interventions and therapeutic instruments. The present article tries to answer the following research question: “Which nursing interventions are considered therapeutic and what are their characteristics?” It was hoped that the meaning of therapeutic instruments would be achieved. A systematic review of the literature on nursing therapeutic interventions was done based on well known data basis through which were achieved 13 articles dated from 1993 to 2006. There is some degree of evidence of the use of therapeutic nursing interventions and general agreement on its meaning. Identified therapeutic instruments used by nurses in therapeutic interventions were: therapeutic letters, bathing and comforting care, humour, music, presence, mindfulness (cognitive therapy), therapeutic touch, information management and emotional management. The concept therapeutic instrument used in nursing needs maturation, justifying a concept clarification study.

Pharmaceutical care in transplant patients in a university hospital: pharmaceutical interventions

A descriptive and prospective study was conducted on the pharmaceutical care in the post-transplant outpatient clinic of Hospital Universitario Walter Cantidio of Universidade Federal do Ceará (HUWC/UFC), in Fortaleza- Ceará in the period of April to October of 2011. The aim of the present study was to describe the pharmaceutical interventions performed in a Pharmaceutical Care service structured in the liver and kidney transplant outpatient clinic of an academic hospital. The Pharmaceutical interventions (PI) were classified according to Sabater et al.(2005), with significance based on Riba et al.(2000) and the Negative Outcomes associated with Medication (NOM) established at the Third Consensus of Granada. Statistical analyses were performed using the Epi Info v.3.5.1 program and hypothesis tests were done with the SigmaPlot v.10.0 program. A chi-squared (X²) test was utilized for statistical analysis of the sample. A total of 97 patients were followed, where 54 problems related to medications were identified and 139 PI performed. Multilevel interventions, implemented at the individual, physician, clinic, health-care organization, and/or community level, increasingly are proposed and used in the belief that they will lead to more substantial and sustained changes in behaviors related to cancer prevention, detection, and treatment than would single-level interventions. It is important to understand how intervention components are related to patient outcomes and identify barriers to implementation. Designs that permit such assessments are uncommon, however. Thus, an important way of expanding our knowledge about multilevel interventions would be to assess the impact of interventions at different levels on patients as well as the independent and synergistic effects of influences from different levels. It also would be useful to assess the impact of interventions on outcomes at different levels. Multilevel interventions are much more expensive and complicated to implement and evaluate than are single-level interventions. Given how little evidence there is about the value of multilevel interventions, however, it is incumbent upon those arguing for this approach to do multilevel research that explicates the contributions that interventions at different levels make to the desired outcomes. Only then will we know whether multilevel interventions are better than more focused interventions and gain greater insights into the kinds of interventions that can be implemented effectively and efficiently to improve health and health care for individuals with cancer. This chapter reviews designs for assessing multilevel interventions and analytic ways of controlling for potentially confounding variables that can account for the complex structure of multilevel data.

Background: Infant and child mortality rates are among the most important indicators of child health, nutrition, implementation of key survival interventions, and the overall social and economic development of a population. In this paper, we investigate the role of coverage of maternal and child health (MNCH) interventions in contributing to declines in child mortality in sub-Saharan Africa. Design: Data are from 81 Demographic and Health Surveys from 35 sub-Saharan African countries. Using ecological time-series and child-level regression models, we estimated the effect of MNCH interventions (summarized by the percent composite coverage index, or CCI) on child mortality with in the first 5 years of life net of temporal trends and covariates at the household, maternal, and child levels. Results: At the ecologic level, a unit increase in standardized CCI was associated with a reduction in under-5 child mortality rate (U5MR) of 29.0 per 1,000 (95% CI: −43.2, −14.7) after adjustment for survey period effects and country-level per capita gross domestic product (pcGDP). Objectifs. L’objectif de ce mémoire est d’améliorer les connaissances quant à l’effet des interventions policières sur la violence imputable aux bandes criminelles. À travers l’évaluation des cinq plus importantes opérations policières réalisées entre 1991 et 2008 au Québec, trois modèles conceptuels sont confrontés : 1) celui du marché qui prévoit une hausse des affrontements entres bandes rivales dont le but est de prendre possession d’un marché criminel maintenant disponible en raison du retrait d’un joueur clé, 2) celui de la dissuasion qui prévoit une baisse des violences criminelles, et 3) celui de l’attrition qui envisage une baisse des violences en raison de l’essoufflement des bandes. Méthodologie. Les données de cette étude proviennent de l’Enquête générale sur les homicides de Statistique Canada. Différents taux d’homicides furent agrégés sur une base annuelle pour différentes provinces et régions métropolitaines de recensement. The focus of trade policy has shifted in recent years from economy-wide reductions in tariffs and trade restrictions toward targeted interventions to facilitate trade and promote exports. Most of these latter interventions are based on the new mantra of "aid-for-trade" rather than on hard evidence on what works and what does not. On the one hand, rigorous impact-evaluation is needed to justify these interventions and to improve their design. On the other hand, rigorous evaluation is feasible because unlike traditional trade policy, these interventions tend to be targeted and so it is possible to construct treatment and control groups. When interventions are not targeted, such as in the case of customs reforms, some techniques, such as randomized control trials, may not be feasible but meaningful evaluation may still be possible. Violence against Women and Girls (VAWG) is a pervasive global problem. It is a violation of basic human rights and a drag on development. Much of the research to-date on the topic-including a major recent World Health Organization study to produce global prevalence rates has focused on better understanding the scale and nature of the problem. The present study builds on this body or research while shifting focus to synthesizing global evidence on potential solutions. This paper, a systematic review of reviews, breaks new ground by synthesizing evidence on the effects of VAWG prevention interventions. It examines the diversity of geographical context, the types of violence addressed, and the numerous approaches that have been used to combat VAWG. Additionally the review summarizes the quality of evidence on efficacy and effectiveness in order to highlight strengths and gaps of interventions on a global scale and could serve as a point of reference for those intending to undertake future design, implementation, and evaluation of interventions. In an effort to bridge the evidence gap, the Independent Evaluation Group (IEG) prepared a systematic review that gathers and analyzes the available impact evaluation evidence in developing countries from 1990 to 2013 on whether early childhood interventions shape future outcomes. Its purpose is not to supplant existing evidence but rather to help practitioners understand how evidence from impact evaluations supports or challenges beliefs about interventions and can be used to inform development policy. This review aims to answer two questions: (1) What is the evidence of attributable effects on outcomes in primary school and beyond from interventions in low- and middle-income countries that occur during the early childhood period? and (2) How do the post-early childhood effects of early childhood interventions vary by socioeconomic status, gender, age at intervention, and age at evaluation, particularly during the first 1,000 days from conception to the childs second birthday and from age three to primary school enrollment at age five to six? Introduction: Internal conflict has become the predominant threat to the security and stability of many of the small island nations of the Southwest Pacific and particularly in the countries of Melanesia. Since the late 1980s, conflicts of varying causes and degrees of intensity have occurred in Papua New Guinea (Bougainville secession attempt), Fiji (coups and attempted coups), Vanuatu (police rebellion) and Solomon Islands (ethnic conflict and coup). These events have seriously debilitated the already fragile national economies and polities of all countries, so much so in the Solomon Islands that that country is now being described by many analysts as a failing, if not failed, state. While most of these countries have so far been able (not without difficulty) to maintain a measure of state integrity, the situation in Solomon Islands has become so precarious that Australia and New Zealand (with the support of most Pacific Island governments and anticipating a request from the Solomons parliament) are preparing to intervene in an attempt to restore the rule of law and rebuild administrative institutions. BACKGROUND: Depression and anxiety are treatable disorders, yet many people do not seek professional help. Interventions designed to improve help-seeking attitudes and increase help-seeking intentions and behaviour have been evaluated in recent times. However, there have been no systematic reviews of the efficacy or effectiveness of these interventions in promoting help-seeking. Therefore, this paper reports a systematic review of published randomised controlled trials targeting help-seeking attitudes, intentions or behaviours for depression, anxiety, and general psychological distress. METHODS: Studies were identified through searches of PubMed, PsycInfo, and the Cochrane database in November 2011. Studies were included if they included a randomised controlled trial of at least one intervention targeting help-seeking for depression or anxiety or general psychological distress, and contained extractable data on help-seeking attitudes or intentions or behaviour. Studies were excluded if they focused on problems or conditions other than the target (e.g., substance use, eating disorder). BACKGROUND It is well known that web-based interventions can be effective treatments for depression. However, dropout rates in web-based interventions are typically high, especially in self-guided web-based interventions. Rigorous empirical evidence regarding factors influencing dropout in self-guided web-based interventions is lacking due to small study sample sizes. In this paper we examined predictors of dropout in an individual patient data meta-analysis to gain a better understanding of who may benefit from these interventions. METHOD A comprehensive literature search for all randomized controlled trials (RCTs) of psychotherapy for adults with depression from 2006 to January 2013 was conducted. Next, we approached authors to collect the primary data of the selected studies. Predictors of dropout, such as socio-demographic, clinical, and intervention characteristics were examined. RESULTS Data from 2705 participants across ten RCTs of self-guided web-based interventions for depression were analysed. Aims: To examine interventions and timing of emergency team calls in hospitals with or without a medical emergency team (MET). Methods: Interventions were recorded, categorized and classified as critical care interventions (e.g. airway intervention, ventilation and use of inotropic drugs); ward level interventions (e.g. fluids, oxygen by mask); assessment, physical examination and investigations. Results: Only 5 of the 2376 calls were free of critical care interventions. For non-cardiac arrest-related calls, MET hospitals had a lower proportion of airway, circulation and drug-related interventions and a higher proportion of ward level interventions. The majority of calls were between 0601 and 1200h and cardiac arrest survival was greatest in the 1200–2400h period. Overall median time at the scene was 25min. Conclusions: Nearly all emergency team calls required critical care type interventions. Emergency team calls show a unique temporal pattern for both MET and control hospitals. OBJECTIVES: Lifestyle interventions in obese pregnant women reduce adverse maternal outcomes of pregnancy. However, the association between weight change due to interventions and the actual reduction in complications is unknown. The objective of this study was to determine the association between gestational weight gain (GWG) and the rate of pregnancy complications. STUDY DESIGN: The authors included randomized controlled trials (RCTs) assessing the effect of lifestyle interventions during pregnancy on GWG and adverse maternal and fetal outcomes. For each outcome they assessed the association between GWG and the risk of adverse pregnancy outcomes. RESULTS: They analyzed data of 23 RCTs (4,990 women). Increased GWG was associated with a nonsignificant increase in the incidence of preeclampsia (PE) (0.2% per gained kg, 95% confidence interval [CI] 0.5 to 0.9%, p > 0.05), gestational diabetes (GDM) (0.3% per gained kg, 95% CI -0.5 to 1.0%, p > 0.05), and induction of labor (IOL) (1.5% per gained kg, 95% CI -0.9 to 3.9%, p > 0.05). CONCLUSIONS: Reduction in GWG due to lifestyle interventions in pregnancy had statistically nonsignificant effects on lowering the incidence of PE, GDM, and IOL. This thesis analyses the economic effects and the political economy causes of actual and proposed trade policy interventions in response to agricultural commodity price spikes. It does so by employing a theoretical model of world trade that incorporates both import and export policies in a common framework. The thesis begins with an analysis of the economic effects of trade interventions by investigating terms of trade effects, welfare implications, and distributional impacts within countries. The international price effect of trade policy interventions of large (or sufficiently large group of small) food-importing countries in response to a price spike is to reinforce the initial exogenous price spike. Insofar as the policy reactions of importing countries trigger exporting countries to respond with trade policies, the price spike is further accentuated. The own-welfare effects also are accentuated. This thesis shows that if countries began with some trade restrictions, the national welfare effects are more complex. The welfare of countries that do not intervene in trade also is affected by interventions of other countries. The within-country distributional effects show that the effects on households' real income depend on initial proportions of their income from different productive factors and the initial proportions of their consumption expenditure on agricultural and non-agricultural tradables and on non-traded goods. Aim: To identify key issues relevant to the delivery and evaluation of population-level mental health interventions for children and adolescents. Methods: The benefits and limitations of clinical, targeted and universal interventions were initially reviewed. Subsequently, experience gained in evaluations of targeted and universal interventions was utilized to identify key challenges that must be addressed by researchers responsible for evaluating population-level interventions and potential solutions to these challenges. Results: To be effective, population-level interventions must engage large numbers of individuals in community or regional areas. Successfully evaluating population-level interventions delivered in routine services requires a clear agreement about outcomes, use of strong research methodologies, and the availability of adequate research funding. Sustaining service-research partnerships over the several-year life of typical population interventions requires careful attention to these issues. Electronic databases with the capacity to efficiently collect, store and allow retrieval of large amounts of data in electronic format are also an essential component of population-level interventions. Finally, research leaders need high-quality administrative skills to manage research teams responsible for evaluating large-scale population-level interventions delivered at a regional or national level. The focus of this research project is the delivery of behavior change interventions based on the brief intervention counseling style via spoken dialogue systems. Brief interventions (BI) are evidence-based, short, well structured, one-on-one counseling sessions. Many challenges are involved in delivering BIs to people in need, such as finding the time to administer them in busy doctors' offices, obtaining the extra training that helps staff become comfortable providing these interventions, and managing the cost of delivering the interventions. Résumé en français Objectifs : Ce mémoire propose d’explorer l’assiduité aux interventions psychosociales chez les personnes atteintes de troubles psychotiques en répondant à trois questions : 1- Quels sont les facteurs influençant l’assiduité aux interventions psychosociales pour une clientèle atteinte de troubles psychotiques? 2- Sont-ils comparables aux facteurs influençant l’observance à la médication? 3- Quel est le taux d’abandon des interventions psychosociales et quels sont les facteurs qui font varier ce taux? Méthodes : Cette étude a permis de faire la synthèse des facteurs influençant l’observance à la médication à partir des revues systématiques publiées sur le sujet, et d’établir les facteurs influençant l’assiduité aux interventions psychosociales à partir des raisons d’abandon citées dans les essais cliniques randomisés publiés. Une méta-analyse a permis de combiner les essais cliniques rapportant les abandons et ainsi d’en établir un taux. Résultats : Nous avons répertorié 92 essais cliniques randomisés sur les interventions psychosociales avec les personnes atteintes de troubles psychotiques. A partir da realidade encontrada em algumas comunidades da etnia Mbyá-Guarani do Rio Grande do Sul aborda-se a relação entre as construções autóctones, a sustentabilidade e as intervenções habitacionais externas. Nestes locais, de acordo com a região, são desenvolvidas diferentes soluções construtivas, utilizando os materiais disponíveis e respeitando os preceitos culturais. Porém, existem dificuldades de acesso aos recursos naturais necessários às construções. Para resolver a falta de moradias, o Governo do Estado vem realizando intervenções habitacionais, utilizando o método de desenho social participativo. Contudo, os Mbyá- Guarani seguem construindo suas casas tradicionais ao lado das intervenções. Esta dissertação tem como objetivo principal analisar comparativamente as diferentes tipologias habitacionais (a autóctone e a proveniente de intervenção externa), observadas em oito comunidades Mbyá-Guarani do estado.
