RESUMO O objeto deste estudo é o acolhimento do usuário em uma unidade da rede básica de serviços públicos de saúde de Ribeirão Preto. Este constituise em um processo, ocorrendo em quaisquer das etapas do processo de trabalho, cuja responsabilidade cabe aos trabalhadores. Sua efetivação implica no estabelecimento de relações humanas, no uso das tecnologias disponíveis para a solução dos problemas, estando estreitamente relacionada ao desenvolvimento de autonomia dos usuários em relação à sua vida e com a conformação do modelo assistencial. Dentre os objetivos buscou-se identificar e analisar os aspectos que o compõem bem como identificar e analisar como os trabalhadores efetuam-no e compreendem seus componentes no processo de trabalho. O acolhimento é um dispositivo técnico e político que visa garantir à população um atendimento em saúde usuário-centrado e com elevada resolutividade em suas ações. O vínculo entre profissional e usuário é a relação que possibilita que a autonomia e a cidadania sejam estimuladas, na participação ativa do usuário no serviço e na construção de saberes sobre saúde. No entanto, visto a relação conflituosa que se estabelece entre anoréxicos e bulímicos, e os profissionais de saúde que tratam desses pacientes, este estudo teve por objetivo avaliar como é o processo de acolhimento dos usuários do Grupo de Assistência em Transtornos Alimentares do Hospital das Clínicas da Faculdade de Medicina - Campus USP (GRATA-HCFMRP-USP) e como se dá o vínculo entre os profissionais e os usuários, ambos inseridos nesse serviço. Este trabalho tem por finalidade analisar uma instituição privada de assistência social, que atende crianças e adolescentes em regime de abrigo, a fim de verificar de que forma o serviço de proteção social especial de alta complexidade está sendo prestado, levando em consideração as exigências legais e a garantia dos direitos da população atendida. Tem por objetivo identificar possíveis falhas e potencialidades neste atendimento. Desta forma, é necessário aprofundar teoricamente a trajetória da assistência social brasileira, a legislação pertinente a esta área e as entidades parceiras da rede sócio-assistencial. A rotatividade de profissionais de enfermagem é impactante para a qualidade e segurança da assistência prestada aos usuários, para o desgaste dos profissionais envolvidos na educação permanente, assim como para o dispêndio de valores para custear os processos demissionais e admissionais nas instituições empregadoras. A reposição do profissional depende de processos de seleção e contratação que demandam tempo para a sua admissão, período em que a equipe fica sobrecarregada pela sua ausência ou pela presença temporária de outro trabalhador não capacitado para as atividades específicas da unidade de trabalho. The Pharmaceutical Assistance Cycle (PA) is the basis for healthcare services through which pharmaceutical assistance and expertise can be implemented in a functional and structured way to meet public health demands, adhere to the Brazilian Ministry of Health vision, and strike a balance in the contemporary struggle for health equity. The aim of this research is to analyze the effectiveness of the PA cycle in the city of Romelândia (SC), through a study conducted at a Primary Healthcare Unit in the municipality. For the study, 120 patients (service users) and seven healthcare professionals were interviewed. The majority of patients confirmed that they were given advice and instruction on dosage during the dispensing process, as well as advice on the recommended use of medicines and course duration. A small number of service users, 4.1%, said that they received instructions on adverse drug interactions; 36.6% commented on a lack of access to medicines available in the municipal pharmacy, and 71.4% of the health professionals interviewed, identified non-availability of medicines in the pharmacy as the most pressing and telling problem facing the service. There is also no local Pharmacy and Therapeutics Committee, and the only selection criteria is the Ministry of Health essential medicines list. This work was concerned to investigate the meaning attributed to anxiolytic drugs by women, in public health service. It proposes a joint analysis through a link between three dimensions: the woman, the drug, and public health service itself, in an hegemonic medical assistance model. It is observed that the relation between these elements has a great influence over the use and construction of a particular meaning, by the user. The medication is analysed as a consumption merchandise and as an health symbol. In this way, it reflects a biologized vision, which believes the drug as a solution for all health problems. It tries to analyse the generalized medical prescription for anxiolythics and it s consequences. It focalizes also the production and utilization of public health services by patients, mainly women. The question related to the use of anxiolytics and the meaning construted by women is analysed focusing the way that relations of masculine/feminine gender are organized in our society. The integrality is the most important principle of the Unified Health System (a National Health System called SUS Sistema ??nico de Sa??de) because it shows that health assistance should consider the particular needs of people or specific groups. There are several meanings which are attributed to this term, however in this research; the whole health care was linked to professional practices at the Center for Psychosocial Care (CAPS). CAPS was chosen as a setting of this research for it has been understood as a space where everyday practices can promote the gathering of knowledge and experience of professionals and users in a rebuilding process for citizenship. Managing the Coordination of Service Delivery in Metropolitan Cities : The Role of Metropolitan Governance

This paper examines different models of governing structure found in metropolitan areas around the world. It evaluates how well these models achieve the coordination of service delivery over the entire metropolitan area as well as the extent to which they result in the equitable sharing of costs of services. Based on theory and case studies from numerous cities in developed and less developed countries, the paper concludes that there is no "one size fits all" model of metropolitan governance. Other observations from the case studies highlight the importance of the process of implementing a metropolitan structure, the need to match fiscal resources with expenditure responsibilities, the need to have a governance structure that covers the entire economic region, and the critical importance of having a strong regional structure that ensures that services are delivered in a coordinated fashion across municipal boundaries.

The report examines the local government sector in Kenya, the reform and decentralization process, and the dynamics of local service delivery. The report is organized in three parts. The first, traces the broad contours of the reform process in Kenya: the inter-governmental system, local government and key local service sectors (such as water, roads, education, and health), and the macro reform processes (such as the public sector reforms, and the Kenya Constitution Review). The second part, reviews the existing systems for local service delivery, including aspects such as institutional arrangements, planning and financing for local services, and the structure, and finances of local governments. This paper examines the design of social investment funds (SIFs) and explores the ways they affect agents incentives to propose, select, and implement good projects. Compared with other forms of decentralized service provision, SIFs possess features of administratively delegated authority and deep political devolution. Where existing political institutions fail to deliver assistance to vulnerable groups, a well-designed SIF may represent a useful administrative alternative. This article reviews several features that provide incentives for both SIF staff and project beneficiaries and concludes with practical guidelines for designing and appraising social investment funds.

After more than 35 years, the elected local government system in Lesotho was reestablished in 2005 through the election of the Local Authorities, i.e. the Community and District Councils (CCs and DCs). Across the political spectrum, the political will to move forward was at its peak. An exemplary campaign to educate the entire population as to the purposes and functioning of the new Local Authorities, and the electoral process preceded the election. The purposes of the new system are the improvement in services and access to government, broad participation of the local population in their own development combined with enhanced accountability to them, and promotion of equitable development in all parts of the country. The core objective of the Country Water Sector Assistance Strategy (CWSAS) is to define an operational plan for World Bank involvement in Lebanon's water sector 2012-2016 in support of implementation of national water strategy. How can the impact of aid be estimated in the presence of fungibility? And how far does fungibility reduce its benefits? These questions are analyzed in a context where a donor wants to target its efforts on a specific sector and specific geographic areas. A traditional differences-in-differences method comparing the change in outcomes between the target and nontarget areas before and after the project risks misestimating the project's benefits. The paper develops an alternative estimation method in which intersectoral fungibility reduces project benefits insofar as government spending has a smaller impact in the sector to which the funds leak than in the target sector, while intrasectoral fungibility reduces benefits insofar as the donor is able to leverage productivity increases in government spending in the target areas. The methods are applied to two contemporaneous World Bank health projects that set out to target assistance on approximately one-half of Vietnam's provinces. China has very serious water problems despite substantial economic development achievements, strong technical expertise, and political stability. But in terms of its potential and the critical pressing needs for water resources management, China could do much better in managing its water resources. The country water resources assistance strategy for Zambia provides an analysis of the role of water in the economy and identifies the specific challenges, development opportunities and policies which inform an agreed framework for priority areas of assistance. Zambia lies entirely within the catchments of the Zambezi and Congo rivers and all internal runoff is shared by downstream and parallel riparian countries. This strategic geographic position in the upper reaches of both these catchments provides an important context for any water resources development. Zambia has played an important role in development of the Southern African Development Community (SADC) revised protocol on shared water courses (2000) and is engaged in the process of developing co-operative mechanisms with riparian states. Mozambique's continuous efforts to sustain economic growth and reduce poverty face a number of constraints including its economic and political history, and its geography and climatic conditions. It is widely accepted that future economic growth of the country will continue to rely on its natural resources base and, specifically, on sustainable use of land and water resources. Mozambique has plentiful land and water resources that provide great potential for the production of a variety of crops, livestock development and industrial growth. However, high climate variability resulting in frequent recurrent droughts and floods, limited water resources availability in the most developed southern part of the country, high dependency on international water resources and very limited water management infrastructure result in the economy being highly vulnerable to water shocks and water being a constraint on growth and poverty reduction. The development of Mozambique Country Water Resources Assistance Strategy (CWRAS) was complementary to the Bank Country Partnership Strategy (CPS) process supporting its approach and priorities.
