A phase II study of UFT with leucovorin administered as a twice daily schedule in the treatment of patients with metastatic colorectal cancer

Prolonged infusions have been shown to be safer and potentially more effective than bolus regimens of 5-fluorouracil (5-FU) as treatment for metastatic colorectal cancer (mCRC). However, infusional 5-FU requires central venous access and costly infusion pumps. Oral fluoropyrimidines enable longer exposures to 5-FU with increased convenience. Tegafur - uracil (UFT) with leucovorin (LV) given thrice daily has improved safety plus comparable survival and response rates to bolus 5-FU/LV. We conducted a phase II clinical study in 98 patients with mCRC to evaluate if UFT with LV given twice daily provided comparable time to progression ( TTP), efficacy and tolerability to that reported for thrice daily in two phase III clinical studies. Secondary objectives included overall response rate ( ORR) and overall survival ( OS). Median TTP was 3.8 months, when compared with 3.5 months for thrice daily. The ORR ( 11%) and median OS ( 12.8 months) with twice daily administration were similar to that of thrice daily administration ( 12% and 12.4 months). The incidence of grade 3/4 treatment-related diarrhoea was 30% on the twice daily and 21% on the thrice daily schedule. These results suggest that twice daily administration has similar efficacy and tolerability to thrice daily administration and is an acceptable alternative for patients who would benefit from UFT with LV therapy.

Infusões prolongadas de 5-fluorouracil são mais seguras e potencialmente mais efetivas no tratamento do câncer de cólon metastático do que infusões rápidas da mesma medicação. No entanto, infusões prolongadas requerem a disponibilidade de um acesso venoso central, bem como de bombas de infusão dispendiosas. O desenvolvimento de fluoropirimidinas orais permitiu que pacientes fossem expostos ao 5-fluorouracil por longo tempo, com maior conveniência. UFT e leucovorin administrados três vezes ao dia demonstraram previamente uma eficácia equivalente, com menor toxicidade, quando comparados a um regime convencional de infusão rápida de 5- fluorouracil e leucovorin. Este estudo com 98 pacientes foi desenhado e conduzido com objetivo de demonstrar equivalência no tempo de progressão com o uso de UFT e leucovorin administrados duas vezes ao dia, com o uso da mesma combinação administrada três vezes ao dia. Neste artigo buscamos explicitar a compreensão de participação na gestão da UFT conforme estudo realizado envolvendo os documentos institucionais e a percepção da comunidade universitária. A discussão acerca da participação na gestão da UFT teve como ponto de partida o entendimento do processo de regulação social resultante da imbricação das esferas pública e privada e das abordagens liberal e histórico-materialista de democracia. Neste sentido, tem sido fortalecida a esfera privada mercantil em detrimento da pública evidenciando-se a mercantilização da educação superior e suas características de natureza neoprofissional, heterônoma, competitiva e operacional. Assim, têm sido fortalecidos nas IFES/UFT os valores da democracia liberal/neoliberal minimalista, operacionalizados na lógica da gestão e da participação estratégico-empresarial. Nesta lógica, os obstáculos à participação no processo de gestão da UFT não diferem dos obstáculos à participação na sociedade de uma forma geral, e têm a ver com o modo de organização e efetivação do sistema de produção e reprodução social capitalista.

Uracil–Tegafur (UFT), an oral fluorinated pyrimidine chemotherapeutic agent, has been used for adjuvant chemotherapy in curatively resected colorectal cancer patients. Past trials and meta-analyses indicate that it is somewhat effective in extending survival of patients with rectal cancer. The objective of this study was to perform a reappraisal of randomised clinical trials conducted in this field. We designed an individual patient-based meta-analysis of relevant clinical trials to examine the benefit of UFT for curatively resected rectal cancer in terms of overall survival (OS), disease-free survival (DFS), and local relapse-free survival (LRFS). We analysed individual patient data of five adjuvant therapy randomised clinical trials for rectal cancer, which met the predetermined inclusion criteria. These five trials had a combined total of 2091 patients, UFT as adjuvant chemotherapy compared to surgery-alone, 5-year follow-up, intention-to-treat-based analytic strategy, and similar endpoints (OS and DFS). In a pooled analysis, UFT had significant advantage over surgery-alone in terms of both OS (hazard ratio, 0.82; 95% confidence interval (CI), 0.70–0.97; P=0.02) and DFS (hazard ratio, 0.73; 95%CI, 0.63–0.84; P<0.0001). This individual patient-based meta-analysis demonstrated that oral UFT significantly improves both OS and DFS in patients with curatively resected rectal cancer.

The feasibility of combining UFT plus leucovorin (LV) with alternating irinotecan and oxaliplatin was investigated in the first-line treatment of patients with advanced colorectal cancer. Twenty-five patients, median age 63 (range 24–79) years, World Health Organisation performance status 0–2 and median four marker lesions, received irinotecan 180 mg m−2 on day 1, oxaliplatin 85–100 mg m−2 on day 15 and UFT 200–300 mg m−2 day−1 with LV 90 mg day−1, days 1–21 of a 28-day cycle. Patients were treated in cohorts of three. At the highest dose (irinotecan 180 mg m−2, oxaliplatin 100 mg m−2 and UFT 300 mg m−2 day−1), three of four patients experienced grade 3 toxicity. Diarrhoea, lethargy and vomiting were dose-limiting. Three of nine patients had grade 2 toxicities at the maximum tolerated dose (irinotecan 180 mg m−2, oxaliplatin 100 mg m−2 and UFT 250 mg m−2 day−1). There were no grade 3 toxicities in the first month of therapy. The overall response rate was 71% in 21 evaluable patients; progression-free survival was 8.8 months. Alternating irinotecan and oxaliplatin plus UFT is an effective and well-tolerated first-line treatment for patients with advanced colorectal cancer. The aim of the study was to evaluate the response rate and safety of weekly paclitaxel (Taxol®) combination chemotherapy with UFT (tegafur, an oral 5-fluorouracil prodrug, and uracil at a 1 : 4 molar ratio) and leucovorin (LV) in patients with advanced gastric cancer. Patients with histologically confirmed, locally advanced or recurrent/metastatic gastric cancer were studied. Paclitaxel 1-h infusion at a dose of 100 mg m−2 on days 1 and 8 and oral UFT 300 mg m−2 day−1 plus LV 90 mg day−1 were given starting from day 1 for 14 days, followed by a 7-day period without treatment. Treatment was repeated every 21 days. From February 2003 to October 2004, 55 patients were enrolled. The median age was 62 years (range: 32–82). Among the 48 patients evaluated for tumour response, two achieved a complete response and 22 a partial response, with an overall response rate of 50% (95% confidence interval: 35–65%). All 55 patients were evaluated for survival and toxicities. Median time to progression and overall survival were 4.4 and 9.8 months, respectively. Major grade 3–4 toxicities were neutropenia in 25 patients (45%) and diarrhoea in eight patients (15%). The sensitivity to 5-fluorouracil (5-FU) has been reported to be associated with target molecule thymidylate synthase (TS), fluoropyrimidine-metabolising enzymes such as orotate phosphoribosyltransferase (OPRT), and dihydropyrimidine dehydrogenase (DPD). We performed an immunohistochemical study on the clinical significance of TS, OPRT, and DPD expression using 151 resected non-small-cell lung cancer (NSCLC) patients postoperatively treated with a combination of tegafur and uracil (UFT). Eighty-two carcinomas were TS-positive, 105 carcinomas were OPRT-positive, 68 carcinomas were DPD-positive. No correlation was observed in the HSCORE between the TS and OPRT expression (r=0.203), between the TS and DPD expression (r=0.098), or between the OPRT and DPD expression (r=0.074). Regarding the survival of NSCLC patients treated with UFT, the 5-year survival rate of patients with TS-negative tumours was significantly higher than that with TS-positive tumours (P=0.0133). The 5-year survival rate of patients with OPRT-positive stage II to III tumours was significantly higher than that with OPRT-negative stage II to III tumours (P=0.0145). In addition, the 5-year survival rate of patients with DPD-negative tumours was also significantly higher than that with DPD-positive tumours (P=0.0004). We conducted this study to ascertain the efficacy and toxicity of docetaxel and cisplatin combined with oral UFT and leucovorin as a first-line treatment for patients with advanced gastric cancer. In all, 52 patients received courses of docetaxel 60 mg m−2 intravenously (i.v.) for 1 h and then cisplatin 75 mg m−2 i.v. for 2 h on day 1. Oral UFT at 400–600 mg day−1, as determined by body surface area, and leucovorin at 75 mg day−1 were administered for 21 consecutive days from day 1, and this was followed by a 7-day drug-free interval. A total of 225 courses were administered, and the median number of courses per patient was four. Four complete responses (7.7%) and 22 partial responses (42.3%) were achieved, giving an overall response rate of 50% (95% Confidence Interval: 36.4–63.6%). The major toxicity was neutropenia, which reached grade 3/4 in 36 patients (69.3%). Grade 3/4 nausea and vomiting was observed in 12 patients (23.1%). Median time to progression was 22 weeks (4 to 156+ weeks), median survival duration was 48 weeks (4 to 156+ weeks), and median response duration was 24 weeks (6–152 weeks). We conclude that docetaxel, cisplatin, oral UFT, and leucovorin combination chemotherapy is effective and tolerable for the treatment of advanced gastric cancer.

Apoptosis and p53 status predict the efficacy of postoperative administration of UFT in non-small cell lung cancer

To examine whether efficacy of postoperative oral administration of UFT, a 5-fluorouracil derivative chemotherapeutic agent, may be influenced by incidence of apoptosis (apoptosis index) or apoptosis-related gene status (p53 and bcl-2) of the tumour, a total of 162 patients with pathologic stage I non-small cell lung cancer were retrospectively reviewed. UFT was administrated postoperatively to 44 patients (UFT group), and not to the other 118 patients (Control group). For all patients, 5-year survival rate of the UFT group (79.9%) seemed higher than that of the Control group (69.8%), although without significant difference (P = 0.054). For patients with higher apoptotic index, 5-year survival rate of the UFT group (83.3%) was significantly higher than that of the Control group (67.6%, P = 0.039); for patients with lower apoptotic index, however, there was no difference in the prognosis between these two groups. Similarly, UFT was effective for patients without p53 aberrant expression (5-year survival rates: 95.2% for the UFT group and 74.3% for the Control group, P = 0.022), whereas not effective for patients with p53 aberrant expression. Bcl-2 status did not influence the efficacy of UFT. In conclusion, apoptotic index and p53 status are useful factors to predict the efficacy of postoperative adjuvant therapy using UFT. © 2001 Cancer Research Campaign http://www.bjcancer.com

A phase I study was carried out to determine the optimal dose and administration schedule for combined UFT plus gemcitabine therapy in patients with non-small cell lung cancer. Twenty-four patients (including 11 patients previously treated with cisplatin as the key drug) received oral UFT 400 mg m−2 on days 1 to 14 with intravenous infusions of gemcitabine (800 mg m−2 on days 8 and 15, or 900 mg m−2 on days 8 and 15, or 900 mg m−2 on days 1, 8 and 15). The most appropriate dosing option appeared to be 400 mg m−2 per day of oral UFT for 14 consecutive days with 900 mg m−2 gemcitabine on days 8 and 15. Eight of the 24 patients achieved partial response. The combination chemotherapy UFT and gemcitabine was well tolerated and may benefit patients with advanced non-small cell lung cancer. A multicentre phase II study using a 3-weekly regimen is in progress.

Tegafur–uracil (UFT) plus leucovorin® (LV, folinic acid) with alternating irinotecan and oxaliplatin were effective and well tolerated in patients with metastatic colorectal cancer (mCRC) in a phase I study. This study expanded the maximum tolerated dose group. Patients aged ⩾18 years had histologically confirmed, inoperable, previously untreated, measurable mCRC. Patients received irinotecan 180 mg m−2 on day 1, oxaliplatin 100 mg m−2 on day 15 and UFT 250 mg m−2 plus LV 90 mg on days 1–21 every 28 days. The phase I/II study comprised 45 patients, 29 at the maximum tolerated dose (MTD). The response rate in 38 evaluable patients was 63% (95% confidence interval (CI): 49–80). Median time to progression and overall survival were 8.7 months (95% CI: 7.9–10.4) and 16.8 months (95% CI: 9.6–25.3), respectively. In the MTD group, one patient had grade 3 leucopaenia; one had grade 3 neutropaenia; three had grade 3 diarrhoea; and one had grade 3 neurotoxicity. No hand–foot syndrome grade >1 was seen. In total, 67% of eligible patients received second-line therapy. UFT plus LV with alternating irinotecan and oxaliplatin is an efficacious first-line treatment for mCRC, with minimal neurotoxicity and hand–foot syndrome.

Previous clinical studies have shown that the oral uracil/tegafur (UFT)/leucovorin (LV) regimen, in which the drugs are taken for 28 consecutive days every 35 days, is equivalent to an infusional 5-fluorouracil (5-FU)/LV regimen for the treatment of colorectal cancer. A 5-days-on/2-days-off schedule for UFT/LV has been proposed as the same schedule for UFT has been reported to be safe and have good compliance. However, few studies have been performed with regards to the feasibility of the UFT/LV regimen. The results of the 5-days-on/2-days-off schedule were compared with those of the consecutive schedule in adjuvant chemotherapy. Twenty-eight patients were treated with the 5-days-on/ 2-days-off schedule of UFT (300 mg/m2/day)/LV (75 mg/body/day), and another 12 patients were treated with the consecutive schedule. In the 5-days-on/2-days-off schedule, 24 of 28 patients (86%) received all the scheduled doses. In the consecutive schedule, 10 of 12 patients (83%) received all the scheduled doses. The mean relative dose intensities for the 5-days-on/2-days-off and consecutive schedules were 0.92 and 0.87, respectively. The toxicities were milder in the 5-days-on/2-days-off schedule compared with the consecutive schedule. The disease-free survival in patients with the 5-days-on/2-days-off schedule tended (P=0.13) to be longer compared with the consecutive schedule. The results of the present study indicate that the 5-days-on/2-days-off schedule of UFT/LV may be feasible and cause no severe toxicities in long-term adjuvant chemotherapy.

This study aims to understand the participation in the FUT management process according to the institutional documents and the university community perception: teachers, students, and administrative staff. To achieve the goal, we used an empirical-theoretical investigation, including bibliographic survey and documentary research, related to the written documentation about the FISEs, especially the FUT, and field research including the university community from seven campuses involved in the study. The study about the participation in the FUT management process had as basis the understanding in the social regulation process, resulting from the overlapping of the public and private spheres, and the liberal and materialist-historical democratic approaches. From this broader discussion, we make explicit the tension between the business-strategical and the participatory-democratical management perspectives and the participation in the business organizations and social institutions as the FISES. This project is part of the teacher training research line in the post-graduation program of the education department at the Federal University of Goiás. It analyzes the career concepts and conceptions which guide the organization of the public teaching career in the State of Tocantins elementary education. It also examines the views of education graduates from the Federal University of Tocantins (UFT) regarding the limits and possible overlaps between initial training and expectations on teaching career and salary in the State and municipal networks in Palmas. Therefore, the research focused on three basic points: a) the organization of the public teaching career for elementary education in the State and municipal networks in Palmas; b) the characteristic of the teaching career in pedagogy projects within education degrees at UFT; c) the understanding and expectation of UFT education graduates regarding career and salary during the training process. O trabalho buscou identificar, analisar e compreender quais foram os modelos e práticas gestoriais presentes no processo de criação e implantação da Universidade Federal do Tocantins (UFT), no contexto da sua relação com a Universidade Estadual do Tocantins (Unitins), e suas reestruturações frente às políticas educacionais desenvolvidas para a educação superior, nos anos 90 do século XX. A análise procurou situar tal processo no contexto mais amplo em que ocorreram mudanças no mundo do trabalho, no papel e na atuação do Estado, ao lado das reformas na administração pública brasileira. O estudo evidenciou que, embora os elementos propostos pelas reformas estejam presentes na gestão da universidade, ainda é comum a presença de relações patrimoniais, carismáticas e, sobretudo, burocráticas, desenvolvidas ao lado de uma gestão do tipo gerencial. ‘A phase II study of oral uracil/ftorafur (UFT®) plus leucovorin combined with oxaliplatin (TEGAFOX) as first-line treatment in patients with metastatic colorectal cancer'

This phase II trial was performed to evaluate the efficacy and tolerability of a new combination of Uracil/Ftorafur (UFT®)/leucovorin (LV) and oxaliplatin in patients (pts) with metastatic colorectal cancer (MCRC) who had not received prior chemotherapy for metastatic disease. Between February 2002 and October 2002, 64 patients received UFT® 300 mg m−2 day−1 and LV 90 mg day−1 from day 1 to day 14 combined with oxaliplatin 130 mg m−2 on day 1, every 3 weeks. All patients were evaluable for safety analysis and 58 of 64 patients were eligible for efficacy. Responses were reviewed by an independent review committee. Of the 58 per-protocol defined assessable patients, 1 complete response and 20 partial responses were observed yielding a response rate of 34% (95% CI: 22–47). The median response duration was 8.74 months (range 1.6–14). The median time to progression and the median survival were 5.88 months (95% CI: 4.34–8.21) and 18.2 months (95% CI: 10–20.7), respectively. Diarrhoea and peripheral neuropathy were the most frequent and predictable toxicities. These events were reversible, noncumulative and manageable. Grade 3 diarrhoea occurred in only 11% of the patients. No grade 4 gastrointestinal toxicity was reported in the study. The incidence of grade 3/4 (National Cancer Institute Common Toxicity Criteria 2: NCI-CTC 2) peripheral neuropathy was 15%. The context of constant transformations on the complex organizational environments require new forms of management, more flexible and participatory structures. The universities face difficulties that the main traditional organizing principles do not solve. In this plot, it is emphasized the importance of having a consolidated human resources management policy in these institutions, in order to establish a humanized organizational structure committed in promoting the health of servers. This research seeks to answer how is developed people management policy of the Federal University of Tocantins (UFT) in relation to health promotion actions of its employees. Specifically, sought to analyze how the Personnel Management from UFT puts into practice the promotion of health of your servers. A estabilidade da economia juntamente com a melhor distribuição de renda proporciona um aumento do consumo de energia elétrica nas edificações, tanto no Brasil como em países desenvolvidos. Com isso, viu-se a necessidade da criação de novos meios que incentivam a conservação de energia, criando assim, o selo PROCEL EDIFICA e seu manual e regulamento. A aplicação do selo no Bloco 3P, da Universidade Federal do Tocantins (UFT), em Palmas, foi realizada com base no Regulamento Técnico da Qualidade do Nível de Eficiência de Edifícios Comerciais, de Serviços Públicos (RTQ-C) e pela Norma de Zoneamento Brasileiro, que levam em consideração a temperatura, radiação, umidade e movimento de ar, como variáveis diretamente relacionadas com o conforto térmico. A análise dos edifícios foi realizada através do método prescritivo, onde se avaliou o sistema de condicionamento de ar, o sistema de iluminação e envoltória e pela simulação computacional, nas quais eles recebem a classificação de A (mais eficiente) a E (menos eficiente). Diante do desafio em se adequar às constantes e rápidas transformações do mundo contemporâneo, as Instituições Federais de Ensino Superior (IFES) vem desenvolvendo novos processos de gestão frente a situação atual. Neste trabalho, destacou-se o Plano de Desenvolvimento Institucional (PDI) como instrumento de planejamento estratégico das IFES, criado pelo Decreto nº 5.773/2006. A pesquisa consistiu em uma análise documental dos PDIs da Universidade Federal do Tocantins (UFT) com o objetivo de avaliar se os PDIs da Universidade estão condizentes com as exigências legais. Através da análise, observou-se uma evolução e aperfeiçoamento na elaboração do PDI, do ponto de vista de que este deixa de ser apenas um instrumento burocrático, para se tornar um instrumento de gestão.
