Oral and pharyngeal transit of a paste bolus in Chagas` disease

We measured the oral and pharyngeal transit of a paste bolus in 20 patients with Chagas` disease and 21 controls. Each subject swallowed of a 10-ml paste bolus prepared with 50 ml of water and 4.5 g of instant food thickener labeled with 55.5 MBq of 99(m) technetium phytate. After the scintigraphic recording of the transit, we delineated regions of interest (ROI) corresponding to mouth, pharynx, and proximal esophagus. Time-activity curves were generated for each ROI. There was no difference between patients with Chagas` disease and controls with respect to the duration of oral and pharyngeal transit, amount of pharyngeal residue, or flux of bolus entry into the proximal esophagus. The amount of oral residue was higher in patients with Chagas` disease (median = 0.71 ml) than in controls (median = 0.45 ml). The pharyngeal clearance duration was longer in patients with Chagas` disease (median = 0.85 s) than in controls (median = 0.60 s). The oral transit duration of the patients with Chagas` disease and dysphagia (median = 0.55 s, n = 14) was shorter than the oral transit duration of chagasic patients without dysphagia (median = 0.80 s, n = 6). We conclude that when swallowing a paste bolus, patients with Chagas` disease may have an increased amount of oral residue and a longer pharyngeal clearance duration than asymptomatic volunteers.

Patients with chronic pancreatitis may have abnormal gastrointestinal transit, but the factors underlying these abnormalities are poorly understood. Gastrointestinal transit was assessed, in 40 male outpatients with alcohol-related chronic pancreatitis and 18 controls, by scintigraphy after a liquid meal labeled with (99m)technetium-phytate. Blood and urinary glucose, fecal fat excretion, nutritional status, and cardiovascular autonomic function were determined in all patients. The influence of diabetes mellitus, malabsorption, malnutrition, and autonomic neuropathy on abnormal gastrointestinal transit was assessed by univariate analysis and Bayesian multiple regression analysis. Accelerated gastrointestinal transit was found in 11 patients who showed abnormally rapid arrival of the meal marker to the cecum. Univariate and Bayesian analysis showed that diabetes mellitus and autonomic neuropathy had significant influences on rapid transit, which was not associated with either malabsorption or malnutrition. O Objetivo desse trabalho é avaliar a Dinâmica Gástrica e o Trânsito Oro-Esofágico em imagens cintilográficas por meio de uma técnica de fluxo óptico chamada Técnica Aprimorada de Demons. Essa técnica permite obter a velocidade dos pixels, também chamada Força de Deformação, em imagens sequenciais aos pares por meio de 3 características das imagens: o gradiente, a intensidade dos pixels e a informação ortogonal ao gradiente. Para alcançar o nosso objetivo, foram utilizadas imagens cintilográficas obtidas no Banco de Imagens do Hospital das Clínicas da USP de Ribeirão Preto. Essas imagens foram adquiridas segundo 2 protocolos: Trânsito Oro-Esofágico e Enchimento Gástrico. Sob o protocolo de Trânsito Oro-Esofágico, foram selecionadas imagens referentes a 9 voluntários saudáveis. Esses, em um período de jejum mínimo de 8 horas, fizeram 10 deglutições de 5 ml de solução fisiológica sendo as deglutições de número 1, 4, 7 e 10 feitas com a solução fisiológica "marcada'' com aproximadamente 26 MBq (cerca de 700 µCi) de 99mTecnécio ligado a moléculas de fitato e adquiridas, pela gamma câmara, como um conjunto de 240 imagens por deglutição. As imagens adquiridas no protocolo de Trânsito Oro-Esofágico foram corrigidas e corregistradas. The epididymal sperm transit time seems to have an important role in the process of sperm maturation, and it seems that alterations to the transit can harm the process. The aim of the present work was to evaluate the influence of altered sperm transit time through the epididymis on sperm parameters and fertility of rats, as well as the role of testosterone in the alterations. Sprague-Dawley adult male rats were randomly assigned to four different groups and were treated for 12 days: (i) 10 mu g/rat/day DES, to accelerate the transit; (ii) 6.25 mg/kg/day guanethidine sulphate, to delay the transit; (iii) same treatment as group 1, plus androgen supplementation; (iv) control animals received the vehicles. Guanethidine treatment delayed the sperm transit time through the epididymal cauda, provoking increased sperm reserves in this region. Animals exposed to DES showed an acceleration of sperm transit time in the epididymis, and consequently decreased sperm density in both epididymal regions, the caput-corpus and cauda, and diminished sperm motility. In both cases sperm production was not altered. Testosterone supplementation was able to restore the transit time to values close to normality, as they were higher than in the control rats. We evaluated the effects of fundectomy and pyloroplasty on the delay of gastric emptying (GE) and gastrointestinal (GI) transit of liquid due to blood volume (BV) expansion in awake rats. Male Wistar rats (N = 76, 180-250 g) were first submitted to fundectomy (N = 26), Heinecke-Mikulicz pyloroplasty (N = 25) or SHAM laparotomy (N = 25). After 6 days, the left external jugular vein was cannulated and the animals were fasted for 24 h with water ad libitum. The test meal was administered intragastrically (1.5 ml of a phenol red solution, 0.5 mg/ml in 5% glucose) to normovolemic control animals and to animals submitted to BV expansion (Ringer-bicarbonate, iv infusion, 1 ml/min, volume up to 5% body weight). BV expansion decreased GE and GI transit rates in SHAM laparotomized animals by 52 and 35.9% (P<0.05). Fundectomy increased GE and GI transit rates by 61.1 and 67.7% (P<0.05) and prevented the effect of expansion on GE but not on GI transit (13.9% reduction, P<0.05). Pyloroplasty also increased GE and GI transit rates by 33.9 and 44.8% (P<0.05) but did not prevent the effect of expansion on GE or GI transit (50.7 and 21.1% reduction, P<0.05). Subdiaphragmatic vagotomy blocked the effect of expansion on GE and GI transit in both SHAM laparotomized animals and animals submitted to pyloroplasty. The present study evaluates the effect of blood volume expansion on the gastrointestinal transit of a charchoal meal (2.5 ml of an aqueous suspension consisting of 5% charcoal and 5% gum arabic) in awake male Wistar rats (200-270 g). On the day before the experiments, the rats were anesthetized with ether, submitted to left jugular vein cannulation and fasted with water ad libitum until 2 h before the gastrointestinal transit measurement. Blood volume expansion by iv infusion of 1 ml/min Ringer bicarbonate in volumes of 3, 4 or 5% body weight delayed gastrointestinal transit at 10 min after test meal administration by 21.3-26.7% (P<0.05), but no effect was observed after 1 or 2% body weight expansion. The effect of blood volume expansion (up to 5% body weight) on gastrointestinal transit lasted for at least 60 min (P<0.05). Mean arterial pressure increased transiently and central venous pressure increased and hematocrit decreased (P<0.05). Subdiaphragmatic vagotomy and yohimbine (3 mg/kg) prevented the delay caused by expansion on gastrointestinal transit, while atropine (0.5 mg/kg), L-NAME (2 mg/kg), hexamethonium (10 mg/kg), prazosin (1 mg/kg) or propranolol (2 mg/kg) were ineffective. These data show that blood volume expansion delays the gastrointestinal transit of a charcoal meal and that vagal and yohimbine-sensitive pathways appear to be involved in this phenomenon. Manometric and pharmacological tests have shown that motor abnormalities may occur in the non-dilated colons of chagasic patients. In order to investigate the presence of abnormalities of colonic function in constipated patients with Chagas’ disease (ChC) without megaesophagus or megacolon, studies of total and segmental colonic transit time with radiopaque markers were performed on 15 ChC patients, 27 healthy volunteers and 17 patients with idiopathic constipation (IC). The values obtained for the control group were similar to those reported in the literature (total colonic time: 34.1 ± 15.6 h; right colon: 9.9 ± 7.3 h; left colon: 10.8 ± 10 h, and rectosigmoid: 12.6 ± 9.9 h). Colonic transit time data permitted us to divide both IC and ChC patients into groups with normal transit and those with slow colonic transit. Colonic inertia was detected in 41% of IC patients and in 13% of ChC patients; left colon isolated stasis (hindgut dysfunction) was detected in 12% of IC patients and 7% of ChC patients, and outlet obstruction was detected in 6% of IC patients and 7% of ChC patients. The objective of the present study was to evaluate associations between fiber intake, colonic transit time and stool frequency. Thirty-eight patients aged 4 to 14 years were submitted to alimentary evaluation and to measurement of colonic transit time. The median fiber intake of the total sample was age + 10.3 g/day. Only 18.4% of the subjects presented a daily dietary fiber intake below the levels recommended by the American Health Foundation. In this group, the median left colonic transit time was shorter than in the group with higher dietary fiber intake (11 vs 17 h, P = 0.067). The correlation between stool frequency and colonic transit time was negative and weak for left colon (r = -0.3, P = 0.04), and negative and moderate for rectosigmoid and total colon (r = -0.5, P<0.001 and r = -0.5, P<0.001, respectively). The stool frequency was lower in the group with slow transit time (0.8 vs 2.3 per week, P = 0.014). In conclusion, most patients with chronic functional constipation had adequate dietary fiber intake. The negative correlation between stool frequency and colonic transit time increased progressively from proximal segments to distal segments of the colon. Patients with normal and prolonged colonic transit time differ in terms of stool frequency.

In recent years helpful guides have appeared in both English and Spanish to assist planners and officials to construct accessible buildings and pedestrian infrastructure which are usable by seniors, persons with disabilities, and all others who especially benefit from universal design. Less has been written about access to public transport systems. Very little guidance is available concerning specific issues which confront those planning Bus Rapid Transit (BRT) systems, mass transit systems which incorporate a spectrum of design and operational features on integrated trunk and feeder routes and which were initiated in Latin America and are now spreading throughout the region and beyond. The guidelines focus on the BRT environment and assume that interested parties can take advantage of existing guidelines to clarify general issues of access to public space, buildings, and pedestrian infrastructure. The guidelines generally follow the travel path of a passenger using a full-featured Bus Rapid Transit system. The accessible travel chain begins with sidewalks and pedestrian crossings and continues into a typical mid-island station served by buses with left-side doors (in countries where traffic drives on the right side). Whether they attempt to build jeepney stops, expand transit access, or improve bus routes, transit projects across much of the developing world are often hampered by (1) the lack of accurate transit maps and data and (2) the weak capacity of transit agencies to acquire and use such data. To address the twin aspects of this long-standing challenge, the World Bank, in collaboration with the Philippines and Australian Aid, developed both a methodology and a suite of open-source software applications based on free, internationally supported open data standards. The solutions have allowed the quick, low-cost production of transit maps; and they have empowered the agencies, and potentially businesses and the rest of government, for the first time to make ambitious planning and investment decisions based on accurate, comprehensive transit data. The global applicability of this approach has been demonstrated by its adoption in six other developing countries to date.

Most of the world’s urban mass transit systems cannot cover operating costs, let alone capital expenses, through farebox revenues. On average, 25 percent of metro operating expenditures are not funded by farebox income. With limited public subsidies, as well as obstacles to raising fares and political sensitivities to road user taxes, metro systems have been increasingly pursuing income from commercial activities connected with their operations. Metro systems earn commercial income, such as from advertising, naming rights, and especially real estate activities, are making inroads in their operating deficits. Commercial revenue in some systems is nearing 20 percent of fare revenue. Although reforms of transit financing structures remain high on the policy agenda, a review of ancillary income streams of metro systems around the world shows that a more entrepreneurial approach to tapping their commercial potential can help them narrow their funding gap.

This study explores the complex process of transit and land-use integration in rapidly growing cities in developing countries. It first identifies barriers to and opportunities for effective coordination of transit infrastructure and urban development. It then recommends a set of policies and implementation measures for overcoming these barriers and exploiting these opportunities. Well-integrated transit and land development create urban forms and spaces that reduce the need for travel by private motorized vehicles. Areas with good access to public transit and well-designed urban spaces that are walkable and bikeable become highly attractive places for people to live, work, learn, play, and interact. Such environments enhance a city's economic competitiveness, reduce local pollution and global greenhouse gas emissions, and promote inclusive development. These goals are at the heart of transit-oriented development (TOD), an urban form that is increasingly important to sustainable urban futures. This book uses a case study approach. Cumbersome transit and customs procedures hinder trade and economic development. Ghana has modified several features of its transit system as part of efforts to improve road-based trade in West Africa. Since 2006 the country's transit reforms have cut the time to process and transport goods crossing the country from five to three days, significantly lowering transport costs. Applying lessons from Ghana could catalyze broader transit reforms in West Africa and other developing regions.

Bogota, the capital city of Colombia, is located near the geographic center of Colombia, 2,640 meters (8,661 ft) sea level. It is the largest and most populous city in the nation, with an estimated 8.2 million inhabitants in the metropolitan area in 2007 and a population density of 3,912 inhabitants per square kilometer. Its economy generates 25 percent of Colombia's total gross domestic product (GDP). The city's roads were highly congested with the significant growth in private car ownership and use. While private cars occupied 64 percent of the road space, they only represented 19 percent of the population, and the daily average commute time was 1 hour and 10 minutes each way. Other issues included high incidences of accidents and extremely high air pollution rates during peak travel hours. Buses are the most common form of urban transit but, in the developing world, are often seen as inefficient and polluting. It is well known that buses that are properly tuned and adjusted tend to be cleaner, safer and consume less fuel than poorly maintained buses. Fuel cost is a relatively large fraction of total cost especially when labor and bus costs are low, as in many developing countries. Hence, reducing fuel use through targeted maintenance of fuel inefficient buses can reduce significant expenditures especially in developing countries, freeing up resources for other improved city services. In addition, if city buses do not receive periodic maintenance that is adequate in quality and quantity, their emissions, both local and global, will suffer. In an effort to catalyze solutions for urban transport, the World Bank Group focused on development of a global Knowledge Product in the form of a 'Guidance Note' (GN) on bus maintenance procedures. Executive Summary Triangle Transit seeks to answer two related questions: • Are lower-income riders more likely than other riders to use a less efficient payment method that contributes to overpayment? • What policies can Triangle Transit adopt to address potential inequities between payments by lower- and higher-income riders? Findings 1. Triangle Transit’s poorer riders are more likely to pay with a pass than with cash. Regression analysis that controls for other factors affecting payment type confirms this finding. 2. Overpayment is a persistent problem among Triangle Transit users, but particularly among cash users. 3. Though access to alternative transportation (cars) is important to employment prospects, it is not a statistically significant predictor of overpayment. Descriptive statistics suggest that income, payment type, and transfers are predictors of overpayment. Lower income, cash use, and higher transfer rates are positive predictors of overpayment. EXECUTIVE SUMMARY POLICY ISSUE (PG 1) Triangle Transit is interested in how much revenue could be generated by the placement of interior and exterior advertisements on the buses and para-transit vehicles that it manages and operates. It would also like to know the impacts of advertisements on Triangle Transit’s brand. In order to answer this question, several major concerns must be addressed: 1. What impacts will an advertisement program have on the agency's brand? 2. What is the most cost-effective way to manage advertisement sales: in-house or under contract? 3. What advertising policy should be issued to avoid future legal challenges to advertisements placed on buses? 4. How much revenue could be generated by allowing interior and exterior advertisements to be placed on Triangle Transit and DATA buses? 5. How much more revenue could be generated if the five transit agencies in the Research Triangle advertise together versus Triangle Transit alone? Warren, Robert; Transit-oriented development (TOD) concept has been a popular planning strategy to reduce car-dependency because it encourages public transit, walking, and bicycling in mixed-use activity nodes around transit stations. There is research that indicates TOD implementation can bring important benefits to specific transit nodes, regions with public transit connection and persons living or working in the region. Through exploring TOD institutional environment and three case studied nodes in Washington D.C., metropolitan area, this study provides detailed evidence and analysis on TOD of dynamics which has received limited attention in the previous research. This study shows that TOD strategy can be implemented in areas having different socio-economic and cultural characteristics. The work contributes to general understanding of TOD in relation to how TOD planning can vary in different transit station areas due to deliberate design by the local governments involved. The institutional environment within which TOD policies and strategies can be adopted and carried out in major metropolitan areas is normally quite complex.
