Em todos os segmentos da sociedade, a utilização de equipamentos eletrônicos projetados e construídos com avançadas tecnologias eletrônicas e controlados por microprocessadores aumenta a cada dia. Na área de saúde, denominados de equipamentos eletromédicos, dão suporte aos setores de diagnóstico, tratamento ou, procedimento cirúrgico, melhorando a qualidade dos serviços e o atendimento aos pacientes. No entanto, o sucesso global do processo deverá considerar a qualidade das instalações elétricas nesses ambientes, bem como a qualidade da energia elétrica fornecida a esses equipamentos. No momento em que a creche integrou o sistema educativo brasileiro, o Estado depara-se com o desafio de estabelecer parâmetros de qualidade e critérios de avaliação do atendimento oferecido às crianças de 0-6 anos. Desta maneira, uma avaliação instrumentalizada da qualidade de ambientes de creches poderia auxiliar nesta tarefa. Dada a inexistência de instrumentos brasileiros, esta pesquisa objetivou verificar a adequabilidade para o nosso contexto da versão traduzida da escala norte-americana Infant/Toddler Environment Rating Scale Revised Edition, utilizada internacionalmente para avaliar a qualidade do atendimento oferecido para crianças de 0-30 meses. Essa escala é composta por 39 itens agrupados em sete subescalas, que contemplam diversas dimensões do ambiente de creches: Espaço e mobiliário (5 itens); Rotinas de cuidado pessoal (6); Falar e compreender (3); Atividades (10); Interação (4); Estrutura do programa (4); Pais e equipe (7). Vivemos atualmente uma era de convergência de tecnologias, motivada por questões tanto econômicas como de caráter operacional, na qual os serviços de dados, voz e vídeo estão migrando rapidamente para uma plataforma IP. Particularmente considerando o paradigma da telefonia IP neste processo de convergência, ocorrem desafios tecnológicos, pois temos de um lado os usuários finais que já possuem uma referência sobre a qualidade da fala, fruto das décadas de uso do sistema telefônico tradicional, e na outra extremidade as operadoras de telecomunicações que em sua última milha dependem de redes estatísticas, sem mecanismos adequados para a garantia de QoS. A avaliação da qualidade dos serviços é um importante instrumento para direcionar o planejamento, a gerência, a alocação de recursos e as políticas na atenção à saúde. O objetivo geral da presente pesquisa foi estudar a qualidade dos serviços na área de saúde, por meio de uma revisão bibliográfica e de uma ferramenta específica. O primeiro estudo, através de uma revisão sistemática qualitativa, teve como objetivo verificar a qualidade nos serviços de saúde utilizando a percepção dos usuários e dos profissionais como indicadores. O resultado constatou que os usuários e os profissionais auxiliam no processo de avaliação da qualidade, e concluiu-se que as expectativas e necessidades dos usuários e profissionais são componentes que contribuem no planejamento da qualidade nos serviços de saúde. O segundo estudo teve como objetivo verificar a possibilidade de planejar a qualidade dos serviços de saúde em quatro Unidades de Saúde da Família na cidade de Piracicaba - SP -Brasil, por meio da ferramenta Desdobramento da Função Qualidade (QFD). What has changed is the increased interest among providers, consumers, and policymakers for ways to measure and compare quality among providers. Satisfaction surveys offer a potentially convenient and cost-effective means for measuring the quality of services. However, concerns about subjectivity and selection bias impede greater use of satisfaction data. This paper analyzes satisfaction data about health and educational services from the 2006 second round of the Governance and Decentralization Survey in Indonesia to assess whether satisfaction data can serve as reliable indicators of quality, despite dubiously high levels of reported satisfaction. The authors use an expectation disconfirmation model that posits that a user s satisfaction with a facility improves with the (positive) difference between the actual quality of the facility and the household s expected standard for quality, which is influenced by its socioeconomic characteristics. The findings show that, after taking into account the expectations of households, reported satisfaction does vary significantly with objective indicators of quality. The analysis also checks for possible selection bias affecting the results by using a two-stage selection model. Foreign Direct Investment in Services and Manufacturing Productivity Growth : Evidence for Chile

During the 1990s, foreign direct investment in producer service sectors in Latin America was massive. Such investment may increase the quality of services, reduce their cost, and offer opportunities for knowledge spillovers to downstream users of the services. This paper examines the effects of foreign direct investment in services on manufacturing productivity growth in Chile between 1992 and 2004. The authors estimate an extended production function where plant output growth depends on input growth and a weighted measure of foreign direct investment in services. The novelty of the approach is that the authors are able to assess the intensity of usage of various types of services at the plant level and use that information in the estimation of the importance of foreign direct investment in those services. The econometric results show a positive and significant effect of foreign direct investment in services on productivity growth of Chilean manufacturing plants which is robust to a multitude of tests. The economic impact of the estimates is that forward linkages from foreign direct investment in services account for almost 5 percent of the observed increase in Chilean manufacturing productivity growth during the sample period. This evidence therefore suggests that reducing the barriers restricting foreign direct investment in services in many developing economies may help accelerate productivity growth in their manufacturing sectors.

Bulgaria has undertaken several significant health sector reforms during the past decade, but a large unfinished policy agenda remains. Compared to other EU countries, the share of out-of-pocket spending is significantly higher in Bulgaria, while government spending on health is relatively low. Various indicators of reported public satisfaction with the health system in Bulgaria are frequently the lowest in the EU. The hospital sector has seen rapid growth in recent years, putting the sustainability of the system in doubt and crowding out expenditures on more pressing priorities such as primary health care and the reimbursement of cost-effective drugs. This report is the second volume of the programmatic Public Expenditure Review (PER) in Belarus. In 2010 the Ministry of Finance requested that the Bank provide sequenced and targeted advice on the fiscal reform options across priority areas of the budget. Responding to this request, a programmatic PER was agreed upon. The first volume was delivered in November 2011 and focused on providing policy options for a sustainable pension system, better targeted social assistance and rationalization of energy and agricultural subsidies. The objective of this volume is to identify reform opportunities to enhance the quality of key public services in a fiscally constrained environment. Specifically, the second PER will analyze three key areas of the budget: intergovernmental fiscal relations and public spending in the education and health sectors. Paying health facilities incentives based on their performance is one form of results-based financing (RBF). Verification of the performance of the providers is a vital part of RBF program implementation. Burundi was one of the first African countries to introduce performance-based financing (PBF). The PBF scheme is implemented in the whole country and is led by the Ministry of Health (MoH). It pays incentives based on quantity of services provided as well as a quality of care component. This study describes the methods used for verification in Burundi, which include monthly verification of the quantity and technical quality of services provided on a quarterly basis; semiannual patient tracing and assessment of patient satisfaction; and counter-verification of the information provided by these three mechanisms. Ethiopia, like most developing countries, has opted to deliver services such as basic education, primary health care, agricultural extension advice, water, and rural roads through a highly decentralized system (Manor 1999; Treisman 2007). That choice is based on several decades of theoretical analysis examining how a decentralized government might respond better to diverse local needs and provide public goods more efficiently than a highly centralized government. Ethiopia primarily manages the delivery of basic services at the woreda (district) level. Those services are financed predominantly through intergovernmental fiscal transfers (IGFTs) from the federal to the regional and then the woreda administrations, although some woredas raise a small amount of revenue to support local services. Since 2006, development partners and the government have cofinanced block grants for decentralized services through the Promoting Basic Services (PBS) Program. Aside from funding the delivery of services, the program supports measures to improve the quality of services and local governments capacity to deliver them by strengthening accountability and citizen voice.

This study presents two case studies, each on a current initiative of contracting for primary health services in Brazil, one for the state of Bahia, the other for the city of Rio de Janeiro. The two initiatives are not linked and their implementation has independently sprung from a search for more effective ways of delivering public primary health care. The two models differ considerably in context, needs, modalities, and outcomes. This paper identifies their strengths and weaknesses, initially by providing a background to universal primary health care in Brazil, paying particular attention to the family health strategy, the driver of the basic health care model. The Government of Tajikistan has identified Primary Heath Care (PHC), and Maternal and Child Health (MCH) as top priorities in its first Comprehensive National Health Sector Strategy (2010-2020). The study findings which closely mirror those of the 2009 World Health Organization (WHO)/United Nation children's Fund (UNICEF) Integrated Management of Childhood Illness (IMCI) survey will be of great concern to the Ministry of Health and should provide the impetus to take immediate remedial actions. Much remains to be done to improve the overall quality of primary health care services for under-five children. Two main issues are highlighted by this study. O presente trabalho teve como objetivo diagnosticar a Qualidade de Vida no Trabalho dos frentistas a partir de três eixos norteadores: a) delinear o perfil da categoria; b) caracterizar o desempenho dos trabalhadores em indicadores de QVT de acordo com o modelo de Fernandes (1996); c) indicar interfaces da QVT com a qualidade dos serviços prestados. O universo foi de 886 sujeitos, dos quais 283 compuseram a amostra. A coleta dos dados foi realizada por meio de questionário estruturado (baseado no modelo de Fernandes, 1996), com uma única questão subjetiva. As questões fechadas foram processadas no software estatístico SPSS, ao passo que a questão aberta teve os resultados categorizados segundo as dimensões do modelo já citado. Como resultado, desempenhos inferiores ocorreram na Compensação e nas Condições de Trabalho, em contraposição às dimensões Imagem da empresa e Comunicação, que obtiveram destaque positivo. The quality of the services is one of the principals’ manager’s preoccupations, a factor that contribute for the client’s satisfaction. The research has as objective the satisfaction analyses as to the quality of services offered by the Natal’s bus terminal (RN), in the scale SERVPERF. The research was crucial in the literature review serving it as base for the compression and results analysis. The data collected was analyzed by a quantitative away, conducted to the conclusion of the applicability. The model adopted has five dimensions that suggest a direct contribution for the general satisfaction’s perception of the passengers in relation to the tangibility, reliability, promptitude, security and empathy, by twenty two items adapted to the terminal. The sample was constituted with the participation of the passengers that embarked in the terminal with the intent to achieve the level of trust and a tolerable error margin. The results of the research signalized that the model’s dimension proposed contribute to the satisfaction of the passengers. In order to design the items for the scale, focus groups were set up with professionals dedicated to attending to individuals with intellectual disabilities. An initial scale of 20 items was constructed by consensus. A total of 1195 family members answered the questionnaire. In order to assess the structure of the scale, EFA recommended deleting 3 overlapping items. The final scale consisted of 17 items (α=95) and was composed of four main dimensions: self-determination (SD), social inclusion (SI), rights (RI) and overall improvement (OI), which explained 74.83% of the variance. Finally, the consistency and validity were assessed. Convergent validity and discriminant validity were satisfactory. Moreover, CFA confirmed the structure of the scale. Main conclusions, limitations and practical implications are discussed.

Monitoring the quality of an Italian public psychiatric service: A four dimensions study of the outcome

Objective: Within the concept of the outcome of hea1th services, the user's satisfaction has to be integrated by other more objective measures of health and quality of life. Debra Srebnik and coworkers have proposed a Survey for Monitoring the Quality of Public Mental Hea1th Services (SMQPMHS), which covers the following dimensions: Satisfaction, Functioning, Quality of Life and Clinica1 Status. The research main goals were as follows: a) to study the psychometric properties of the Ita1ian version of the Survey, and b) to study possible differences between the outcome of psychiatric patients applying to our outpatient facilities vs. the American sample. Methods: The Italian version of SMQPMHS was proposed to all patients receiving care at 2 Italian outpatient psychiatric facilities over a period of 2 months. 291 subjects accepted to participate in the study and fi1led in adequately the questionnaire. Results: Mean scores of the 13 variables of the Survey were very similar to those observed in the Arnerican sample. A matrix of correlations between each variable and each of the others indicated adequate internal consistency. A principal component analysis supported the four-dimensions model of the Survey. Patients recently hospitalized showed a poorer Clinical Status and a lower Satisfaction. Patients unemployed at the time of the survey showed a poorer Clinical Status.
