É sabido que o paciente com doença renal crônica que tem um acompanhamento nefrológico precoce apresenta melhores condições clínicas e melhor prognóstico ao entrar em diálise. Porém o acompanhamento precoce pelo nefrologista melhora também o enfrentamento psíquico da doença e do tratamento? Foram estudados 39 pacientes com insuficiência renal estágio 5 no momento em que entravam em hemodiálise: 19 pacientes com acompanhamento por nefrologista 6 meses (Grupo 1) e 20 pacientes sem qualquer acompanhamento nefrológico prévio (Grupo 2). Todos os pacientes participaram de entrevista semiestruturada, composta por 17 questões abordando informações, crenças, expectativas e fantasias relacionadas à doença e ao tratamento. As respostas foram decompostas em categorias através da técnica da Análise de Conteúdo. Os dados demográficos e laboratoriais também foram coletados. A classe socioeconômica foi avaliada e classificada como baixa, média ou alta. Na análise estatística foram utilizados o teste de Fisher, do Quiquadrado, t de Student ou de Wilcoxon como apropriado e os resultados apresentados como média ± DP. O proposito deste estudo e avaliar o impacto psicologico da vivencia hospitalar de individuos que sofrem complicacoes pos-operatoria, partindo do pressuposto de que a intercorrencia agrava o quadro clinico com repercussoes na esfera psiquica destes pacientes. Teoricamente enfoca a evolucao do conceito de trauma na teoria freudiana, partindo do desamparo primordial (hilflosigkeit) ate a nova concepcao de angustia, levando em consideracao o fator economico, a nocao de a posteriori (nachträglichkeit) e a compulsao a repeticao. OBJECTIVES--To assess the psychological impact of first episode of genital herpes, and to determine whether this changes over time. SETTING AND SUBJECTS--The Departments of Genitourinary Medicine (GUM), and Dermatology, Middlesex Hospital London. The study group consisted of patients attending the department of GUM with a clinically proven first episode of genital herpes. Two control groups were recruited; firstly patients without herpes attending the GUM Department and secondly patients attending the Dermatology Department out patients with chronic dermatoses. METHODS--Patients and controls completed an 87 item, self-administered psychological questionnaire at 3 monthly intervals for a year. The questionnaire consisted of the General Health Questionnaire (GHQ); the Hospital Anxiety and Depression Questionnaire (HADQ); Illness Attitude Scales and Illness Concern. Patients were also asked questions about their sexual behaviour. RESULTS--Ninety one patients (68 women, 23 men) with genital herpes, 61 GUM controls (42 women, 19 men) and 56 dermatology controls (36 women, 20 men) participated. There were no statistically significant demographic differences between patients and controls. At first visit the proportion of patients classified as "cases" by the GHQ (GHQ cases) were similar for primary herpes patients 62% (56/91) and Dermatology controls 52% (29/56) while a significantly smaller proportion of GUM controls 34% (21/61) were classified as GHQ cases. The primary herpes group were significantly more concerned about their illness than either the GUM controls or the Dermatology controls (p < 0.002). The proportion of primary herpes patients classified as "cases" by the GHQ reduced significantly over the initial three month period with 67% of patients classified as "cases" at their first visit becoming "noncases" after three months (p < 0.0001). Also 50% of those classified as "cases" at first visit by the HADQ become "noncases" after the initial three months (p = 0.007). This paper describes the development and psychometric properties of a new scale for assessing the psychological impact of genetic susceptibility testing for Alzheimer’s disease (AD). The new instrument, The REVEAL Impact of Genetic Testing for Alzheimer’s disease (IGT-AD) was designed to examine the unique nature of genetic information and the disease course of AD. The scale was tested as a part of a multicenter clinical trial designed to evaluate the impact of AD risk assessment and data was collected from 276 participants in the study. Using an iterative process of Principal Component Analysis and Cronbach’s alpha, the final 16 item IGT-AD was found to have a two factor structure with excellent internal reliability. Construct validity was established by patterns of correlation with other standardized self-reported measures. This scale should be useful in the identification of patients who maybe susceptible to the negative effects of receiving genetic information, monitoring of patients who have received genetic information, and as a tool for researchers who wish to study the effects of genetic susceptibility testing for AD.

An increasing number of patients with colorectal cancer (CRC) receive genetic counselling within 1 year after diagnosis. Little is known whether specific subgroups are more vulnerable for genetic testing related distress. A literature review was conducted to identify the psychological impact of CRC in the first year, and the additional impact of genetic testing. The electronic databases of PubMed, PsychInfo, Embase and the Cochrane Library were searched to identify all reports published between January 1997 and October 2007 on the psychological impact of (1) CRC-diagnosis up to 1 year after treatment and of (2) genetic testing for Lynch syndrome in patients with CRC. Studies on the psychological impact of genetic testing in newly diagnosed patient with CRC were not available. Either CRC patients diagnosed several years ago were studied and the focus was also often on the psychological impact of genetic testing prior to DNA-test disclosure. They show that limitations in emotional and social functioning can persist up to 1 year after CRC treatment, especially in those with a stoma or diagnosed before age 60. Female patients and male patients diagnosed before age 50 appear to be more vulnerable to genetic test-related distress. It is well known that being treated for CRC has great impact on psychological functioning. Little is known about the psychological impact during the first year after diagnosis and very little is known about the additional psychological effect of genetic testing for hereditary cancer in this period. We found presumptive evidence that specific subgroups of patients with CRC are more vulnerable for genetic-testing-related distress.

This study examines a cohort of persons quarantined during the 2003 SARS outbreak in Canada and describes their understanding of, difficulties and compliance with, and the psychological impact of the quarantine experience. A mailed questionnaire was administered to 1912 eligible adults and included the Impact of Events Scale – Revised (IES-R) to assess symptoms of post-traumatic stress disorder (PTSD). Self-reported compliance with all required quarantine measures was low (15·8±2·3%), although significantly higher when the rationale for quarantine was understood (P=0·018). Health-care workers (HCW) experienced greater psychological distress, including symptoms of PTSD (P<0·001). Increasing perceived difficulty with compliance, HCW, longer quarantine and compliance with quarantine requirements were significant contributors to higher IES-R scores. The low compliance with quarantine requirements introduces concerns about the effectiveness of quarantine as a public health measure. Improvements in compliance and reduced psychological distress may be possible by minimizing duration, revising requirements, and providing enhanced education and support.

Introduction and Objectives: The objectives were to assess the relationship between perceived smile aesthetics and perceived psychological impact as measured by the Psychosocial Impact of Dental Aesthetics Questionnaire (PIDAQ), and their own perception of it using the Aesthetic Component of the Index of Orthodontic Treatment Need (IOTN-AC) and a Visual Analog Scale (VAS); relate the IOTN-AC and VAS to the PIDAQ; and study the predictive capacity of the scales for psychological impact. Material and Methods: A cross-sectional study was conducted in 447 college students in Spain and Portugal (average age 20.4 years, 33.1% men and 66.9% women). The online self-completed surveys used the recently-validated Spanish and Portuguese versions of the PIDAQ to assess the self–reported psychological impact of the students’ dental aesthetics and IOTN-AC and an ad hoc 100 mm VAS for their perception of their dental aesthetics. Results: PIDAQ was linearly correlated with IOTN AC and VAS. Pearson’s coefficient was 0.55 for PIDAQ and IOTN-AC (CI 95% 0.48-0.61) and -0.72 for PIDAQ and VAS (CI 95% -0.66 - -0.76). VAS and IOTN-AC were predictive variables in a linear regression model of the total PIDAQ score. This research provides the first support for a possible psychological universal: Human beings around the world derive emotional benefits from using their financial resources to help others (prosocial spending). In Study 1, survey data from 136 countries were examined and showed that prosocial spending is associated with greater happiness around the world, in poor and rich countries alike. To test for causality, in Studies 2a and 2b, we used experimental methodology, demonstrating that recalling a past instance of prosocial spending has a causal impact on happiness across countries that differ greatly in terms of wealth(Canada, Uganda, and India). Finally, in Study 3, participants in Canada and South Africa randomly assigned to buy items for charity reported higher levels of positive affect than participants assigned to buy the same items for themselves, even when this prosocial spending did not provide an opportunity to build or strengthen social ties. Our findings suggest that the reward experienced from helping others may be deeply ingrained in human nature, emerging in diverse cultural and economic contexts.

This paper examines the psychological effect of orphanhood in a case study of 193 children in Rakai district of Uganda. Studies on orphaned children have not examined the psychological impact. Adopting parents and schools have not provided the emotional support these children often need. Most adopting parents lack information on the problem and are therefore unable to offer emotional support; and school teachers do not know how to identify psychological and social problems and consequently fail to offer individual and group attention.

The concept of the locus of control is used to show the relationship between the environment and individuals’ assessment of their ability to deal with it and to adjust behaviour. Most orphans risk powerful cumulative and often negative effects as a result of parents’ death, thus becoming vulnerable and predisposed to physical and psychological risks.

The children were capable of distinguishing between their quality of life when their parents were alive and well, when they became sick, and when they eventually died. Most children lost hope when it became clear that their parents were sick, they also felt sad and helpless. When they were adopted, many of them felt angry and depressed. Children living with widowed fathers and those living on their own were significantly more depressed. These children were also more externally oriented than those who lived with their widowed mothers.

Teachers need to be retrained in diagnosing psycho-social problems and given skills to deal with them. Short courses should be organized for guardians and community development workers in problem identification and counselling.; no

Les agressions sexuelles envers les enfants sont un fléau social important. Elles portent atteinte à l’intégrité physique et psychologique des enfants qui en sont victimes, entraînant des conséquences délétères pour l’enfant et ses parents. Ces derniers sont d’ailleurs considérés comme des personnes clés dans le rétablissement des enfants. Toutefois, les connaissances scientifiques dans ce domaine ont été principalement acquises auprès des mères, reléguant ainsi le père à une place secondaire. Cette thèse a pour objectif général d’élargir notre compréhension du vécu et du rôle des pères ayant à faire face au dévoilement de l’agression sexuelle de leur enfant. La présente thèse est composée de deux articles empiriques. L’objectif du premier article est de décrire le vécu des pères non agresseurs ayant un ou plusieurs enfants victimes d’agression sexuelle extra ou intrafamiliale sous l’angle de leurs réactions émotionnelles, cognitives et comportementales à la suite du dévoilement de l’AS. Pour ce faire, 17 pères provenant soit du Centre d’Expertise Marie-Vincent de Montréal, soit du Centre d’Intervention en Abus Sexuels pour la Famille de Gatineau ou encore soit de la population générale et ayant un ou plusieurs enfants victimes d’agression sexuelle intra ou extra-familiale ont été rencontrés pour une entrevue individuelle semi-dirigée. Este estudo avaliou o impacto psicológico de um grupo educativo de cirurgia cardíaca em pacientes de um hospital universitário. Participaram 73 sujeitos, dos quais 62% eram homens. A média de idade foi 52 anos, desvio-padrão foi 16. Os pacientes foram submetidos a uma sessão multidisciplinar com exposição dialogada sobre cardiopatias, procedimento cirúrgico e sintomas psicológicos. Os resultados mostraram que, para poucos pacientes (36%), o grupo ajudou a esclarecer dúvidas sobre o tratamento, demonstrando a necessidade de ajustes nas estratégias didáticas para melhor compreensão dos conteúdos. A análise das emoções percebidas após participação no programa revelou que 70% responderam estar esperançosos. A contraposição entre "esperança" e "tranquilidade" foi uma resposta emocional esperada frente à vivência da cirurgia, avaliada como uma condição de ameaça ambiental. Conclui-se que o grupo educativo foi benéfico para o ajustamento psicossocial do paciente à cirurgia. Genetic testing has been available for Huntington's disease for longer than any other adult onset genetic disorder. The discovery of the genetic mutation causing Huntington's disease made possible the use of predictive testing to identify currently unaffected carriers. Concerns have been raised that predictive testing may lead to an increase in deaths by suicide among identified carriers, and these concerns set in motion research to assess the psychological impact of predictive testing for Huntington's disease. This review article provides an overview of the literature and draws implications for clinical practice. About 10%-20% of people at risk request testing when approached by registries or testing centres. Most of the evidence suggests that non-carriers and carriers differ significantly in terms of short term, but not long term, general psychological distress. Adjustment to results was found to depend more on psychological adjustment before testing than the testing result itself. Although risk factors for psychological sequelae have been identified, few adverse events have been described and no obvious contraindications for testing people at risk have been identified. Dissertação de mestrado em Direitos Humanos; Com o desenvolvimento de um debate cada vez mais preocupado em torno do tráfico de seres humanos, por influência de instâncias transnacionais como a Organização das Nações Unidas (ONU), nomeadamente através do Protocolo de Palermo, começou a emergir, a partir de finais da década de 90, um novo discurso social, jurídico e político que define o tráfico de pessoas como uma violação dos direitos humanos à escala mundial. Muitas vezes chamada de nova forma de escravatura, é hoje comummente aceite que o tráfico de seres humanos deve ser entendido como um crime da maior gravidade, na medida em que engloba um vasto conjunto de violações graves de direitos humanos básicos, como a vida, a liberdade e a integridade física e psicológica, com consequências terríveis para as vítimas. El propósito del estudio es explorar el impacto psicológico percibido de las lesiones deportivas en el ámbito deportivo y personal del deportista, estudiando su relación con respecto al bienestar psicológico y la ansiedad competitiva. Los participantes del estudio fueron 26 deportistas de edades comprendidas entre 12 y 18 años, de los cuales 17 eran gimnastas y 9 tenistas. Inicialmente, se registró el impacto psicológico de las lesiones producidas sobre la vida deportiva y personal del deportista a través de una escala analógica ad hoc. Al inicio de la temporada deportiva, se valoró el estado de bienestar general a través de la Escala de Bienestar Psicológico de Ryff y, al iniciar la temporada competitiva, próximos a una competición, se utilizó la Escala de Ansiedad Competitiva SAS-2. Los resultados muestran que el impacto psicológico de la lesión tiene una relación negativa con el ‘dominio del entorno’ como dimensión del bienestar psicológico en la ‘vida deportiva’ de los participantes ‘experimentados’. Crianças de rua do Sri Lanka, entre as idades de 5 e 11 anos, foram avaliadas psicologicamente quanto ao impacto da violência e de abuso, testando-se a quantidade de: agressãopercebida; habilidades de enfrentamento; e impacto do abuso. Os resultados indicaram que as crianças mais novas manifestaram mais agressão do que as mais velhas e que os meninos eram mais agressivos do que as meninas. Esta agressão ocorreu principalmente ante a condição de abuso sexual. As crianças mais velhas pareceram ser capazes de um enfrentamento melhor do que as menores sendo que, as meninas, em geral, estavam mais capacitadas para o enfrentamento do que os meninos. O impacto psicológico do abuso pareceu ser mais grave quando sob as formas de depressão e de pensamentos suicidas. Muitas crianças sentiam que o abusado se tornaria um abusador ou um criminoso. Abstract The participation of victims of crime in the judicial process can have many repercussions on their recovery. Many authors have interested themselves in the psychological impact that victims of violent criminal acts endure after their contact with the police and the criminal jn the subject seem to be ambiguous. Objective and method: The authors conducted a systematic review to examine how different variables related to the judicial and criminal process may have psychological effects on victims of violent criminal acts. The following words were utilized in the search within three databases: Crime victims, victims of crime, Mental health, well‐being, psycholog*, Criminal justice proceeding, criminal justice, court, police, criminal procedures. The systematic review included 14 articles. The quality of the studies was assessed with the critical list Critical Review Form – Quantitative Studies (Law et al., 1998). Results: Many studies suggest a significant and positive relationship between negative experiences within the criminal process and psychological distress for victims, namely for victims of sexual assault. They concluded that these victims were at risk for increased post‐traumatic stress symptoms and therefore face secondary victimisation when they participate in the judicial system.
