Na UTI Neonatal, além da exposição contínua a ruídos e luminosidade, o prematuro é submetido a inúmeros procedimentos diagnósticos e terapêuticos, os quais estão associados à dor. A aspiração endotraqueal é um procedimento freqüente e doloroso, causador de estresse no neonato. O principal objetivo deste estudo é avaliar os efeitos da contenção facilitada na resposta de estresse de neonatos prematuros durante a aspiração endotraqueal. Trata-se de um ensaio clínico controlado e aleatorizado, no qual participaram 36 recémnascidos prematuros da Unidade de Terapia Intensiva Neonatal do CAISM/UNICAMP. Os pacientes foram aleatoriamente distribuídos em dois grupos, controle e propósito. Os indivíduos do grupo propósito foram submetidos à aspiração endotraqueal com a utilização da contenção facilitada enquanto que os recém-nascidos do grupo controle foram tratados de acordo com o protocolo convencional. OBJECTIVE: to analyze the total Premature Infant Pain Profile scores of premature infants undergoing arterial puncture during music and 25% glucose interventions, and to assess their association with neonatal and therapeutic variables. METHOD: a randomized clinical trial with 80 premature infants; 24 in the Experimental Group 1 (music), 33 in the Experimental Group 2 (music and 25% glucose), 23 in the Positive Control Group (25% glucose). CONTEXT: Premature infant lung development may be affected by lung injuries during the first few weeks of life. Lung injuries have been associated with changes in lung mechanics. OBJECTIVE: To evaluate an association between lung mechanics and lung structural alterations in very low birth weight infants (birth weight less than 1500 g). DESIGN: A cross-sectional evaluation of pulmonary mechanics (lung compliance and lung resistance) and high resolution computed tomography of the chest at the time of discharge, in 86 very low birth weight infants born at Instituto Fernandes Figueira, a tertiary public healthcare institution in Rio de Janeiro, Brazil. Lung compliance and resistance were measured during quiet sleep. High resolution computed tomography was performed using Pro Speed-S equipment. MAIN MEASUREMENTS: Statistical analysis was performed by means of variance analysis (ANOVA/ Kruskal Wallis). The significance level was set at 0.05. RESULTS: Abnormal values for both lung compliance and lung resistance were found in 34 babies (43%), whereas 20 (23.3%) had normal values for both lung compliance and lung resistance. The mean lung compliance and lung resistance for the group were respectively 1.30 ml/cm H2O/kg and 63.7 cm H2O/l/s. CONTEXT AND OBJECTIVE: It is challenge to assess and treat pain in premature infants. The objective of this study was to compare the multidimensional pain assessment of preterm neonates subjected to an acute pain stimulus at 24 hours, 72 hours and seven days of life. DESIGN AND SETTING: Prospective cohort study, at Universidade Federal de São Paulo. METHODS: Eleven neonates with gestational age less than 37 weeks that needed venepuncture for blood collection were studied. The exclusion criteria were Apgar score < 7 at five minutes, presence of any central nervous system abnormality, and discharge or death before seven days of life. Venepuncture was performed in the dorsum of the hand, and the heart rate, oxygen saturation and pain scales [Neonatal Facial Coding System (NFCS), Neonatal Infant Pain Scale (NIPS), and Premature Infant Pain Profile (PIPP)] were assessed at 24 hours, 72 hours and 7 days of life. NFCS and NIPS were evaluated prior to procedure (Tpre), during venepuncture (T0), and two (T2) and five (T5) minutes after needle withdrawal. Brain abnormality in surviving premature infants is associated with an enormous amount of neurodevelopmental disability, manifested principally by cognitive, behavioral, attentional, and socialization deficits, most commonly with only relatively modest motor deficits. The most recognized contributing neuropathology is cerebral white matter injury. The thesis of this review is that acquired cerebellar abnormality is a relatively less recognized but likely important cause of neurodevelopmental disability in small premature infants. The cerebellar disease may be primarily destructive (eg, hemorrhage, infarction) or primarily underdevelopment. The latter appears to be especially common and relates to a particular vulnerability of the cerebellum of the small premature infant. Central to this vulnerability are the extraordinarily rapid and complex developmental events occurring in the cerebellum. Although sensitive maternal behaviors improve later quality of mother-infant interaction and subsequently infant development, little is known regarding how an intervention might promote early premature infant social interactive behavior. This study compared the frequency of premature infant engagement and disengagement behaviors during two maternally administered interventions, the multi-sensory auditory, tactile, visual and vestibular intervention (ATVV) and kangaroo care (KC) for 26 infants between 31 and 46 weeks PMA. The ATVV intervention elicited more disengagement (M = 24 vs. 12, p = .0003), trended toward more engagement (M = 21 vs. 15.7, p = .06) and more potent engagement (M = 24 vs. 12, p = .0003), subtle disengagement (M = 25 vs. 11.9, p < .0001), and potent disengagement (M = 22.9 vs. 14, p = . 006) behaviors than did KC. The ATVV intervention may be an intervention to promote the infant’s learning how to regulate engagement and disengagement behaviors.

Necrotizing enterocolitis (NEC) is an inflammatory disease of the newborn bowel, primarily affecting premature infants. Early intestinal colonization has been implicated in the pathogenesis of NEC. The objective of this prospective case-control study was to evaluate differences in the intestinal microbiota between infants who developed NEC and unaffected controls prior to disease onset. We conducted longitudinal analysis of the 16S rRNA genes of 312 samples obtained from 12 NEC cases and 26 age-matched controls with a median frequency of 7 samples per subject and median sampling interval of 3 days. We found that the microbiome undergoes dynamic development during the first two months of life with day of life being the major factor contributing to the colonization process. Depending on when the infant was diagnosed with NEC (i.e. early vs. late onset), the pattern of microbial progression was different for cases and controls. The difference in the microbiota was most overt in early onset NEC cases and controls. In proximity to NEC onset, the abundances of Clostridium sensu stricto from Clostridia class were significantly higher in early onset NEC subjects comparing to controls. Background: Moderately premature infants, defined here as those born between 30 (frac{0}{7}) and 34 (frac{6}{7}) weeks gestation, comprise 3.9% of all births in the United States and 32% of all preterm births. While long-term outcomes for these infants are better than for less mature infants, morbidity and mortality are still substantially increased in comparison to infants born at term. There is an added survival benefit resulting from birth at a tertiary neonatal care center, and although many of these infants require tertiary level care, delivery at lower level hospitals and subsequent neonatal transfer are still common. Objective: Our primary aim was to determine the impact of maternal characteristics and antenatal medical management on the early neonatal course of the moderately premature infant. The secondary aim was to create a clinical prediction rule to determine which infants require intubation and mechanical ventilation in the first 24 hours of life. Such a prediction rule could inform the decision to transfer maternal-fetal patients prior to delivery to a facility with a Level III Neonatal Intensive Care Unit (NICU), where optimal care could be provided without the requirement for a neonatal transfer. OBJECTIVE: To investigate the impact on mothers and families of caring for a premature infant receiving home oxygen therapy (HOT). METHODS: Standard questionnaires were used to compare the functioning of mothers and families of 10 premature infants receiving HOT, 10 premature infants who were discharged from hospital on HOT but who no longer required it and 20 premature infants who had never required treatment with HOT. RESULTS: After adjustment for gestational age, chronological age, birthweight and place of residence (urban/rural), the care required by premature infants receiving HOT had a significantly greater impact on their families than the care of infants not receiving HOT. Mothers of premature infants receiving HOT reported significantly less vitality and more mental health problems than mothers of infants not receiving HOT. CONCLUSIONS: The use of HOT for premature infants may have a significant adverse impact on their mothers and families.

OBJECTIVETo compare the total sleep time of premature infant in the presence or absence of reducing sensory and environmental stimuli in the neonatal unit.METHODLongitudinal study in a Neonatal Intermediate Care Unit of a public hospital in Sao Paulo. The sample consisted of 13 premature infants. We used polysomnograph and unstructured observation for data collection. We analyzed 240 and 1200 minutes corresponding to the periods of the presence and absence of environmental management, respectively. Data were compared in proportion to the total sleep time in the two moments proposed by the study.RESULTSThe total sleep time in periods without environmental management was on average 696.4 (± 112.1) minutes and with management 168.5 (± 27.9) minutes, proportionally premature infant slept an average of 70.2% during periods with no intervention and 58.0% without management (p=0.002).CONCLUSIONReducing stimulation and handling of premature infant environment periods was effective to provide greater total sleep time.

O estudo teve como objetivo identificar diagnósticos de enfermagem de prematuros assistidos em uma unidade de cuidado intermediário neonatal do interior de São Paulo, Brasil. Tratou-se de um estudo retrospectivo a partir de prontuários de 118 prematuros hospitalizados. Os diagnósticos de enfermagem mais frequentes foram: Privação do Sono (83,1%), Risco de Infecção (76,3%) e Processos Familiares Alterados (75,4%), pertencentes aos respectivos domínios da NANDA: atividade/repouso, segurança/proteção e relacionamentos de papel. Purpose: Observe whether there are differences in visual functions among premature infants with treated retinopathy of prematurity (ROP) in relation to preterm infants with ROP and spontaneous regression; and among these two groups with ROP and the control group without ROP. Methods: Crosssectional observational no blind study. Premature infants were born between 06/199206/2006 and were exam between 06/200912/2010; registered in data of Hospital de Olhos Sandalla Amin Ghanem; with gestational age less than or equal to 32 weeks and 1,599 g born weigh; without ROP and ROP stages II or III, in one of the eyes, with spontaneous regression or with treatment; at least three visits during the selection period at maximum 6 months in the first exam and minimum 4 years of age in reassessment (chronological age) were include. This qualitative study aims to identify and analyze the meanings the health team attributes to the parents' participation in the care of premature children hospitalized at a public hospital. Data were recorded and collected through semi-structured interviews performed with 23 professionals. The results show that parents' participation in the care of these children is still in an initial stage at the hospital's neonatal unit. However, there is interest from the health team to implement it because its importance is recognized in improving the clinical stability, the growth and development process of premature children. In addition to allowing for mother-child interaction and affective bonding, it prepares the mother for the child's discharge. The presence of the mother helps the nursing team by giving maternal care to the hospitalized child. On the other hand, the parents' presence interferes in the environment of the neonatal unit. It affects the work dynamics and creates insecurity among team workers, who feel supervised. Besides, there is concern regarding hospital infection. The overall purpose of this longitudinal exploratory study was to describe the interactive behaviors of American Indian mothers and their prematurely born infants. The first study examined behaviors of American Indian mothers and their premature infants when the infants were 3, 6, and 12 months of age corrected for prematurity as well as the effects of infant illness severity (NBRS) and socioeconomic status (maternal education) on these behaviors. Higher infant illness severity was associated with more maternal touch, more infant vocalization, and fewer opportunities for variety in daily stimulation. Higher maternal education was associated with more maternal talk; more infant vocalization; less infant touch; and higher scores on the provision of appropriate play materials, parental involvement with the child, and opportunities for variety in daily stimulation subscales of the HOME. OBJECTIVETo compare the total sleep time of premature infant in the presence or absence of reducing sensory and environmental stimuli in the neonatal unit.METHODLongitudinal study in a Neonatal Intermediate Care Unit of a public hospital in Sao Paulo. The sample consisted of 13 premature infants. We used polysomnograph and unstructured observation for data collection. We analyzed 240 and 1200 minutes corresponding to the periods of the presence and absence of environmental management, respectively. Data were compared in proportion to the total sleep time in the two moments proposed by the study.RESULTSThe total sleep time in periods without environmental management was on average 696.4 (± 112.1) minutes and with management 168.5 (± 27.9) minutes, proportionally premature infant slept an average of 70.2% during periods with no intervention and 58.0% without management (p=0.002).CONCLUSIONReducing stimulation and handling of premature infant environment periods was effective to provide greater total sleep time.; OBJETIVOComparar o tempo total de sono dos recém-nascidos pré-termo na vigência ou não da redução de estímulos sensoriais e ambientais da unidade neonatal.MÉTODOEstudo longitudinal realizado em uma Unidade de Cuidados Intermediários Neonatal de hospital público de São Paulo. El estudio tuvo por objetivo comprender el motivo para la permanencia materna en el hospital durante la internación del hijo prematuro en Unidad de Terapia Intensiva Neonatal. Se efectuó con doce madres de recién nacidos prematuros, en un hospital maternidad municipal de Rio de Janeiro, Brasil, en 2007. Se adoptó como soporte metodológico la Fenomenología Sociológica de Alfred Schütz. La entrevista fenomenológica se utilizó para colectar el testimonio de las madres cuya acción intencional fue determinada a través de las siguientes categorías: Cuidar del hijo - enfrentando el desafío de tener un pequeño bebé; Estar cerca del hijo prematuro - la presencia materna contribuyendo a una más rápida recuperación; Ayuda recíproca entre las madres - la esperanza es reforzada cada día. Esse estudo tem como objetivo identificar e analisar os significados atribuídos pela equipe de saúde acerca da participação da mãe/pais no cuidado ao filho prematuro hospitalizado em um hospital público. Trata-se de um estudo com delineamento na abordagem qualitativa. Os dados foram coletados através de entrevista semi-estruturada e gravada de 23 profissionais. Os resultados mostraram que esta participação ainda é incipiente na unidade neonatal do hospital, mas há interesse da equipe de saúde em implementá-la, reconhecendo a sua importancia ao favorecer a estabilidade clínica do prematuro e seu processo de crescimento e desenvolvimento, possibilitar a interação mãe-filho e o estabelecimento do vínculo afetivo, bem como o treinamento materno para a alta do filho. Percebemos ainda, que a mãe ajuda a enfermagem nos cuidados do filho hospitalizado, executando cuidados de maternagem. Por outro lado, a presença dos pais modifica o ambiente da unidade neonatal, pois interfere com a dinâmica do trabalho, gera insegurança na equipe que se sente fiscalizada e há preocupação com as infecções hospitalares. Assim, esses significados em consonância com outros estudos em diferentes paises, nos levam refletir na necessidade de fundamentar a assistência ao prematuro em termos de construir coletivamente uma filosofía de cuidado que recupere conceitos sobre direitos humanos...

OBJETIVO: analizar los puntajes totales del Premature Infant Pain Profile de los prematuros sometidos a punción arterial bajo intervención de música y de glucosa al 25%, y verificar la asociación de estos con variables neonatales y terapéuticas. MÉTODO: ensayo clínico aleatorio con 80 prematuros: 24 Grupo Experimental 1 (música), 33 Grupo Experimental 2 (música y glucosa al 25%) y 23 Grupo Control Positivo (glucosa al 25%). Todos los prematuros fueron filmados y una canción de cuna fue escuchada por 10 minutos antes de la punción en el Grupo Experimental 1 y en el Grupo Experimental 2, glucosa al 25% administrada en el Grupo Experimental 2 y en el Grupo Control Positivo, dos minutos antes de la punción. RESULTADOS: 60,0% de los prematuros presentaron dolor moderado o intenso, los puntajes de dolor y los grupos de intervención no demostraron ser estadísticamente significativos.
