OBJECTIVE: The goal was to review published studies of analgesic effects of sweet solutions, to ascertain areas with sufficient evidence of effectiveness and areas of uncertainty. METHODS: Databases searched included Medline, Embase, the Cumulative Index to Nursing and Allied Health Literature database, and PsycINFO, using the terms pain*, infant*, neonat*, newborn*, sucrose, glucose, and alternative sugars. Publications were sorted according to type, year, painful procedure studied, placebo/no-treatment groups, population studied, and country of publication. RESULTS: A total of 298 relevant unique publications involving human infants were identified; 125 (42%) were primary research studies, of which 116 (93%) were randomized controlled trials. Healthy preterm or term newborns were included in 82 studies (65%), and sick or very low birth weight infants were included in 22 (18%). Most studies included single episodes of painful procedures, with only 3 (2%) conducted over long periods. Procedures investigated most frequently were heel lance (49%), venipuncture (14%), and intramuscular injection (14%). OBJECTIVES. The purpose of this study was to obtain data on the association of antiphospholipid antibodies with clinical manifestations in childhood and to enable future studies to determine the impact of treatment and long-term outcome of pediatric antiphospholipid syndrome. PATIENTS AND METHODS. A European registry extended internationally of pediatric patients with antiphospholipid syndrome was established as a collaborative project of the European Antiphospholipid Antibodies Forum and Lupus Working Group of the Pediatric Rheumatology European Society. To be eligible for enrollment the patient must meet the preliminary criteria for the classification of pediatric antiphospholipid syndrome and the onset of antiphospholipid syndrome must have occurred before the patient`s 18th birthday. RESULTS. As of December 1, 2007, there were 121 confirmed antiphospholipid syndrome cases registered from 14 countries. Fifty-six patients were male, and 65 were female, with a mean age at the onset of antiphospholipid syndrome of 10.7 years. Sixty (49.5%) patients had underlying autoimmune disease. Venous thrombosis occurred in 72 (60%), arterial thrombosis in 39 (32%), small-vessel thrombosis in 7 (6%), and mixed arterial and venous thrombosis in 3 (2%). OBJECTIVE: HIV transmission has been associated with offering a child food prechewed by an HIV-infected caregiver. We assessed awareness of prechewing and oral prewarming of food by an adult before offering it to a child among HIV-infected pregnant women and clinical investigators in 3 Latin American countries. METHODS: HIV-infected pregnant women at 12 sites (Eunice Kennedy Shriver National Institute of Child Health and Human Development International Site Development Initiative Perinatal Longitudinal Study in Latin American Countries, a prospective cohort trial) in Argentina, Brazil, and Peru were administered a screening survey about prechewing/prewarming of infant foods and cautioned against these feeding practices. Survey responses were analyzed, overall, and stratified according to country. RESULTS: Of the 401 HIV-infected pregnant women interviewed, 34% had heard about prechewing (50% from Argentina, 32% from Brazil, and 36% from Peru), 23% knew someone who prechewed food for infants, and 4% had prechewed food in the past. Seventeen percent had heard about oral prewarming of food, 13% knew someone who prewarmed food for infants, and 3% had prewarmed food for an infant in the past. Women who reported knowing someone who prechewed were more likely to also know someone who prewarmed food (P < .0001). OBJECTIVE: The study goal was to compare the efficacy of expressed breast milk (EBM) versus 25% glucose on pain responses of late preterm infants during heel lancing. METHODS: In a noninferiority randomized controlled trial, a total of 113 newborns were randomized to receive EBM (experimental group [EG]) or 25% glucose (control group [CG]) before undergoing heel lancing. The primary outcome was pain intensity (Premature Infant Pain Profile [PIPP]) and a 10% noninferiority margin was established. Secondary outcomes were incidence of cry and percentage of time spent crying and adverse events. Intention-to-treat (ITT) analysis was used. RESULTS: Groups were similar regarding demographics and clinical characteristics, except for birth weight and weight at data collection day. There were lower pain scores in the CG over 3 minutes after lancing (P<.001). A higher number of infants in the CG had PIPP scores indicative of minimal pain or absence of pain (P = .002 and P = .003 on ITT analysis) at 30 seconds after lancing, and the mean difference in PIPP scores was 3 (95% confidence interval: 1.507-4.483). A noção de integralidade engloba três conjuntos de sentidos: a prática dos profissionais de saúde, os atributos da organização do serviço e as respostas governamentais aos problemas de saúde. Este estudo teve como objetivo analisar e discutir os sentidos relacionados à integralidade nas ações da equipe de saúde de uma unidade de internação pediátrica de um hospital universitário no interior do estado do Rio Grande do Sul. Trata-se de uma pesquisa qualitativa cujos dados foram coletados a partir de entrevistas e analisados mediante análise temática. A integralidade na prática dos profissionais está centrada na dimensão relativa à atuação profissional, em que eles buscam associar assistência e prevenção, vislumbrando a criança hospitalizada no seu contexto econômico e social. Introdução e objetivo: A cetoacidose diabética (CAD) é a principal causa de hospitalização e morte em crianças com diabetes mellitus tipo 1 (DM1). A mortalidade está principalmente relacionada ao edema cerebral, considerado uma complicação do tratamento. O objetivo desse estudo foi avaliar as características dos pacientes com CAD tratados na Pediatria do Hospital de Clínicas da UNICAMP. Métodos: Estudo retrospectivo e descritivo de variáveis clínicas e laboratoriais de 74 internações por CAD no período de janeiro de 1994 a dezembro de 2003. Resultados: As 74 internações corresponderam a 49 pacientes. Doze pacientes tiveram mais de uma internação, 27 (55%) eram do sexo feminino e a idade variou de 0,9 a 14,5 anos. O tempo médio de DM1 foi de 3 ± 3,1 anos, sendo em 20 casos a primeira manifestação do DM1. Quando comparados o número de internações de pacientes já com diagnóstico de DM1 com o de pacientes com primeira descompensação em relação ao ano de internação, houve diferença a partir do ano de 2000, quando houve predomínio de internações por primeira descompensação. Quando realizada a mesma comparação com relação à idade, houve predomínio de internações por primeira descompensação em pacientes menores de 9 anos. Vaccination of health care workers is an efficient way to reduce the risk of occupational infection and to prevent nosocomial transmission to vulnerable patients. Despite this, achieving high immunization rates among those professionals is a challenge. We assessed the immunization status of Residents in Pediatrics at the Federal University of São Paulo from June to December 2008. Their immunization records were checked and evaluated according to the Brazilian Immunization Schedule for health care workers. Considering all required vaccines, only 3.1% of the 64 Residents were up-to-date with their immunizations. Influenza was the vaccine with the lowest uptake (3.1%) and measles and rubella were diseases with the highest evidence of immunity (62.5% each). Only 37.5% of Residents had received three hepatitis B vaccine doses with a subsequent serology confirming seroconversion. Moreover, the vast majority of Residents in Pediatrics who were not up-to-date were unaware of the fact. Both medical schools and Pediatric Residence programs should not only offer information but also check vaccination records in an effort to keep their healthcare workers´ vaccinations up-to-date.

OBJECTIVES: to evaluate attitudes in childcare and primary care in pediatrics, as well as aspects of training and medical education for residents in pediatrics consistent with the current medical paradigm. METHODS: the subjects were 133 residents of six pediatrics residency services in the city of Porto Alegre, RS. A cross-sectional, descriptive study was performed consisting of a questionnaire and an attitude measurement scale (Likert scale). Thirty propositions related to the issues of child care and primary care in pediatrics were simultaneously submitted to small groups of residents at their residency settings. RESULTS: women outnumbered men in a ratio of three to one in the residency programs. The majority aimed at a specialty in pediatrics, principally the ones involving high-tech procedures. Choices were influenced by prior training and by professors or tutors, although they stated they would like to work in prevention in the future. As for the scale, 87% had the right attitudes, both positive and negative, according to the Golden standard applied. CONCLUSIONS: the result obtained in this study related to attitude was very satisfactory but future professional choices are a concern to the medical educational system. The authors suggest that studies on attitude be further developed and improved to become a source of input for new strategies in the area of pediatrics education.

This technical report discusses recent advances in electronic prescribing (e-prescribing) systems, including the evidence base supporting their limitations and potential benefits. Specifically, this report acknowledges that there are limited but positive pediatric data supporting the role of e-prescribing in mitigating medication errors, improving communication with dispensing pharmacists, and improving medication adherence. On the basis of these data and on the basis of federal statutes that provide incentives for the use of e-prescribing systems, the American Academy of Pediatrics recommends the adoption of e-prescribing systems with pediatric functionality. This report supports the accompanying policy statement from the American Academy of Pediatrics recommending the adoption of e-prescribing by pediatric health care providers.

The most common adult chronic diseases affect 1 in 3 adults and account for more than three-quarters of US health care spending. The major childhood drivers of adult disease are distinctly nonmedical: poverty, poor educational outcomes, unhealthy social and physical environments, and unhealthy lifestyle choices. Ideally, well-child care (WCC) would address these drivers and help create healthier adults with more productive lives and lower health care costs. For children without serious acute and chronic medical problems, however, traditional pediatric preventive services may be largely ineffective in addressing the outcomes that really matter; that is, improving lifelong health and reducing the burden of adult chronic disease. In this article, we examine what role WCC has in addressing the major childhood drivers of adult disease and consider various models for the future of WCC within pediatrics.

Discussions of “big data” in medicine often revolve around gene sequencing and biosamples. It is perhaps less recognized that administrative data in the form of vital records, hospital discharge abstracts, insurance claims, and other routinely collected data also offer the potential for using information from hundreds of thousands, if not millions, of people to answer important questions. However, the increasing ease with which such data may be used and reused has increased concerns about privacy and informed consent. Addressing these concerns without creating insurmountable barriers to the use of such data for research is essential if we are to avoid a “missed opportunity” in pediatrics research.

This study deals with a historical, descriptive and exploratory approach aiming to recall the origin and trajectory of the Pediatrics Hospital at Universidade Federal do Rio Grande do Norte. This research also deals with the insertion of the nursing department in the same Hospital. This was realized through existing records and discourse collected through interview of professionals-doctors, nurses, midwifes, nursing attendants and psychologists. Thus, a network was established and consisted of qualified informants, composed through reference analysis. Data treatment and analysis was performed based on the collection of oral data. The data was considered according to the font s context, all of which depending on process of comprehension and interpretation. The research was based on the main theme, through oral history used in order to build a historical background. These main themes were then subdivided and other discourses were made present such as: the historical scene, the dream came true and the insertion of nursing, present in the history construction; all of which enabled the research. Thus, in this process, it was possible to identify the most important characters of the origin of children s health services organization and assistance at Rio Grande do Norte. Introdução: A literatura nacional e internacional oferece parâmetros para dimensionar pessoal de enfermagem em unidades de pacientes adultos. Contudo, existe uma lacuna de conhecimento no que se refere à proposição de indicadores de tempo de cuidado para atender os pacientes em unidades de assistência pediátrica, o que dificulta a operacionalização dos métodos de dimensionamento nessa área. Objetivo: Identificar o tempo médio de assistência de enfermagem, por paciente, nas 24 horas, utilizado para assistir o paciente pediátrico internado na área de Terapia Intensiva Pediátrica (UTIP) da Unidade de Terapia Intensiva Pediátrica e Neonatal (UTIPN) do Hospital Universitário da Universidade de São Paulo (HU-USP), segundo o grau de dependência da equipe de enfermagem. Método: Estudo descritivo, observacional, transversal, de natureza quantitativa, realizado na UTIP da UTIPN do HU-USP. Participaram da pesquisa todos os profissionais de enfermagem que trabalharam na UTIP e todos os pacientes internados, no período de 03 a 09 de novembro de 2014. CONTEXT AND OBJECTIVE: Infant hearing deficiency is a human disorder with devastating effects and serious implications for the development of speech and language. Early diagnosis of hearing loss should be the objective of a multidisciplinary team, and early-intervention programs should immediately follow this. The aim of this study was to investigate the knowledge and conduct of pediatricians and pediatric residents in a tertiary teaching hospital regarding deafness. DESIGN AND SETTING: Cross-sectional study in a tertiary hospital in the state of São Paulo, Brazil. METHODS: Eighty-eight questionnaires were randomly distributed to pediatricians and pediatric residents. RESULTS: Thirty-six questionnaires were analyzed. Most respondents (61.1%) were residents in pediatrics and/or neonatology. Eighty-three percent of them performed special procedures on babies presenting a high risk of deafness, and 55% reported that they had no knowledge of techniques for screening hearing. Most of them were unaware of the classifications of level and type of hearing loss. According to 47.2% of them, infants could begin to use a hearing aid at six months of age. OBJECTIVE: To evaluate the profile and the scientific production of researchers in Pediatrics with scholarship from the National Counsel of Technological and Scientific Development. METHODS: The Lattes curricula of 34 researchers in Pediatrics with active scholarships, from 2006 to 2008 were included in the analysis. The variables of interest were: gender, affiliation, time since PHD, tutoring of undergraduate students, mentorship of masters and doctors, scientific production and the papers' impact. RESULTS: In a total of 411 researchers in Medicine, 34 (8%) belonged to Pediatrics. Males (77%) and scholars in the category 2 of productivity (62%) prevailed. Three states of Brazil were responsible for approximately 90% of the researchers: São Paulo (53%), Minas Gerais (21%), and Rio Grande do Sul (15%). During their academic career, the Pediatrics researchers have published 3,122 articles with a median of 89 articles per researcher (interquartile range - IQ=51-119). Of the total, 40 and 59% articles were indexed in the Web of Science and Scopus databases, respectively. CONTEXT AND OBJECTIVE: Infant hearing deficiency is a human disorder with devastating effects and serious implications for the development of speech and language. Early diagnosis of hearing loss should be the objective of a multidisciplinary team, and early-intervention programs should immediately follow this. The aim of this study was to investigate the knowledge and conduct of pediatricians and pediatric residents in a tertiary teaching hospital regarding deafness. DESIGN AND SETTING: Cross-sectional study in a tertiary hospital in the state of São Paulo, Brazil. METHODS: Eighty-eight questionnaires were randomly distributed to pediatricians and pediatric residents. RESULTS: Thirty-six questionnaires were analyzed. Most respondents (61.1%) were residents in pediatrics and/or neonatology. Eighty-three percent of them performed special procedures on babies presenting a high risk of deafness, and 55% reported that they had no knowledge of techniques for screening hearing. Most of them were unaware of the classifications of level and type of hearing loss. According to 47.2% of them, infants could begin to use a hearing aid at six months of age. Recent changes in medical education have highlighted the importance of experiential learning. Simulation is one model that has gained significant attention in the last decade and has been widely adopted as a training and assessment tool in medical education. Pediatric simulation has been utilized to teach various skills including resuscitation and trauma management, procedural skills, and team training. It is also a valuable tool for health care educators, as it allows learners to achieve competence without putting patients at risk. Recent literature demonstrates increased retention of knowledge and skills after simulation-based training. Further research is required to improve current simulation curriculums, develop validated assessment tools, and to demonstrate improved clinical outcomes after simulation-based training. We conducted an online search of original and review articles related to simulation and pediatric medical education and provide an overview of the role and utility of simulation in pediatrics.

A vacinação de profissionais da saúde representa maneira eficiente de reduzir o risco ocupacional a infecções e de prevenir a transmissão nosocomial de doenças a pacientes vulneráveis. Apesar disso, atingir altas taxas de cobertura vacinal entre estes profissionais continua sendo um desafio. Avaliamos a situação vacinal dos residentes de Pediatria da Universidade Federal de São Paulo entre junho e dezembro de 2008. Suas carteiras de vacinação foram checadas e avaliadas de acordo com a orientação do calendário nacional para profissionais da saúde. Considerando as vacinas propostas, apenas 3,1% dos 64 residentes estavam em dia com sua imunização. A vacina para Influenza foi a mais negligenciada (3,1%) e sarampo e rubéola, as doenças com maior evidência de imunidade (62,5%). Apenas 37,5% dos residentes haviam recebido três doses da vacina para hepatite B e possuíam sorologia confirmando soroconversão. Além disso, a grande maioria dos residentes que estavam com atraso vacinal desconhecia este fato.
