The public sector is facing turbulent times and this also challenges the health professions who are expected to serve both the interests of the citizens and the cost-containment and austerity policies of governments. This article seeks to explore the changing role of the health professions. I introduce an approach on ‘citizen professionals’ as active players in the policy process and mediators between the state/policymakers and the citizens/patients. The aim is to highlight a transformative potential of professionalism and the connectedness with other sets of governance, like management. Empirical material from a German case study and a comparative European study serve to illustrate the arguments, drawing on policy analysis and secondary sources. The results bring the complexity of transformations and new emergent forms of professionalism into view that cannot be understood in traditional categories of conflict, exclusion and jurisdiction. An open-system approach to medical professionalism: a controversy within the sociology of professions

This article focuses on a specific debate within theories of professions. Despite different trends, there has been difficulty in theorizing some dimensions of the dominant perspectives because of predominant institutional attention. The aim here was to reinforce the debate with complementary conceptualization of professionalism and, consequently, to foster new understandings of professional relationships. The argument lies in shifting the analytical level from the professions to professionals, as a basic step in conceptualizing individual action within professional contexts. This concern becomes increasingly important as the professional discretion structurally held by individuals becomes greater, since the ability to choose leads more explicitly to internal differentiation of professions. Systemic influence is felt given that social structures are intrinsically involved in actions, which represent exteriorizations of individually internalized processes. Consequently, it becomes necessary to consider the reasons for behaviors and the meanings individually conferred on professional dimensions.

AIMS: To ascertain views about constraints on the progress of audit experienced by members of four of the therapy professions: physiotherapy, occupational therapy, speech and language therapy, and clinical psychology. METHODS: Interviews in six health service sites with a history of audit in these professions. 62 interviews were held with members of the four professions and 60 with other personnel with relevant involvement. Five main themes emerged as the constraints on progress: resources; expertise; relations between groups; organisational structures; and overall planning of audit activities. RESULTS: Concerns about resources focused on lack of time, insufficient finance, and lack of access to appropriate systems of information technology. Insufficient expertise was identified as a major constraint on progress. Guidance on designing instruments for collection of data was the main concern, but help with writing proposals, specifying and keeping to objectives, analysing data, and writing reports was also required. Certain U.S. racial and ethnic minority groups traditionally have been underrepresented in the health professions. There have been significantly smaller proportions of health professionals who are blacks, Hispanics, and American Indians than members of those groups in the population at large. In the 1960s and 1970s various pieces of legislation were passed that were intended to increase the total number of health professionals and address the underrepresentation of minorities and other disadvantaged persons. The Special Health Careers Opportunity Grant (SHCOG) Program and its successor, the Health Careers Opportunity Program (HCOP) have had, as their specific charge, responsibility for awarding funds to undergraduate colleges, health professions schools, and other entities to increase the number of minority students admitted to, and graduated from, health professions schools. Since 1972, during the existence of the SHCOG and HCOP Programs, the number of black students in health professions schools has risen from 4,099 to nearly 6,000. The number of Hispanic students has doubled to more than 2,360 and that of American Indian health professions students has increased from 125 to 355. Minority student applicants to medical school have improved their scores on the medical college admission test (MCAT) and their mean grade point average (GPA) at a higher rate than nonminority students. Students from HCOP-supported schools had higher acceptance rates--even with lower MCAT and GPA scores--than students from schools lacking HCOP support.

This paper compares the efforts of physiotherapy and chiropractic to move towards the development of a scientific body of knowledge through research. The history of both professions is discussed with particular reference to the education of the clinical practitioners and the educational faculty. Four cited factors contributing to the paucity of research in chiropractic, namely anti-scientific elements in philosophy, lack of suitable role-models, lack of funds and a lack of understanding of research methodology are also seen as barriers to physiotherapy research. These obstacles are discussed in the light of the strategies being employed by both professions to deal with this issue and their success or otherwise. It is stressed that research by members of both professions is fundamental to ensure professional survival. Solutions are principally concerned with increasing the research output from those institutions which educate future professionals. This new generation will likely ensure that the professions are soundly based on scientific principles. The paper considers, therefore, the pre-professional curriculum primarily, although not exclusively, from a Canadian perspective.

Understanding and applying nutrition knowledge and skills to all aspects of health care are extremely important, and all health care professions need basic training to effectively assess dietary intake and provide appropriate guidance, counseling, and treatment to their patients. With obesity rates at an all-time high and the increasing prevalence of diabetes projected to cost the Federal government billions of dollars, the need for interprofessional nutrition education is paramount. Physicians, physician assistants, nurses, nurse practitioners, pharmacists, dentists, dental hygienists, occupational therapists, physical therapists, speech and language pathologists, and others can positively affect patient care by synchronizing and reinforcing the importance of nutrition across all specialty areas. The Pathways for Students into Health Professions program is one of four nationally funded programs by the Maternal and Child Health Bureau of the US Department of Health and Human Services to support the training of undergraduate students, particularly from under-represented minority groups to pursue maternal and child health professions. To assess the program’s impact on student ratings, knowledge, and interest in maternal and child health professions. A baseline survey on student ratings and knowledge in maternal and child health topics and careers, public health topics, and career development topics was provided to 32 students at the beginning of their first year in the program and approximately 1 year after participation. Half of the students (16 students) in the program from 2009–2011 were from traditionally underrepresented minority groups. After participation, students reported significantly higher ratings of interest in maternal and child health topics and careers and in receiving adequate academic and career guidance. Im Zusammenhang damit gewinnt die Aufgabe des „Managements“ von Projekten an Bedeutung: Wie können kontingente projektförmige Arbeitsprozesse so geplant, kontrolliert und gesteuert werden, dass die vereinbarten Ziele, Termine und Kosten eingehalten werden? Mit dem Aufstieg und der Ausdifferenzierung des Projektmanagements als Dauerfunktion wird häufig die Erwartung verbunden, dass sich als Träger für die anspruchsvolle Methodik des Projektmanagements ein eigener Beruf entwickelt. Eine systematische Bearbeitung der Entwicklung des „Projektmanagement“ ist bisher weitgehend ausgeblieben. Diese Lücke schließt die vorliegende berufssoziologische Arbeit. Ce mémoire vise à retracer les carrières des escortes indépendantes montréalaises et les tensions qui les traversent, afin de rendre compte de la complexité du « drame social » que constitue cette activité. Nos résultats montrent que cette profession présente de nombreuses similarités avec d’autres professions, en même temps que sa position particulière dans une matrice sociale stigmatisante et dans une relation de service intime lui confère toute sa singularité. Partie de la question « Comment commence-t-on et poursuit-on dans l’activité d’escorte, alors que celle-ci est stigmatisée ? », nous avons réalisé une enquête de terrain auprès d’escortes indépendantes, composée essentiellement de sept entrevues approfondies et de l’observation de leur environnement professionnel informatisé. Nous avons décidé de nous écarter du débat actuel, tant scientifique que militant, qui divise sur le sujet du travail du sexe. Notre cadre conceptuel est, dans un perspective interactionniste, à la croisée des sociologies des professions, de la déviance et du stigmate. Le contexte socio-économique marqué par la division du travail favorise les fermetures de marché. La professionnalisation représente un type de fermetures par lequel un groupe occupationnel cherche à obtenir et à maintenir le contrôle de l'environnement interne et externe de son activité économique afin d'éviter que ce contrôle s'exerce exclusivement de l'extérieur. En maintenant le contrôle sur un champ de compétence ou sur une fermeture de marché, un groupe assure la maîtrise de sa survie professionnelle et socio-économique. La recherche de fermeture de marché est une explication possible des considérations sousjacentes à l'intérêt d'un groupe occupationnel pour la professionnalisation. Cette considération ne compromet pas nécessairement la mission de protection du public qui doit guider les ordres professionnels dans l'exercice de leurs pouvoirs de contrôle. En effet, le processus de légitimation favorise au contraire le respect de cette mission. Par ce processus, un groupe occupationnel maintient non seulement la reconnaissance sociale de sa compétence et de son utilité mais également l'exercice des pouvoirs de contrôle qui lui sont octroyés. La légitimité ainsi acquise permet de maintenir le degré de crédibilité nécessaire à la survie du groupe. Nesta revisão são discutidos os aspectos conceituais e princípios básicos para a construção de currículos para os cursos de gradaução das profissões da saúde. Serão abordados alguns modelos, fundamentos e passos essenciais para a construção de currículos, em paralelo ao contexto das Diretrizes Curriculares Nacionais. No planejamento e na organização dos currículos, é destacada a relevância do comprometimento do professor, aluno e equipe; do emprego de estratégias apropriadas no processo de ensino-aprendizagem e dos instrumentos de avaliação que possam contribuir para a revisão e melhorias do currículo proposto.; In this review we present the concepts and the basic principles for curriculum design in health professions education (HPE), and present some curriculum design models as well as the essential steps for those who are facing this challenge. All the discussion brings the perspective of National Curriculum Guidelines for HPE in Brazil. It is highlighted the importance of the stakeholders’ engagement in this process and the correct use of appropriate strategies of teaching-learning, students’ assessment tools that can contribute to the implementation/review and improvement of curriculum in HPE courses.

Este é um estudo sobre a história da formação de Educadoras Sanitárias e Enfermeiras de Saúde Pública na primeira metade do século 20, com destaque para acontecimentos em São Paulo e no Rio de Janeiro. A sociologia histórica proporciona um instrumental teórico-metodológico fundamental para análise e interpretação das relações entre instituições, poder e identidades profissionais e, nesse sentido, permite o estudo do processo de demarcação de um território de decisões e de atuação feminina que não fosse marcado pela subalternidade à profissão médica. O presente estudo se apóia na literatura histórico-sociológica que discute o movimento de configuração do campo da saúde pública iniciado nos anos de 1920 e estreitamente ligado aos debates sobre a nacionalidade e a constituição de identidades profissionais no campo do Cuidar. A década de 1920 instaurou novas práticas e concepções na relação entre Estado e sociedade, acentuando-se a especialização em saúde pública ou higiene. INTRODUCTION: The Health Professions Council of South Africa (HPCSA) monitors compulsory continuing professional development (CPD) to ensure that healthcare professionals update their knowledge and skills to the benefit of their clients. One of the objectives of the Clinical Skills Unit (CSU) in the School for Allied Health Professions at the University of the Free State (UFS) was to use it for CPD as part of a responsibility towards alumni. This study investigated the needs of qualified dieticians (DT), occupational therapists (OT) and physiotherapists (PT) to assist in the provision of relevant CPD activities at the Unit. METHODS: A descriptive, comparative study was conducted. Two hundred and fifty-eight questionnaires were distributed in the Free State and Northern Cape provinces, to members of the professional groups mentioned above. To ensure reliability, 10% of the sample was re-tested after one month. RESULTS: One hundred and twenty-seven professionals responded to the questionnaire, with 55.6% being from the Free State. CPD activities were attended in Bloemfontein by 65.9%. Most of the CPD activities (70.6%) attended prior to the study were theoretical in nature and 85% of respondents would have prefered to observe experts and learn by doing. This article reports on the development of a staff development framework for the School for Allied Health Professions (SAHP) at the University of the Free State. The aim of compiling such a framework was to create a structure to serve as a master plan for staff development planning. A comprehensive literature study was done to identify the factors that influence staff development at Higher Education Institutions. A questionnaire was used to capture staff perceptions of skill development. Indicators from the literature as well as data from the completed questionnaires determined which components were included in the framework. The framework consists of three phases namely, planning, operational and evaluation phases. Each phase showed the complex interaction of factors that should be taken into account. The framework can serve as basis for the development of own modules for staff development within the different departments in the SAHP as well as other departments within Allied Health Professions in South Africa.

This article focuses on a specific debate within theories of professions. Despite different trends, there has been difficulty in theorizing some dimensions of the dominant perspectives because of predominant institutional attention. The aim here was to reinforce the debate with complementary conceptualization of professionalism and, consequently, to foster new understandings of professional relationships. The argument lies in shifting the analytical level from the professions to professionals, as a basic step in conceptualizing individual action within professional contexts. This concern becomes increasingly important as the professional discretion structurally held by individuals becomes greater, since the ability to choose leads more explicitly to internal differentiation of professions. Systemic influence is felt given that social structures are intrinsically involved in actions, which represent exteriorizations of individually internalized processes. Consequently, it becomes necessary to consider the reasons for behaviors and the meanings individually conferred on professional dimensions.
