No Brasil, como em outros países, podem ser encontrados milhões de trabalhadores com distúrbios e problemas resultantes da exposição ocupacional a vibrações mecânicas. No entanto, são escassos, os estudos e as pesquisas voltados a esse agente, seja com relação ao seu reconhecimento, à sua avaliação ou ao seu controle. Neste trabalho estimou-se a exposição ocupacional a vibrações em mãos e braços dos trabalhadores que atuam nas atividades de acabamento, em marmorarias no município de São Paulo. Os principais critérios técnicos internacionais relacionados ao agente, sua aplicação e interpretação são discutidos frente à legislação nacional. Para a obtenção de um sinal de vibração representativo da exposição dos trabalhadores, utilizou-se um procedimento alternativo de medição, mediante uso de um mini-acelerômetro triaxial e um adaptador de palma fixado diretamente nas mãos dos operadores. Dessa forma, procurou-se contabilizar de modo mais representativo as diversas posturas, forças e posições aplicadas durante a execução das atividades. Objetivo Avaliar o perfil da exposição ocupacional a agentes químicos, em laboratórios, e verificar os impactos na avaliação de risco decorrentes das mudanças nos limites de exposição ocupacional (LEO) e na legislação federal brasileira. Introdução: O câncer de mama é o segundo câncer mais incidente no mundo e o mais freqüente entre as mulheres, entretanto, sua etiologia continua desconhecida. O câncer de mama está associado a múltiplos fatores de risco, dentre eles, os fatores ocupacionais e ambientais. Objetivo: Fazer revisão bibliográfica sobre câncer de mama e exposição ocupacional e ambiental a solventes. Método: Artigos em inglês, português e espanhol foram selecionados no Pubmed e no Latin American and Caribbean Center on Health Sciences Information LILACS entre dezembro de 1981 a março de 2011. Os termos utilizados foram: breast cancer, breast neoplasm, solvent, occupational exposure and environment exposure. Foram excluídos os artigos sobre fatores genéticos, marcadores biológicos, polimorfismos, campos eletromagnéticos e nutrição. Baseado na revisão bibliográfica foi construído questionário sobre o tema e realizado teste com 29 casos e 29 controles, em um hospital da cidade de Santo André SP. Resultados: Não foram identificados artigos no Lilacs. No Pubmed foram encontrados 39 artigos. Após as exclusões 33 estudos foram selecionados: 11 casos-controle, 12 revisões, 09 coorte e 01 transversal. Introdução: O fisioterapeuta utiliza-se de vários equipamentos eletro-eletrônicos que emitem radiação eletromagnética para fins terapêuticos. Entre esses, o equipamento de diatermia por micro-ondas de alta frequência, sendo essa de 2,45 GHz. Nesse procedimento terapêutico, é possível ter perdas variáveis da energia irradiada por reflexão e por dispersão, desde o aplicador até a área a ser tratada. Assim, o profissional que opera o equipamento de diatermia por micro-ondas, é exposto a essa radiação diariamente durante anos. Adicionalmente a esse cenário ocupacional, observa-se que o fisioterapeuta, de modo geral, tem raras atitudes de proteção ocupacional durante essas aplicações, tornando o ambiente de trabalho, bem como a si mesmo, passivos de situações de riscos não controlados e, consequentemente, efeitos adversos podem ocorrer. Com a finalidade de contribuir na elucidação de indicadores que despertem o interesse das autoridades relacionadas com a vigilância e legislação em saúde no que se refere à segurança de profissionais envolvidos direta ou indiretamente com a terapia de diatermia por micro-ondas, realizou-se este estudo. Noise is the most frequent type of occupational exposure and can lead to both auditory and extra-auditory dysfunction as well as increasing the risk of work accidents. The purpose of this study was to estimate the attributable fraction of work accidents related to occupational noise exposure in a medium-sized city in Southeast Brazil. In this hospital-based case-control study, including 600 cases and 822 controls, the odds ratio of work accidents (controlled for several covariables) was obtained classifying occupational noise exposure into four levels and determining the prevalence at each level. Based on these data, the calculated attributable fraction was 0.3041 (95%CI: 0.2341-0.3676), i.e., 30% of work accidents in the study area were statistically associated with occupational noise exposure. The authors discuss the causes of this association and the implications for the prevention of work accidents.

The speeches were recorded, transcribed and qualitative analysis was performed with the aid of QUALIQUANTISOFT® software. The Collective Subject Discourse (CSD) was obtained.RESULT:The feeling most frequently experienced was related to the fear of contagion. Most accidents occurred during the handling of sharp dental instruments. Respondents attributed the occurrence of accidents especially the lack of attention, carelessness while handling sharp instruments, and lack of use of Personal Protective Equipment. As regards the measures taken right after the exposure, they washed the local area. Other respondents reported they continued the dental treatment. They complained mostly about the fear of having been infected, and because they had to leave the faculty to take blood exams for HIV screening. As part of the learning experience the injured reported they paid more attention when handling sharp instruments. The handling of waste can be responsible for occupational exposure to particles and fungi. The aim of this study was to characterize exposure to particles and fungi in a composting plant. Measurements of particulate matter were performed using portable direct-reading equipment. Air samples of 50L were collected through an impaction method with a flow rate of 140L/min onto malt extract agar supplemented with chloramphenicol (0.05%). Surfaces samples were also collected. All the samples were incubated at 27ºC for 5 to 7 days. Particulate matter data showed higher contamination for PM, and PM10 sizes. Aspergillus genus presents the highest air prevalence (90.6%). Aspergillus niger (32.6%), A. fumigatus (26.5%) and A. flavus (16.3%) were the most prevalent fungi in air sampling, and Mucor sp. (39.2%), Aspergillus niger (30.9%) and A. fumigatus (28.7%) were the most found in surfaces. the results obtained claim the attention to the need of further research.

Chrysonilia sitophila is a common mould in cork industry and has been identified as a cause of IgE sensitization and occupational asthma. This fungal species have a fast growth rate that may inhibit others species’ growth causing underestimated data from characterization of occupational fungal exposure. Aiming to ascertain occupational exposure to fungi in cork industry, were analyzed papers from 2000 about the best air sampling method, to obtain quantification and identification of all airborne culturable fungi, besides the ones that have fast-growing rates. Impaction method don’t allows the collection of a representative air volume, because even with some media that restricts the growth of the colonies, in environments with higher fungal load, such as cork industry, the counting of the colonies is very difficult. Otherwise, impinger method permits the collection of a representative air volume, since we can make dilution of the collected volume. Besides culture methods that allows fungal identification trough macro- and micro-morphology, growth features, thermotolerance and ecological data, we can apply molecular biology with the impinger method, to detect the presence of non-viable particles and potential mycotoxin producers’ strains...

Evaluation of HIV-induced IL-2 production by peripheral blood mononuclear cells (PBMC) and HIV-specific T helper and cytotoxic T lymphocyte (CTL) responses in health care workers (HCW) occupationally exposed to HIV reveals a high rate of response to HIV among non-seroconverters. IL-10 is also known to interfere with HIV infection in vitro. To evaluate the induction of IL-10 by HIV antigens in HCW occupationally exposed to HIV, 18 HCW with percutaneous injury were enrolled in this study, 9 of them exposed to HIV-contaminated blood, and 9 exposed to HIV-negative blood. PBMC were incubated on plates coated with HIV-1 antigens, and IL-10 was measured in supernatants by ELISA. Five of nine HCW exposed to HIV-contaminated blood presented HIV-induced IL-10. Two of nine HCW exposed to HIV-negative source patients also had detectable levels of HIV-induced IL-10, one of them in the sample obtained on the day of accidental exposure. There was a relationship between the type of device involved in injury and IL-10 production. Individuals exposed to hollow needles or scalpels presented HIV-induced IL-10, whereas those exposed to solid needles and to digital puncture did not, suggesting a relationship between infectious load and IL-10. We assessed the neuropsychological test performances of 26 patients (mean age = 41.5 ± 6.1 years; mean years of education = 9.8 ± 1.8; 20 males) diagnosed with chronic occupational mercurialism who were former workers at a fluorescent lamp factory. They had been exposed to elemental mercury for an average of 10.2 ± 3.8 years and had been away from this work for 6 ± 4.7 years. Mean urinary mercury concentrations 1 year after cessation of work were 1.8 ± 0.9 µg/g creatinine. Twenty control subjects matched for age, gender, and education (18 males) were used for comparison. Neuropsychological assessment included attention, inhibitory control, verbal and visual memory, verbal fluency, manual dexterity, visual-spatial function, executive function, and semantic knowledge tests. The Beck Depression Inventory and the State and Trait Inventory were used to assess depression and anxiety symptoms, respectively. The raw score for the group exposed to mercury indicated slower information processing speed, inferior performance in psychomotor speed, verbal spontaneous recall memory, and manual dexterity of the dominant hand and non-dominant hand (P < 0.05). In addition, the patients showed increased depression and anxiety symptoms (P < 0.001). This study investigated the hypotheses that occupational exposure to acid mists is positively associated with periodontal disease, assessed by periodontal attachment loss. The study sample included 530 male workers at a metal processing factory. Data were obtained from interviews and oral examinations. Periodontal attachment loss was defined as > 4mm at probing, in at least one tooth. A job exposure matrix was utilized for exposure evaluation. Exposure to acid mists was positively associated with periodontal attachment loss > 4mm at any time (prevalence ratio, PRadjusted = 2.17), past (PRadjusted = 2.11), and over 6 years of exposure (PRadjusted = 1.77), independently of age, alcohol consumption, and smoking, and these results were limited to workers who did not use dental floss. Exposure to acid mists is a potential risk factor for periodontal attachment loss, and further studies are needed, using longitudinal designs and more accurate exposure measures.

Environment monitoring has an important role in occupational exposure assessment. However, due to several factors is done with insufficient frequency and normally don´t give the necessary information to choose the most adequate safety measures to avoid or control exposure. Identifying all the tasks developed in each workplace and conducting a task-based exposure assessment help to refine the exposure characterization and reduce assessment errors. A task-based assessment can provide also a better evaluation of exposure variability, instead of assessing personal exposures using continuous 8-hour time weighted average measurements. Health effects related with exposure to particles have mainly been investigated with mass-measuring instruments or gravimetric analysis. However, more recently, there are some studies that support that size distribution and particle number concentration may have advantages over particle mass concentration for assessing the health effects of airborne particles. Several exposure assessments were performed in different occupational settings (bakery, grill house, cork industry and horse stable) and were applied these two resources: task-based exposure assessment and particle number concentration by size. The results showed interesting results: task-based approach applied permitted to identify the tasks with higher exposure to the smaller particles (0.3 μm) in the different occupational settings. The data obtained allow more concrete and effective risk assessment and the identification of priorities for safety investments.

Estimating the burden of occupational bladder cancer in Ontario using the CAREX Canada database

Objective: This study attempts to estimate the proportion of incident cases of bladder cancer in Ontario, Canada that is due to exposure to occupational carcinogens. Methods: The population attributable risk approach is used to estimate the proportion of bladder cancer in Ontario that is due to occupation. Risk ratios were obtained from a review of epidemiologic literature using a priori inclusion and exclusion criteria. Summary risk estimates for each bladder carcinogen included were calculated using RevMan 4.2. The CAREX Canada database provided Ontario-specific estimates of the proportion of workers exposed to bladder carcinogens. Results: In Ontario, the proportion of bladder cancer due to occupational exposure is approximately 5.6% (95% CI 0.2% to 14%). Based on the incident number of bladder cancer cases in 2001 in Ontario, it is estimated that approximately 52 new cases of bladder cancer were due to occupational exposure to polycyclic aromatic hydrocarbons (PAHs), diesel exhaust, aromatic amines and 2-naphthylamine. An alternate interpretation is if these occupational exposures were eliminated, 52 cases of bladder cancer per year in Ontario alone could be avoided. Conclusion and Recommendations: The current study advances our knowledge of the extent to which specific occupational bladder carcinogens contribute to the overall bladder cancer burden in Ontario. The current study highlights the utility of the CAREX Canada database in advancing current knowledge on the burden of occupational cancer in Ontario. Este estudo descritivo teve como objetivo avaliar as facilidades e barreiras enfrentadas por enfermeiros com cargo de chefia quanto às medidas preventivas à exposição ocupacional envolvendo material biológico, tendo como base o Modelo de Crenças em Saúde de Rosenstock. O estudo foi realizado com 87 enfermeiros de um hospital-escola do interior paulista em 2006. A study was developed aiming to elucidate if there is occupational co-exposure to aflatoxin B1 (AFB1) and ochratoxin (OTA) in Portuguese swine production. To assess occupational exposure to both mycotoxins, a biomarker of internal dose was used. The same blood samples from workers of seven swine farms and controls were consider to measure AFB1 and OTA. Twenty one workers (75%) showed detectable levels of AFB1 with values ranging from <1 ng/ml to 8.94 ng/ml and with significantly higher concentration when compared with controls. In the case of OTA, there wasn't found a statistical difference between workers and controls and the values for workers group ranged from 0.34 ng/ml to 3.12 ng/ml and 1.76 ng/ml to 3.42 ng/ml for control group. The results suggest that occupational exposure to AFB1 occurs. However, in the case of OTA results, seems that food consumption plays an important role in both groups exposure. The results claim attention for the possible implications on health of this co-exposure.

Malignant mesothelioma is an uncommon but rapidly fatal disease for which the principal aetiological agent is exposure to asbestos. Mesothelioma is of particular significance in Australia where asbestos use was very widespread from the 1950s until the 1980s. Exposure to asbestos includes occupational exposure associated with working with asbestos or in workplaces where asbestos is used and also 'take-home' exposure of family members of asbestos exposed workers. Asbestos exposure may also be non-occupational, occurring as a consequence of using asbestos products in non-occupational contexts and passive exposure is also possible, such as exposure to asbestos products in the built environment or proximity to an environmental source of exposure, for example an asbestos production plant. The extremely long latency period for this disease makes exposure assessment problematic in the context of a mesothelioma registry. Este estudo descritivo teve como objetivo avaliar as facilidades e barreiras enfrentadas por enfermeiros com cargo de chefia quanto às medidas preventivas à exposição ocupacional envolvendo material biológico, tendo como base o Modelo de Crenças em Saúde de Rosenstock. O estudo foi realizado com 87 enfermeiros de um hospital-escola do interior paulista em 2006. Os dados foram coletados através de um roteiro semi-estruturado, com questões abertas e fechadas e analisados pela técnica de Análise de Conteúdo. O equipamento de proteção individual foi citado como a maior facilidade para a prevenção de acidentes, porém a falta de adesão ao uso e o uso incorreto foram referidos como barreiras para a prevenção de acidentes e como os principais motivos para a ocorrência destes. Occupational exposure to acid mists and periodontal attachment loss

This study investigated the hypotheses that occupational exposure to acid mists is positively associated with periodontal disease, assessed by periodontal attachment loss. The study sample included 530 male workers at a metal processing factory. Data were obtained from interviews and oral examinations. Periodontal attachment loss was defined as > 4mm at probing, in at least one tooth. A job exposure matrix was utilized for exposure evaluation. Exposure to acid mists was positively associated with periodontal attachment loss > 4mm at any time (prevalence ratio, PRadjusted = 2.17), past (PRadjusted = 2.11), and over 6 years of exposure (PRadjusted = 1.77), independently of age, alcohol consumption, and smoking, and these results were limited to workers who did not use dental floss. Exposure to acid mists is a potential risk factor for periodontal attachment loss, and further studies are needed, using longitudinal designs and more accurate exposure measures.
