CONTEXTUALIZAÇÃO: A Síndrome de Marfan (SM) é uma doença autossômica dominante do tecido conjuntivo que envolve os sistemas ocular, cardiovascular e musculoesquelético, causada por mutações no gene da fibrilina1, gerando flacidez nos ligamentos articulares, favorecendo a hipermobilidade articular e redução na contenção do crescimento ósseo. OBJETIVOS: Avaliar as medidas antropométricas, alterações musculoesqueléticas e a frequência do tratamento fisioterapêutico nos pacientes com SM. MÉTODOS: Participaram deste estudo 26 pacientes, sendo 17 do gênero feminino, com idade de 13,23±2,77 anos, massa corpórea de 51,5±24-68 Kg, altura de 1,70±1,40-1,81 m e envergadura de 1,73±0,12 cm, e nove do gênero masculino, com idade de 14,44±2,18, massa corpórea de 61,0±42-72 Kg, altura de 1,83±1,66-1,97 m e envergadura de 1,93±0,13. Foram obtidas medidas antropométricas, alterações ME de forma padronizada, sendo o pectus e a escoliose, por avaliação radiológica, e a angulação (â) da curva escoliótica, pelo método de Cobb; a aracnodactilia, pelo sinal do polegar e teste de Walker-Murdoch, e a dolicostenomelia, pela envergadura em relação à altura. A Biomecânica Ocupacional procura aprimorar as condições de trabalho objetivando prevenir e reduzir as lesões, aumentar o desempenho dos operários reduzindo sobrecargas. Pesquisas nesta área indicam que não apenas a manipulação de materiais pesados, mas também posturas pouco naturais e movimentos súbitos e inesperados provocam lesões no sistema osteomuscular que afetam principalmente o pescoço, membros superiores e região lombar. Alguns profissionais apresentam alto risco ao aparecimento desses distúrbios envolvendo principalmente os membros superiores e região lombar, particularmente os digitadores, operadores de caixa de supermercados, dentistas, motoristas profissionais e operadores de trens metropolitanos. Objetivo: Comparar a sobrecarga no sistema musculoesquelético e na qualidade de vida de trabalhadores em turnos fixos matutinos, vespertinos e noturnos. Métodos: A amostra deste estudo foi composta de 254 trabalhadores de uma empresa ligada ao setor automotivo. As coletas foram realizadas em uma cidade brasileira de médio porte no ano de 2011. Os trabalhadores foram divididos em grupos de turnos: G1. 87 Trabalhadores do Turno Matutino (6:00 - 14:00); G2. 87 Trabalhadores do Turno Vespertino (14:00 - 22:00); G3. 80 Trabalhadores do Turno Noturno (22:00 - 6:00). Foram aplicados os seguintes instrumentos: Questionário Sociodemográfico, Questionário Nórdico de Sintomas Musculoesqueléticos e Questionário de Qualidade de Vida SF-36. Foi estimada a prevalência, com respectivo intervalo de 95% de confiança, para as queixas músculoesqueléticas. Análises bivariadas foram usadas para identificar possíveis associações entre as queixas musculoesqueléticas e as variáveis independentes. As décadas de 80 e 90 caracterizaram-se por ações intensas na área de saúde ocupacional. Porém os índices de acometimentos e queixas álgicas em MMSS continuam sendo um grande problema. Os trabalhadores que realizam atividades de preparação de alimentos apresentam uma grande sobrecarga em MMSS. O objetivo deste estudo foi analisar a sobrecarga musculoesquelética e o desconforto postural em trabalhadores de duas cozinhas industriais através do Método RULA e do mapa de desconforto postural. Foram filmadas e avaliadas 151 tarefas durante 10 dias em cada restaurante. Cada tarefa foi avaliada por dois observadores independentes visando a identificação do escore do método RULA e fatores que pudessem influenciar na sobrecarga musculoesquelética como mobiliário, instrumentos de trabalho, mecanização do processo de trabalho, postura (análise cinesiológica e biomecânica) e utilização de alimentos pré-processados. Os trabalhadores preencheram também o mapa de desconforto postural. Como resultado evidenciou-se que não houve diferença significativa nos escores do método RULA entre os restaurantes estudados. A mucopolissacaridose tipo II (MPS II) é uma doença de depósito lisossômico rara causada pela deficiência da atividade da enzima iduronato-2-sulfatase. Essa enzima é responsável pelo catabolismo de dois Glicosaminoglicanos (GAGs) diferentes, o sulfato de dermatan e o sulfato de heparan. O acúmulo lisossômico desses GAGs causa disfunção de células, tecidos e órgãos, sendo que o envolvimento do Sistema Músculo-esquelético se deve ao acúmulo essencial em articulações e no tecido conectivo. A MPS II abrange muitos aspectos e dois fenótipos são reconhecidos, o leve e o grave, que representam os dois pontos extremos de um vasto espectro da gravidade clínica. O objetivo desse estudo foi caracterizar o Sistema Músculo-esquelético de uma amostra de sujeitos com MPS II, relacionando as conseqüências biomecânicas e funcionais. With the purpose of increasing performance and reducing the risk of injury, substantial advances were accomplished to pursuit this goal, either on the laboratory techniques as well as modelling and simulation. Traditionally, the main focus was the assessment of kinematics, kinetics and electromyography data to describe the main mechanics and neuromuscular behaviour, when performing a certain movement. The use of methodologies that enable the quantification of the effect of a particular joint moment of force in the entire body or the contribution of an individual muscle force to accelerate the centre of mass of the body is quite relevant in biomechanical analysis. This is particularly important when dealing with explosive movements such as those that occur in sports activities, or in the clinical field when dealing with abnormal movement. In the municipality of Pedreira in São Paulo State, Brazil, a large number of workers from the ceramic industry have left their jobs because of work related musculoskeletal disorders. The objectives of this study were to describe the work conditions pertaining to the ceramic industry, to determine the prevalence of musculoskeletal pain and to identify the associations between symptoms and organizational, biomechanical, psychosocial and individual variables. Nine ceramic manufacturers participated. The activities of 18 individuals were analyzed through direct observation. All workers answered a questionnaire about work and health (n = 235). The results found that the general working conditions in the pottery manufacturers were poor. A 38.5% prevalence of musculoskeletal pain was found. Repetition, tool use, lack of control over decisions, worries regarding work demands, relationship issues, work dissatisfaction and the wish to move on to another function were all associated with pain. We concluded that musculoskeletal pain is one of the outcomes of elevated human requirements resulting from working conditions and organization in the ceramic industry.

The ability to combine physiology and engineering analyses with computer sciences has opened the door to the possibility of creating the "Virtual Human" reality. This paper presents a broad foundation for a full-featured biomechanical simulator for the human musculoskeletal system physiology. This simulation technology unites the expertise in biomechanical analysis and graphic modeling to investigate joint and connective tissue mechanics at the structural level and to visualize the results in both static and animated forms together with the model. Adaptable anatomical models including prosthetic implants and fracture fixation devices and a robust computational infrastructure for static, kinematic, kinetic, and stress analyses under varying boundary and loading conditions are incorporated on a common platform, the VIMS (Virtual Interactive Musculoskeletal System). Within this software system, a manageable database containing long bone dimensions, connective tissue material properties and a library of skeletal joint system functional activities and loading conditions are also available and they can easily be modified, updated and expanded. Application software is also available to allow end-users to perform biomechanical analyses interactively. The National Institutes of Health–sponsored workshop “Translational Models for Musculoskeletal Tissue Engineering and Regenerative Medicine” was held to describe the utility of various translational models for engineered tissues and regenerative medicine therapies targeting intervertebral disc, cartilage, meniscus, ligament, tendon, muscle, and bone. Participants included leaders in the various topics, as well as National Institutes of Health and Food and Drug Administration. The Food and Drug Administration representatives provided perspectives and needs for studies supported by animal models. Researchers described animal models for specific tissues and addressed the following questions: (1) What are the unmet musculoskeletal clinical needs that may be addressed by tissue engineering and regenerative medicine? (2) Are there appropriate models available? (3) Are there needs to develop standardized animal models? (4) What are the translational pathways that lead to clinical trials and therapeutic development? The workshop provided an effective and succinct summary of the status of various animal models in musculoskeletal regenerative medicine. Traditional finite element (FE) analysis is computationally demanding. The computational time becomes prohibitively long when multiple loading and boundary conditions need to be considered such as in musculoskeletal movement simulations involving multiple joints and muscles. Presented in this study is an innovative approach that takes advantage of the computational efficiency of both the dynamic multibody (MB) method and neural network (NN) analysis. A NN model that captures the behavior of musculoskeletal tissue subjected to known loading situations is built, trained, and validated based on both MB and FE simulation data. It is found that nonlinear, dynamic NNs yield better predictions over their linear, static counterparts. The developed NN model is then capable of predicting stress values at regions of interest within the musculoskeletal system in only a fraction of the time required by FE simulation.

[Purpose] The purpose of the present study was to investigate the effect of trumpet and marching euphonium performance posture on the trunk and lower limb musculoskeletal system. [Subjects] The subjects were 10 female university students. [Methods] Subjects maintained a resting position, a trumpet performance posture, and a marching euphonium performance posture. The angles and muscle activities of the trunk and lower limbs were then measured. [Results] The anterior tilt angle of the trunk decreased significantly in the trumpet and marching euphonium performance postures compared with the resting standing position, as well as in the marching euphonium performance posture compared with the trumpet performance posture. The muscle activity of the cervical paraspinal muscles, upper fibers of the trapezius, and lumbar paraspinal muscles increased significantly in the marching euphonium performance posture compared with the resting standing position, as well as in the marching euphonium performance posture compared with the trumpet performance posture. The night and day cycle governs the circadian (24 hourly) rhythm of activity and rest in animals and humans. This is reflected in daily changes of the global gene expression pattern and metabolism, but also in the local physiology of various tissues. A central clock in the brain co-ordinates the rhythmic locomotion behaviour, as well as synchronizing various local oscillators, such as those found in the musculoskeletal system. It has become increasingly recognized that the internal molecular clocks in cells allow a tissue to anticipate the rhythmic changes in their local environment and the specific demands of that tissue. Consequently, the majority of the rhythmic clock controlled genes and pathways are tissue specific. The concept of the tissue-specific function of circadian clocks is further supported by the diverse musculoskeletal phenotypes in mice with deletions or mutations of various core clock components, ranging from increased bone mass, dwarfism, arthropathy, reduced muscle strength and tendon calcification. Functional imaging, which provides information of how tissues function rather than structural information, is well established in neuro- and cardiac imaging. Many musculoskeletal structures, such as ligaments, fascia and mineralized bone, have by definition a mainly structural role and clearly don’t have the same functional capacity as the brain, heart, liver or kidney. The main functionally responsive musculoskeletal tissues are the bone marrow, muscle and nerve and, as such, magnetic resonance (MR) functional imaging has primarily addressed these areas. Proton or phosphorus spectroscopy, other fat quantification techniques, perfusion imaging, BOLD imaging, diffusion and diffusion tensor imaging (DTI) are the main functional techniques applied. The application of these techniques in the musculoskeletal system has mainly been research orientated where they have already greatly enhanced our understanding of marrow physiology, muscle physiology and neural function. Going forwards, they will have a greater clinical impact helping to bridge the disconnect often seen between structural appearances and clinical symptoms, allowing a greater understanding of disease processes and earlier recognition of disease, improving prognostic prediction and optimizing the monitoring of treatment effect.

Ultrasonography of the postoperative musculoskeletal system plays an important role in the Epub ahead of print accurate diagnosis of abnormal lesions in the bone and soft tissues. Ultrasonography is a fast and reliable method with no harmful irradiation for the evaluation of postoperative musculoskeletal complications. In particular, it is not affected by the excessive metal artifacts that appear on computed tomography or magnetic resonance imaging. Another benefit of ultrasonography is its capability to dynamically assess the pathologic movement in joints, muscles, or tendons. This article discusses the frequent applications of musculoskeletal ultrasonography in various postoperative situations including those involving the soft tissues around the metal hardware, arthroplasty, postoperative tendons, recurrent soft tissue tumors, bone unions, and amputation surgery.

Veränderungen im muskulo-skelettalen System mit verminderter Knochendichte, erhöhter Frakturneigung und Veränderungen von Markern des Knochenstoffwechsels bei juveniler-idiopathischer Arthritis (JIA) wurden in der Literatur mehrfach beschrieben. Auch die Hypothese einer bereits im Kindesalter vorliegenden Osteoporose wurde dabei mehrfach gestellt. Allerdings wurden diese Ergebnisse mit Messverfahren gewonnen, die eine Aussage über die tatsächliche Knochendichte, die Geometrie und die beteiligten Kompartimente der Muskel-Knochen-Einheit nicht zulassen. In der vorliegenden Querschnittsstudie wurden 57 Patienten mit JIA untersucht. Vor allem die unterschiedlichen Einflussgrößen auf den Knochen, die Geometrie des Knochens und der Muskel konnten mit Hilfe der pQCT-Technik untersucht und mit pädiatrischen Normwerten verglichen werden. Sowohl eine Auswertung hinsichtlich des Lebensalters als auch hinsichtlich des Größenalters wurde durchgeführt. Dabei stand neben der Knochendichtemessung die Frage im Vordergrund, ob es sich bei den Veränderungen um primäre Knochenveränderungen, sekundäre Folgen einer Affektion der Muskel-Knochen-Einheit oder um Mischeffekte handelt. Zum genaueren Verständnis wurden 35 weitere Patienten in Subgruppen zu bestimmten Fragestellungen und ein experimentelles Tiermodell mit kollagen-induzierter Arthritis in gleicher Technik untersucht. INTRODUCTION: Musculoskeletal pain is one of the most frequent complaints among teachers, compromising their health and quality of life. OBJECTIVE: To estimate the prevalence of musculoskeletal pain among teachers, assessing their occurrence according to sociodemographic characteristics, general health and well-being at work. METHODS: An exploratory cross-sectional study conducted with 525 teachers. During activities of continuing education, the teachers completed a self-administered questionnaire containing questions about sociodemographic factors, general health, well-being at work and musculoskeletal pain. RESULTS: The overall prevalence of musculoskeletal pain was equal to 73.5%. The most common musculoskeletal pains were localized in the shoulders (31.6%), upper back (27.8%), neck (27.2%) and ankles and/or feet (24.0%). Circulatory and respiratory problems and Common Mental Disorders were associated with pain in the shoulders, upper back, neck and ankles and/or feet. Well- being at work is associated with pain in the shoulders, neck and ankles and/or feet. O objetivo da presente pesquisa foi identificar a ocorrência de sintomas musculoesqueléticos em professores do Ensino Fundamental, bem como a sua localização corporal, aspectos de qualidade de vida e tarefas do trabalho docente percebidas como mais fatigantes. Trata-se de um estudo transversal, com 157 professores efetivos da rede estadual e municipal de ensino da cidade de São João da Boa Vista. Os instrumentos compreenderam um inventário sobre dados gerais e ocupacionais, uma adaptação da parte geral do Questionário Nórdico; o questionário genérico de avaliação da qualidade de vida (SF-36) e a escala RPE (Rating of Perceived Exertion) de Borg. Neoplasm seeding in biopsy tract of the musculoskeletal system. A systematic review

To identify, through a systematic literature review, the characteristics of neoplasm seeding in biopsy performed on the musculoskeletal system. We performed a search on PubMed, MEDLINE, LILACS and SciELO from August to October 2010. We included articles that addressed the neoplasm seeding in biopsy performed on the musculoskeletal system. The search was limited to English, Spanish and Portuguese as publication languages, but it was not limited by year of publication. We retrieved 2858 articles, but only seven were selected based on inclusion and exclusion criteria. Other four papers were found in the references of selected articles, totalizing 11 articles that were used to perform this systematic review. Issues may be raised in the literature: age and gender don't seem to influence the occurrence of neoplasm seeding; without resection of the biopsy tract, the possibility of local recurrence is very real; the influence of the type of tumor in the occurrence of neoplasm seeding is uncertain; it is impossible to conclude whether the closed biopsy technique has a lower chance of neoplasm seeding; it is likely that adjuvant chemotherapy has a protective effect against neoplasm seeding; an unfavorable prognosis is expected according to neoplasm seeding results.

Study Design: Cross-sectional. Objectives: (i) To identify the prevalence of musculoskeletal disorders, (ii) to identify musculoskeletal disorders correlates and (iii) to identify the existence of relationship between musculoskeletal disorders and healthcare expenditures in adults attended to by the public healthcare system. Methods: Crosssectional study involving 963 adults (over 50 years of age and of both sexes) treated in basic healthcare units (BHU). The participants answered questionnaires about self-reported morbidity, economic condition, musculoskeletal symptoms, occupational activity behavior, physical therapy consultation and prescription drugs purchased. Weight and height were obtained for body mass index computation. Healthcare expenditures in the last 12 months were computed. Results: The prevalence of musculoskeletal symptoms was 65%. Patients diagnosed with musculoskeletal system/connective tissue diseases (OR= 4.11 [2.98 – 5.67]) and people who needed to lift loads at work (OR= 1.45 [1.27 – 1.66]) presented higher likelihood to report the outcome. Moreover, occurrence of musculoskeletal symptoms in shoulders, low back, knees and ankles/feet were positively related to increased prescription drugs used and medical consultations. It is likely that everyone will, at some time, suffer from a problem related to the musculoskeletal system, ranging from a very common problem such as osteoarthritis or back pain to severely disabling limb trauma or rheumatoid arthritis. Many musculoskeletal problems are chronic conditions. The most common symptoms are pain and disability, with an impact not only on individuals’ quality of life but also, importantly, on people’s ability to earn a living and be independent. It has been estimated that one in four consultations in primary care is caused by problems of the musculoskeletal system and that these conditions may account for up to 60% of all disability pensions. In contrast, teaching at undergraduate and graduate levels - and the resulting competence and confidence of many doctors - do not reflect the impact of these conditions on individuals and society. Many medical students do not have any clinical training in assessing patients with bone and joint problems. Under the umbrella of the Bone and Joint Decade 2000-2010, experts from all parts of the world with an interest in teaching have developed recommendations for an undergraduate curriculum to improve the teaching of musculoskeletal conditions in medical schools.
