A síndrome de Rett (SR) é um distúrbio neurológico progressivo de causa genética que afeta quase exclusivamente o sexo feminino. É causada por mutações, geralmente esporádicas, do gene MECP2, localizado no cromossomo X. Apresenta como características principais: estagnação no desenvolvimento neuropsicomotor, perda de comunicação, do contato visual, do interesse por pessoas e objetos e estereotipias manuais. Em conseqüência do grave comprometimento cognitivo e motor, as portadoras de SR têm muita dificuldade em realizar as tarefas do dia-a-dia. O objetivo desse trabalho foi avaliar as habilidades funcionais e averiguar as necessidades de assistência do cuidador, conforme determinadas pelo Inventário de Avaliação Pediátrica de Incapacidade (PEDI). Esse instrumento de avaliação, que possui 197 itens nas áreas de autocuidado, mobilidade e função social, foi aplicado em 64 portadoras de SR que preenchiam os critérios para a forma clássica da doença. Elas tinham idade entre 2 e 26 anos, com média de 10 anos. Entre as 73 atividades da área de autocuidado do PEDI, 52 (71,2%) não foram realizadas por qualquer criança; na área de mobilidade, entre as 59 atividades propostas, 8 (13,5%) não foram feitas pelas portadoras de SR; e finalmente na área de função social...

Introdução: Dor e deformidade nos pés são queixas comuns na Artrite Reumatóide (AR) promovendo alterações no equilíbrio e na mobilidade funcional. A informação somatossensorial dos pés e tornozelos para a regulação da postura e manutenção do equilíbrio tem sido discutida na literatura e apregoa-se que a integridade do pé e da sua informação sensorial são fundamentais para a estabilidade postural. Os programas propostos neste trabalho para estes pacientes integram atenção para a dor e para as deformidades enfocando a melhora da funcionalidade. Aims: To evaluate the spatio-temporal variables of gait and the isometric muscle strength component of the ankle in patients with peripheral diabetic neuropathy. Also, verify the relationship between these variables and gait parameters. Methods: This study involved 25 diabetic peripheral neuropathy (DPN) participants (62.4 ± 8.36 years) and 27 age-matched healthy control individuals (64.48 ± 6.21 years). The assessment of the spatio-temporal parameters of gait was performed using an electronic baropodometry treadmill. Prior to the collection data, each participant was instructed to walk on the treadmill in her/his habitual self-selected speed. Results: Diabetic neuropathy group showed impairment of gait, with a smaller stride and length speed of the cycle, and increased duration of support time. Restricted dorsiflexion mobility and increased plantarflexion mobility were found, with a decrease in muscle strength of the dorsiflexors and plantiflexors. There was a significant relationship between plantiflexor muscle strength and the length and speed of the gait cycle. Also the muscle strengths of the plantiflexors and dorsiflexors, and the range of motion of dorsiflexion were predictors of gait performance. RESUMO:O objectivo deste estudo foi analisar a relação entre mobilidade funcional, risco de queda, nível de actividade física e percepção de saúde de 34 indivíduos praticantes (n=18) e não praticantes (n=16) de exercício físico duas ou mais vezes por semana durante pelo menos 45 minutos, residentes na comunidade e ambulatórios. Tipo de estudo: transversal exploratório-descritivo. Metodologia: foi feito um levantamento das variáveis de caracterização (idade, sexo, habilitações literárias, situação sócio-económica, situação familiar) e do estado cognitivo e estado emocional/depressão (Mini-Mental State Examination e Geriatric Depression Scale). Factors associated with functional balance and mobility among elderly diabetic outpatients

OBJECTIVES: To characterize balance and mobility among diabetic elderly outpatients and to estimate the extent to which functional balance and mobility abnormalities can be influenced by sociodemographic, clinical and other functional factors in a cross-sectional study. METHODS: Ninety-one elderly (65+ years) outpatients were assessed. Mobility was evaluated by the Timed Up and Go Test (TUGT) and the balance, by the Berg Balance Scale (BS). RESULTS: TUGT mean score was 15.65 ± 5.9 seconds and BS mean score was 49.31 ± 7.3 points. Using linear regression analysis (α < 0.05), significant and independent positive relationships were obtained between TUGT and age, daily activities (ADL/IADL), step strategy, and proprioceptive sensitivity. Factors negatively associated with BS were: ADL/IADL, step strategy, proprioceptive sensitivity, orthostatic hypotension (OH) and conflictive sensory conditions. CONCLUSION: Elderly diabetic outpatients show abnormal balance and mobility related mainly to advanced age, disability, absence of step strategy, absence of proprioceptive sensitivity and presence of OH.

Objective: to investigate the association between nutritional status and functional limitation and disability in an elderly population in southern Brazil. Methods: epidemiological, cross-sectional household-based study carried out with 477 elderly of both sexes (60 to 100 years). Body mass index (BMI) served to assess the nutritional status: underweight (BMI < 22 kg/m2) and overweight (BMI > 27 kg/m2). The sum score (0-5) obtained in three tests: "chair stand" and "pick up a pen" (measured by time) and standing balance (four static measurements) assessed the functional limitation. The disability was evaluated by the difficulty in performing one or more self-reported tasks related to basic activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Crude and adjusted analyzes (3 models) were carried out using Poisson regression; prevalence ratios (PR) and 95% confidence intervals (CI) were calculated. Obesity in middle and old age predicts mobility limitation; however, the cumulative effect of overweight and/or obesity over the adult life course is unknown. The association between overweight and/or obesity in young, middle, and late adulthood and its cumulative effect on incident mobility limitation was examined among community-dwelling US adults aged 70–79 years at baseline (1997–1998) in the Health, Aging and Body Composition Study (n = 2,845). Body mass index was calculated by using recalled weight at ages 25 and 50 years and measured weight at ages 70–79 years. Mobility limitation (difficulty walking 1/4 mile (0.4 km) or climbing 10 steps) was assessed semiannually over 7 years of follow-up and was reported by 43.0% of men and 53.7% of women. Men and women who were overweight or obese at all 3 time points had an increased risk of mobility limitation (hazard ratio = 1.61, 95% confidence interval: 1.25, 2.06 and hazard ratio = 2.85, 95% confidence interval: 2.15, 3.78, respectively) compared with those who were normal weight throughout. Furthermore, there was a significant graded response (P < 0.0001) on risk of mobility limitation for the cumulative effect of obesity in men and overweight and/or obesity in women. Onset of overweight and obesity in earlier life contributes to an increased risk of mobility limitation in old age.

In this study, the authors aimed to determine whether higher activity energy expenditure, assessed by using doubly labeled water, was associated with a reduced decline in mobility limitation among 248 older community-dwelling US adults aged 70–82 years enrolled in 1998–1999. Activity energy expenditure was calculated as total energy expenditure (assessed over 2 weeks by using doubly labeled water) minus resting metabolic rate (measured with indirect calorimetry), with adjustment for the thermic effect of food. Across sex-specific tertiles of activity energy expenditure, men in the lowest activity group experienced twice the rate of mobility limitation as men in the highest activity group (41% (n = 18) vs. 18% (n = 8)). Conversely, women in the lowest and highest activity groups exhibited similarly high rates of mobility limitation (40% (n = 16) vs. 38% (n = 15)). After adjustment for potential confounders, men with higher activity energy expenditure levels continued to show reduced risk of mobility limitation (per standard deviation (284 kcal/day): hazard ratio = 0.61, 95% confidence interval: 0.41, 0.92). The purpose of this qualitative descriptive study was to examine reasons for participation in clinical research among older adults with mobility limitation. A purposive sample of 20 men and 20 women aged 70 years or older was recruited. Data were collected by audiotaped telephone interviews using a semi-structured interview guide and transcribed verbatim. Participants expect privacy, professionalism by research staff, and respectful treatment. Benefits to protocol adherence include personal education, comparison of their health status with that of others, opportunity to maintain vitality, and altruism. Barriers to protocol adherence are apprehension, in particular a negative impact on their health care, randomization to the control group, and experimental drugs; and inconvenience. Factors promoting study completion are obligation, reciprocity, receipt of test results, health promotion, and socialization. Implications include meeting expectations, providing health education and study results to participants, reducing barriers to participation, and presenting opportunities for interaction with others.

The Role of Neighborhood Safety in Recovery from Mobility Limitations: Findings from a National Sample of Older Americans (1996–2008)

Prior research has documented a link between perceived neighborhood safety and functional limitations including incident mobility limitation, yet no research has explored the association between perceived neighborhood safety and recovery from functional limitations. This study investigates whether perceived neighborhood safety independently predicts recovery. Using longitudinal data from the Health and Retirement Study (HRS) (1996–2008), discrete-time event history models with multiple competing events were estimated using multinomial logistic regression. Robust standard errors were used to adjust for clustering. In the unadjusted model, the odds ratio for recovery from mobility limitations was 1.22 (CI: 1.17, 1.27) for respondents reporting greater neighborhood safety, while, in the fully adjusted model, the odds ratio was 1.11 (CI: 1.05, 1.17). Even after controlling for sociodemographic characteristics and numerous health risk factors, perceived neighborhood safety was a robust predictor of mobility limitation recovery. This research provides further evidence that environmental factors shape functional health outcomes including recovery.

Objectif: Évaluer les défis de la mobilité chez les personnes âgées atteintes de dégénérescence maculaire reliée à l’âge (DMLA), de glaucome ou de dystrophie cornéenne de Fuchs et les comparer avec les personnes âgées n’ayant pas de maladie oculaire. Devis: Étude transversale de population hospitalière Participants: 253 participants (61 avec la DMLA, 45 avec la dystrophie cornéenne de Fuchs, 79 avec le glaucome et 68 contrôles) Méthodes: Nous avons recruté les patients parmi ceux qui se font soigner dans les cliniques d’ophtalmologie de l’Hôpital Maisonneuve-Rosemont (Montréal, Canada) de septembre 2009 à octobre 2010. Les patients atteints de la DMLA ou de la maladie de Fuchs ont une acuité visuelle inférieure à 20/40 dans les deux yeux, tandis que les patients avec du glaucome ont un champ visuel dans le pire oeil inférieur ou égal à -4dB. RESUMEN: Introducción: Las sillas de ruedas son un producto de apoyo que brinda a los niños discapacitados oportunidades de movimiento, para explorar el medio y continuar con su desarrollo de una forma más plena. El objetivo de esta revisión, fue identificar en la literatura existente el impacto de la silla de ruedas en los niños con algún tipo de discapacidad física, entendemos como impacto, la repercusión de este producto de apoyo, de sus modalidades y adaptaciones, en la vida diaria de estos niños. Materiales y métodos: se realizó una búsqueda bibliográfica en las bases de datos Medline y PEDro ultilizado como palabras clave “Wheelchairs” “Child” , “Child Preschool”y “Mobility Limitation” incluyendo un total de seis artículos, de acuerdo a los criterios de inclusión y exclusión. Resultados: la silla de ruedas proporciona oportunidades para explorar el medio a niños con movilidad reducida, y a ser más autónomos a la hora de explorar el medio. Objectives: to translate and assess the psychometric properties of the Brazilian short-form of the Mobility Assessment Tool (MAT-sf) in Brazilians community elderly. Methds: the 12 items MAT-sf went through the process of translation and back translation into Portuguese of the Brazil, considering the semantic and cultural adaptation. In a sample of 150 aged 65-74 years living in the community, the instrument's psychometric properties were evaluated by analysis of convergent and construct validity and test-retest reliability. The main outcomes measures used for validation included in the battery of tests of physical ability, self-repor measures of functional limitations, health, depression, cognitive and sex. The test-retest reliability of the instrument was assessed using the intra-class correlation coefficient (ICC), 40 subjects were reassessed after an interval of 14 days of assessment. Results: the concurrent validity for the MAT-sf was evidenced by significant correlations with SPPB (r = 0,53), number of functional limitations (r = -0,62) and depressive symptoms (r = -0,45). Quantitative comparison of mobility and gross motor function in Brazilian children with cerebral palsy

Background: Cerebral palsy (CP) presents changes in posture and movement as a core characteristic, which requires therapeutic monitoring during the habilitation or rehabilitation of children. Besides clinical treatment, it is fundamental that professionals use systems of evaluation to quantify the difficulties presented to the individual and assist in the organization of a therapeutic program. The aim of this study was to quantitatively verify the performance of children with spastic di-paresia type CP. Methods: The Pediatric Evaluation of Disability Inventory (PEDI) and Gross Motor Function Classification System (GMFM) tests were used and classification made through the GMFCS in the assessment of 7 patients with CP, 4 females and 3 males, average age of 9 years old. Results: According to GMFCS scales, 17% (n=1) were level II and 83% (n=6) were level III. The PEDI test and 88 GMFM items were used in the area of mobility. We observed that there was high correlation between mobility and gross motor function with Pearson's correlation coefficient =0.929) showing the likely impact of these areas in the functional skills and the quality of life of these patients. Conclusion: We suggest the impact of the limitation of the areas in functional skills and quality of life of these patients.; USP; USP

Functional mobility of children with cerebral palsy (CP) is influenced by personal and environmental factors, serving as barriers and/or facilitators and impacting on children's strategies and functional outcome. OBJECTIVES: To describe typical mobility methods used by children with CP at home, school and community and to compare them across family's socioeconomic levels (SES). METHODS: The Functional Mobility Scale was used to assess mobility of 113 children with CP of high and low SES at home, school, and community. RESULTS: Differences in mobility methods of participants classified as Gross Motor Function Classification System levels II, III and IV were found between home and community. For levels III and IV, differences were also found between home and school. At home, participants from higher SES used wheelchairs more frequently while those from lower SES used floor mobility (crawling). CONCLUSIONS: Environmental settings and families' socioeconomic status influence mobility and use of mobility devices by children with CP.

OBJECTIVE—To investigate the influence of pain severity, knee extensor muscle weakness, obesity, depression, and activity on the association between recent knee pain and limitation of usual and fast paced walking, and ability to rise from a chair.
METHODS—A cross sectional analysis of 769 older women (mean age 77.8, range 65-101) with physical disability, but no severe cognitive impairment. Severity of knee pain in the past month was classified as none, moderate, or severe. Mobility was measured using timed performance tests.
RESULTS—The prevalence of recent knee pain was 53% (408/769). One third of the women with pain reported it to be severe. In general, knee pain was only significantly associated with limited mobility if severe. Obesity, activity and, to a lesser extent, depression intensified the effects of pain. Knee extensor weakness did not. Obesity was a distinctive risk factor in that it substantially increased the risk of mobility limitation, but only in women with pain. In women who had severe pain, activity (walking more than three city blocks in the past week) increased the risk of walking disability more than inactivity. Depression had a minor, but statistically significant effect on walking ability, but not the ability to rise from a chair.

Modelo do estudo: Estudo transversal. Objetivo: O objetivo do presente estudo foi analisar a associação entre composição corporal, mobilidade e equilíbrio de idosos com idade igual ou superior a 80 anos. Métodos: A amostra foi composta por 123 idosos com idade entre 80 e 95 anos (83,2±2,7 anos), sendo 78 mulheres (83,2±2,9 anos) e 45 homens (83,2±2,4 anos) residentes na área urbana de Presidente Prudente - SP. A avaliação da composição corporal foi feita pelo método de Absorciometria por Dupla Emissão de Raios-X (DEXA). A mobilidade e o equilíbrio foram avaliados por meio dos testes de velocidade para caminhar, equilíbrio estático e força de membros inferiores. Para tratamento estatístico realizou-se o teste qui-quadrado, o software utilizado foi SPSS (13.0) e o nível de significância estabelecido foi de 5%. Resultados: No grupo masculino, os idosos com maior desempenho nos testes funcionais apresentaram maiores valores percentuais de massa muscular (MM) (35,6%) comparados aos de menor desempenho (15,6%), p=0,026. No grupo feminino, as idosas com maior desempenho nos testes apresentaram maiores valores de densidade mineral óssea (DMO) (30,8%) comparadas as de menor desempenho (20,5%) p=0,041. O objetivo foi avaliar capacidade funcional, mobilidade e função cognitiva de idosos com a doença de Alzheimer (DA), bem como o nível de sobrecarga de seus cuidadores, verificando possíveis associações entre essas variáveis. Foram selecionados 28 idosos (77,8±8,3 anos) diagnosticados com DA por meio do manual diagnóstico e estatístico das doenças mentais e da Classificação Internacional de Doenças; e também os respectivos cuidadores (58,0±13,9 anos), todos participantes da Associação Brasileira de Alzheimer em Goiás. Foram avaliadas função cognitiva, mobilidade e capacidade funcional dos idosos, por meio do miniexame do estado mental, Southampton assessment of mobility e Disability assessment for dementia, respectivamente. O nível de sobrecarga dos cuidadores foi avaliado pela Zarit burden interview. As associações foram calculadas pelo teste de correlação de Spearman e o nível de significância fixado em 0,05. Obtiveram-se correlações fracas significativas entre o nível cognitivo e a escolaridade dos idosos (r=0,389; p=0,041), nível de funcionalidade dos idosos e nível de sobrecarga dos cuidadores (r=-0,398; p=0,036), e mobilidade e tempo de diagnóstico da DA (r=0,401; p=0,042). Os resultados sugerem que o deficit cognitivo não interferiu na capacidade funcional e a perda cognitiva foi proporcionalmente maior que a perda da mobilidade. Background: Sarcopenia is a geriatric syndrome, which affects the functional status and mobility of individuals. Objective: To identify the prevalence of sarcopenia and undernutrition, and to assess the association between sarcopenia and mobility, and sarcopenia and basic activities of daily living (ADL) in a group of elderly women. Subjects and methods: A cross-sectional study was performed in patients attending a geriatric service at a government hospital in Mexico City. Sarcopenia was identified applying Baumgartner's equation by obtaining skeletal muscle mass index (SMI) and undernutrition was diagnosed using mininutritional assessment instrument (MNA). The SENECA questionnaire and Katz index were used to evaluate mobility and ADL, respectively. Statistical analysis: lineal regression models were constructed fitting SMI as a dependent variable and SENECA, and ADL scores as independent variables, adjusted for age. Results: 90 women with a mean age of 78.2 (± 6.8) were studied. Undernutrition was identified in 15.5% of the patients. The prevalence of sarcopenia was 41.1%.
