O envelhecimento da população brasileira tornou-se uma das principais preocupações dos gestores da área de saúde. Para manter o idoso com um envelhecimento ativo é necessária a preservação da independência e da autonomia. A institucionalização interfere nessa autonomia, deixando, muitas vezes, o idoso mais frágil. O objetivo deste estudo foi avaliar o nível de fragilidade de idosos que vivem em Instituições de Longa Permanência (ILPI) e sua relação com a independência funcional. Trata-se de um estudo quantitativo, observacional e prospectivo. Foram entrevistados 112 idosos residentes em ILPIs da cidade de Maceió, Alagoas. A coleta de dados foi realizada no período de agosto de 2012 a janeiro de 2013. O instrumento de coleta de dados foi composto por dados demográficos e socioeconômicos, Mini Exame do Estado Mental (MEEM), presença de comorbidades/problemas de saúde, Escala de Depressão Geriátrica (GDS), Índice de Barthel e Escala de Fragilidade de Edmonton (EFS). A análise dos dados realizou-se por meio de distribuições de frequência e de medidas de tendência central e dispersão. Foi objetivo desta pesquisa analisar a produção cientifica de fatores de risco para quedas, a partir do diagnóstico da North American Nursing Diagnosis Association, na literatura cientifica brasileira e estrangeira, de 2005 a 2010. Revisão integrativa, na qual foram utilizados os descritores: acidente por quedas e idoso, nas bases de dados da Cumulative Index to Nursing and Allied Health Literature e Literatura Latino-Americana e do Caribe em Ciências da Saúde, sendo selecionados 32 artigos para análise de conteúdo. Os resultados são apresentados conforme os fatores de riscos indicados na North American Nursing Diagnosis Association, sendo eles: fatores de riscos ambientais, como, recinto com móveis e objetos/tapetes espalhados pelo chão, pouca iluminação, piso escorregadio; fatores de riscos cognitivos, tais como, estado mental rebaixado; fatores de riscos em adultos, como, idade acima de 65 anos; fatores de riscos fisiológicos, como, equilíbrio prejudicado, dificuldades visuais, incontinência, dificuldade na marcha, neoplasia; fatores de riscos para uso de alguns medicamentos. Objective: to verify the inter and intra-rater reproducibility of the Brazilian adapted version of the Edmonton Frail Scale (EFS) in an elderly group of residents. Method: in order to test the inter-rater reproducibility, two assessments were independently conducted by two researchers on the same day but at different times, in a sample of 103 elderly. Concerning the intra-rater reproducibility, the instrument was administered to 83 elderly (80.6% of the initial sample) by the same researcher in a time gap of 15 days between the two assessments. Results and Discussion: in relation to the inter-rater test, the Kappa was 0.81 (CI 0.61-1.00) and the Intraclass Correlation Coefficient (ICC) corresponded to 0.87 (CI 0.82-0.91, p<0.001). In relation to the intra-rater test, the Kappa was 0.83 (CI 0.72-0.94) and the ICC 0.87 (CI 0.81-1.00, p<0.001). Conclusion: the results show that the EFS is reliable and can be used as a tool to improve geriatric nursing care in Brazil.

Objective To explores nursing faculty members’ attitudes towards older people, their thoughts about gerontological nursing education. Method Five focus groups and a survey were used with nursing faculty members 132 at the three nursing schools to explore their attitudes towards the care of older people and the perceived status of gerontological nursing education. The survey was given to 132 faculty members, including 76 clinical instructors, 40 associate professors and 16 professors. The nursing faculty in general had a positive attitude toward older people (M=3.36, SD 0.25), and teachers’ attitudes were higher than those of their nursing students (M=3.18, SD0.29). Results This study results suggests that Saudi nursing curricula should include more extensive gerontology content and clinical experience with older people. Conclusion This is the first time in Saudi Arabia that research has listened to their voices and examined their commitments toward gerontology education.

OBJECTIVE: To compare the occurrence of the nursing diagnosis of delayed surgical recovery among the adult and elderly population. METHODS: This was an observational, cross-sectional study conducted with 72 patients followed after the fifth postoperative day. For data collection, a form with conceptual and operational definitions of the defining characteristics and related factors of the nursing diagnosis was used. RESULTS: The rate of the diagnosis of delayed surgical recovery was slightly higher in the elderly, which was 77.1%, compared to adults, which was 75.7%, but there was no significant difference (0.421). Different defining characteristics prevailed: "difficulty moving about" (0.045), "perception that more time is needed to recover," and "requires help to complete self-care" (0.000). CONCLUSION: To differentiate adult and elder care favors the specialization of perioperative nursing care and surgical recovery in the time desired.

The need to systematically and comprehensively manage data to support nursing services asserts itself in all areas and contexts where nursing services are delivered to patients and families. While most applications have targeted inpatient nursing services, several systems to sustain ambulatory nursing care have been proposed as well. In this paper, we outline the purpose and general structure of a custom-designed computerized database management system to support the clinical, administrative, and research operations of a geriatric nursing outreach program in rural Virginia.

Background: The increase of multidrug-resistant organisms (MDROs) causes problems in geriatric nursing homes. Older people are at increased a growing risk of infection due to multimorbidity and frequent stays in hospital. A high proportion of the elderly require residential care in geriatric nursing facilities, where hygiene requirements in nursing homes are similar to those in hospitals. For this reason we examined how well nursing homes are prepared for MDROs and how effectively protect their infection control residents and staff.

8 p.; il.; É freqüente o fenômeno de re-internações de idosos. Assim, é necessário conhecer as respostas humanas de idosos, no momento da admissão hospitalar, para que se possa traçar atendimento adequado. Esta pesquisa objetivou: analisar o perfil de diagnósticos de enfermagem (DE) do domínio Segurança/Proteção, da Taxonomia II da NANDA, na admissão hospitalar de idosos. O estudo descritivo foi realizado na clínica médica de um Hospital Escola da Região Centro-Oeste, de abril a outubro de 2007. Foi utilizado um protocolo previamente validado para a coleta de dados. Para análise dos dados foi adotado o processo de raciocínio clínico proposto por Risner, e procedimentos de estatística descritiva. Participaram 28 idosos, sendo 75% do sexo masculino, 86,4% tinham entre 60 a 79 anos, 71,4% casados, 75% católicos, 42,9% estudaram de um a três anos e 67,8% possuíam rendimento de até dois salários. Foi identificado que 100% dos idosos apresentaram o diagnóstico Dentição Prejudicada, 100% Risco de Infecção, 57,1% Risco de Quedas, 14,2% Integridade da Pele Prejudicada e 3,5% Hipertermia. Examines the culture of two nursing homes using an ethnographic method which is underpinned by ethnomethodology as a philosophical approach. The study shows how older and disabled people are seen to deviate from social norms and how society responds to their situation. Las caídas en los ancianos es un síndrome geriátrico de gran relevancia en la actualidad. Esto es debido tanto a su prevalencia como a sus consecuencias, pudiendo ser desde lesiones leves hasta muy graves, llegando incluso a la muerte en las situaciones más comprometidas. Por ello, provoca un gran impacto en la salud del anciano y su calidad de vida, además de suponer un elevado coste social y sanitario. Conocer los factores de riesgo implicados en las caídas resulta imprescindible para poner en marcha programas de prevención. Para conocerlos se debe realizar una valoración del ambiente, lo que ayuda a conocer los factores de riesgo extrínsecos, además de realizar una valoración geriátrica integral del anciano, para reconocer los factores de riesgo intrínsecos. En este último apartado nos resultara útil la aplicación de escalas de valoración. We compared the clinical presentation and treatment outcomes of corneal and scleral perforations in geriatric nursing home residents, geriatric community residents, and non-geriatric population. The medical records of patients who were treated for corneal and scleral perforations at the Prince of Wales Hospital, Hong Kong between January 1, 2004 and May 1, 2013, were reviewed retrospectively. Of 144 cases, 53 (37%) occurred in the geriatric population, of which 16 (11%) lived in nursing homes, and 37 (26%) were community residents. There were 91 (63%) patients in the non-geriatric group. The mean age of the patients in nursing home geriatric group was 86.5 years (87.5% females). The most common etiology of perforation was trauma. Rupture due to fall was more common in geriatric patients (P < 0.001) whereas laceration due to penetrating eye injury was more common in non-geriatric patients (P < 0.001). There were more cases of infection leading to spontaneous perforation in geriatric nursing home group compared to the other groups (P = 0.001). In the geriatric nursing home group, visual acuity at presentation (P < 0.001) and postoperative visual acuity (P = 0.012) was worse compared to the other groups. Our study showed that corneal and scleral perforations in the geriatric nursing home residents carry a poor visual prognosis. This article reports a critical analysis of gerontological nursing practice guided by Leininger's theory of Culture Care Diversity and Universality. A LILACS database search was performed from January 1970 to June 2006. The analysis suggests that gerontological nursing care based on Leininger's theory needs further development; there is very little published research. The pluralism point of view of the process of taking care of the elderly requires close approximation, active involvement, respect, commitment, and responsibility from health care providers. Appropriate methods of taking care of the elderly should focus on the promotion and maintenance of health, emancipated care, and independence and autonomy. The report of this critical analysis might encourage further development of nursing care to the elderly.

Con el fin de comprender las situaciones que motivan a las familias a asilar un miembro anciano de su familia se realizó un estudio de abordaje metodológica cualitativa, de la cual participaron seis familiares responsables por ancianos recluidos. La colecta de las informaciones ocurrió por medio de entrevistas abiertas. El análisis condujo a dos categorías denominadas: El aislamiento como forma de atender a las necesidades del anciano y Buscar el mejor lugar para que el anciano viva y mantenga el vínculo con la familia. Vivir el proceso de asilar un familiar anciano es un periodo de transición en que diferentes factores son considerados y contribuyen para esta decisión. Cuando hay esta deliberación, la familia busca la mejor residencia para el viejo en términos de estructura física, cuidados y convivencia social con sus congéneres y otras personas. The present study had the objectives of identifying the characteristics of gerontogeriatric nursing and of reflecting on this teaching, based on identified characteristics, in the light of Complexity. A qualitative research, its data sources were nine proceedings volumes of Brazilian Nursing congresses and seven Brazilian nursing publications of the 1991-2000 period. The main body of this article was comprised of 16 scientific articles; QRSNUD* IST4 was used for analysis. Results showed that this teaching does take place, and it is carried out through: mandatory disciplines in the curriculum; practical activities; activities during curricular training; extra-curricular activities; participation on specific or general Nursing events in which studies are presented; other activities. The objective of this study was to analyze the scientific production regarding risk factors for falls in the elderly, based on the North American Nursing Diagnosis Association found in the Brazilian and international literature from 2005 to 2010. This integrative review was performed using the descriptors: accidental falls and elderly, utilizing the following databases: Cumulative Index to Nursing and Allied Health Literature and Latin-American and Caribbean Health Sciences Literature. The results are presented according to the risk factors indicated by the North American Nursing Diagnosis Association, which are: environmental risk factors, such as rooms with excessive furniture and objects/rugs on the floor, poor illumination, and slippery floors; cognitive risk factors such as reduced mental state; risk factors in adults such as age above 65 years; physiological risk factors such as impaired balance, visual difficulties, incontinence, difficulty in walking, and neoplasms; and risk factors associated with the use of certain medications. O objetivo da pesquisa foi identificar e analisar o perfil de Diagnósticos de Enfermagem do Domínio Promoçãode Saúde de acordo com a Taxonomia II da North American Nursing Diagnosis Association, nos idosos atendidos naclínica médica, em alta hospitalar. Pesquisa descritiva, realizada com 25 idosos (> 60 anos) em processo de alta na clínicamédica de um Hospital Escola da Região Centro-Oeste, no período de abril a outubro de 2007. Para a identificação dosdiagnósticos foi utilizado o processo de raciocínio clínico de acordo com Risner. Todos os idosos (100%) apresentaramdiagnósticos de enfermagem no domínio de Promoção de Saúde, e os prevalentes foram: disposição para o controleaumentado do regime terapêutico (64%), controle ineficaz do regime terapêutico (56%), disposição para nutrição melhorada(52%). The study objective was to identify conceptions of the elderly care, designed by the nursing staff, regarding to complications in the postoperative period. It is a descriptive and qualitative study, conducted in a surgical clinic of a teaching hospital in Curitiba, Paraná. The subjects of the study were ten nursing professionals according to established criteria for participation. Information was collected through semi-structured interviews and analyzed by the collective subject discourse technique. Six core ideas emerged from this study: 1) surgical complication is unexpected complications in the postoperative period, triggered by the surgical procedure. 2a) Respiratory complications suggest illnesses. 2b) Complications related to surgical wounds are common. 3a) Non-specific Care, performed with help from the companion. 3b) A paternal caring and childish face. 4) Caring is a priority during hygiene and on the curative care standard. Cooperative learning strategies to teach nutrition to geriatric nursing staff

The objective of this study was to test the hypothesis that cooperative learning strategies will help to increase nutrition knowledge of nurses and nursing assistants caring for the elderly in different institutional communities of the Basque Country, Spain. The target population was a sample of volunteers, 16 nurses and 28 nursing assistants. Training consisted of 12 nutrition education sessions using cooperative strategies conducted over a period of 3 consecutive weeks. The assessment instruments included two pretest and two posttest questionnaires with questions selected in multiplechoice format. The first questionnaire was about general knowledge of applied nutrition (0-88 point scale) and the second one on geriatric nutrition knowledge (0-18 point scale). Data were analyzed using SPSS vs. 11.0. The outcomes indicated a significant increase in general nutrition knowledge (difference between the pre- and posttest mean score: 14.5±10.1; P<0.001) and in geriatric nutrition knowledge for all participants (difference between the pre- and post-test mean score: 4.6±4.6; P<0.001). So the results indicated that cooperative learning strategies could improve the nutrition knowledge of nursing staff. Additionally, the results of this study provide direction to continuing nutrition education program planners regarding appropriate content and methodology for programs.
