Comparison of signal-to-cutoff values in first, second, and third generation enzyme immunoassays for the diagnosis of HTLV-1/2 infection in ""at-risk"" individuals from Sao Paulo, Brazil

Data obtained during routine diagnosis of human T-cell lymphotropic virus type 1 (HTLV-1) and 2 (HTLV-2) in ""at-risk"" individuals from Sao Paulo, Brazil using signal-to-cutoff (S/C) values obtained by first, second, and third generation enzyme immunoassay (EIA) kits, were compared. The highest S/C values were obtained with third generation EIA kits, but no correlation was detected between these values and specific antibody reactivity to HTLV-1, HTLV-2, or untyped HTLV (p = 0.302). In addition, use of these third generation kits resulted in HTLV-1/2 false-positive samples. In contrast, first and second generation EIA kits showed high specificity, and the second generation EIA kits showed the highest efficiency, despite lower S/C values. Using first and second generation EIA kits, significant differences in specific antibody detection of HTLV-1, relative to HTLV-2 (p = 0.019 for first generation and p < 0.001 for second generation EIA kits) and relative to untyped HTLV (p = 0.025 for first generation EIA kits), were observed. These results were explained by the composition and format of the assays. In addition, using receiver operating characteristics (ROC) analysis, a slight adjustment in cutoff values for third generation EIA kits improved their specificities and should be used when HTLV ""at-risk"" populations from this geographic area are to be evaluated. The 1314 km border shared by Portugal and Spain is simultaneously a conflict generator, due to joint access to common resources such as water, and a motive for transboundary cooperation in terms of development projects based on common concerns. Transboundary cooperation associated with Environment Impact Assessment (EIA) has been encouraged through the enactment of the Espoo Convention (1997). The European Union has adopted a Directive (97/11/CE Directive) under which taking transboundary impacts into consideration during EIA processes has become mandatory for member states. As a consequence, Portugal and Spain have approved related provisions. This paper aims to critically analyse the legal and procedural features of bilateral cooperation through the comparison of two case studies related to water management projects (the Sela and Alqueva dams). The paper highlights procedural weaknesses and puts forward a “Good Practice Model” for cooperation under transboundary EIA processes. The model focuses on the ways in which EIA-based bilateral cooperation should proceed through the specification of phases and procedures for collaboration between Portugal and Spain in the identification and evaluation of transboundary impacts, as well in the public participation procedures.

Expression studies of the early EIIa transcription unit (EIIaE) of the adenovirus EIa-deletion mutant dl312 in murine embryonal carcinoma stem cells suggested that these cells contain an activity that substitutes for the viral EIa. To further characterize this cellular EIa-like activity, we analyzed expression of the EIIaE promoter as well as the binding activity of the cognate E2F transcription factor after infection of F9 embryonal carcinoma cells and their differentiated derivatives with wild-type adenovirus, EIa (dl312), or EIV (dl808) deletion mutants. We show that, in contrast to the viral EIa proteins that transactivate the EIIaE promoter in F9 cells only after differentiation, the viral EIV products activate the EIIaE promoter most efficiently in undifferentiated F9 cells. We also show that the EIV products induce a specific modification of the E2F transcription factor leading to its cooperative binding to the EIIaE promoter. Although the EIa-dependent transactivation of EIIaE in differentiated cells is also in part mediated by E2F, it does not by itself correlate with the simultaneous binding of two E2F molecules. In these cells E2F dimer binding only occurs as a secondary effect of EIa that also stimulates EIV expression. Our results suggest that EIa and EIV act through separate pathways...

A split-sample study was conducted to evaluate the clinical performance of an enzyme immunoassay that detects the human parvovirus B19 virus (B19V) immunoglobulin M (IgM) or IgG in the sera of pregnant women. The initial study compared a baculovirus-expressed VP2 enzyme immunoassay (BVP2 EIA) (Biotrin International Inc., Dublin, Ireland) with the currently available and commonly used Escherichia coli-expressed VP1 enzyme immunoassay (EVP1 EIA) (MRL Diagnostics, Cypress, Calif.). There was a high degree of agreement between the two assays in the detection of IgM antibodies (283 of 307 [92.2%]) or IgG antibodies (279 of 311 [89.7%]), with the majority of discrepancies (IgM, 17 of 24 [71%]; IgG, 16 of 31 [50%]) being due to equivocal data obtained with the EVP1 EIA. Specimens with discordant BVP2 EIA and EVP1 EIA results (23 of 24 IgM and 32 of 32 IgG results) were analyzed further by baculovirus-based VP1 immunofluorescence assays (BVP1 IFAs) (Biotrin International). The BVP2 EIA and BVP1 IFA results for 20 of 23 and 28 of 32 specimens for IgM and IgG, respectively, were concordant. We evaluated a newly commercial enzyme immunoassay (EIA) (Biotest Legionella Urin Antigen EIA; Biotest AG, Dreieich, Germany) for detection of antigens of all Legionella pneumophila serogroups with a relatively wide spectrum of cross-reactivity as well as antigens of other Legionella spp. by comparing its sensitivity and specificity with those of an EIA for detection of L. pneumophila serogroup 1 antigen (Legionella urinary antigen EIA; Binax, Portland, Maine). Both tests were performed with both concentrated and nonconcentrated urine samples. We also evaluated the capabilities of both EIAs to detect extracted soluble antigens of American Type Culture Collection (ATCC) Legionella strains (L. pneumophila serogroups 1 to 14, L. bozemanii, and L. longbeachae). The sensitivity of the Biotest EIA was 66.66% in nonconcentrated urine and 86.66% in concentrated urine. The sensitivity of the Binax EIA was 63.76% and 88.88% in nonconcentrated and concentrated urine, respectively. The specificity was 100% in nonconcentrated and concentrated urine for both assays. The Binax EIA and Biotest EIA detected extracted soluble antigens of L. pneumophila serogroups 1 to 14 and L. bozemanii ATCC strains. The cross-reactions observed with the Binax EIA were probably due to common epitopes directly related to lipopolysaccharide. During adenovirus infection, the EII promoter is positively regulated by products of the EIa region. We have studied this regulation by fusing a DNA segment containing the adenovirus EII promoter to a dihydrofolate reductase cDNA segment. Expression of this hybrid gene is stimulated in trans when cell lines containing an integrated copy are either transfected with plasmids carrying the EIa region or infected with adenovirus. This suggests that EIa activity regulates transcription of the EII promoter in the absence of other viral proteins and that this stimulation can occur when the EII promoter is organized in cellular chromatin. Transcription from the EII promoter is initiated at two sites in cell lines lacking EIa activity. Introduction of the EIa region preferentially stimulated transcription from one of these two sites. A sensitive, stable cotransfection assay was used to test for specific EII sequences required for stimulation. EIa activity stimulates all mutant promoters; the most extensive deletion retained only 18 base pairs of sequences upstream of the initiation site. We suggest that regulation of a promoter by the EIa region does not depend on the presence of a set of specific sequences, but instead reflects a characteristic of promoters that have been exogenously introduced into cells. Insertion of the 72-base-pair repeat of simian-virus 40 in cis enhances transcription from the EII promoter. The stimulatory effects of EIa activity and of the simian virus 40 sequence are additive and appear to differ mechanistically.

Activation of the Ras-Raf-extracellular signal-regulated kinase (ERK) pathway causes not only proliferation and suppression of apoptosis but also the antioncogenic response of senescence. How these contrasting effects are reconciled to achieve cell transformation and cancer formation is poorly understood. In a system of two-step carcinogenesis (dedifferentiated PC EIA, transformed PC EIA-polyoma-middle T [PC EIA + Py] and PC EIA-v-raf [PC EIA + raf] cells], v-raf cooperated with EIA by virtue of a strong prosurvival effect, not elicited by Py-middle T, evident toward serum-deprivation-and H2O2-induced apoptosis. Apoptosis was detected by DNA fragmentation and annexin V staining. The prosurvival function of v-raf was, in part, mitogen-activated protein kinase/ERK kinase (MEK)-dependent, as shown by pharmacological MEK inhibition. The MEK-dependent antiapoptotic effect of v-raf was exerted despite a lower level of P-ERK1/2 in EIA + raf cells with respect to EIA + Py/EIA cells, which was dependent on a high tyrosine phosphatase activity, as shown by orthovanadate blockade. An ERK1/2 tyrosine phosphatase was likely involved. The high tyrosine phosphatase activity was instrumental to the complete suppression of senescence, detected by β-galactosidase activity...

The background to the establishment of the Snowy Mountains Hydro-electric Scheme is examined, with an eye to any environmental considerations taken into account, either during planning, establishment or construction. The environmental assessment processes involved in the establishment of such a scheme today are, naturally, more involved than those surround the initial planning of the Scheme. Had these modern processes existed at the time of the establishment of the Scheme, a different outcome could have arisen. The ANZECC guidelines are used as a framework with which to examine the establishment of the Scheme. The current state of the environment surrounding the Scheme is examined, and while some areas are found wanting, others, namely concerns due to erosion, have been adequately examined with appropriate safeguards implemented. The modern framework of Environmental Impact Assessments prove to offer valuable insights in the implementation of large scale projects, and illuminates the processes by which the Scheme managed without an EIA, namely by concentrating on only those aspects of the natural environment which could directly feedback into and effect the viability of the Scheme. "Belastungsasthma", im angloamerikanischen auch als "exercise-induced asthma (EIA)" oder "exercise-induced bronchoconstriction (EIB)" bezeichnet, ist mit einer Prävalenz von 4-20% der Normalbevölkerung und 10-50% unter Sportlern eine weit verbreitete Erkrankung. Die Diagnosestellung des EIA bereitet immer noch Probleme, da sich bis dato bei allen gängigen Testverfahren die Bronchokonstriktion mit der auslösenden Belastung nur indirekt in Zusammenhang bringen lässt, d.h. der Nachweis der Bronchokonstriktion gelingt entweder erst nach der Belastung oder es wird gar nicht die Belastung selbst simuliert sondern ein anderer auslösender Faktor (medikamentöse Provakationstests). Zudem ist der Nachweis einer Bronchokonstriktion mittels eines Lungenfunktionstests immer von der Motivation der zu untersuchenden Person abhängig. Ziel der vorliegenden Arbeit war es, die Bronchokonstriktion zum Zeitpunkt ihrer Entstehung mittels Lungenfunktionsdiagnostik noch während einer hochintensiven Ausdauerbelastung nachzuweisen und somit ein objektives Diagnoseverfahren für EIA zu entwickeln. Hierfür wurde eine Personengruppe (n=10) mit vordiagnostiziertem Belastungsasthma, sowie eine Kontrollgruppe (n=10) einem hochintensiven Ausdauerbelastungstest unterzogen. Dabei wurden vor und nach der Belastung Lungenfunktionstests mit Bestimmung der Einsekundenkapazität (FEV1) durchgeführt. Während der Ausdauerbelastung wurde die Atemstromgeschwindigkeit kontinuierlich aufgezeichnet. Die Ergebnisse der vorliegenden Arbeit können wie folgt zusammengefasst werden: - Die Verringerung des exspiratorischen Peakflow zu 80 und 93% einer erschöpfenden hochintensiven Ausdauerbelastung um mehr als 3% stimmt signifikant mit einer Verringerung des FEV1 um >10% vor und nach Belastung überein und kann somit als Grundlage eines neues...

No Brasil, a Avaliação de Impacto Ambiental (AIA), devido a exigência legal e por estar vinculada ao sistema de licenciamento ambiental, é o instrumento/procedimento de gestão ambiental mais conhecido. Porém somente após a publicação da Resolução CONAMA 001/86 é que a AIA, por meio da elaboração do EIA/RIMA e sua análise e discussão, passou efetivamente a ser implementada. Apesar de existirem exemplos de aplicação bem sucedida do processo, verifica-se que os resultados esperados não têm sido satisfatórios para o que se pretende com ele. Falhas tanto na elaboração dos EIAs como na condução das demais fases da AIA têm sido verificadas. Empresários vêem o processo de AIA, principalmente a elaboração do EIA/RIMA, como empecilho legal e burocrático. Enfim, têm-se verificado deficiências na condução, na eficácia da AIA e naquilo que se pretende com esse instrumento de política pública, que é a busca da sustentabilidade do desenvolvimento. Alguns o enxergam, inclusive, como um processo falido. Diante desse quadro, o presente trabalho buscou, principalmente com base em documentos disponíveis no órgão ambiental estadual e em informações obtidas junto às empresas, avaliar a eficácia da AIA como instrumento de gerenciamento ambiental junto de empreendimentos minerais localizados no Quadrilátero Ferrífero de Minas Gerais. No Brasil, previne-se a transmissão por transfusão sangüínea do vírus da hepatite C com a triagem dos doadores de sangue realizada por meio da pesquisa do anti VHC pelo método de EIA e dosagem dos níveis séricos da ALT. A especificidade da triagem do anti-VHC por EIA, em populações de baixa soroprevalência, tem sido questionada. Assim, para avaliar-se a eficácia desta triagem, pesquisou-se a presença, com o uso da RT-PCR, do RNA viral em amostras de doadores de sangue, procurando-se, também, estabelecer as associações entre os resultados desta reação e os testes de EIA e RIBA. Procurou-se, ainda, detectar marcadores teciduais para o VHC, com a utilização de Um anticorpo monoclonal específico, pela técnica de imuno-histoquímica eimunofluorescência em cortes de tecido hepático parafinado. En este artículo se expone la historia, la evolución y las experiencias del concurso de puentes realizado anualmente en la Escuela de Ingeniería de Antioquia, EIA. Se hace una descripción de los sistemas estructurales básicos usados en puentes, se plantean los puntos débiles de cada uno y se presentan algunos modelos del concurso, analizando su forma de falla según los criterios estructurales. Por último, se describen los puentes ganadores de las cuatro versiones del evento y se expone cómo a través de la experiencia, tanto concursantes como organizadores han logrado superar y elevar el nivel del concurso.; The history, evolution and experience of the bridge contest carried out every year by the Antioquia School of Engineering (EIA) is exposed. The basic structural systems used for bridges are described, the weak points of every system are brought out and some of the contest models are analyzed, according to their structural system, in the way they fault. At last, winner bridges of the four versions of the event are described, and a study of how, through experience, the contestants and organizers had attained and raised the level of contest is exposed.

El presente artículo presenta una propuesta de modelo para la creación de un laboratorio de prospectiva en gerencia en la Escuela de Ingeniería de Antioquia, EIA. Esta proposición se fundamenta en la identificación de las necesidades que las organizaciones tienen de proyectarse en el futuro, como resultado de la globalización y del entorno cambiante. Inicialmente se realiza un análisis corto del marco teórico de la prospectiva así como un resumen del resultado de entrevistas con gerentes de algunas instituciones sobre la necesidad de entender el futuro como clave para el éxito sostenible de los negocios. Seven hundred thirty-two female urogenital samples were collected for Chlamydia trachomatis testing by both the Sanofi Diagnostics Pasteur (Chaska, Minn.) Chlamydia Microplate EIA by the shortened protocol and the Syva (San Jose, Calif.) MicroTrak II EIA, and the results were compared with those obtained by cell culture. For the analysis of samples from female patients, the patients were divided into high- and low-risk categories. An additional 121 male urethral samples were collected and tested by the Sanofi Microplate EIA and cell culture; for the analysis of samples from male patients, the patients were divided into asymptomatic and symptomatic categories. All specimens positive by enzyme immunoassay (EIA) were confirmed by a blocking assay following the respective manufacturer's instructions. Specimens negative by EIA that fell within a gray zone 30% below the cutoff and negative cultures with one or more corresponding positive EIA results were tested further by cytocentrifugation and direct immunofluorescent assay. The overall sensitivity, specificity, positive predictive value, and negative predictive value for Syva versus culture were 94, 98.8, 85.5 and 99.6%, respectively. A construção de uma hidrelétrica provoca grandes transformações na paisagem regional, provocando rápida degradação ambiental, contrária ao processo milenar de sua formação. A implantação de hidrelétricas é definida por pré-requisitos, como a disponibilidade de água, topografia e geologia adequadas e, assim, o estudo integral da paisagem – sistema em constante transformação, não faz parte do processo. A Usina Hidrelétrica do Funil, UHEFunil, foi construída no alto do rio Grande, próximo à cidade de Lavras – MG. Os primeiros estudos são da década de 1960; os estudos e relatórios de impacto ambiental – EIA-RIMA de 1991/1992 e, em 2003, iniciou-se a operação comercial. Objetivou-se, nesta pesquisa, analisar toda a documentação referente ao EIA-RIMA, para perceber como a paisagem na área de influência da usina foi considerada antes, durante e após sua construção; objetivouse também verificar se os estudos apontaram alternativas para o aproveitamento ou a preservação dos recursos naturais, questionando-se sobre a relação entre os benefícios da geração de energia elétrica e os conseqüentes danos ambientais. Os resultados conduziram a uma análise temporal com três estágios de transformação da paisagem: dos primórdios da ocupação antrópica até a época do EIA-RIMA; modificações durante a construção e enchimento da barragem e o surgimento de novas paisagens após a construção da usina.; The building of a hydroelectric power station provokes great transformations in the regional landscape...

Entre 1992 e 1997 foram avaliados, ambulatorialmente, 790 doadores de sangue com teste anti-HCV EIA-2 fortemente reagente (relação entre a densidade ótica da amostra / "cut-off" >; 3), que haviam sido detectados na triagem sorológica do banco de sangue. Todos eram negativos para doença de Chagas, sífilis, hepatite B (HBsAg) e AIDS. Amostras de sangue foram coletadas, na primeira consulta ambulatorial, para a realização de hemograma, exames bioquímicos e novos testes sorológicos para a HVC (anti-HCV EIA-2). Em 226 doadores anti-HCV EIA-2 repetidamente reagentes, realizou-se o teste suplementar de "immunoblot" para a HVC (RIBA-2). Em 209 doadores, pesquisou-se a presença do RNA do VHC pelo teste do PCR, através de exame automatizado (HCV-AMPLICOR, ROCHE). A ultra-sonografia abdominal foi realizada em 366 doadores e a biópsia hepática em 269 concordantes. Notou-se que 95,6% eram EIA-2 repetidamente reagentes, 94% eram assintomáticos e que apenas 2% referiram icterícia pregressa. Em 47% detectou-se, pelo menos, um fator de risco para a transmissão do VHC, sendo o uso de drogas E.V. o principal deles (27,8%). A transfusão de sangue foi o segundo fator na transmissão da HVC (27,2%). Hepatomegalia foi encontrada em 54%. Esplenomegalia e sinais de hipertensão portal foram raramente encontrados no exame físico...

Evaluation of the concomitant use of two different EIA tests for HIV screening in blood banks

OBJECTIVE: In 1998, the Brazilian Ministry of Health made it mandatory for all blood banks in the country to screen donated blood for human immunodeficiency virus (HIV) concomitantly using two different enzyme immunoassay (EIA) tests. Concerned with the best use of available resources, our objective with this study was to evaluate the usefulness of conducting two EIA screening tests instead of just one. METHODS: We analyzed data from 1999 through 2001 obtained by testing 698 191 units of donated blood using two EIA HIV screening tests concomitantly at the Pro-Blood Foundation/Blood Center of São Paulo (Fundação Pró-Sangue/Hemocentro de São Paulo), which is a major blood center in the city of São Paulo, Brazil. All samples reactive in at least one of the two EIA tests were submitted for confirmation by a Western blot (WB) test, and the persons who had donated those samples were also asked to return and provide a follow-up sample. RESULTS: Out of the 698 191 blood units that were donated, 2 718 of them (0.4%) had to be discarded because they were reactive to at least one of the EIA tests. There were two WB-positive donation samples that were reactive in only one HIV EIA screening test. On their follow-up samples, both donors tested WB-negative.
