O que acontece no encontro do médico com a morte do seu paciente diz de algo estruturante da condição humana que transcende a formação acadêmica e mobiliza por este estar na presença de outro ser humano. Trata-se de uma pesquisa qualitativa que teve como objetivo principal descrever e conhecer o que existe de estruturante no encontro do médico com a morte do seu paciente. Este estudo utiliza o método fenomenológico de pesquisa, entendido como um convite para o exercício reflexivo visando à construção de novos paradigmas na produção de conhecimento. O autor de base que fundamenta a análise teórica foi Maurice Merleau-Ponty (2006). Foram realizadas quatro entrevistas com médicos oncologistas clínicos, embasadas na pergunta norteadora: O que significa para você cuidar do paciente oncológico, sem possibilidade de cura, que vivencia o seu processo de morrer e posteriormente perdê-lo?. Após a realização das entrevistas, os relatos (ingênuos) foram literalizados, dos quais foram levantadas as unidades de análise e transformadas em categorias analisadas fenomenologicamente, que possibilitaram o diálogo intersubjetivo e objetivo com os pressupostos teóricos sobre o tema em pauta. Compreendendo a mobilidade do trabalho como um fenômeno que promove o deslocamento espacial, setorial e profissional de trabalhadores, com o objetivo de o capital explorar força de trabalho e acumular excedente econômico e observando que os médicos no Brasil trabalham de forma particularmente diversificada, transversa e longitudinalmente, em múltiplos sub-empregos, procura-se apreender dimensões da mobilidade da força de trabalho de médicos no sistema brasileiro de serviços de saúde. This article maps the new emergent modes of control in a comparative perspective and aims to better understand the relationship between medicine and management as hybrid and context-dependent. Theoretically, we critically review approaches into the managerialism-professionalism relationship; methodologically, we expand cross-country comparison towards the meso-level of organisations; and empirically, the focus is on processes and actors in a range of European hospitals. Methods The research is explorative and was carried out as part of the FP7 COST action IS0903 Medicine and Management, Working Group 2. Comprising seven European countries, the focus is on doctors and public hospitals. We use a comparative case study design that primarily draws on expert information and document analysis as well as other secondary sources. Results The findings reveal that managerial control is not simply an external force but increasingly integrated in medical professionalism. These processes of change are relevant in all countries but shaped by organisational settings, and therefore create different patterns of control: (1) ‘integrated’ control with high levels of coordination and coherent patterns for cost and quality controls; (2) ‘partly integrated’ control with diversity of coordination on hospital and department level and between cost and quality controls; and (3) ‘fragmented’ control with limited coordination and gaps between quality control more strongly dominated by medicine...

Understanding the relationship between periodontal disease (PD) and systemic health (SH) is necessary for the accurate diagnosis and treatment of both. The aim of this study was to evaluate the knowledge of medical doctors in Turkey with regard to the association between PD and SH. This study was carried out using self-reported questionnaires that were sent to medical doctors who work at various universities and public and private hospitals in different cities in Turkey. The questionnaires consisted of questions about the demographic information of the medical doctors, as well as the knowledge of those doctors about the relationship between PD and SH. In total, 1,766 responses were received and 90.8% of the participants agreed that there was a relationship between PD and SH. Diabetes mellitus was the most frequent systemic disease (66.8%) known to be related to PD. Of the participants, 56.5% of the medical doctors referred their patients to periodontists for different reasons. Gingival bleeding was the most frequent reason for patient referrals, with 44% of doctors giving such referrals. Doctors who worked in basic medical sciences were significantly less aware of the relationship between PD and SH than the doctors in other specialties. STUDY OBJECTIVE—To investigate the suicide risk of doctors in England and Wales, according to gender, seniority and specialty.
DESIGN—Retrospective cohort study. Suicide rates calculated by gender, age, specialty, seniority and time period. Standardised mortality ratios calculated for suicide (1991-1995), adjusted for age and sex.
SETTING—England and Wales.
SUBJECTS—Doctors in the National Health Service who died by suicide between 1979 and 1995, identified by death certificates. Population at risk based on Department of Health manpower data.
MAIN RESULTS—Two hundred and twenty three medical practitioners in the National Health Service who died by suicide or undetermined cause were identified. The annual suicide rates in male and female doctors were 19.2 and 18.8 per 100 000 respectively. The suicide rate in female doctors was higher than in the general population (SMR 201.8; 95% CI 99.7, 303.9), whereas the rate in male doctors was less than that of the general population (SMR 66.8; 95% CI 46.6, 87.0). The difference between the mortality ratios of the female and male doctors was statistically significant (p=0.01), although the absolute suicide risk was similar in the two genders. A postal survey of 1,500 doctors who had qualified between 1935 and 1959 and were in active practice in Great Britian elicited 988 replies (a 66 per cent response rate). Nine hundred and thirty-two of the replies were suitable for analysis. There were few differences between hospital doctors and general practitioners in reported medical histories: respiratory tuberculosis had occurred more often among hospital doctors; other respiratory diseases were slightly more common among general practitioners, as were diseases of the digestive tract.

This note describes some of the problems found in recruiting new doctors to the government health care system, drawing on the recent experiences in three cases: the Central Health Service (CHS), the Uttar Pradesh (UP) government health care system and Tamil Nadu government health care system. In comparing the different experiences of recruitment in the three cases, the policy note highlights institutional issues and the incentives built into an employment package as important for further consideration in the aim to scale up the numbers of government doctors.

Persuading medical doctors to work in rural areas is one of the main challenges facing health policy makers, in both developing and developed countries. Discrete Choice Experiments (DCEs) have increasingly been used to analyze the preferences of health workers, and how they would respond to alternative incentives associated with working in a rural location. Previous DCE studies focusing on the rural recruitment and retention problem have sampled either in-service health workers or students in the final year of their training program. This study is the first to sample both of these groups in the same setting. It carry out a DCE to compare how doctors and final-year medical students in Vietnam value six job attributes, and use the results to simulate the impact of alternative incentive packages on recruitment in rural areas. The authors report on a study in which unannounced visits were made to health clinics in Bangladesh with the intention of discovering what fraction of medical professionals were present at their assigned post. This survey represents the first attempt to quantify the extent of the problem on a nationally representative scale. Nationwide the average number of vacancies over all types of providers in rural health centers is 26 percent. Regionally, vacancy rates (unfilled posts) are generally higher in the poorer parts of the country. Absentee rates at over 40 percent are particularly high for doctors. When separated into level of facility, the absentee rate for doctors at the larger clinics is 40 percent, but at the smaller sub-centers with a single doctor, the rate is 74 percent. Women are disproportionately poorly represented in the rural Australian medical workforce and appropriate retention policies should recognise the factors contributing to a positive rural experience for female doctors. This study describes the professional, social and family experiences of 10 female general practitioners (GPs) working in rural South Australia. Variety of work, the opportunity to provide continuity of care and being valued by the community were important factors in rural recruitment and retention. Successful retention of a female rural medical workforce also required flexible on-call and after-hours arrangements, provision of additional support and employment opportunities for non-medical spouses and partners; and provision of appropriate child care services to rural medical families. To date, there have been few initiatives specifically addressing the concerns of female rural medical practitioners. Background Due to difficulties attracting Australian trained medical practitioners to work outside metropolitan areas, many rural communities are recruiting OTDs to provide medical services. Consequently health services in rural and remote areas are heavily dependent on OTDs to maintain their complement of medical practitioners. The experience of overseas trained medical doctors working in rural areas may be culturally and professionally highly challenging with wide-ranging implications for personal, familial and social life. Objectives The objective of this review was to identify the experiences, views, attitudes, and perceptions of OTDs or international medical graduates (IMGs) towards working and living in an Australian rural context. Search strategy and selection criteria Published studies from the electronic databases such as PubMed, CINAHL, JBI Library of Systematic Reviews, ERIC, AUSTROM, ProQuest, Scopus, Current Contents, PsychINFO and unpublished studies from Dissertation Abstract International, and Australian Government/Rural and Remote Professional bodies or association web sites were searched between 1990-2010. Qualitative studies examining OTDs’ experiential accounts of working in rural and remote communities in Australia were sought. Participants were OTDs or general practitioners from non-English speaking backgrounds regardless of duration of working experience in rural areas. Objective. The health and well being of medical doctors is vital to their longevity and safe practice. The concept of resilience is recognised as a key component of well being and is an important factor in medical training to help doctors learn to cope with challenge, stress, and adversity. This study examined the relationship of resilience to personality traits and resilience in doctors in order to identify the key traits that promote or impair resilience. Methods. A cross sectional cohort of 479 family practitioners in practice across Australia was studied. The Temperament and Character Inventory measured levels of the seven basic dimensions of personality and the Resilience Scale provided an overall measure of resilience. The associations between resilience and personality were examined by Pearson product-moment correlation coefficients, controlling for age and gender (α = 0.05 with an accompanying 95% confidence level) and multiple regression analyses. This paper is one of several inputs prepared for a comprehensive Health Sector Review that the Government of Indonesia is currently conducting. It compiles, analyzes and interprets available information on Indonesia's health service providers; doctors, midwives and nurses. Within the limitations imposed by questions about the accuracy and timeliness of current workforce data, the paper describes the stock and distribution of health workers. The paper draws attention to weaknesses in the workforce planning methods in use, and then reviews the human resource policies, including governance structures and the regulatory framework, that affect health workers. It concludes by describing future challenges and some suggested ways of addressing these challenges. In brief, this paper identifies a number of main challenges in the health workforce in Indonesia. The current study seeks to investigate expert’s testimony in the context of the resolution of judicial demands on the civil liability of doctors. For this purpose, the research first focuses on relevant theories and raises the question of expert testimony and its relationship to the other evidence permitted under law in civil lawsuits, with an emphasis on the principle of rational persuasion of the judge. Thereafter, the research focuses on the jurisprudential analysis of the Court of Justice of Minas Gerais State in Brazil. OBJETIVO: Descrever as relações entre médico prescritor, advogado e indústria farmacêutica em ações judiciais contra o Estado. MÉTODOS: Estudo descritivo retrospectivo com base nas informações dos expedientes administrativos dos processos judiciais com demandas por medicamentos contra o Estado de Minas Gerais movidos entre outubro de 1999 e outubro de 2009. As variáveis estudadas foram: sexo, idade e doença dos beneficiários das ações, origem do atendimento médico (público ou privado), médico prescritor, tipo de representação jurídica e medicamento solicitado. Foi realizada análise descritiva das variáveis com a distribuição de frequências. RESULTADOS: Foram analisadas 2.412 ações judiciais referentes a 2.880 medicamentos solicitados, com 18 fármacos diferentes. Entre esses, 12 são fornecidos pelas políticas de assistência farmacêutica do Sistema Único de Saúde (SUS). Os medicamentos mais solicitados foram adalimumabe, etanercepte, infliximabe e insulina glargina. As principais doenças dos beneficiários foram artrite reumatóide, espondilite anquilosante, diabetes mellitus e doenças pulmonares obstrutivas crônicas. Diferentes pesquisas já identificaram dificuldades, entre médicos, de lidar com o morrer humano, mas tais dificuldades estariam associadas a formas mais amplas e coletivas de lidar com o tema. O presente estudo buscou explorar tal perspectiva investigando grupos fora do âmbito dos profissionais da saúde. Dois grupos, cariocas e mineiros, foram entrevistados sobre suas visões e atitudes em relação ao tema morte. Todos na faixa etária 50-60 anos, com nível superior de escolaridade e padrão socioeconômico e cultural semelhantes e sem atuação em áreas relacionadas à saúde. A pesquisa foi de natureza exploratória e a metodologia de análise foi a do discurso do sujeito coletivo - (DSC) que se baseia na teoria da representação social. Os resultados sugeriram diferenças locais e de gênero. Os mineiros incluíram laços familiares e de amizade como elementos da qualidade de vida. Entre as mulheres (cariocas e mineiras) houve referências frequentes a elementos familiares como pai, mãe, avó e tia. No conjunto, cariocas e mineiros expressaram um padrão geral: preferem não pensar na morte, escolhem pensar na qualidade de vida para envelhecer bem. Monitoramento de terminologia na mídia: o Programa Mais Médicos; Terminology monitoring in the media: the Program More Doctors

No presente estudo, analisa-se a cobertura da Folha de São Paulo em relação ao Programa Mais Médicos, no período de 28 de janeiro de 2013 a 30 de junho de 2014. Utiliza-se como método o monitoramento de 28 termos-chave extraídos dos objetivos da Lei no 12.871, de 22 de outubro de 2013, que instituiu o programa. Verificou-se que apenas dez dos 28 termos-chave estão entre os 100 lexemas mais frequentes no corpus. O monitoramento dos termos demonstrou-se capaz de comprovar o distanciamento da cobertura do jornal em relação aos objetivos da lei que instituiu o Programa Mais Médicos.; In the present study we analyze the coverage of Folha de São Paulo regarding the Program More Doctors, from January 28th 2013 to June 30th 2014. The method used was the monitoring of 28 key terms extracted from the objectives of the Brazilian law nr. 12.871, of October 22nd 2013, which established the Program. It was found that only 10 of the 28 key terms are among the 100 most frequent lexemes in the corpus. The monitoring of terms proved to be able to show the distance of newspaper’s coverage in relation to the objectives of the law which established the Program in Brazil.
