Perfil para medicamentos na forma farmacêutica comprimidos contendo uma substância ativa pode ser obtido usando diferentes conjuntos de propriedades, incluindo aspectos físicos e químicos. As primeiras medições realizadas em comprimidos são de características físicas, também chamadas características póscompressão. Tais dados podem ser valiosos para auxiliar na detecção de falsificações de medicamentos e ser utilizados em uma perspectiva de inteligência forense, quando inseridos em bancos de dados. Este trabalho está focado nas características físicas dos comprimidos de Cialis® e Viagra® apreendidos pela Polícia Federal no Estado brasileiro do Rio Grande do Sul. Com o emprego do Teste de Fisher (ANOVA), todas as amostras falsificadas de Viagra® (n = 28) e de Cialis® (n = 40) foram diferenciadas das amostras autênticas pelas seguintes características pós-compressão: comprimento (maior e menor), espessura e massa. INTRODUCTION: The emergence of drug resistance is one of the main problems concerning malaria treatment. The use of counterfeit and/or substandard antimalarial drugs can contribute to the development of parasite resistance. Thus, the aim of this study was to evaluate the quality of antimalarial drugs distributed in Brazil. METHODS: Samples containing chloroquine phosphate, mefloquine hydrochloride, primaquine phosphate, and quinine sulfate tablets were delivered to the Rio de Janeiro central storeroom (CENADI), state storerooms (SS), and Basic Health Units (BHUs) in the north region of Brazil - a total of 10 sample sets. After 5 months of storage, the samples were collected, and in vitro quality control analyses according to official and published methods were performed. RESULTS: Inadequate drug storage conditions were found in two SS and in all BHUs evaluated. There were no quality deviations found in the chloroquine samples. The quinine samples exhibited weight variation above the allowed limits. The primaquine samples were found to have packaging deficiency. The release of mefloquine in samples from some regions showed a statistically significant difference when compared with the CENADI samples. CONCLUSIONS: It is important to periodically evaluate the quality and storage conditions of essential drugs. The quality deviations found with the primaquine and quinine samples are not related to storage conditions and must be addressed urgently. The profile of tablets containing an active pharmacological ingredient can be obtained using different sets of properties, including physical and chemical aspects. The first measurements carried out on tablets are the physical characteristics also called post-tabletting batch (post-TB) characteristics. These data may be valuable to assist in the detection of pharmaceutical product forgery and may also be used in a forensic intelligence perspective when inserted into databases. This work is focused on the physical characteristics of Cialis® and Viagra® tablets seized by the Brazilian Federal Police in the Rio Grande do Sul state. Using the F-test (ANOVA), all samples of counterfeit Viagra® (n = 28) and Cialis® (n = 40) were well distinguished from authentic samples by the following post-TB characteristics: length (major and minor), thickness, and mass. Using the exploratory statistical technique of Hierarchical Cluster Analysis (HCA), tablets with similar physical profiles were grouped. This result may indicate a common illicit production. We observed the validity of using post-TB properties to generate - in a fast and reliable manner and with no sample preparation - a technological profile that joins itself to the other analytical methods assisting in routine forensic detection of counterfeit Viagra® and Cialis®.

Counterfeit drugs are inherently dangerous and a growing problem; counterfeiters are becoming increasingly sophisticated. Growth of the counterfeit medication market is attributable in part to phosphodiesterase type 5 inhibitor (PDE5i) medications for erectile dysfunction (ED). Millions of counterfeit PDE5is are seized yearly and account for the bulk of all counterfeit pharmaceutical product seizures. It has been estimated that up to 2.5 million men in Europe are exposed to illicit sildenafil, suggesting that there may be as many illegal as legal users of sildenafil. Analysis of the contents of counterfeit PDE5is shows inconsistent doses of active pharmaceutical ingredients (from 0% to > 200% of labelled dose), contaminants (including talcum powder, commercial paint and printer ink) and alternative ingredients that are potentially hazardous. In one analysis, only 10.1% of samples were within 10% of the labelled tablet strength. Estimates place the proportion of counterfeit medications sold over the Internet from 44% to 90%. Of men who purchase prescription-only medication for ED without a prescription, 67% do so using the Internet. Growth of international free trade and inadequate drug regulation have led to the expansion of trade in counterfeit drugs worldwide. Technological protection is seen to be the best way to avoid this problem. Different technologies came into existence like overt, covert, and track and trace technologies. This review emphasises ideal technological characteristics, existing anti-counterfeit technologies, and their adoption in different countries. Developed countries like the USA have implemented RFID while the European trend is towards 2D barcodes. The Indian government is getting sensitised about the extent of the problem and has formulated rules mandating barcodes. Even the pharmaceutical companies have been employing these technologies in order to detain illegitimate drugs in their supply chain.

Background Awareness of pharmacists about counterfeit drugs is necessary for health improvement in community. The purpose of the present study is to assess the knowledge and measure the professional attitude and practice of Iranian pharmacist about counterfeit drugs. In August 2008, a knowledge, attitude and practice (KAP) study was performed in a national sample of 794 pharmacists who participated in an Iranian Pharmacist Association congress. A questionnaire was prepared to collect Demographic and professional characteristics, Knowledge, attitude and practice of pharmacists regarding counterfeit drugs. The mean percent of participants who answer each practice questions correctly is 13.62% and none of questions have more than 14.7% of correct answer, while the participants’ attitude towards the subject is at high level. None of demographic factors represented a significant relationship with knowledge and the only related parameters with attitude, were age and gender. Increasing age of pharmacists resulted in attitude improvement (p = 0.013) and women›s attitudes were better than men (p = 0.05).The only related parameters with practice, were the number of working hours per a week and attitude. During this PhD work new analytical methods for the analysis of counterfeit drugs were developed. The Colorimeter is a simple instrument for preliminary and fast screening of suspicious counterfeit drug. It employs an “Eye one” spectrophotometer, that acquires the reflectance spectrum of solid surfaces in the visible; it is portable and very cheap. Since the color both of tablets and of peculiar regions of the packaging are specific to a drug, it is very unlikely that a counterfeit product can imitate them in an accurate way. Thus the reflectance spectrum of a suspicious drug can be compared to that of the original one, obtained through a statistical analysis of a representative amount of original samples. By this comparison it is evaluated if the analyzed drug is probably a counterfeit, hence it needs more in depth analyses, or if it is original. A solid state approach was developed by combining FT-IR, DSC, TGA. This approach is rapid as sample treatments are not necessary. By this approach it is possible to identify the ingredients of an unknown drug, in particular both active substances and excipients can be detected. "Counterfeit Drugs Kill" is the slogan the World Health Organization (WHO) uses in its anti-counterfeiting campaign. International organizations, governments of developed and developing countries, and the pharmaceutical industry created the IMPACT initiative (International Medical Products Anti-Counterfeiting Taskforce) to take on the thriving illegal industry that makes profits by selling fake drugs. However, before committing resources, policy makers want to assess the burden caused by counterfeit drugs in comparison with other health problems that compete for the limited resources available.

Despite the fundamental role of pharmaceuticals, there remains a profound gap between the benefit which pharmaceuticals have to offer and the reality that for millions of people -- particularly poor and disadvantaged people -- medicines are unavailable, unaffordable, unsafe or improperly used. This case study analyses the challenges of establishing a pharmaceutical and medical supplies system in a Timor-Leste post-conflict context. In the aftermath of its separation from Indonesia, the Timor-Leste health infrastructure was in total disarray, with more than a third of health facilities destroyed, and those remaining severely damaged. The crisis in human resources was severe, as more than 80 percent of qualified public-sector staff had returned to Indonesia. The resultant heavy dependence on expatriates was complicated by language incompatibility, and nationals were not well integrated into planning and implementation processes, as an entire public sector infrastructure was being established de nouveau. There is a strong political will in the Western Balkan states to align the region's pharmaceutical legislation and practice with that of the European Union. Accordingly, recent policy changes were aimed at harmonization of policies with other European countries. Several national drug laws were updated or completely re-written in the last several years. More specifically, provisions were made for the simplification of drug registration requirements, licensing of professionals and businesses in the sector, implementation of ethics standards, price controls and reimbursement of drugs through national health insurance systems. Countries in the region have introduced various measures for cost containment, mostly through positive lists with various co-payment levels or expenditure caps for prescribing physicians. Some institutional buyers are using pooled procurement with open tenders to ensure lower prices. O direito ao acesso a medicamentos encontra respaldo na Constituição Federal brasileira, isto porque o direito à vida, à saúde, à dignidade da pessoa humana, e ao desenvolvimento são princípios constitucionais, possuindo origem e não poderia ser diferente na própria Constituição. De outro modo, o inciso XLIII do art. 5º da Carta Magna, determina que haja tratamento penal mais severo à prática de crimes hediondos, dentre outros. A falsificação de medicamentos relaciona-se com desvios de qualidades, produtos sem registro ou com registro vencido. A má distribuição de riqueza o acesso aos cuidados de saúde, aliados aos avanços tecnológicos industriais e da informação possibilitam uma nova área de expansão de negócios ilícitos no comércio de medicamentos. Para evitar a ocorrência dessa máfia de medicamentos os órgãos de fiscalização conjuntamente editam novas leis visando frear o combate à falsificação de medicamentos. Relacionado a esse tema está a Anvisa que tem fiscalizado veementemente desde a fabricação, a prevenção, e a distribuição destes produtos. A ocorrência de casos relacionados com medicamentos falsificados são comuns e muitas vezes levam aos que necessitam dos remédios à morte. Muitas são as formas de detectar a existência da ilegalidade do medicamento. The counterfeit medicines discussion is an example of how the use of a turbid rationale for greater intellectual property protections serves sophisticated private interests while potentially harming the public interest. The risk of harm created by counterfeit medicines provides a compelling counter-narrative to the access to medicines critique of intellectual property rights. Intellectual property advocates and the pharmaceutical industry have portrayed poor global enforcement of intellectual property rights as contributing to the proliferation of dangerous counterfeit medications. Yet, the deliberate linkage in the literature between weak intellectual property rights and the harms caused by counterfeit medicines provides a justification for new international treaties, such as the recent Anti-Counterfeiting Trade Agreement, that require increased government enforcement of intellectual property rights, even where the public interest justifications are relatively weak. The counterfeit medicines narrative gives private industry a public interest rationale instead of a profit-oriented rationale for demanding government enforcement of private intellectual property rights. Depuis quelques années, une nouvelle criminalité est en pleine expansion, celle de la contrefaçon de biens. Plusieurs études ont démontré que la contrefaçon de médicaments plus particulièrement est très importante, et cela à travers le monde. Les consommateurs ne sont pas à l’abri de ces contrefaçons et plusieurs conséquences sur leur santé ainsi que sur l’économie des populations touchées se font voir. Ce fléau est aujourd’hui peu connu et peu développé par les chercheurs, nous trouvions donc pertinent et nécessaire d’effectuer une étude exploratoire afin d’en apprendre davantage sur ce type particulier de criminalité. Nous avons une chance rare d’avoir accès à des données de saisies de médicaments contrefaits analysées en laboratoire par une entreprise pharmaceutique. Ces médicaments contrefaits ont été saisis sur une période de 8 ans, dans 35 pays différents, des analyses de réseau, spatiales et temporelles ont donc pu être effectuées. C’est la première fois qu’une étude sur ce type de criminalité est basée sur autant de saisies, c’est-à-dire 391 saisies. Although drug counterfeiting is a major issue in many regions of the world, the U.S. drug supply is generally safe due to extensive federal and state oversight and steps taken by drug manufactures, distributors, and pharmacies to prevent drug counterfeiting. The U.S., however, now faces an increasing counterfeit drug threat due to advances in counterfeiting technology and increased financial incentives for criminals to introduce counterfeit drugs into the U.S. drug supply. The first half of this paper describes the threat posed by counterfeit drugs, discusses the scope of the counterfeit drug problem, explains how counterfeit drugs may be introduced into the U.S. drug supply, and discusses past efforts taken by U.S. authorities to secure the drug supply. The second half of this paper analyzes the FDA’s 2004 recommendations on how to best fight the counterfeit drug threat, addressing the merits of the FDA’s recommendations as well as the FDA’s success in implementing such recommendations thus far.

Pharma frauds are on the rise, counterfeit drugs are giving sleepless nights to patients, pharmaceutical companies and governments. The laws prohibiting the sales of counterfeit drugs cannot succeed without technological interventions. Several analytical techniques and tools including spectroscopy, holograms, barcoding, differentiated packing, radio frequency identification, fingerprints, hyperspectral imaging etc. have been employed over the years in combating this menace; however this challenge is becoming more sophisticated with the evolution of the World Wide Web and online pharmacies. This paper presents a review on the contribution of Information and Communication Technology (ICT) as a drug counterfeit countermeasure.

Objetiva-se neste trabalho descrever as normas federais que regulam a cadeia farmacêutica e os mecanismos previstos para combate da falsificação, avaliar os medicamentos falsificados no período de 2005 a 2009 e especialmente delinear o escopo da Lei n. 11.903/09 e as principais implicações na cadeia mediante pesquisa bibliográfica e aplicação de questionário a associações de indústrias farmacêuticas. Foi observado neste trabalho um aumento no número de lotes de medicamentos falsificados no período de 2005 a 2009 no Brasil. A inclusão do mecanismo de autenticidade (etiqueta de segurança) e de rastreabilidade (código bidimensional) em todas as unidades de medicamentos prevista pela Lei n. 11.903/09 pode reduzir o número de casos de medicamentos falsificados, entretanto, outras medidas devem ser realizadas concomitantemente para o combate do mercado ilegal, tais como a fiscalização contínua e conscientização da população sobre os riscos de medicamentos falsificados. A implantação do novo sistema acarretará em aumento de gastos em todos os atores da cadeia farmacêutica, em especial na indústria farmacêutica, principalmente, se for ratificada a inclusão da etiqueta. OBJETIVO: Identificar os principais medicamentos falsificados apreendidos pela Polícia Federal brasileira e os estados em que houve a apreensão. MÉTODOS: Estudo retrospectivo descritivo dos laudos periciais elaborados por Peritos Criminais da Polícia Federal sobre medicamentos contrafeitos entre janeiro de 2007 e setembro de 2010. RESULTADOS: Os medicamentos com maior número de apreensões foram inibidores seletivos da fosfodiesterase 5, utilizados para tratar a disfunção erétil masculina (Cialis® e Viagra®, em média 66%), seguidos por esteróides anabolizantes (Durateston® e Hemogenin®, 8,9% e 5,7%, respectivamente). A maior parte dos medicamentos falsos foi apreendida nos estados do Paraná, São Paulo e Santa Catarina, com incremento superior a 200% no número de medicamentos inautênticos encaminhados à perícia no período. Houve aumento nas apreensões de medicamentos contrabandeados arrecadados em conjunto com os falsos; 67% das apreensões incluíram no mínimo um medicamento contrabandeado. CONCLUSÕES: A falsificação de medicamentos é um grave problema de saúde pública. A identificação das classes de medicamentos falsos no País e os principais estados brasileiros com essa problemática podem facilitar ações futuras de prevenção e repressão pelos órgãos brasileiros responsáveis.; OBJETIVO: Identificar los principales medicamentos falsificados incautados por la Policía Federal Brasileña y los estados donde hubo incautación.
