Quality cancer treatment depends upon careful coordination between multiple treatments and treatment providers, the exchange of technical information, and regular communication between all providers and physician disciplines involved in treatment. This article will examine a particular type of organizational structure purported to regularize and streamline the communication between multiple specialists and support services involved in cancer treatment: the multidisciplinary treatment care (MDC) team. We present a targeted review of what is known about various types of MDC team structures and their impact on the quality of treatment care, and we outline a conceptual model of the connections between team context, structure, process, and performance and their subsequent effects on cancer treatment care processes and patient outcomes. Finally, we will discuss future research directions to understand how MDC teams improve patient outcomes and how characteristics of team structure, culture, leadership, and context (organizational setting and local environment) contribute to optimal multidisciplinary cancer care.

Background: Placebo groups are used in randomised clinical trials (RCTs) to control for placebo effects, which can be large. Participants in trials can misunderstand written information particularly regarding technical aspects of trial design such as randomisation; the adequacy of written information about placebos has not been explored. We aimed to identify what participants in major RCTs in the UK are told about placebos and their effects. Methods and Findings: We conducted a content analysis of 45 Participant Information Leaflets (PILs) using quantitative and qualitative methodologies. PILs were obtained from trials on a major registry of current UK clinical trials (the UKCRN database). Eligible leaflets were received from 44 non-commercial trials but only 1 commercial trial. The main limitation is the low response rate (13.5%), but characteristics of included trials were broadly representative of all non-commercial trials on the database. 84% of PILs were for trials with 50∶50 randomisation ratios yet in almost every comparison the target treatments were prioritized over the placebos. Placebos were referred to significantly less frequently than target treatments (7 vs. 27 mentions, p<001) and were significantly less likely than target treatments to be described as triggering either beneficial effects (1 vs. 45...

Background: Patient-provider communication is a major challenge in resource-limited settings with large catchment areas. Though mobile phone usership increased 20-fold in Africa over the past decade, little is known about acceptability of, perceptions about disclosure and confidentiality, and preferences for cell phone communication of health information in the region. Methods: We performed structured interviews of fifty patients at the Immune Suppression Syndrome clinic in Mbarara, Uganda to assess four domains of health-related communication: a) cell phone use practices and literacy, b) preferences for laboratory results communication, c) privacy and confidentiality, and d) acceptability of and preferences for text messaging to notify patients of abnormal test results. Results: Participants had a median of 38 years, were 56% female, and were residents of a large catchment area throughout southwestern Uganda. All participants expressed interest in a service to receive information about laboratory results by cell phone text message, stating benefits of increased awareness of their health and decreased transportation costs. Ninety percent reported that they would not be concerned for unintended disclosure. A minority additionally expressed concerns about difficulty interpreting messages...

Technical capabilities for improved communication, surveillance, and navigation (CNS) over the oceans are currently available. However, all aircraft operators will not equip simultaneously because of the high costs required. Consequently, as these CNS systems are integrated into oceanic air transportation architecture, the controller will have to manage the current low frequency surveillance and communication paths in parallel with future enhanced CNS. The cognitive effects of the mixed equipage environment were studied through field studies and experimental analysis. Field studies at New York Center, Oakland Center, and Reykjavik Center in Iceland were conducted to identify human-centered systems issues with the emerging mixed equipage environment. Findings show that the integration of varying communication latencies influences controller planning. The fusion of multiple surveillance sources and the application of varying separation standards based on equipage was found to limit the cognitive processes of the controller. These limitations may constrain the controller from providing full efficiency benefits to aircraft equipped with the highest capabilities, which would reduce the incentives for equipping. Experimental analysis was conducted to further study the integration of high and low frequency surveillance and the use of varying separation standards. Results show that workload increases and situation awareness degrades in the mixed surveillance environment...

The purpose of this sourcebook is to pull together into a single document a collection of common sense tips and recommendations based on actual practices and experience around the world. The sourcebook aims first and foremost to help strengthen Monitoring and Evaluation (M&E) capacity at the national and sub-national levels, and to ensure a consistency of approach and methodology so that, at the global level, sufficient reliable and timely information can be accessed from the different countries and used to make cross-country comparisons and to calculate development indicators at the global level. The sourcebook is specifically targeted towards countries where conditions are less-than-ideal, particularly with respect to the availability of relevant information. The sourcebook also shows how a service delivery approach can be used to select indicators which can generate useful, easy-to-measure early outcome measures. It suggests that greater use be made of qualitative indicators, such as access, use and satisfaction. The sourcebook devotes considerable attention to the need for a strong statistical infrastructure and reviews the range of different statistical instruments available.

THE COMMUNICATION OF TECHNOLOGICAL RISK HAS BECOME A SUBJECT OF RESEARCH INTEREST BECAUSE OF INDISPUTABLE PRACTICAL PROBLEMS IN THE EXCHANGE OF TECHNICAL INFORMATION ON MATTERS OF HEALTH AND SAFETY BETWEEN DIFFERENT GROUPS IN SOCIETY. AN IMPORTANT ASSUMPTION IS IMPLICIT IN MUCH OF THE WORK IN THE FIELD: THAT THE EFFECTIVENESS OF COMMUNICATIONS CAN BE JUDGED BY THE EXTENT TO WHICH THEY LEAD TO A RESOLUTION OF PERCEPTUAL DIFFERENCES AND CONFLICTS OF OUTLOOK BETWEEN DIFFERENT SOCIAL GROUPS. THIS PAPER ARGUES THAT, JUDGED BY SUCH A CRITERION, SUCCESSFUL RISK COMMUNICATION IS FUNDAMEN- TALLY UNATTAINABLE, BECAUSE SOCIAL CONFLICT IS A SIGN OF VALUE DIFFERENCES AND CONFLICTS OF INTEREST WITHIN SOCIETY. PAST RESEARCH IN RISK PERCEPTION AND COGNITIVE SCIENCE, AS WELL AS BASIC DEMOCRATIC ETHICS INDICATES THAT THE EFFECTIVENESS OF RISK COMMUNICATIONS SHOULD BE JUDGED IN A DIFFERENT MANNER; THAT IS, BY THE EXTENT TO WHICH THEY SATISFY THE NEEDS OF THEIR AUDIENCE IN RESOLVING UNCERTAINTIES ABOUT HOW TO BEHAVE IN THE FACE OF PERCEIVED THREATS; NA-NOT AVAILABLE

This report documents a program for the numerical solution of large sparse systems of algebraic and implicitly defined stiff differential equations. The principal use is intended to be the solution of differential equations arising from time dependent partial differential equations when the finite element method is used to discretize the space domain. The use of compact matrix storage techniques and iteration for the solution of the quasi-Newton iterates in Gear's method makes the program extremely efficient both in terms of storage requirements and execution times.; http://archive.org/details/programfornumeri00fran; N0001476WR60052

This short country report, a result of larger Information for Development Program (infoDev)-supported survey of the Information and Communication Technologies (ICT) in education in Africa, provides a general overview of current activities and issues related to ICT use in education in the country. The penetration levels of ICTs in Zambia's education institutions remains low, with those schools that are equipped mostly utilizing second-hand and refurbished computers. The integration of ICTs in learning and teaching practice has been limited, although the introduction of computer studies as a school study subject has begun to change this. The recent adoption of a national ICT policy, as well as the development of a draft ICT policy for education and an associated implementation framework, provides an enabling policy environment to promote far greater access and use of ICTs across all sectors of Zambia's education system, including a system for enhancing education management, administration, and teaching and learning. While the goals and targets set in these policy documents seem realistic...

The present work investigates how the processes of production and diffusion of technical and scientific information occur among specialists in the assistance area aiming to contribute to the construction of knowledge in Oncology. The main interest is to know specialists information needs and uses and to understand how they cope with existing knowledge and means of communication available. In other words, how this effort is viewed in terms of information flow towards knowledge generation. In this attempt, the concept of information is linked to the institutional culture context to give a better understanding of how social practices among professional specialists in palliative care influence knowledge sharing. The study was developed in two stages. The first one was towards designing information behavior of INCA specialists. The second was intended to describe how information transfer happens among specialists of palliative care in their assistance practices. It is found high interrelation among specialists in relation to problems solving on routine activities in assistance practices. The specialists develop an efficient communication network to obtain and diffuse information aiming knowledge production to solve specific problems. It was also founded that there is a gap between informal and formal communication of technical and scientific information originated by assistance practices. This needs to be improved at institutional basis in order to increase knowledge production in Oncology; O presente trabalho investiga como os processos de produção e difusão da informação técnica e científica ocorrem entre os profissionais especialistas da área assistencial...

Developing effective instructions for complex, sequential tasks is a challenging information design problem. The focus of this thesis is on the non-verbal, visual explanations. This thesis attempts to define a set of instructional communication standards necessary to develop highly functional graphic design solutions to complex instructional information. These standards are then applied to free-weight instructions to improve their clarity and usability. Free weights, or dumbbells, are heavy-weight, metal bars that are lifted in repetitive steps to gain or tone muscles through resistance. When attempting to use currently available free-weight instructions, most people are unsure how to interpret them. This can lead to unnecessary confusion, and even injury in some cases. An established set of standards that simplifies and clarifies sequential visual explanations will result in less confusion and fewer injuries. It is hoped that these standards for instructional design will be useful to a wide range of similar design problems where complex, sequential instructions need to be conveyed with clarity, simplicity and grace.
