Nutritional status and lipid profile of HIV-positive children and adolescents using antiretroviral therapy

OBJECTIVE: To describe nutritional status, body composition and lipid profile in children and adolescents receiving protease inhibitors. METHODS: Fifty-nine patients, 23 treated with protease inhibitors (group 1) and 36 not using protease inhibitors (group 2). Their dietary intake, anthropometry, bioimpedance analysis and lipid profile variables were measured. RESULTS: There was no difference in nutritional status or body composition between groups at the beginning of the study. After 6 months of follow-up, there was an increase in weight and height in both groups, as well as in waist circumference and subscapular skinfold thickness. In group 2, body mass index and triceps skinfold thickness adequacy were significantly higher after 6 months of follow-up. The groups had similar energy and macronutrient intake at any time point. After 6 months, group 1 had a higher cholesterol intake and group 2 had a higher fiber intake. Triglyceride serum levels were significantly different between the groups, with higher values in G1, at any time point [G1: 153 mg/dl (30-344); 138 (58-378) versus G2: 76 mg/dl (29-378); 76 (29-378)]. Introdução O reconhecimento da violência doméstica contra crianças e adolescentes no Brasil se deu no fim da década de 80, com o estabelecimento do ECA, que tornou obrigatória a notificação, mesmo dos casos suspeitos, numa medida de proteção aos direitos das crianças e dos adolescentes. Ainda que tenha sua magnitude desconhecida, é apontada como um fenômeno crescente em todo o mundo e seu conhecimento encontra-se em processo de construção. Em função das graves conseqüências que acarreta às vítimas e às suas famílias, seu estudo se justifica, no intuito de dar visibilidade ao tema, a partir dos casos atendidos no Instituto Médico Legal de Fortaleza. Objetivos - Analisar as características da violência doméstica contra crianças e adolescentes atendidos no Instituto Médico Legal (IML) de Fortaleza, visando contribuir para a formulação de políticas públicas de enfrentamento deste problema na população estudada, a partir de estratégias apropriadas de prevenção e controle. Esta pesquisa de abordagem qualitativa teve por objetivo investigar como os Centros para Crianças e Adolescentes (CCAs) vêm tratando a questão da inclusão do público com deficiência em seus espaços. Nas suas ações, a educação não formal figura como perspectiva educativa subjacente às ações de proteção social, visando à promoção do desenvolvimento de competências sociais, cognitivas e afetivas, marcadas por valores de inclusão e protagonismo social, complementando, assim, o processo educacional escolar num conceito ampliado de educação. Nessa lógica, os CCAs podem ser considerados potencialmente qualificados como instituições aptas a desenvolver ações socioeducativas com crianças e adolescentes com deficiência, inclusive em complementaridade à educação escolar, indicando oportunidades para que a abordagem da deficiência seja refletida e reedificada em ambos os espaços. Os procedimentos metodológicos para a coleta de dados foram: entrevistas semiestruturadas com doze profissionais de três CCAs eleitos como campo de pesquisa, análise de documentos e observação participante; ainda, a análise dos dados das entrevistas referenciou-se na análise de conteúdo. A presente pesquisa teve como objetivo identificar e analisar as representações sociais de violência contra crianças e adolescentes construídas por educadores que denunciaram casos de violência doméstica ao Conselho Tutelar. O fenômeno pode ser definido como todo ato ou omissão praticado por pais, parentes ou outros responsáveis contra crianças e adolescentes, capaz de causar dano físico, sexual e/ou psicológico à vítima. O estudo referenciou-se epistemologicamente na teoria e método de análise das Representações Sociais (Moscovici), que se baseia na análise das práticas discursivas dos sujeitos da investigação. Os dados foram coletados por meio de entrevistas individuais semiestruturadas, realizadas com 12 educadores da rede municipal de educação da cidade de Uberlândia/MG. Violence against children and adolescents, ranging from negligence to sexual abuse, is an imperative public health problem. The ill treatments are considered as nonaccidental traumas, actions or omissions against children, who suffer physical or emotional violence. The main perpetrators are fathers, mothers, guardians, family, friends or the children‘s primary caretaker. It is very important to identify abuse cases, because this allows proper medical and legal intervention. In Brazil, the law n. 8.069, of July 13, 1990, known as the ECA -Statute for Children and Adolescents (SCA), stipulates the obligation to report suspected or confirmed cases of ill treatment against a child or adolescent to the Guardianship Council of the respective locality. The health professionals play a crucial role with regard to identifying cases of violence, collecting information, making early diagnosis of suspect cases and reporting such cases to the authorities. The dentists can contribute significantly, as most injuries occur in the orofacial region. Bite marks, burns, bruising, among others, are easily identified during a dental consultation. Objective To conduct a systematic review about the long-term response to cognitive-behavioral therapy (CBT) for anxiety disorders (ADs) in children and adolescents. Methods The PubMed and ISI Web of Science databases were consulted. Search in the databases was performed in November 2012 and included cohort studies after CBT for ADs in children and adolescents with a follow-up period over 12 months. Results A total of 10 papers met the inclusion criteria. The follow-up period ranged from 12 months to 13 years and the results generally showed maintenance of the short-term benefits with CBT. However, the studies presented limitations, especially regarding methods, such as lack of a control group and losses to follow-up. Conclusion The long-term benefits of CBT were identified, however it would be interesting to conduct other studies with more frequent assessment periods, in order to minimize losses to follow-up, in addition to evaluating children and adolescents in the various stages of their development.

OBJECTIVE - To assess the diagnostic value, the characteristics, and feasibility of tilt-table testing in children and adolescents. METHODS - From August 1991 to June 1997, we retrospectively assessed 94 patients under the age of 18 years who had a history of recurring syncope and presyncope of unknown origin and who were referred for tilt-table testing. These patients were divided into 2 groups: group I (children) - 36 patients with ages ranging from 3 to 12 (mean of 9.19±2.31) years; group II (adolescents) - 58 patients with ages ranging from 13 to 18 (mean of 16.05±1.40) years. We compared the positivity rate, the type of hemodynamic response, and the time period required for the test to become positive in the 2 groups. RESULTS - The positivity rates were 41.6 % and 50% for groups I and II, respectively. The pattern of positive hemodynamic response that predominated in both groups was the mixed response. The mean time period required for the test to become positive was shorter in group I (11.0±7.23 min) than in group II (18.44±7.83 min). No patient experienced technical difficulty or complications. CONCLUSION - No difference was observed in regard to feasibility, positivity rate, and pattern of positive response for the tilt-table test in children and adolescents. Pediatric patients had earlier positive responses.

Subjective well-being has been defined as an association among positive affect, negative affect, and life satisfaction. This study aimed to investigate the subjective well-being of children and adolescents attending school and living with their families as compared to those living in youth offenders institutions. Two hundred ninety-seven children and adolescents in situations of social vulnerability aged between seven and sixteen years old (M = 11.22 years, SD = 2.13), 155 male (52.2%) and 142 female (47.8%) participated in the study. The instruments were a structured interview, a positive and negative affect scale, and a multidimensional life satisfaction scale. The results confirmed that institutionalized children/adolescents experienced more negative affect regarding development contexts. However, institutionalized children/adolescents did not differ from those living with their families in terms of life satisfaction and positive affect.

The magnitude of mental health problems in children and adolescents has not been recognized sufficiently by many governments and decision-makers. This paper reviews the epidemiology of these problems as a basis for planning of services; the situation of mental health services for children and adolescents in the various regions of the world; the principles and strategies of intervention for mental health disorders in children and adolescents; and the role of international organizations and advocacy groups. It is concluded that old myths, treatments and policies are no longer to be tolerated and that there is now the opportunity to develop and implement evidence-based interventions, modern training programs and effective policies.

Obesity is associated with autonomic nervous system dysfunction. The aim of the study was to evaluate baroreflex sensitivity, an indicator of autonomic nervous function, in 20 obese children and adolescents in comparison with 20 age- and sex-matched nonobese subjects. All subjects were examined in the supine position over a period of 50 min. Systolic blood pressure (SBP) and RR intervals were monitored continuously. Baroreflex sensitivity was assessed by cross-spectral analysis of SBP and RR interval oscillations (BRS index) and SBP and heart rate oscillations (BRSf index) within the low frequency range (0.04-0.15 Hz). Sensitivity was determined in 3 time intervals of 3 min each to evaluate changes during rest. The BRS index was significantly lower in obese children and adolescents than in the nonobese control group (p = 0.002). Significant changes in the BRS index over time (p = 0.004) were found only in nonobese subjects. In contrast, the BRSf index increased over time in both groups (p = 0.01), and no significant between-group difference was detected. In conclusion, obese children and adolescents show decreased resting baroreflex sensitivity with less variation compared with nonobese subjects. While there is a growing body of literature on the mental health status of adult refugees, children have been relatively neglected in research, particularly in Australia. This study investigated the prevalence of emotional and behavioural problems and patterns of service utilisation among 530 refugee children and adolescents aged 4–17 years living in South Australia. Parents and teachers of children aged 4–17 years and adolescents aged 13–17 years completed the appropriate versions of the Strengths and Difficulties Questionnaire. Of the 11.0% of children and adolescents found to have borderline or abnormal emotional and behavioural problems, only 13.0% accessed professional help. OBJECTIVE: To analyze the frequency of human leukocyte antigens class II-DR in children and adolescents with systemic lupus erythematosus. PATIENTS AND METHODS: Fifty-fiveBrazilian systemic lupus erythematosus children and adolescents and 308 healthy individuals were studied. Gender, race, and age of onset of systemic lupus erythematosus were recorded. The human leukocyte antigens typing of class II-DR was carried out by polymerase chain reaction amplification with sequence-specific primers (PCR-SSP). Data were analyzed statistically using the chi square test with Yates' correction, Fisher's exact test, and Bonferroni's correction. RESULTS: Human leukocyte antigen-DR 15 was the most frequently detected antigen in this group of children and adolescents, and it also occurred more frequently in the female group, in children with onset of systemic lupus erythematosus between 0 and 9 years and between 10 to 14 years, and in the Black race group, but these associations were not statistically significants. CONCLUSION: In this group of children and adolescents with a high degree of racial admixture, we could not verify a significant association between human leukocyte antigens class II-DR and systemic lupus erythematosus.

The KIDSCREEN is a European cross-cultural and standardized instrument that assesses ten quality of life dimensions in children, adolescents and their parents. This instrument is used to validate evidences to support general inferences on quality-of-life measures obtained by the Portuguese version of KIDSCREEN10 for children and adolescents, in the context of a survey research carried on in Portugal. The present study focuses only on the KIDSCREEN children and adolescents' versions. A sample of 8072 Portuguese children and adolescents attending the 5th grade (19.3%), 6th grade (19.2%), 7th grade (20.3%), 8th (21.6%) and 10th grade (19.7%) in Portuguese schools were inquired, with a mean age of 13.2; SD 2.06, randomly distributed regarding gender. The Portuguese version of KIDSCREEN-10 instrument showed a good Internal Consistency of .78. Chosen fit indexes indicate good fit to the data. Specifically in the final solution the RMSEA was lower than .03 and the upper limit of 90% confidence interval was lower than .05, and CFI was higher than .95. Results indicated that the current 10-item structure is invariant across age groups, nationality and socio-economic level. The KIDSCREEN-10 questionnaire is a reliable instrument to estimate the perception of quality of life in children and adolescents. Psychometric guidelines are presented for the Portuguese population aged 10 to 16 years old.

O trabalho apresenta uma reflexão sobre a abordagem pedagógica utilizada em processos educativos junto às crianças e adolescentes “de e na” rua. OBJETIVO: Analisar a freqüência dos antígenos de histocompatibilidade leucocitária de classe II-DR em crianças e adolescentes com o lúpus eritematoso sistêmico. PACIENTES E MÉTODOS: Cinqüenta e cinco crianças e adolescentes lúpicos brasileiros e 308 indivíduos sadios foram estudados. Os sexos, os grupos étnicos e as idades de início da doença foram anotados. A tipagem de histocompatibilidade leucocitária de classe II-DR foi realizada pela reação de polimerase em cadeia com amplificação de sondas de seqüência específica (PCR-SSP). Na análise estatística foram utilizados o teste de qui-quadrado com correção de Yates, o teste exato de Fisher e a correção de Bonferroni. RESULTADOS: A histocompatibilidade leucocitária-DR15 foi a mais freqüente neste grupo de crianças e adolescentes, sendo também mais freqüente nas mulheres, nas crianças com idade de início da doença entre zero e nove anos e entre 10 e 14 anos e nas crianças de raça negra, mas estas correlações não foram estatisticamente significativas. Validity and reliability of the Spanish sign language version of the KIDSCREEN-27 health-related quality of life questionnaire for use in deaf children and adolescents

Objective: The aim of this paper was to test the validity and reliability of a Spanish sign language (SSL) adaptation of KIDSCREEN-27, a health-related quality of life (HRQoL) questionnaire for use in deaf children and adolescents. Methods: We performed an observational cross-sectional study of 114 deaf children and adolescents aged 8 to 18 years old. The Spanish version of the KIDSCREEN-27 was adapted to SSL through the translation-back translation technique. The adapted questionnaire was then administered using a web tool to ensure complete access to study participants. Floor and ceiling effects were calculated. Structural and cultural validity were tested using exploratory and confirmatory factor analysis. Cronbach´s a was used to assess internal consistency. The questionnaire was administered for a second time to the entire sample after 2 to 4 weeks (test-retest reliability). Results: In the SSL version of the KIDSCREEN-27, as in the original Spanish scale, five dimensions explained 59% of the variance. None of the participants obtained the minimum or maximum scores on the scale (floor and ceiling effect, respectively). Confirmatory factor analysis showed the goodness-of-fit of the factor solution with five dimensions of the SSL version. The Cronbach´s a of both the total scale and of each of the distinct dimensions was above 0.75. The intra-class correlation coefficient of the test-retest scale was considered acceptable in all the dimensions. Conclusions: The reliability and validity of the SSL version of the KIDSCREEN-27 are similar to those of the original Spanish version...

Background and Objectives: Obsessive compulsive disorder (OCD) is a frequent psychiatric disorder. Despite its significant influence on personal development, little is known about its long-term course in children and adolescents. The aim was to follow children and adolescents with OCD for 7 years and to compare patients gaining remission with patients experiencing symptoms. Methods: The study was a prospective follow-up study in which the patients were interviewed 7 years after their initial contact with the healthcare system. The patients had been described at baseline. Results: Among the 95 participants, 53 participants described symptoms at follow-up and 44 participants described remission. More patients with persistent symptoms described an anxious personality trait. The schizoid/compulsive personality traits were represented at a higher rate in the group with persistent symptoms than in the group with remission symptoms. Important predictive factors included predisposition to OCD, the occurrence of comorbid disorders and older referral age. The occurrence of magic obsessions and repetitive compulsions increased the risk of OC symptoms at follow-up. The group “late responders” differed from the non-responder group by predisposition to depression. Objective: The aim of this study was to analyze substance abuse and health self-perception in children and adolescents from the province of Cádiz (Spain). Methodology: Participants were 738 students, 50.9 boys and 49.1% girls, from elementary school to high school (1st to 12th grade, mean age 12.2. years), who responded a Spanish adaptation of the Health Behavior in School-aged Children Inventory. Results: No difference was observed in percentage of boys and girls reporting to smoke or drink alcohol. Smoking and drinking habits increased with age, being higher in 11th and 12th grade students. Significant differences were observed among the percentage of smokers and non smokers recognizing to have parents, older brothers or friends who smoke. Rates of drug abuse were generally low, being cannabis the most frequently consumed illicit drug. Most respondents had a perception of excellent or good health, with no significant gender differences. About half of subjects considered that health exclusively depends on self behavior, but an important percentage also gave a relevant role to luck. Stomach-ache was the most frequent physical complaint, followed by headache. Psychological complaints mainly corresponded to nervousness and bad temper. Only a small percentage of subjects declared the use of medicines or tablets without prescription. No significant difference in the percentage of respondent who perceived an excellent or good health was observed among substance users and non users. Conclusions: Substance abuse and health self-perception in children and adolescents are the result of the interaction of a complex series of individual and social factors. Activities aimed to improve health focusing on partial aspects of adolescents' lifestyle most probably will have limited results...
