Apesar dos estudos epidemiol??gicos informarem melhorias da situa????o de sa??de bucal, observada globalmente, a c??rie dent??ria permanece com altas preval??ncias em quase todas as idades. Nesta tese foi analisado, atrav??s de um estudo ecol??gico e de um estudo de base individual, o cumprimento de duas metas da Organiza????o Mundial de Sa??de para a c??rie dent??ria no ano 2000: o percentual de crian??as livres de c??rie aos cinco anos de idade e o CPO-D aos 12 anos de idade. No estudo ecol??gico o objetivo foi analisar o cumprimento das referidas metas em 80 munic??pios do estado de Minas Gerais Brasil, identificando fatores associados ?? varia????o no CPOD m??dio e na ocorr??ncia de CPOD???3, entre crian??as de 12 anos, e no percentual de livres de c??ries aos cinco anos. ABSTRACT The attention deficit and hyperactivity disorder (ADHD) is the most common developmental disorder among students and may trigger changes across the life course of the individuals and their families. Objectives: To investigate whether ADHD can be considered a risk factor for oral alterations. Methods: The dissertation was developed in two independent studies. In the first study, a review article was developed in which two researchers conducted the searches independently, in the MEDLINE and Web of Science databases. We analyzed all original articles on the subject, regardless the type of the study, published since the beginning of the database to August 2012. In the second study we developed a cross-sectional study with a representative sample of 851 children aged between 7 and 12 years, randomly selected from schools of Diamantina city. Socioeconomic data were collected through questionnaires sent to parents. Considerando-se a aus?ncia de dados sobre a preval?ncia de c?rie dental em uma popula??o tipicamente amaz?nida, foi feito um levantamento epidemiol?gico em 103 escolares na regi?o da Ilha de Sirituba, no Munic?pio de Abaetetuba, Estado do Par?, Brasil. O CPOD m?dio encontrado foi de 6,5 e o ceo de 5,4. Apesar da primitividade do local, o alto ?ndice de c?rie pode estar relacionado a diversos fatores, dentre os quais deve ser inclu?da a influ?ncia urbana na alimenta??o, principalmente pelo a??car, criando novos h?bitos alimentares que alteraram fundamentalmente a dieta da regi?o.; ABSTRACT: In view of the lack of data as to the prevalence of dental caries among typical amazonian populations, the authors carried out an epidemiological study of 103 schoolchildren from Sirituba Island, Abaetetuba, Par?, Brazil. The mean values found for DMFT and deft were 6.5 and 5.4 respectively. Although the region is very primitive, this high prevalence may be linked to several factors, among which is the urban influence, mainly through the use of sugar, on food habits in such a way as fundamentally to alter the diet of the amazonian population.

A infec??o pela Helicobacter pylori ? uma das mais comuns em humanos e apesar de possuir tropismo pelo est?mago, pode ser encontrada na cavidade oral, mantendo uma rela??o comensal com o hospedeiro, enquanto a c?rie dental tamb?m ? uma doen?a infecciosa e resulta do metabolismo da placa bacteriana. Ambas as infec??es apresentam alta preval?ncia em pa?ses em desenvolvimento, pois estas popula??es est?o mais expostas a fatores ambientais de risco, e normalmente s?o adquiridas durante a inf?ncia. Neste estudo foram avaliadas as condi??es de sa?de bucal de diab?ticos acompanhados pelo Programa Sa?de da Fam?lia no munic?pio de Bel?m-Par?. As condi??es analisadas foram: c?rie dent?ria, doen?a periodontal, uso e necessidade de pr?tese dent?ria e les?es de mucosa. A amostra foi composta de 268 diab?ticos e 270 indiv?duos n?o diab?ticos compondo o grupo controle. Para a coleta dos dados, foram utilizados os ?ndices recomendados pela Organiza??o Mundial de Sa?de. Os exames foram realizados nos domic?lios dos indiv?duos selecionados por examinadores previamente calibrados. Os dados foram processados no Programa de computa??o Epi info vers?o 3.3.2 para o Sistema Operacional Windows e estatisticamente analisados. Os resultados demonstraram que: n?o houve diferen?a na preval?ncia de c?rie. O CPOD foi igual a 25,7 e 25,4 para diab?ticos e grupo controle, respectivamente, com uma grande perda dent?ria para ambos os grupos. N??o h?? relatos de levantamentos epidemiol??gicos referentes ??s condi????es de sa??de bucal conduzidos no munic??pio de ??gua Santa, RS, Brasil, que permitam adequar os servi??os de sa??de ?? realidade local. Sendo assim, o estudo objetivou conhecer a preval??ncia de c??rie dent??ria, necessidades de tratamento e fluorose dent??ria na popula????o de escolares de 12 anos de idade. Realizou-se um censo destes escolares, com um total de 91 exames realizados segundo a Organiza????o Mundial da Sa??de em 1997. O CPOD m??dio encontrado foi de 2,73. Os primeiros molares permanentes representaram 69,35% na composi????o do ??ndice CPOD. As necessidades de tratamento existentes foram poucas e de baixa complexidade. A fluorose dent??ria teve baixa preval??ncia com 90,1% dos escolares considerados sem fluorose. M??todo: Cinquenta crian??as com idades entre cinco e oito anos,atendidas na Cl??nica Infantil da Faculdade de Odontologia da Universidade Federal de Pelotas, foram submetidas ao exame cl??nico e radiogr??fico interproximal bilateral realizado pelo ??nico operador treinado, seguindo t??cnica padronizada. Dados demogr??ficos e de higiene bucal foram coletados da entrevista com a m??e. O diagn??stico da imagem radiogr??fica foi conduzido por dois avaliadores calibrados (Kappa 0,85), a partir dos crit??rios de Rolla et al.,1996 modificados. Os resultados foram analisados utilizando de estat??stica descritiva e teste T (??=0.05). AIM: To evaluate pre-school children caries prevalence related to demographic (sex, age), socioeconomic (schooling, family income and kind of school), behavioral (sugar-intake and hygiene) and biological (visible plaque occurrence) factors. METHODOLOGY: Observational cross-sectional study of primary teeth stage children from 1 to 5 years of age enrolled at pre-school level in Pelotas RS. A semistructured questionnaire which focused on demographic, socioeconomic and family behavior related variables was devised for children s parents. Physical exams were performed by three trained and qualified examiners and the dmft (decayed missing filled teeth) WHO index was adopted as a condition diagnosis criterion, including enamel lesions (dmft m) as well as severity dmft m = 0,1-4≥5. RESULTS: Of a total of 570 children examined, 71,4% and 50,9% were caries-free, taking into account the dmft and dmft m indices, respectively. There was an increase in the dmft m index in older children; this also occurred when enamel lesions were included; 38% of the five year old children group were caries-free. The aim of this study was to evaluate the effect of additional etching with phosphoric acid at 35% in the bond strength and Weibull modulus of two adhesive systems applied to sound and affected dentin. Ninety human molars with coronal carious lesion were sectioned to produce flat coronal superficies with effected dentin surrounded by sound dentin, and allocated in six experimental groups: SB-CT: Single Bond?? applied to manufacture s instructions; SB+15: Single Bond?? with additional etching for 15s; SB+30: Single Bond?? with additional etching for 30s; CF-CT: Clearfil SE Bond?? applied to manufacture s instructions; CF+15: Clearfil SE Bond?? with additional etching for 15s; CF+30: Clearfil SE Bond?? with additional etching for 30s. Afterwards, the crowns were built up with a composite resin and, after 24h, mesiondistal sectioned to produce 2 or 3 slices in each substratum. The black stain are dental extrinsic pigmentations that has been associated with low caries risk. This study was divided in two parts, the first was a cross-sectional study with schoolchildren between 8 to 12 years old; the aim was determine the prevalence of black stain on the teeth and his relation with demographic and socioeconomics conditions, oral hygiene habits, presence of plaque and dental caries. 1175 children were included from 20 private and public school of Pelotas??s city. Was performed a questionnaire to parents of schoolchildren, interviews and clinical examinations to the students. In the second part was a case control study performed based on the oral survey in schoolchildren with black stain on the teeth and the control are also students that had participated in the survey, using for each case a control. The objectives were evaluated the association between the use or ferrous sulfate in childhood, the ingestion of foods containing iron ant the water source with the presence of black stain on the teeth; and describe the clinical characteristics of pigmentation, evaluated 56 children (28 case and 28 controls). The orthodontic treatment has been related to caries lesions development, since the devices used are biofilm retentive. This thesis aimed to evaluate the effect of ligation type and type of orthodontic bracket in the mineral loss and biofilm accumulation to these devices. To accomplish this goal, three studies were conducted. The first study consisted in a systematic literature review, where electronic databases (PubMed, Embase, Cochrane Central Register of Controlled Trials, ISI Web of Knowledge, TRIP, Scopus, and SciELO) were searched up to July 2011. Clinical trials or in situ studies that assessed the effect of types of brackets or ligatures on biofilm accumulation and/or enamel demineralization were selected. Non-controlled studies, in vitro studies, or studies not reporting on the established outcomes were excluded. The second study assessed the enamel demineralization around six types of brackets/archwire ligation combinations. The aim of this study was to test the accuracy of three partial protocols of oral examination, assessing only first molars, to estimate dental caries prevalence, severity and its association with risk factors. A representative sample of 1,211 children with 8-12 years of age from 20 public and private schools from urban area of Pelotas, Brazil was assessed. DMFT index was obtained by full mouth examination and three partial protocols: P1 all first molars; P2 e P3 first molars from two random diagonal quadrants (1 and 3, 2 and 4). Socioeconomic, behavioral and oral health variables were also collected. Sensitivity, absolute and relative bias and inflation factor were calculated for each partial protocol using dental caries prevalence (DMFT ??? 1).Risk factors were assessed for all protocols using Poisson Regression for both dental caries prevalence and severity (mean DMFT). Prevalence of dental caries was 32.4% for full mouth examination, 30.2% for P1, 22.2% for P2 and 21.4% for P3. In comparison with full-mouth examination, P1 showed a higher sensitivity [93.1% (CI 95% 91.5; 94.5)]. All protocols showed similar magnitude of association measures for all risk factors investigated, using caries prevalence and mean DMFT. The aim of this study was to perform a systematic review of prevalence of black stains and their association with dental caries. Also, this study aimed to estimate the prevalence of dental black stains from a cohort of children born in 2004 in Pelotas, RS, Brazil, evaluating the association with dental caries. For the systematic review, the following electronic databases were accessed: Cochrane Central Register of Controlled Trials, Web of Science, EMBASE, MEDLINE, PubMed, LILACS, BBO. No language restriction was used and the combinations of the following keywords were used: prevalence, black stain teeth, black tooth stain, and their equivalents in Portuguese. The search was limited to studies in children and adolescents with deciduous and permanent teeth, and all studies showing prevalence of black stains were included. The selection of studies was done by three reviewers independently. The cross-sectional study included a sample of 1129 five years old children belonging to the Pelotas birth cohort. Children were visited to carry out examination and a questionnaire including questions about demographics conditions, habits and behaviors related to oral health. Micro-organisms belonging to the resident oral microflora are organized into complex communities, the biofilms, and are the direct cause of the caries process. It is necessary to understand the degree to which biofilm physicochemical and nutrient environments affect the plaque development in individuals differing in their oral environments. In this context, microcosm biofilm models could be used to answer specific questions related to caries. The aim of this study was to test the hypothesis that microcosm biofilms growth from inoculums of caries free children (CF), childrenwith early childhood caries (ECC) and with severe early childhood caries (S-ECC) would provide the same cariogenic response under regular sucrose exposure. Also, microcosm biofilms formed from saliva of children and their mothers considering cariogenic response and microbiota composition were compared. Three children of each group were selected: CF, ECC and S-ECC and their mothers. The mother-child pairs were examined and the dental status was recorded. Saliva samples were collected and screened for microbial composition. Microcosm plaque biofilms were initiated from saliva, growth on bovine enamel discs in artificial saliva enriched with mucin (DMM) for up to 10 days in 24-well microplates under anaerobic conditions. This study aimed to investigate the prevalence of use and needs of dental prosthesis at age 24 and determinants of needs of dental prosthesis in this population. The study was nested in a population-based birth cohort started in 1982 in Pelotas, Brazil. A representative sample of 720 young adults was evaluated. Dental examinations and interviews were performed at 15 and 24 years old. Use and need of prosthesis was the outcome. Exploratory variables included demographic and socioeconomic (at birth, 15 and 24 years old), oral health and dental service utilization patterns (at ages 15 and 24). Prevalence of outcomes was calculated and Poisson Regression was used to determine risk factors to needs of dental prosthesis. The prevalence of use and needs of dental prosthesis was 2.08% and 29.72%, respectively. Multivariable analysis showed that poorest family socioeconomic trajectory [PR= 1.56 IC95% (1.08-2.26)], lower mother s educational level at children birth (p=0.023), lower family income at children birth [RP= 1.37 (1.01-1.86)], no oral hygiene instruction by the dentist [RP=1.64 IC95% (1.11-2.41)] and caries presence (high DMFT tertile) (p<0,001) were associated with prosthetic treatment needs. This paper assesses clinical/radiographic changes after incomplete carious dentine removal and tooth sealing. 32 teeth with deep caries lesions were studied. The treatment consisted of incomplete excavation, application of Ca (OH)layersealing 2 , for a 6-7 month period, and then restored. Standardized bitewing radiographs were taken after the temporary sealing, and at 6-7, 14-18, and 36-45 month intervals. The digitized images were analyzed: qualitatively in regard to the radiolucent zone (RZ) depth and tertiary dentine (6-7 and 36-45 months) and quantitatively in regard to radiographic density by image subtraction (6-7, 14-18, and 36-45 months). The subtractions were performed in the RZ beneath the restoration and in two control areas (CA). During the 6-7 month period, there was 1 pulp necroses and 1 pulp exposure, and at the 36-45 month interval, 3 fractures and 3 withdrawals. Nine cases showed a decrease in RZ depth in the 6-7 month period and 3 more cases, at the 36- 45 month period. Four cases displayed tertiary dentine. Despite the decline observed in the occurrence of oral problems, studies among schoolchildren has revealed that dental pain is still a problem of high prevalence, and may have a negative impact on quality of life of individuals and society. Considering the relevance of the topic and that there are few epidemiological studies that assess the phenomenon of pain, this cross-sectional study aimed to evaluate the report of dental pain in a school-based sample of children in South Brazil and test the association with socioeconomics, demographic, psychosocial and clinical variables. Also, aimed to investigate the consequences of dental pain on oral health perception and impact on children s daily life. A minimum sample size of 922 children was estimated. The sample, random and stratified, was selected by multistage sampling technique and evaluated in 20 schools, located in Pelotas/RS in 2011.
