The objective of this study was to validate the Piper Fatigue Scale-Revised (PFS-R) for use in Brazilian culture. Translation of the PFS-R into Portuguese and validity and reliability tests were performed. Convenience samples in Brazil we as follows: 584 cancer patients (mean age 57 +/- 13 years; 51.3% female); 184 caregivers (mean age 50 +/- 12.7 years; 65.8% female); and 189 undergraduate nursing students (mean age 21.6 +/- 2.8 years; 96.2% female); Instruments used were as follows: Brazilian PFS, Beck Depression Inventory (BDI), and Karnofsky Performance Scale (KPS). The 22 items of the Brazilian PFS loaded well (factor loading > 0.35) on three dimensions identified by factor analysis (behavioral, affective, and sensorial-psychological). These dimensions explained 65% of the variance. Internal consistency reliability was very good (Cronbach`s alpha ranged from 0.841 to 0.943 for the total scale and its dimensions). Cancer patients and their caregivers completed the Brazilian PFS twice for test-retest reliability and results showed good stability (Pearson`s r a parts per thousand yenaEuro parts per thousand 0,60, p < 0,001). Correlations among the Brazilian PFS and other scales were significant, in hypothesized directions, and mostly moderate contributing to divergent (Brazilian PFS x KPS) and convergent validity (Brazilian PFS x BDI).

Microbial ecology and chemical composition of Brazilian kefir beverage was performed. The microorganisms associated with Brazilian kefir were investigated using a combination of phenotypic and genotypic methods. A total of 359 microbial isolates were identified. Lactic acid bacteria (60.5%) were the major isolated group identified, followed by yeasts (30.6%) and acetic acid bacteria (8.9%). Lactobacillus paracasei (89 isolates), Lactobacillus parabuchneri (41 isolates), Lactobacillus casei (32 isolates), Lactobacillus kefiri (31 isolates), Lactococcus lactis (24 isolates), Acetobacter lovaniensis (32 isolates), Kluyveromyces lactis (31 isolates), Kazachstania aerobia (23 isolates), Saccharomyces cerevisiae (41 isolates) and Lachancea meyersii (15 isolates) were the microbial species isolated. Scanning electron microscopy showed that the microbiota was dominated by bacilli (short and curved long) cells growing in close association with lemon-shaped yeasts cells. During the 24 h of fermentation, the protein content increased, while lactose and fat content decreased. The concentration of lactic acid ranged from 1.4 to 17.4 mg/ml, and that of acetic acid increased from 2.1 to 2.73 mg/ml. The production of ethanol was limited, reaching a final mean value of 0.5 mg/ml.

Nine Brazilian scientists with an outstanding profile of international publications were invited to publish an original article in the same issue of a Brazilian Journal (Anais da Academia Brasileira de Ciências). The objective was to measure the impact of the papers on the number of citations to the articles, the assumption being that these authors would carry their international prestige to the Brazilian periodical. In a 2-year period there was a larger number of citations of these articles compared to others published in the same journal. Nevertheless, the number of citations in Brazilian journals did not equal the number of citations obtained by the other papers by the same authors in their international publications within the same 2-year period. The reasons for this difference in the number of citations could be either that less significant invited articles were submitted or that it was due to the intrinsic lack of visibility of the Brazilian journals, but this could not be fully determined with the present data. Also relevant was a comparison between the citations of Brazilian journals and the publication in Brazilian journals by these selected authors. A clear imbalance due to a remarkable under-citation of Brazilian authors by authors publishing in Brazilian journals raises the possibility that psychological factors may affect the decision of citing Brazilian journals.

The objective of this study was to characterize patterns of the Brazilian endemic clone of methicillin-resistant Staphylococcus aureus (MRSA) from hospitals throughout Brazil. We studied 83 MRSA strains isolated from patients hospitalized in 27 public and private hospitals in 19 cities located in 14 Brazilian states from September, 1995, to June, 1997. The MRSA strains were typed using antibiograms, bacteriophage typing and pulsed field gel electrophoresis (PFGE). The analysis of genomic DNA by PFGE showed that 65 isolates presented the same PFGE pattern. This pattern was present in all of the hospitals studied indicating the presence of an endemic MRSA clone widely disseminated throughout Brazilian hospitals (BEC). All isolates belonging to the BEC proved to be resistant to ciprofloxacin, erythromycin, lincomycin, trimethoprim-sulphamethoxazole, and tetracycline. Variable susceptibility to these drugs was found only in isolates belonging to clones other than the BEC. The results show that, among MRSA, the BEC is common in Brazil. The best method for mapping changes in the frequency of this clone among MRSA is pulsed field gel electrophoresis. Use of molecular mapping is an important tool for monitoring the spread of potentially dangerous microbes.

Brazilian spotted fever, caused by Rickettsia rickettsii, has been increasingly reported in Brazil especially in the southeastern states. The severe and fulminant forms of the disease are not unusual but most of the reported fatal cases have shown some typical clinical clue, which leads the attending physician to a correct diagnosis. We report a probable case of atypical fulminant Brazilian spotted fever that presented full-blown septic shock associated with Adult Respiratory Distress Syndrome (ARDS) and delayed uncharacteristic rash with an over four-fold increase in reciprocal IgM, but not IgG titer against Rickettsia rickettsii. Brazilian practitioners should be aware of the possibility of Brazilian spotted fever as a cause of fulminant primary sepsis with ARDS; improved laboratory methods are necessary for the rapid diagnosis of such cases.

The elderly population will grow rapidly over the next 25 years, however there is little information about hospital infections in this group of patients in Brazil. We examined the prevalence of nosocomial and community infections in elderly (>65 years) patients and their relationship with intrinsic and extrinsic risk factors in a Brazilian University Hospital. A total of 155 hospitalized elderly patients were evaluated, and clinical and demographic information about each patient was obtained from hospital records. The rates of nosocomial and community infections were 16.1% and 25.6%, respectively. When the elderly group with and without nosocomial infections was analyzed, practically all the risk factors considered (use of antibiotics, invasive devices, surgery and time of hospitalization) were significantly more associated with the patients with hospital infection. All patients with nosocomial infections were taking antibiotics and most of them (56.0%) were being treated with two or more antibiotics; the length of hospitalization was double (p=0.007) compared to patients who had not acquired hospital infection. The most frequent sites of nosocomial and community infections were surgical (56.0%) and the skin (37.1%), and most of the patients (47.5%) were in the surgical clinic wards. In conclusion...

The present study was conducted with 55 patients native from western Brazilian Amazonia, who were HBV-DNA positive after seroconversion of HBeAg. It is a descriptive case study, with the patients separated into two groups: with hepatitis and without hepatitis on histological examination. The aim of the present study was to describe the clinical and molecular characteristics of patients who are chronic carriers of HBsAg. The prevalence of hepatitis was 63.64%, with a predominance of males (41.82%) and a mean age of 42.5 years, occurring mostly in natives of the southeast sub-region (32.73%). Time was a variable proportional to the course of the disease and the most frequent symptoms were: dyspepsia, asthenia and loss of libido with the majority of the patients having history of prior contact with HBV or positive family history. Splenomegalia was the most frequent sign (40%). Among the tests, platelet count, serum albumin and prothrombin activity were significant in the diagnosis of hepatitis. Alpha-fetoprotein was greater in patients with hepatitis, and hepatocellular carcinoma was detected in 3.63% of the patients with hepatic cirrhosis. Three types of HBV genotypes were diagnosed: A, D and F in the samples amplified for gene S. Genotype A (AA) was observed in 54.54% of the cases with hepatitis...

A lethal case of Brazilian spotted fever (BSF) is presented. Clinical features were initially of gastrointestinal involvement and evolved with progression to septic shock, meningoencephalitis and death on the 6th day of illness. Indirect immunofluorescence assay (IFA) for spotted fever group rickettsia (SFGR) was non-reactive. Diagnosis was confirmed by the polymerase chain reaction (PCR) and the nucleotide sequencing of a fragment of the ompA gene showed 100% homology to Rickettsia rickettsii. BSF has not been reported in the city of Rio de Janeiro in the last three decades, and the present description should alert the clinicians to its presence in urban Rio de Janeiro, and to the differential diagnosis with dengue fever, gastroenteritis, leptospirosis and bacterial septic shock, among others.

We evaluate the relationship of positive cases of rabies with the continuing expansion of livestock production, and analyse the trends of this zoonosis in human population in the state of Pará, Brazilian Amazon. The distribution of rabies cases was recorded between 1999 and 2004. Of 148 cases of rabies, 21% were in humans and 79% in non-human mammals. The rapid growth in livestock numbers seems to be associated with the increase of positive cases in bovine livestock transmitted by vampire bats. This idea is supported by positive and significant relationship of both events in time (p < 0.01), but failed when spatial distribution among regions of the state was considered. However, rabies cases tend to occur toward the northeastern of the state of Pará, where rabies cases are proportionally five times greater than other mesoregions, suggesting that increased livestock production may influence the increase of this zoonosis.

Brazilian transport system accounts for negative externalities in terms of energy consumption, carbon dioxide, local pollution emissions, social costs and infrastructure expenditures. These elements results on unsustainablemobility system. Resources usage is considerable and trend lines depict growing concerns in the following years. Brazilian cities continuous expansion increasing passenger mobility demand as well as social improvements (including C social level group) combined with public transportation lowquality causes exponential increments on external transport system costs. For methodological means, the Brazilian cities case study was based on documental and literature research within public administration in Brazil. Therefore, this paper purpose is to present external costs and urban transportation tendencies in Brazil. Research data states significant growth in public infrastructure expenditures provoked by exaggerated energy consumption in the last years. In conclusion, based on literature and the aforementioned data basis, it can be inferred that sustainable transport system relies on public transportation. Pursuing such scenario will lead Brazilian cities to save relevant amount of financial and natural resources.

This study analyzes the Management Practices and Results constructs from several ISO 9001:2000 certified and non-certified organizations in the Brazilian leather-footwear chain in order to identify possible differences in performance between those and to determine the Motivation construct that led these organizations to attain the certification. To achieve these objectives, a research survey was made by sending out an invitation by e-mail to complete a suitable questionnaire on the Internet. We received 130 replies, 45 from certified  organizations and 85 from non-certified organizations. A validation process of the questionnaire was carried out. The factors extracted of the constructs presented positive and significant correlations in the certified organizations. These organizations also performed better in Management Practices and obtained superior Results than those of the non-certifiedorganizations. It is suggested that the non-certified companies in the Brazilian leather-footwear chain should develop a specific motivation to obtain the ISO 9001:2000 certification in order to improve their businesses. The outcome of the research favors the implementation of such joint policies for quality along the value chain to improve industry competitiveness.

Styrax camporum is a common shrub species from the cerrado regions of São Paulo State, Brazil. Although its leaves do not have a morphologically delimited pulvinus, the apical leaves are oriented parallel to the sun (paraheliotropic leaves), most notably around noon. Horizontal leaves at the base of shoots are oriented perpendicular to the sun's rays (diaheliotropic leaves). The ecophysiological significance of this is not fully understood. We investigated how paraheliotropism influences daily CO2 assimilation (A) and other gas exchange rates, water relations, leaf temperature (Tl), and how these relate to leaf structure as assessed by anatomical leaf descriptions. Paraheliotropic leaves had greater A and stomatal conductance (g s), which led to greater transpiration rates (E) during most of the day. However, no categorical data proved that these responses were responsible for the lower Tl for vertical leaves, which only occurred at 1100 h and 1600 h. There was no indication that Tl influenced A, E, or g s. However, diaheliotropic leaves did not assimilate less carbon because of a smaller stomatal aperture. Both leaf types had satisfactory stomatal apertures and the same g s range, resulting in similar intercellular CO2 levels. Paraheliotropic leaves had higher A rates under increased irradiance...

The article investigates some of the most relevant factors for the voting decision in the 2002 presidential election by testing some of the main hypotheses about electoral behaviour in the country by means of logistic regression analyses based on data from the ESEB (Brazilian Electoral Study), a post-electoral survey conducted on a national sample of voters. In the models, taken as a whole, political opinions did not have much weight in the voting decision. Furthermore, they are unable to "explain" a very large share of voters' positioning on a left-right scale or on a scale of voters' "party sentiments". All these "political" variables taken as a whole, in turn, "explain" only part of the evaluations that voters make of the government's performance. The analysis shows that Brazilian voters' voting decision seems rather varied, since some variables were shown to be relevant to "explain" the vote for a candidate, but not for the others. The variables shown to be more frequent (for all four candidates analysed) and with more considerable weight were: voters' religion, their "party sentiments", their positioning on a left-right scale, the evaluations made of the then current government (in actual fact important only for the vote for Serra...

Brazilian politics has been usually analyzed as a case full of pathologies by scholars and political journalists alike. Fragmentation, volatility, clientelism and inefficiency have become bywords for describing the performance of Brazil's political institutions. As a counter to this view, this work argues that the country's democracy in the post-1988 period presents enough evidence in favor of classical hypotheses about electoral politics in the contemporary world, theories that invariably are based on premises of rationality in the behavior of voters and political parties. These theories include the median voter theorem, Duverger's law on the mechanical and psychological effects of electoral systems, and the model of retrospective voting. The article also contends that the passing of time has contributed to make Brazilian politics more rational and efficient in the mould of older democracies.
