Tese de Doutoramento en Engenharia Civil - Estruturas; Em Portugal e noutros países, existe um número significativo de pontes em serviço que se encontram sujeitas a cargas muito superiores aquelas para as quais foram dimensionadas. Por outro lado, o investimento reduzido na conservação das pontes faz com que muitas dessas estruturas se deteriorem significativamente durante a sua vida útil e que a capacidade de carga esteja drasticamente reduzida. Para evitar um decréscimo acentuado do desempenho estrutural ou mesmo o colapso de algumas dessas pontes, é de crucial importância avaliar se ainda verificam os níveis de segurança adequados para esse maior nível de carregamento. Estas preocupações podem, em muitos casos, ser analisadas usando métodos tradicionais de avaliação da segurança. In this study, different career exploration assessment procedures, adjusted to childhood, adolescence and adulthood, are presented and discussed, taking in account theoretical and empirical developments of the construct. The first procedure, aimed for 3 to 10 year-old children, includes the use of the Career Development Child Interview Grid (Araújo & Taveira, 2000; Araújo & Taveira, 2006) and the Self Exploration Guided Task (Araújo, Taveira, & Lemos, 2004; adapted from Gordon, 1968). The second procedure, designed to assess the career exploration process assessment of adolescents aged 14 years-old through adulthood, involves the use of the Career Exploration Survey (Taveira, 1997; adapted from Stumpf, Colarelli, & Hartman, 1983). The last procedures, developed for adults, include the use of the Career Repertory Grid (Silva & Taveira, 2005) and the Career Narrative for adults with low levels of schooling (RVCC-AEP, 2005; Gonçalves, Taveira, & Pereira, 2007). Loiasis is a neglected tropical disease caused by infection with the filarial parasite Loa loa, transmitted by Chrysops vectors. Loiasis has recently emerged as a disease of public health importance when neurologic serious adverse events (SAEs) were reported in individuals with high L. loa microfilaraemia after ivermectin treatment. This had a negative impact on the control of onchocerciasis and lymphatic filariasis in areas of co-endemicity with loiasis. Microfilarial prevalence of 20% has been suggested as the threshold above which there is an unacceptable risk of SAEs with ivermectin treatment.The African Programme for Onchocerciasis Control (APOC) undertook large scale mapping of loiasis in 11 potentially endemic countries using a rapid assessment procedure for loiasis (RAPLOA) that uses a simple questionnaire on the history of eye worm. A geostatistical analysis method called kriging applied to the results in 4798 sampled villages generated a contour map of eye worm prevalence, providing the first global map of loiasis based on actual survey data. This map showed high risk levels of loiasis in 10 countries where an estimated 14.4 million people live in high risk areas.

Interest in filariasis has found a new impetus now that neglected tropical diseases have their own journal. However, some of the advances published in renowned international journals have completely ignored previous publications on the subject, particularly those in languages other than English. The rapid assessment procedure for loiasis and the mapping of lymphatic filariasis provide two perfect illustrations of this. This problem may seem a bit outdated, given that all “good authors” now publish exclusively in English. It certainly is outdated for most areas of medicine. But, surely, this should not be the case for neglected tropical diseases, for which certain long-standing findings are every bit as important as what may be presented as new discoveries. One possibility would be for certain journals, such as PLOS Neglected Tropical Diseases, to include a specific heading permitting the publication in English of older studies that initially appeared in a language other than English. The texts would be English versions respecting the entirety of the original text. Submission should be accompanied by a presentation of the problem, with details and explanatory comments, with submission at the initiative of the authors of the former article in question or their students or sympathizers.

The single-stimulus (SS) preference assessment procedure has been described as more appropriate than the paired stimulus (PS) procedure for “lower functioning” individuals, but this guideline’s vagueness limits its usefulness. We administered the SS and PS preference assessment procedures with food items to seven individuals with severe or profound developmental disabilities who scored at level 2 of the Assessment of Basic Learning Abilities (ABLA) and seven who scored at ABLA level 3. Thirteen of the 14 participants also received these assessments (PS and SS), with non-food items. The two procedures were about equally effective for both groups, and with both types of stimuli, although the PS procedure produced more refined preference hierarchies. Most participants showed moderate to high correlations in preference scores between the two procedures for both food and non-food items. These results suggest that, for individuals who score at either ABLA level 2 or ABLA level 3, the SS and the PS procedures are equally likely to identify preferred stimuli.

Recent laboratory studies have shown that men display more risk-taking behavior in decision-making tasks following stress, whilst women are more risk-aversive or become more task-focused. In addition, these studies have shown that sex differences are related to levels of the stress hormone cortisol (indicative of activation of the hypothalamus-pituitary-adrenocortical-axis): the higher the levels of cortisol the more risk-taking behavior is shown by men, whereas women generally display more risk-aversive or task-focused behavior following higher levels of cortisol. Here, we assessed whether such relationships hold outside the laboratory, correlating levels of cortisol obtained during a job-related assessment procedure with decision-making parameters in the Cambridge Gambling Task (CGT) in male and female police recruits. The CGT allows for discriminating different aspects of reward-based decision-making. In addition, we correlated levels of alpha-amylase [indicative of activation of the sympatho-adrenomedullary-axis (SAM)] and decision-making parameters. Currently there is little guidance available on an experimentally-validated detailed seismic assessment procedure for vintage flexible timber diaphragms such as are routinely encountered in New Zealand unreinforced masonry buildings. The results from recent testing of full-scale diaphragms are presented and interpreted with particular attention given to the definition of shear stiffness and shear strength values, whilst acknowledging that the recommendations derive from a small data set. References are provided to information previously published elsewhere to justify the theoretical framework adopted, and the procedure is linked to ASCE 41-13 for guidance regarding diaphragm scenarios that have not been studied by the authors. A procedure is provided to account for the effects on diaphragm response of supplementary stiffness due to masonry end walls. The performance of several diaphragms that were improved with either overlays or underlays is reported as potential proof-tested standard solutions. Zum jetzigen Zeitpunkt gibt es keine Untersuchungsverfahren, die konsens-basierte Empfehlungen zur Differentialdiagnostik von Schädel-Hirn-verletzten Patienten mit Bewusstseinsstörungen in ausreichendem Maße berücksichtigen. In dieser Arbeit wird die Validierung eines neuen Verfahrens, des Instruments zur Differentialdiagnostik von Bewusstseinsstörungen (IDB), beschrieben. Bewusstseinsgestörte Patienten einer neurologischen Frührehabilitationseinrichtung (29 männlich und 11 weiblich) wurden mit dem IDB sowie der Disability Rating Scale (DRS) und der Coma-Remission Scale-Revised (CRS-R) zum Zeitpunkt der Aufnahme und der Entlassung aus der Klinik untersucht. Mit dem IDB ist eine diagnostische Einstufung als Wachkoma (Vegetative State, VS), minimal bewusster Zustand (MCS) oder bewusst bei allen Patienten unabhängig von der Krankheitsätiologie möglich. Die Summenscores des IDB zeigten eine hohe Korrelation mit denen der DRS und CRS-R. Bei 6 von der CRS-R als VS eingestuften Patienten konnten mit dem IDB kognitive Verarbeitungskompetenzen nachgewiesen werden. Der initiale IDB-, DRS- und CRS-R-Wert waren signifikante Prädiktoren für das Kurzzeitoutcome. Das IDB ist ein valides, reliables und im Vergleich zur CRS-R und DRS sensibleres Verfahren zur Differentialdiagnostik von Bewusstseinsstörungen und eignet sich zur Vorhersage des Behandlungserfolgs. In 2011, Armenia joined the Russia Education Aid for Development (READ) trust fund program, the goal of which is to help countries improve their capacity to design, carry out, analyze, and use assessments for improved student learning. As part of the READ Trust Fund program, and in order to gain a better understanding of the strengths and weaknesses of its existing assessment system, Armenia participated in a formal exercise to benchmark its student assessment system under The World Bank's Systems Approach for Better Education Results (SABER) program. SABER is an evidence-based program to help countries systematically examine and strengthen the performance of different aspects of their education systems. SABER-student assessment is a component of the SABER program that focuses specifically on benchmarking student assessment policies and systems. The goal of SABER-student assessment is to promote stronger assessment systems that contribute to improved education quality and learning for all. The importance of assessment is linked to its role in: providing information on levels of student learning and achievement in the system; monitoring trends in education quality over time; supporting educators and students with real-time information to improve teaching and learning; and holding stakeholders accountable for results. The SABER-student assessment framework is built on the available evidence base for what an effective assessment system looks like. The framework provides guidance on how countries can build more effective student assessment systems. A Fiduciary System Assessment (FSA) was carried out in accordance with OP/BP 9.0 that evaluated the fiduciary systems pertaining to the Health System Quality and Efficiency Improvement Program for Results. The integrated fiduciary assessment comprised separate assessments of the fiduciary risks relating to the programs: (i) procurement; (ii) financial management; and iii) governance. The objective of the assessment was to provide reference that could be used to monitor fiduciary system performance during the implementation of the above referenced operation, as well as to identify actions, as relevant, to enhance the performance of the systems. Competency-based training models in surgical education challenge programs to adequately document and assess trainees’ clinical decision-making, problem solving, and procedural skills in the operating room. While a variety of tools for the assessment of procedural skills exist, validated intraoperative assessment tools are scarce. Those that have been validated often employ checklists or numerical rating scales, which are prone to bias and provide limited feedback to residents. The Surgical Procedure Feedback Rubric (SPR) was developed by the Department of Surgery at Queen’s University to document the quality of resident performance during a single, directly observed operative encounter and provide targeted feedback to support learning. It differs from other assessment tools because it defines performance criteria by increasing complexity through the use of behavioural anchors, thus embedding standards of performance in the tool. This study begins the process of building a validity argument for the SPR. Validity in educational assessment is an evaluation of the appropriateness of inferences made with the use of an assessment tool. Kane and Stobart’s validity frameworks guided this study. Introdução - É realizada uma reflexão sobre o significado das palavras avaliação e gestão e sua contextualização em um complexo hospitalar e faculdade de medicina. Analisa-se a contribuição para o aprimoramento contínuo da qualidade de serviços de saúde por meio de processo sistemático de avaliação da gestão, em que são apontados pontos fortes e oportunidades de melhoria. Objetivo - Analisar o processo interno de avaliação da gestão utilizado pelo Sistema Faculdade de Medicina da Universidade de São Paulo - Hospital das Clínicas (FMUSP/HC) no período de 2003 a 2006. Método - A avaliação interna da gestão é realizada anualmente, baseada nos fundamentos da excelência do Prêmio Nacional da Qualidade em sua versão para a saúde: Prêmio Nacional da Gestão em Saúde. Foram analisadas as pontuações obtidas nos oito critérios, a saber: liderança, estratégias e planos, clientes, sociedade, informação e conhecimento, pessoas, processos e resultados. Por se tratar de um Sistema que utiliza o mesmo modelo de avaliação da gestão, os dados foram analisados em conjunto, não individualizando cada uma das dez Unidades que compõem o Sistema FMUSP/HC (oito hospitais de alta complexidade, uma unidade de reabilitação e a faculdade de medicina). The Water Safety Plan (WSP) for the city of Salta (Argentina) is presented and discussed. To develop this WSP, we used an adapted version of the methodology proposed by the World Health Organization (WHO). The new method included a preliminary weighting procedure to assess the relative importance of different parts of the system, and a more systematic estimation of the magnitude of control measures. These modifications allowed the definition of a variety of risk reduction strategies. The risk assessment step was performed during participatory workshops with members of the local water company. The Initial Risk for the entire system was 30.2%, with variations among processes, subprocesses and components. More than 60% of the hazardous situations identified require control measures to reduce the risk below an acceptable threshold. If all control measures were successfully implemented, the Final Risk could be lowered to 17.7%. Assessment of written performance: tracing raters' decision making process

Written performance assessment is mostly carried out using direct tasks the marking of which involves trained raters. Thus, the final judgement is a result of interplay between the task, the measurement instrument, and the rater. The present study gives an insight into the way raters arrive at a decision. Five Hungarian teachers of German and English took part in rating compositions in a nationwide survey in Hungary. They were asked to produce think-aloud protocols during the rating process, which were later transcribed and analysed. The presented data show the procedure that raters followed in marking the scripts.; La valoraci??n de la expresi??n escrita se realiza mayormente con ejercicios directos cuya evaluaci??n necesita evaluadores entrenados. Onchocerciasis is an endemic disease in Ondo state, Nigeria. Community directed distribution of ivermectin is currently on-going in some local government areas of the state. Randomly selected persons (2 331 males and 2 469 females) were interviewed using a modified rapid assessment procedure for Loa loa (RAPLOA) to assess community directed treatment with ivermectin. The retrospective study evaluated the coverage, impacts and adverse reactions to the drug treatment. A questionnaire was administered by house-to-house visit in six local government areas, implementing community directed treatment with ivermectin (CDTI) in this bioclimatic zone. A total of 2,398 respondents were reported to have participated in the treatment. The overall ivermectin coverage of 49.96% was recorded (range 0 - 52% in different communities). Adverse reactions from ivermectin administration were experienced in 38% of individuals. Diverse adverse reactions experienced included predominantly itching (18.50%); oedema, especially of the face and the limbs (8.2%); rashes (3.4%) and body weakness (2.4%). Objective: To evaluate diabetes patients´ self-monitoring of blood glucose using a community pharmacy-based quality assurance procedure, to investigate whether the procedure improved the quality of the patient performance of self monitoring of blood glucose, and to examine the opinions of the patients taking part in the study. Methods: The results of patient blood glucose measurements were compared to the results obtained with HemoCue Glucose 201+ by pharmacy employees in 16 Norwegian community pharmacies. Patient performance was monitored using an eight item checklist. Patients whose blood glucose measurements differed from pharmacy measurements by more than 20% were instructed in the correct use of their glucometer. The patients then re-measured their blood glucose. If the results were still outside the set limits, the control procedure was repeated with a new lot of glucometer strips, and then with a new glucometer. The patients returned for a follow-up visit after three months. Results: During the first visit, 5% of the 338 patients had measurements that deviated from pharmacy blood glucose values by more than 20% and user errors were observed for 50% of the patients. At the moment, the health-environmental risk analysis is used to decision-making targets in the contaminated sites management; this procedure allows to assess the quantitative health risk related to the pollutants presence in environmental compartments, as soil and waters. As regards potentially contaminated agricultural soils, the ingestion of food from vegetable and/or animal source, produced inside the contaminated area, is the most suitable way to assess the health risk. As an official procedure to this assessment is not available, the National Institute for Health (Istituto Superiore di Sanità, ISS) has worked out an operating procedure, organized into several phases, depending on the available specific-site know-how. In this document, agricultural soils potentially contaminated in two sites have been studied; the sites are the following: Brescia Caffaro and Torviscosa.

Nowadays digital ultrasound-cardiovascular devices are able to send out directly digital images and films. Thanks to the large adoption of such devices, the echocardiographic world is facing new ways of exchanging images and collaborating. What we present in this paper is a review of the experimental projects carried in Lombardy, meant to support the work of specialists by means of second opinion and telemedicine services. On the medical point of view echocardiography is a widely used activity where operators are perfectly accustomed to do repetitive operations and steps. Tele-echocardiography (T-E) introduces new methods and technologies into stable and everyday medical practice, causing disruptions either on the side of the specialists' way of working or on the new opportunities and service. Introducing such a service means to properly model it in order to reduce the changes in the operators' way of working while maximizing the benefits. A proper method of modelling the operators' needs is then a key factor which must be correctly addressed. This paper will present some successful projects and the assessment procedure but it will also discuss a possible service modelling method, which has been adopted for the described experiences.
