Abatacept in children with juvenile idiopathic arthritis: a randomised, double-blind, placebo-controlled withdrawal trial

Background Some children with juvenile idiopathic arthritis either do not respond, or are intolerant to, treatment with disease-modifying antirheumatic drugs, including anti-tumour necrosis factor (TNF) drugs. We aimed to assess the safety and efficacy of abatacept, a selective T-cell costimulation modulator, in children with juvenile idiopathic arthritis who had failed previous treatments. Methods We did a double-blind, randomised controlled withdrawal trial between February, 2004, and June, 2006. We enrolled 190 patients aged 6-17 years, from 45 centres, who had a history of active juvenile idiopathic arthritis; at least five active joints; and an inadequate response to, or intolerance to, at least one disease-modifying antirheumatic drug. All 190 patients were given 10 mg/kg of abatacept intravenously in the open-label period of 4 months. Of the 170 patients who completed this lead-in course, 47 did not respond to the treatment according to predefined American College of Rheumatology (ACR) paediatric criteria and were excluded. Objective. To evaluate the antiinflammatory effects of RC-3095 in 2 experimental models of arthritis, collagen-induced arthritis (CIA) and antigen-induced arthritis (AIA), and to determine the mechanisms of action involved. Methods. RC-3095 was administered daily to mice with CIA and mice with AIA, after induction of disease with methylated bovine serum albumin. Disease incidence and severity were assessed using a clinical index and evaluation of histologic features, respectively. In mice with CIA, gastrin-releasing peptide receptor (GRPR) was detected by immunohistochemical analysis, while in mice with AIA, migration of neutrophils, presence of glycosaminoglycans, and lymphocyte proliferation, determined using the MTT assay, were assessed. Expression of cytokines interleukin-17 (IL-17), IL-1 beta, and tumor necrosis factor alpha (TNF alpha) was evaluated in all mouse knees using enzyme-linked immunosorbent assay. In order to investigate a putative role for nitric oxide (NO) in the central nociceptive processing following carrageenan-induced arthritis in the rat temporomandibular joint (TMJ), we analyzed the immunoreactivity, gene expression and activity of nitric oxide synthases (NOS) in the caudal part of the spinal trigeminal nucleus (Sp5C) during the acute (24 h), chronic (15 days) and chronic-active (14 days-24 h) arthritis. In addition, evaluation of head-withdrawal threshold was carried out in all phases of arthritis under chronic inhibition of nNOS with the selective inhibitor 7-nitroindazole (7-NI). Neurons with nNOS-like immunoreactivity (nNOS-LI) were concentrated mainly in the lamina II of the Sp5C, showing no significant statistical difference during arthritis. Only a discrete percentage of nNOS-LI neurons expressed Fos immunoreactivity. The mRNA expression for both nNOS and endothelial nitric oxide synthases (eNOS) presented no noticeable differences among the groups. No expression of inducible nitric oxide synthase (iNOS) was detected in the Sp5C by either immunohistochemistry or reverse-transcription polymerase chain reaction (RTPCR). Artrite reumatóide é uma doença auto-imune que apresenta acentuado quadro inflamatório e proliferação celular o que, provavelmente, determina a alta prevalência de doenças cardiovasculares quando comparados a população mundial. A mortalidade e a morbidade conseqüentes das doenças cardiovasculares estão 2 vezes aumentadas em pacientes com artrite reumatóide e um dos principais fatores de risco relacionados ao desenvolvimento da aterosclerose é a dislipidemia. Esse importante fator de risco vem sendo associado à artrite reumatóide e as concentrações plasmáticas de lípides são constantemente avaliadas, já que se encontra bem estabelecido a relação entre dislipidemia e alta incidência de doença cardiovascular. No entanto, o verdadeiro impacto das alterações lipídicas na artrite reumatóide não é bem conhecido, já que os resultados de perfil lipídico são contraditórios. Alterações nas concentrações plasmáticas de lípides não necessariamente acompanham distúrbios no metabolismo das lipoproteínas plasmáticas. A artrite idiopática juvenil frequentemente afeta a articulação temporomandibular, compromete o crescimento facial e pode ser fator de risco para cáries e doenças periodontais. Entretanto, são escassos os estudos que avaliam em conjunto todas essas alterações. O objetivo deste estudo foi avaliar a condição dentária, as características faciais e a articulação temporomandibular de 35 pacientes com artrite idiopática juvenil, evolução poliarticular, tratados em hospital escola, comparativamente a um grupo controle de 35 crianças saudáveis. O estudo constou de avaliação sistemática das estruturas dentárias, orais e faciais através de: questionário de anamnese, índice CPO-D, índice de placa, índice de sangramento gengival, relação dentária, perfil facial, índice de Helkimo e classificação das alterações condilares observadas nas radiografias panorâmicas da face. A idade média foi de 11,4±3,47 e 10,4±2,56 anos, respectivamente para os grupos estudo e controle. Therapeutic options for patients suffering from the more severe forms of spondyloarthritis have been rather limited in the last decades. There is now accumulating evidence that antitumor necrosis factor therapy is highly effective in spondyloarthritis, especially in ankylosing spondylitis and psoriatic arthritis. Based on the data recently published on more than 500 patients with ankylosing spondylitis and psoriatic arthritis, this treatment seems to be even more effective than in rheumatoid arthritis. The antitumor necrosis factor-alpha agents currently available, infliximab (Remicade), etanercept (Enbrel), and adalimumab (Humira), are approved for the treatment of rheumatoid arthritis in the United States and partly in Europe. The situation in spondyloarthritis is different from that of rheumatoid arthritis because there is an unmet medical need, especially in ankylosing spondylitis: no therapies with disease-modifying antirheumatic drugs are available for severely affected patients, especially with spinal disease. BACKGROUND: Psoriatic arthritis is an inflammatory arthritis associated with psoriasis. Its prevalence in patients with psoriasis varies from 7 to 42% but its exact prevalence is unknown. OBJECTIVES: Considering the lack of national data related to its diagnosis in patients with psoriasis, this study aims to describe the clinical, laboratorial and radiological manifestations of psoriatic arthritis in these patients. METHODS: We evaluated 133 patients with psoriasis, treated as outpatients. These patients were asked to fill in the forms with data about the disease and were submitted to a clinical evaluation by a dermatologist and a rheumatologist. Suspected cases of arthritis were referred for further investigation and were classified according to presence or absence of psoriatic arthritis according to CASPAR criteria. RESULTS: The number of patients with psoriatic arthritis was 47 (35%), 17 of them were new cases. There was no difference between the groups regarding the type of psoriasis, nail involvement, presence of scalp lesions and psoriatic arthritis. Patients with psoriatic arthritis had more enthesitis and dactylitis (46.7%) than those without arthritis. INTRODUCTION: Epstein-Barr virus exposure appears to be an environmental trigger for rheumatoid arthritis that interacts with other risk factors. Relationships among anti-cyclic citrullinated peptide antibodies, the shared epitope, and smoking status have been observed in patients with rheumatoid arthritis from different populations. OBJECTIVE: To perform an association analysis of anti-Epstein-Barr nuclear antigen-1 antibodies, anti-cyclic citrullinated peptide antibodies, the shared epitope, and smoking status in Brazilian patients with rheumatoid arthritis. METHODS: In a case-control study, 140 rheumatoid arthritis patients and 143 healthy volunteers who were matched for age, sex, and ethnicity were recruited. Anti-Epstein-Barr nuclear antigen-1 antibodies and anti-cyclic citrullinated peptide antibodies were examined using an enzyme-linked immunosorbent assay, and shared epitope alleles were identified by genotyping. Smoking information was collected from all subjects. A comparative analysis of anti-Epstein-Barr nuclear antigen-1 antibodies, anti-cyclic citrullinated peptide antibodies, the shared epitope, and smoking status was performed in the patient group. Previous studies using clinic or convenience samples have indicated that not only patients with arthritis are at increased risk of depression, but there may also be a link between depression and disability in people with arthritis. We examined the prevalence of psychological distress in a population sample with and without arthritis and the association with health-related quality of life. The WANTS Health and Well-being Survey was a population household telephone interview survey of adults (age ≥18 years) in three states of Australia. Data obtained were weighted to provide population-representative estimates. The survey included questions regarding arthritis, SF-12, the Kessler 10 index of psychological distress and presence of mental health conditions. A total of 7,473 interviews providing information on arthritis were completed, with 1,364 (18.3%) reporting arthritis. Self-reported mental health conditions were more frequent in those with arthritis (14.9 vs 12.0%, p = 0.004), and a higher proportion were at a medium or high risk for anxiety or depression (39.0 vs 31.0%, p < 0.001). Bei Kindern mit juveniler idiopathischer Arthritis (JIA) entstehen durch die chronische Arthritis Schädigungen der Gelenke und deren Funktion. Im Rahmen der Arthritis, sowie der damit verbundenen Schonung kann es zu einer Abnahme der Muskelkraft kommen. Im Verlauf kann die Knochendichte abnehmen und eine sekundäre Osteoporose auftreten. Eine Kontrolle der Gelenkfunktion ist für die Beurteilung des Krankheitsverlaufes und Therapieerfolges bei Kindern mit JIA wünschenswert und wichtig. Die Elektrogoniometrie stellt ein einfaches, kostengünstiges Verfahren zur Beurteilung des Bewegungsumfanges von Gelenken dar. Sie erlaubt die Berechnung der Gelenkgeschwindigkeit und der Beschleunigung sowie der Muskeldrehmomente. Diese lassen Rückschlüsse auf die Kraft und indirekt auf die Knochenfestigkeit zu. Die vorliegende Arbeit beschäftigt sich mit der Möglichkeit der elektrogoniometrischen Messung der Muskeldrehmomente zur Beurteilung der Gelenkfunktion bei Kindern mit JIA. Zuerst werden Verfahren zur Erhebung der Extremitätenmassen überprüft, welche notwendig zur Berechnung von kinetischen Parametern sind. Veränderungen im muskulo-skelettalen System mit verminderter Knochendichte, erhöhter Frakturneigung und Veränderungen von Markern des Knochenstoffwechsels bei juveniler-idiopathischer Arthritis (JIA) wurden in der Literatur mehrfach beschrieben. Auch die Hypothese einer bereits im Kindesalter vorliegenden Osteoporose wurde dabei mehrfach gestellt. Allerdings wurden diese Ergebnisse mit Messverfahren gewonnen, die eine Aussage über die tatsächliche Knochendichte, die Geometrie und die beteiligten Kompartimente der Muskel-Knochen-Einheit nicht zulassen. In der vorliegenden Querschnittsstudie wurden 57 Patienten mit JIA untersucht. Vor allem die unterschiedlichen Einflussgrößen auf den Knochen, die Geometrie des Knochens und der Muskel konnten mit Hilfe der pQCT-Technik untersucht und mit pädiatrischen Normwerten verglichen werden. Sowohl eine Auswertung hinsichtlich des Lebensalters als auch hinsichtlich des Größenalters wurde durchgeführt. Dabei stand neben der Knochendichtemessung die Frage im Vordergrund, ob es sich bei den Veränderungen um primäre Knochenveränderungen, sekundäre Folgen einer Affektion der Muskel-Knochen-Einheit oder um Mischeffekte handelt. Die Collagen induzierte Arthritis ist eine experimentelle Autoimmunkrankheit in Mäusen, die als ein Modell der rheumatoiden Arthritis und der juvenilen idiopathischen Arthritis gilt. CD137 (4-1BB) ist ein kostimulatorisches Molekül, das in der Frühphase der T-Zell-Aktivierung exprimiert wird. Ein agonistischer anti-CD137-Antikörper verhindert die humorale Immunantwort gegen T-Zell-abhängige Antigene. Dies geschieht in vivo nicht durch eine direkte Beeinflussung der Funktion von B-Zellen, sondern durch die Induktion einer lang anhaltenden Anergie von T-Helfer-Zellen. Da die T-Helfer-Zellen eine wesentliche Rolle in der Pathogenese der CIA spielen, wurde in der vorliegenden Arbeit der Einfluss eines spezifischen, agonistischen anti-CD137-Antikörpers auf die Induktion und den Verlauf von CIA untersucht. DBA/1J-Mäuse wurden mit bovinem Collagen II immunisiert und mit einem agonistischen anti-CD137-Antikörper behandelt. Die Tiere wurden zweimal pro Woche untersucht und ihr anti-Collagen-II-Antikörper-Spiegel im Serum gemessen. Außerdem wurden histologische Untersuchungen und FACS-Analysen durchgeführt. Die Juvenile Idiopathische Arthritis (JIA) ist die häufigste rheumatische Erkrankung im Kindesalter. Auch wenn es zahlreiche Hinweise darauf gibt, dass es sich um eine Autoimmunerkrankung handelt, sind die der Pathogenese zu Grunde liegenden Vorgänge dieses Krankheitsbildes noch weitestgehend unbekannt. Ein suffizientes Tiermodell zur Erforschung dieser Krankheit ist ebenfalls bisher nicht gefunden worden. In der K/BxN-Mauslinie entwickelt sich am Tag 25 bis 35 spontan eine Arthritis der distalen Gelenke. Die Krankheit in diesem Mausmodell wird ausgelöst durch eine Autoimmunreaktion gegen das systemische Antigen Glukose-6-phosphat-Isomerase (GPI), ein zytosolisches Enzym der Glykolyse und Glukoneogenese. Die Krankheit kann durch Transfer von anti-GPI-Antikörpern in gesunde Wildtyp-Mäuse übertragen werden. Die Symptomatik erkrankter Mäuse deckt sich in auffallender Weise mit dem Erkrankungsbild menschlicher Arthritiden. Daher sollte hier untersucht werden, inwieweit eine humorale Autoimmunreaktion gegen GPI für die Pathogenese der JIA von Bedeutung ist. In dieser Arbeit konnte gezeigt werden, dass eine humorale Immunantwort gegen GPI kein allgemeines, pathogenetisches Prinzip bei der JIA ist. Background: “Arthritis” is a common musculoskeletal condition but the knowledge of what type of arthritis people have, may be limited but may have changed over time in response to campaigns, increased awareness and improved health literacy. This paper describes people who did not know what type of arthritis they had, by a range of relevant demographic and socioeconomic variables, and assesses changes over time in the proportion of people who report having arthritis but do not know what type, using representative population surveillance data. Methods: Data were collected using the South Australian Monitoring and Surveillance System (SAMSS), a risk factor surveillance system where each month, a representative random sample of South Australians is selected from the Electronic White Pages, with interviews conducted using computer assisted telephone interviewing (CATI). Data were used for the period January 2006 to December 2008 (n = 16465) for respondents aged 18 years and over. OBJECTIVE: It has been shown that the temporomandibular joint is frequently affected by juvenile idiopathic arthritis, and this degenerative disease, which may occur during facial growth, results in severe mandibular dysfunction. However, there are no studies that correlate oral health (tooth decay and gingival diseases) and temporomandibular joint dysfunction in patients with juvenile idiopathic arthritis. The aim of this study is to evaluate the oral and facial characteristics of the patients with juvenile idiopathic arthritis treated in a large teaching hospital. METHOD: Thirty-six patients with juvenile idiopathic arthritis (26 female and 10 male) underwent a systematic clinical evaluation of their dental, oral, and facial structures (DMFT index, plaque and gingival bleeding index, dental relationship, facial profile, and Helkimo's index). The control group was composed of 13 healthy children. RESULTS: The mean age of the patients with juvenile idiopathic arthritis was 10.8 years; convex facial profile was present in 12 juvenile idiopathic arthritis patients, and class II molar relation was present in 12 (P = .032). The indexes of plaque and gingival bleeding were significant in juvenile idiopathic arthritis patients with a higher number of superior limbs joints involved (P = .055). OBJECTIVES: To investigate the association of body cell mass loss with disease activity and disability in rheumatoid arthritis patients. INTRODUCTION: Rheumatoid cachexia, defined as the loss of body cell mass, is important but under-recognized and contributes to morbidity and mortality in patients with rheumatoid arthritis. METHODS: One hundred forty-nine rheumatoid arthritis patients and 53 healthy, non-rheumatoid arthritis control subjects underwent anthropometric measurements of body mass index and waist and hip circumferences. Bioelectrical impedance analysis was used to determine the subjects' body compositions, including fat mass, skeletal lean mass, and body cell mass. The disease activity of rheumatoid arthritis was assessed using C-reactive protein serum, the erythrocyte sedimentation rate and the 28-joint disease activity score, while disability was evaluated using a health assessment questionnaire. RESULTS: Rheumatoid arthritis patients had lower waist-to-hip ratio (0.86 ± 0.07 vs. 0.95 ± 0.06; p<0.001) and lower skeletal lean mass indexes (14.44 ±1.52 vs. 15.18 ± 1.35; p = 0.002) than those in the healthy control group. INTRODUCTION: Epstein-Barr virus exposure appears to be an environmental trigger for rheumatoid arthritis that interacts with other risk factors. Relationships among anti-cyclic citrullinated peptide antibodies, the shared epitope, and smoking status have been observed in patients with rheumatoid arthritis from different populations. OBJECTIVE: To perform an association analysis of anti-Epstein-Barr nuclear antigen-1 antibodies, anti-cyclic citrullinated peptide antibodies, the shared epitope, and smoking status in Brazilian patients with rheumatoid arthritis. METHODS: In a case-control study, 140 rheumatoid arthritis patients and 143 healthy volunteers who were matched for age, sex, and ethnicity were recruited. Anti-Epstein-Barr nuclear antigen-1 antibodies and anti-cyclic citrullinated peptide antibodies were examined using an enzyme-linked immunosorbent assay, and shared epitope alleles were identified by genotyping. Smoking information was collected from all subjects. A comparative analysis of anti-Epstein-Barr nuclear antigen-1 antibodies, anti-cyclic citrullinated peptide antibodies, the shared epitope, and smoking status was performed in the patient group. OBJETIVO: A articulação temporomandibular é freqüentemente afetada pela artrite idiopática juvenil, e esta doença degenerativa, durante o crescimento facial, resulta em disfunção mandibular grave. No entanto, não há estudos que avaliam conjuntamente alterações na saúde oral (cáries e doenças gengivais) e na articulação temporomandibular decorrentes da artrite idiopática juvenil. O objetivo deste estudo é avaliar a condição dentária e a função mandibular de pacientes com artrite idiopática juvenil tratados em um hospital escola. MÉTODO: Trinta e seis pacientes com artrite idiopática juvenil (26 meninas e 10 meninos) foram submetidos a uma avaliação clínica sistemática de suas estruturas dentárias, orais e faciais (índice CPO-D, índice de placa e sangramento gengival, relação dentária, perfil facial e índice de Helkimo para articulação temporomandibular). O grupo controle foi composto por 13 crianças saudáveis. RESULTADOS: A média de idade dos pacientes com diagnóstico de artrite idiopática juvenil foi 10,8 anos; o perfil facial convexo foi encontrado em 12 pacientes com artrite idiopática juvenil e relação molar classe II esteve presente em 12 deles (p=0,032).
