O presente estudo aborda a amputação desde o momento em que passa a fazer parte das inquietações da autora, integrando seu mundo-vida, por meio de interrogações relativas à questão da amputação, suas implicações e sentimentos experimentados pela pessoa que a vivencia. Constitui-se numa investigação de caráter qualitativo, desenvolvida à luz do referencial teórico-metodológico da fenomenologia e que busca compreender, então, a vivência de uma amputação, a partir da visão da pessoa que a experiencia. Inicialmente, a fim de compreender o fenômeno que se mostra diante de meus olhos, pedindo um aclaramento, realizei um levantamento bibliográfico, o que me possibilitou conhecer a amputação sob vários enfoques, além de permitir a apropriação de algumas idéias do pensamento filosófico de Merleau-Ponty, abordando a percepção, o corpo que percebe e é percebido, na sua relação com o mundo, e o enraizamento do espírito neste corpo. Após conhecer a amputação, sob o ponto de vista literário, busquei o seu compartilhar com a pessoa a ela submetida, habitando seu mundo. Este trabalho tem como objetivo discutir as possibilidades e dificuldades de um psicanalista quando passa a compor uma equipe multidisciplinar de atendimento a pacientes amputados em reabilitação com prótese. A equipe de saúde tem papel fundamental no atendimento ao paciente amputado que realiza reabilitação. Pensa-se que há diferença para este trabalho o fato de haver na equipe um psicanalista, que atua de um lugar diferente dos outros membros, representantes do discurso médico. Para se compreender o que ocorre quando há um psicanalista na equipe de saúde são abordadas as diferenças entre os campos da psicologia hospitalar e da psicanálise dentro de uma equipe; as diferenças entre os discursos médico e psicanalítico, o estatuto do corpo para a psicanálise, e as bases de sustentação para que um analista se constitua enquanto tal. Além disso, são tratados o estatuto do corpo na contemporaneidade e a questão da deficiência, pontos cruciais no atendimento ao paciente com amputação. Estudo quantitativo, descritivo e retrospectivo de internações hospitalares de pacientes com e sem o diagnóstico de diabetes submetidos a amputação não traumática de extremidades inferiores, no período de 2001 a 2008, em Ribeirão Preto - SP - Brasil. Este estudo teve como objetivos caracterizar as internações hospitalares de pacientes com e sem o diagnóstico de diabetes mellitus submetidos a amputação não traumática de extremidades inferiores segundo sexo, idade, período da internação hospitalar, tipo de atendimento e condições da alta; relacionar as internações hospitalares com as variáveis tipo de alta e atendimento, sexo, idade e tempo de internação, e analisar a tendência das internações hospitalares em Ribeirão Preto-SP, no período de 2001-2008. Utilizaram-se dados de fonte secundária de internações hospitalares, do Sistema de Informações do Centro de Processamento de Dados Hospitalares. A 35-year-old African Brazilian patient had sickle cell anemia complicated with recurrent vasoocclusive (VOC) crises and refractory painful leg ulcers for 16 years. The ulcers started over both medial malleoli and expanded gradually. The ulcer on the left leg spread from the foot to the knee circumferentially and was refractory to all forms of therapy within the frame work of multi-disciplinary care. The patient agreed to a below the knee amputation of the left leg. He felt much better after the amputation but developed severe neuropathic phantom pain that was well controlled medically. He could differentiate the sickle cell anemia and ulcer pain from the neuropathic pain. About 6 months after the amputation he had dengue fever with fatal outcome. This is the first report of treatment of refractory sickle cell anemia leg ulcer with amputation and probably the first report of a Brazilian patient with sickle cell anemia and dengue fever.

Introdução: A amputação, especialmente dos membros inferiores, é um procedimento cirúrgico antigo que permite excelentes resultados quando realizada nas indicações apropriadas. Na década de 1940 , Ertl desenvolveu uma técnica reprodutível para amputação transtibial com o princípio de osteomioplastia , que restaurou a pressão intra-óssea por meio da obliteração do canal medular e ampliou a área de apoio terminal , formando uma ponte óssea entre a fíbula e a tíbia distais (sinostose). Objetivos: O objetivo deste estudo foi investigar a eficácia de uma modificação da técnica original Ertl, em que um retalho pediculado osteoperiosteal é usado para formar a ponte óssea na amputação transtibial em adultos . Resultados: Todos os nove pacientes apresentaram cotos indolores e capazes de apoio terminal total em 24 semanas pós-operatório. OBJECTIVE: Identify the etiology and incidence, as well to assess functional outcomes of patients, undergoing lower limb amputation after failure or complication of total knee arthroplasty. These patients were treated at the Center for Knee Surgery at the National Institute of Traumatology and Orthopedics (INTO), during the period of January 2001 to December 2010. METHODS: The patients were interviewed and their charts were retrospectively analyzed to evaluate their functional outcome. RESULTS: The incidence of amputation due to failure or complication of total knee arthroplasty was 0.41% in 2409 cases. Recurrent deep infection was the cause of amputation in 81% of cases, being Staphylococcus aureus and Pseudomonas aeruginosa the most frequent germs. Vascular complications and periprosthetic fracture associated to metaphyseal bone loss were also causes of amputation. In our study, 44% of amputees patients were using orthesis and 62.5% have had the ability to walk. CONCLUSION: Incidence of 0.41%, being the main cause recurrent infection. The functional outcome is limited, and the fitting achieved in 44% of patients and only 62.5% are ambulatory.

PURPOSE: To investigate immunohistochemical aspects of the myenteric plexus of valves constructed in the colon of rats to verify whether any denervation occurs both at the operative site and in those areas adjacent to the third valve. METHODS: Thirty six male Wistar rats divided into the following three groups were used: Control Group (CG); Amputated Group (AG); Amputated Group with Valves (AGWV). In AG was held in the rectum amputation and the colon was sutured to the skin elaborating the perineal colostomy. In AGWV was held in the rectum amputation. A laparotomy was performed for the manufacture of valves (seromyotomy) in the colon. After this step, the colon was sutured to the skin elaborating the perineal colostomy. The density of the neural elements in the muscular wall as marked specifically using Protein Gene Product (PGP) 9.5 and utilising the proper tools of the KS300 software for measuring the area. From these measurements, a relation and three proportions were drawn and analysed according to the mean of the averages obtained from the measured images. Objective To quantify the quality of life of people with a lower limb amputation, and to propose a relationship between the quality of life and the International Classification of Functioning, Disability and Health (ICF). Materials and methods After a retrospective study of medical records, 15 amputees met the inclusion criteria. The characteristics of the amputation and quality of life were evaluated, and both were correlated with the ICF. The 36-Item Short-Form Survey (SF-36) was used to assess quality of life. Results It was possible to establish ICF codes for levels of amputation and the quality of life. A high and significant correlation was found between quantitative descriptors of the ICF and SF-36 scores (r = -0.9376, p < 0.0001). Conclusion People with a lower limb amputation showed a reduced quality of life, which was reflected in scores from a generic questionnaire and their correlation with the ICF.

A qualitative, descriptive and exploratory study was conducted through semi-structured interviews with 18 people undergoing amputation in the period 2008-2010, in public hospitals in Florianópolis. The objectives were to analyze the care provided by health professionals throughout the amputation process in the perspective of the amputee patient; and discuss the process of health care to the person with amputation in the bioethical analysis perspective. Data were analyzed according to content analysis. Three thematic categories emerged: the process of amputation; team performance and rehabilitation. Bioethics permits reflection on the care provided to amputees and problematizes the relationship of the health care process with support available through public health policies. The professional involved in this process has to take responsibility for putting the process in practice and interdisciplinary is essential for the recovery of the amputated patient.

This study aimed to understand the implications and feelings associated with the experience of amputation. The literature review allowed the knowledge of this phenomenon under several perspectives, besides the appropriation of some ideas of the philosopher Merleau-Ponty about body and perception. After this initial knowledge, interviews were performed with people who underwent amputation, by living in their world and listening to their statements. By sharing these moments, it was possible to understand meanings from the perspective of those who experience it, which led to six thematic categories. In addition to understanding those who underwent amputation in their context and in their essence, some facets of this phenomenon were revealed through the phenomenological referential.

Traumatic amputation of the hand is an incapacitating injury, requiring skilled surgical care. Achieving functional recovery in addition to salvage of the amputated extremity should be the goal of the patient care team, after the life of the patient is secured. This is a report of the first five cases of hand replantation after complete traumatic amputation in Manaus (Amazonas, Brazil), carried out at Hospital Pronto-Socorro Dr. João Lúcio Pereira Machado. The patients were all young males, mean age of 22.8 years, presenting at the emergency room with complete sharp amputation of the hand. After initial procedures according to the ATLS® protocol, the following surgical actions were taken: heparinization of the blood vessels of the amputated extremity, fixation of the bones, wide opening of the carpal tunnel, primary vascular anastomoses and repair of nerves and tendons. In four cases, viability and partial functional recovery of the replanted hands were achieved. The unsuccessful case was due to extensive venous thrombosis.Through this preliminary experience, we can state that hand replantation can be performed successfully in a nonspecialized hospital and that optimal results depend on adequate prehospital care, multidisciplinary surgical treatment and careful postoperative assistance.

OBJECTIVES: Limited information is available concerning the post-treatment neutrophil-lymphocyte ratio in critical limb ischemia patients who receive conservative therapy. Accordingly, this study was designed to evaluate the predictive value of the post-treatment neutrophil-lymphocyte ratio in critical limb ischemia patients without surgery. METHOD: From January 2009 to January 2011, critical limb ischemia patients were admitted to a vascular center. The demographic data, patient histories, comorbidities and risk factors were documented, and the differential cell count was determined at admission and seven days later after conservative therapy. The cutoff value of the post-treatment neutrophil-lymphocyte ratio was determined by an ROC curve. Patients were divided into groups A and B according to the cutoff value. Amputation-free survival was compared between groups. Univariate and multivariate analyses were used to identify independent risk factors. RESULT: A total of 172 patients were identified with a mean age 71.98±10.09 years; among them, 122 were male. A value of 3.8 was identified as the cutoff value of the post-treatment neutrophil-lymphocyte ratio. In particular, both quantitative and qualitative changes in the synthesis of soluble proteins in stump tissues, including those of the heat shock protein family (HSP70-1ike) were examined. peer-reviewed; Successful adjustment to amputation can be quantified in a number of ways. Physical measures such as prosthesis use, mobility indices, and activities of daily living have traditionally been suggested as outcome measures in the literature. More recently, the importance of psychological outcomes is gaining attention. This study was based in a regional prosthetic, orthotic, and limb-absence rehabilitation unit. It was developed in order to provide an overview of a range of issues affecting patients, in a multidisciplinary setting. Sixty-two participants completed self-report questionnaires on a range of constructs while attending the unit. From this general study, a central study evolved looking specifically at the relationship between adjustment to amputation and psychological distress, and the possibility of a moderating and/or mediating effect of posttraumatic stress and posttraumatic growth on this relationship. Significant relationships between adjustment to amputation and psychological distress were found. No moderating effect of PTS or PTG was found on the relationship between adjustment to amputation and psychological distress; however, a mediating effect of PTS was apparent. Zielsetzung: Ziel der durchgeführten Studie ist es, bei Patienten mit malignen Extremitätentumoren die alternativen Behandlungsmethoden Amputation und extremitätenerhaltende Operation bezüglich der postoperativen Lebensqualität sowie Funktionalität zu vergleichen und daraus Behandlungsempfehlungen abzuleiten. Patientenkollektiv und Methodik: Insgesamt umfasst die Studie 43 Patienten, die im Zeitraum von 08.02.1990 bis 30.06.2007 an der Orthopädischen Universitätsklinik Tübingen operativ versorgt worden waren. 13 Patienten bilden die Amputationsgruppe und 30 Patienten die extremitätenerhaltende Gruppe. Dabei ergibt sich ein mittleres Follow-Up von 7,8 Jahren. Die Lebensqualität wurde durch den modifizierten Short Form (SF) – 36 Health Survey und die Funktionalität mit Hilfe des Functional Independence Measure (FIM) und Musculoskeletal Tumor Society (MSTS) Score erhoben. Ergebnisse: Hinsichtlich der postoperativen Lebensqualität wurde kein signifikanter Unterschied zwischen den Behandlungsgruppen nachgewiesen. The aim of the study was to investigate how kinematic and kinetic adjustments between level and slope locomotion of persons with transtibial amputation are related to their individual muscular and functional capacities. A quantified gait analysis was conducted on flat and slope surfaces for seven patients with transtibial amputation and a control group of eight subjects to obtain biomechanical parameters. In addition, maximal isometric muscular strength (knee and hip extensors) and functional scores were measured. The results of this study showed that most of the persons with transtibial amputation could adapt to ramp ascent either by increasing ankle, knee, and hip flexion angles of the residual limb and/or by recruiting their hip extensors to guarantee enough hip extension power during early stance. Besides, 6-minute walk test score was shown to be a good predictor of adaptation capacities to slope ascent. In ramp descent, the increase of knee flexion moment was correlated with knee extensor strength and residual-limb length. However, no correlation was observed with functional parameters. CONTEXT AND OBJECTIVE: Diabetic patients present high risk of having to undergo minor or major amputation during their lifetimes, because of ischemia or infection. The aim of this study was to identify and quantify risk factors for major amputation in diabetic patients with foot infections. DESIGN AND SETTING: Retrospective clinical-surgical trial at the Vascular Surgery Service of Santa Casa de São Paulo. METHODS: Ninety-nine patients with diabetic foot infections who underwent 129 hospitalizations in the Vascular Surgery Unit were analyzed in accordance with a pre-established protocol to compare two groups of diabetic patients: one that underwent major amputations and the other that underwent minor amputations or debridements. The patients were predominantly male, in their sixth decade of life, and had type 2 diabetes mellitus. Chronic arterial insufficiency, age, diabetes mellitus duration, ascending lymphangitis, calcaneal lesions, Wagner's classification, laboratory tests and different microorganisms in deep tissue cultures were the risk factors evaluated in all patients. Diante da inquietação com a questão da amputação, suas implicações e sentimentos experimentados pela pessoa que a vivencia, realizou-se esse estudo, com a proposta de compreender esse fenômeno. Inicialmente, a revisão da literatura possibilitou conhecer esse tema sob diversos enfoques, além de permitir a apropriação de algumas idéias do pensamento filosófico de Merleau-Ponty sobre o corpo e a percepção. Após o conhecimento sob essa ótica, realizou-se entrevistas com as pessoas submetidas à amputação, habitando seu mundo, ouvindo seus depoimentos. Compartilhando desses momentos, pôde-se compreender seus significados, a partir da experiência de quem os vivencia, expressando-os sob a forma de seis categorias temáticas. Desse modo, foi possível desvelar algumas facetas do fenômeno amputação, à luz do referencial fenomenológico, além de compreender a pessoa amputada em sua situcionalidade, tal como ela se mostra em si mesma, em sua essência.; Frente a las circunstancias de amputación, sus implicaciones y sentimientos vividos, este estudio fue realizado con la propuesta de comprender este fenómeno. We studied 40 elderly with lower limb amputation (above the malleolus), who attended a hospital outpatient clinic (Orthoses and Prostheses Unity, University Hospital, State University of Campinas), from June 1994 to June 1999. Our objectives were to evaluate the independence level according to the Barthel's Index and to identify relationships between the mentioned Index and the type and level of amputation, as well as the prosthetic use. The subjects had a high degree of independence, according to the Barthel's Index. Diabetes-related lower-extremity amputation incidence and risk factors: a prospective seven-year study in Costa Rica

OBJECTIVE: To analyze the incidence and determinants of lower-extremity amputation (LEA) in people with diabetes in a low-income community in Costa Rica. METHODS: Data on LEA incidence were collected during a seven-year follow-up (2001-2007) in a diabetes patient cohort (n = 572). Risk factors were analyzed using the Cox proportional hazards regression model and baseline variables from the year 2000 (socio- demographic characteristics, comorbidity, metabolic control, treatment, and chronic microvascular complications). RESULTS: LEA incidence was 6.02 per 1 000 person-years (8.65 in men and 4.50 in women). Known risk factors (sex, years of diabetes, elevated glycated hemoglobin [HbA1c], retinopathy, insulin therapy, and prior amputation) were highly significant. CONCLUSIONS: Those most likely to undergo LEA among Costa Rican diabetic patients were men with 10 or more years of diabetes and average HbA1c > 8% who used insulin and had diabetic retinopathy. Patients on insulin therapy were at greatest risk, especially those with a previous amputation. Diabetic patients with the above-mentioned profile should be considered to be at very high risk of LEA and followed closely by the health care system.
