Purpose: We tested whether the combination of 4 established cell cycle regulators (p53, pRB, p21 and p27) could improve the ability to predict clinical outcomes in a large multi-institutional collaboration of patients with pT3-4N0 or pTany Npositive urothelial carcinoma of the bladder. We also assessed whether the combination of molecular markers is superior to any individual biomarker. Materials and Methods: The study comprised 692 patients with pT3-4N0 or pTany Npositive urothelial carcinoma of the bladder treated with radical cystectomy and bilateral lymphadenectomy (median followup 5.3 years). Scoring was performed using advanced cell imaging and color detection software. The base model incorporated patient age, gender, stage, grade, lymphovascular invasion, number of lymph nodes removed, number of positive lymph nodes, concomitant carcinoma in situ and adjuvant chemotherapy. Results: Individual molecular markers did not improve the predictive accuracy for disease recurrence and cancer specific mortality. Combination of all 4 molecular markers into number of altered molecular markers resulted in significantly 1 higher predictive accuracy than any single biomarker (p < 0.001.). Carcinoma epidermóide de palato mole é relativamente raro, representando em torno de 15% das neoplasias da orofaringe, onde predominam os tumores de base de língua e tonsila palatina. Não há consenso sobre a melhor forma de tratamento para estes tumores, pois apesar de todos os avanços e associações terapêuticas, as taxas de sobrevida têm se alterado pouco nas últimas décadas. Mesmo com os avanços da cirurgia reparadora, minimizando as seqüelas da cirurgia oncológica, a radioterapia exclusiva e/ou associada à quimioterapia é a forma de tratamento mais utilizada em muitos serviços, principalmente para os tumores em estádio clínico avançado. Entretanto, muitos pacientes não apresentam resposta a tal terapêutica. Desse modo, selecionar um grupo de pacientes com maiores possibilidades de resposta à irradiação poderia orientar a melhor indicação terapêutica para cada caso. Avaliou-se a influência da exposição ao flúor (F) através da água de beber, da velocidade de crescimento das unhas, da idade e do gênero na concentração deste elemento nas unhas das mãos e dos pés. Em adição, verificou-se a correlação entre as concentrações de F na saliva total, saliva do ducto da parótida e unhas das mãos e dos pés. Participaram do estudo 300 indivíduos, das faixas etárias de 3-7,14-20, 30-40 e 50-60 anos, residentes de cinco comunidades brasileiras, sendo duas no Estado de São Paulo (Pirajuí e Bauru, com água não fluoretada e 0,7 mgF/L na água de abastecimento, respectivamente) e três no Estado da Paraíba (Cajazeirinhas, Brejo dos Santos e Brejo das Freiras, com 0,2, 0,7 e 1,7 mgF/L na água de consumo, respectivamente). Foram coletadas duas ou três (apenas em Bauru) amostras de água de beber, além de duas amostras de: unhas dos pés, unhas das mãos, saliva total e saliva do ducto da parótida de cada indivíduo. Os marcadores de estresse músculo-esqueletal (MSMs), assim denominados, surgem pela primeira vez em 1995 no trabalho pioneiro de Hawkey e Merbs. De lá pra cá, diversos estudos se prestaram a aprimorar o método desenvolvido pelos pesquisadores e, ainda, associar de maneira mais concreta os graus de desenvolvimento de enteses a atividades desenvolvidas ao longo da vida de um indivíduo. Sob tais circunstâncias, buscamos neste trabalho a verificação da implementação de uma metodologia de análise revisada, testada e aprimorada dos marcadores de estresse músculo-esqueletal e a possibilidade de correlação entre os graus de desenvolvimento das enteses estudadas com as possíveis atividades desenvolvidas pelos indivíduos analisados, provenientes do sambaqui costeiro catarinense Jabuticabeira II (JAB-II). A escolha pela análise dos remanescentes ósseos deste sítio decorreu da vasta informação disponível em relação aos mesmos e ao sítio em si, provindos de outros trabalhos desenvolvidos previamente. Introdução: A evolução dos marcadores sorológicos da hepatite B em pacientes com hepatite B crônica coinfectados com o vírus da imunodeficiência humana (HIV) tem sido pouco documentada. Objetivos: O objetivo deste estudo foi descrever a evolução dos marcadores sorológicos AgHBe e AgHBs, com ênfase na avaliação da frequência de perda definitiva ou transitória desses marcadores, neste grupo de pacientes. Buscamos, também, comparar as variáveis clínicas e demográficas desses pacientes segundo a evolução desses marcadores sorológicos. Pacientes e métodos: A população de estudo foi composta por pacientes atendidos em um ambulatório de referência para atendimento a pacientes infectados pelo HIV em São Paulo, Brasil. Todos os pacientes selecionados eram portadores de HIV e de hepatite B crônica. Foram incluídos nesse estudo pacientes AgHBs positivos, com confirmação da presença desse marcador em, no mínimo, duas sorologias consecutivas, com intervalo mínimo de seis meses entre elas. Em mamíferos, a espermatogênese é mantida pela autorrenovação e diferenciação das células-tronco espermatogoniais (SSC). Apesar da grande importância do SSC para a fertilidade masculina, em Bos taurus pouco se sabe sobre a sua identificação e biologia celular. Para roedores, mais de 30 marcadores para células germinativas indiferenciadas já foram descritos. No entanto, ainda não é conhecido um marcador específico apenas para SSC. Quase todos são também expressos por gonócitos, espermatogônias mais diferenciadas ou mesmo células somáticas. Yin Yang 2 (YY2) é um factor de transcrição expresso nas células com a morfologia de gonócitos e SSC, sendo um candidato a marcador de SSC. Assim, a identificação de novos marcadores para SSC e factores que afectam a sua expressão, tais como a idade, são fundamentais para o desenvolvimento da biotecnologia como transgenia e tratamento de infertilidade, nos quais as SSC poderiam ser ferramentas biológicos importantes. Methods- This was a cohort study with 96 patients undergoing cardiac surgery assessed at the following 4 different time periods: the day before surgery (D0); the 3rd postoperative day (D3); between the 7th and 10th postoperative days (D7-10); and the 30th postoperative day (D30). During each period, we evaluated demo- graphic variables (sex and age), surgical variables (type and duration , extracorporeal circulation), and serum dosages of the markers of myocardial injury and inflam- matory response. Results - Of all patients, 12 (12.5%) met the clinical criteria for a diagnosis of postpericardiotomy syndrome, and their mean age was 10.3 years lower than the age of the others (P=0.02). The results of the serum markers for tissue injury and inflammatory response were not signifi- cantly different between the 2 assessed groups. Serum hyaluronan and collagen IV as non-invasive markers of liver fibrosis in patients from an endemic area for schistosomiasis mansoni: a field-based study in Brazil

Non-invasive markers of fibrosis have been used to diagnose liver fibrosis in a variety of diseases. Hyaluronic acid (HA) and collagen IV (C-IV) levels were measured in the sera of patients from an endemic area for schistosomiasis in Brazil to diagnose and to rank the intensity of liver fibrosis. Seventy-nine adult patients with schistosomiasis, in the age range of 21-82 years (49 ± 13.4) were submitted to clinical and ultrasonographic examinations. Ultrasound was employed to diagnose and categorise liver fibrosis according to World Health Organization patterns. Serum HA and C-IV levels were measured using commercial ELISA kits. Ultrasound revealed six patients with intense liver fibrosis, 21 with moderate, 23 with light and 29 without. Serum HA was able to separate individuals with fibrosis from those without (p < 0.001) and light from intense fibrosis (p = 0.029), but C-IV was not (p = 0.692). The HA diagnostic accuracy for fibrosis was 0.89. The 115.4 ng/mL cut-off level diagnosed patients with fibrosis (sensitivity 0.98, specificity 0.64). HA correlated positively with portal hypertension. Periportal fibrosis (subjective evaluation), age and collateral circulation predicted HA increase. In conclusion, we propose that serum HA can be used to identify patients with liver fibrosis in an endemic area for schistosomiasis mansoni in Brazil.

Type 1 (T1R) and Type 2 (T2R) leprosy reactions are complications in the clinical management of leprosy patients because they can lead to neural damage and impairment, resulting in irreversible deformities and disabilities. This thesis, presented as research article/manuscript has investigated potential markers for the diagnosis and prognosis of leprosy reactions. In the first study, we evaluated a multicentric cohort of leprosy patients with single skin lesion which is considered the earliest clinical manifestation of the disease. In this study, at the moment of diagnosis a skin biopsy was collected for histopathology and for the investigation of Mycobacterium leprae DNA by polymerase chain reaction (ML-PCR). After diagnosis patients were treated with single dose of Rifampicin, Ofloxacin and Minocyclin (ROM) and were clinically monitored during 3 years .During follow up, around 15% of patients developed T1R. In multivariate analysis, age > 40 years and MLPCR positivity were associated with T1 manifestation. Introduction: Dengue is the most important emerging and reemerging disease in morbidity and mortality today. Among those affected. 2.5% die, and is increasing the number of deaths in children annually. In this group, in particular, early diagnosis is more difficult. Objectives: To identify clinical and laboratory markers for diagnosis and severity of dengue infection cases reported in the period 2001 to 2009 in children under 15 years old living in the city of Goiania, laboratory confirmed, describe sociodemographic data, tourniquet test and admissions performed. Methodology: Database Information System for disease surveillance of the City Health Department to Goiania was used to do a retrospective, descriptive, exploratory quantitative analysis, using the test of statistical significance t-student test, with significance level 5% and the final statistical analysis in SPSS 17.0. Results: In the period 2001 to 2009 were reported in the city of Goiania, 124,794 cases of dengue, of these 113,744 (91%), referring to residents of the capital. Among those under 15 years reported in this period, 22,278 were residents of Goiania. A Case-Only Genome-wide Association Study of Gender- and Age-specific Risk Markers for Childhood Leukemia

Males and age group 1 to 5 years show a much higher risk for childhood acute lymphoblastic leukemia (ALL). We performed a case-only genome-wide association study (GWAS), using the Illumina Infinium HumanCoreExome Chip, to unmask gender- and age-specific risk variants in 240 non-Hispanic white children with ALL recruited at Texas Children’s Cancer Center, Houston, Texas. Besides statistically most significant results, we also considered results that yielded the highest effect sizes. Existing experimental data and bioinformatic predictions were used to complement results, and to examine the biological significance of statistical results. Our study identified novel risk variants for childhood ALL. The SNP, rs4813720 (RASSF2), showed the statistically most significant gender-specific associations (P < 2 x 10-6). Likewise, rs10505918 (SOX5) yielded the lowest P value (P < 1 x 10-5) for age-specific associations, and also showed the statistically most significant association with age-at-onset (P < 1 x 10-4). Two SNPs, rs12722042 and 12722039, from the HLA-DQA1 region yielded the highest effect sizes (odds ratio (OR) = 15.7; P = 0.002) for gender-specific results, and the SNP, rs17109582 (OR = 12.5; P = 0.006), showed the highest effect size for age-specific results. Sex chromosome variants did not appear to be involved in gender-specific associations. The HLA-DQA1 SNPs belong to DQA1*01:07and confirmed previously reported male-specific association with DQA1*01:07. BACKGROUND: Transfusion of packed red blood cells (PRBCs) saves lives in the neonatal critical care setting and is one of the most common interventions in the preterm infant. The number and volume of PRBC transfusions are associated with several major neonatal morbidities, although a direct causal link between transfusion and major neonatal morbidity is still to be proven. Transfusion-related immunomodulation (TRIM) may underlie these adverse outcomes, yet it has received little attention in the high-risk preterm infant. METHODS: One transfusion event was studied in infants ≤28 wk gestation between 2 and 6 wk postnatal age (n = 28). Plasma inflammatory cytokines and markers of endothelial activation were measured in the infants before and 2–4 h after transfusion, as well as in the donor pack. RESULTS: Median (range) age at transfusion was 18 (14–39) days with the pretransfusion hemoglobin level at 9.8 (7.4–10.2) g/dl. Interleukin (IL)-1β (P = 0.01), IL-8 (P = <0.001), tumor necrosis factor-α (P = 0.008), and monocyte chemoattractant protein (P = 0.01) were increased after transfusion. A similar elevation in markers of endothelial activation was seen after transfusion with increased plasma macrophage inhibitory factor (P = 0.005) and soluble intracellular adhesion molecule-1 (P = <0.001). CONCLUSION: Production of inflammatory cytokines and immunoactivation of the endothelium observed after the transfusion of PRBCs in the preterm infant may be a manifestation of TRIM. The detection of fibrosis within nonalcoholic fatty liver disease (NAFLD) is important for ascertaining prognosis and the stratification of patients for emerging therapeutic intervention. We validated the Original European Liver Fibrosis panel (OELF) and a simplified algorithm not containing age, the Enhanced Liver fibrosis panel (ELF), in an independent cohort of patients with NAFLD. Furthermore, we explored whether the addition of simple markers to the existing panel test could improve diagnostic performance. One hundred ninety-six consecutively recruited patients from 2 centers were included in the validation study. The diagnostic accuracy of the discriminant scores of the ELF panel, simple markers, and a combined panel were compared using receiver operator curves, predictive values, and a clinical utility model. The ELF panel had an area under the curve (AUC) of 0.90 for distinguishing severe fibrosis, 0.82 for moderate fibrosis, and 0.76 for no fibrosis. Simplification of the algorithm by removing age did not alter diagnostic performance. Addition of simple markers to the panel improved diagnostic performance with AUCs of 0.98, 0.93, and 0.84 for the detection of severe fibrosis, moderate fibrosis, and no fibrosis, respectively. Die aktuelle Forschung beobachtet Zusammenhänge zwischen Depression und Neurodegeneration. Dabei steht eine late-onset-Depression (LOD), mit einem Beginn nach dem 60ten Lebensjahr, im Zusammenhang mit dementiellen Prozessen, sowohl als Prodrom wie auch während einer Demenz (Blazer 2003). Eine early-onset-Depression (EOD), mit einem Beginn vor dem 60ten Lebensjahr, dahingegen stellt aufgrund neurotoxischer Effekte einen Risikofaktor für die Entwicklung einer Alzheimer Demenz (AD) dar (Ownby et al 2006). Um die Relation von Depression und Neurodegeneration zu untersuchen sind auch neuropsychologische Verfahren geeignet. Rapp und Kollegen (2005) zeigten, dass Personen mit einer LOD und vaskuläre Risikofaktoren, Defizite der Exekutivfunktionen hatten, wohingegen Personen mit einer EOD Gedächtniseinschränkungen aufwiesen. Neben Gedächtnismaßen eignen sich jedoch auch die Riechfähigkeit (Berg 2008) und Amyloidpeptide im Serum (Song et al 2011) zur AD-Früherkennung. Ausgehend von den Vorbefunden wurde erwartet, dass Personen mit einer EOD aufgrund anhaltender neurotoxischer Effekte auf hippocampale Strukturen schlechtere Gedächtnisleistung aufweisen. Associations between life course socioeconomic position (SEP) and novel biological risk markers for coronary heart disease such as inflammatory markers are not well understood. Most studies demonstrate inverse associations of life course SEP with C-reactive protein (CRP), interleukin-6 (IL-6) and fibrinogen, however little is known about associations between life course SEP and other inflammatory markers including intercellular adhesion molecule-1 (ICAM-1), tumor necrosis factor II (TNFR2), lipoprotein phospholipase A2 (Lp-PLA2) activity, monocyte chemoattractant protein-1 (MCP-1) or P-selectin. The objectives of this analysis were to determine whether three life course SEP frameworks ("accumulation of risk", "social mobility" and "sensitive periods") are associated with the aforementioned inflammatory markers. We examined 1413 Framingham Offspring Study participants (mean age 61.2+/-8.6 years, 54% women), using multivariable regression analyses. In age- and sex-adjusted regression analyses, cumulative SEP ("accumulation of risk" SEP framework), for low vs. high SEP, was inversely associated with CRP, IL-6, ICAM-1, TNFR2, Lp-PLA2 activity, MCP-1 and fibrinogen. Background: Reliable antenatal identification of pre-eclampsia and small for gestational age is crucial to judicious allocation of monitoring resources and use of preventative treatment with the prospect of improving maternal/perinatal outcome. The purpose of this systematic review was to determine the accuracy of five serum analytes used in Down's serum screening for prediction of pre-eclampsia and/or small for gestational age. Methods: The data sources included Medline, Embase, Cochrane library, Medion (inception to February 2007), hand searching of relevant journals, reference list checking of included articles, contact with experts. Two reviewers independently selected the articles in which the accuracy of an analyte used in Downs's serum screening before the 25th gestational week was associated with the occurrence of pre-eclampsia and/or small for gestational age without language restrictions. Two authors independently extracted data on study characteristics, quality and results. Results: Five serum screening markers were evaluated. 44 studies, testing 169,637 pregnant women (4376 pre-eclampsia cases) and 86 studies, testing 382,005 women (20,339 fetal growth restriction cases) met the selection criteria. Microsatellite markers may provide evidence of faulty DNA mismatch repair (MMR) via the detection of microsatellite instability (MSI). The choice of microsatellite markers may impact on the MSI detection rate. In hereditary non-polyposis colon cancer (HNPCC), several informative microsatellite markers have been recommended. Two of these, BAT 25 and BAT 26, are quasi-homozygous, enabling analysis of tumour DNA in the absence of paired normal DNA. Sixty-six breast cancer patients under 45 years of age at diagnosis were examined for MSI at BAT 25 and BAT 26. Tumour DNA was extracted from paraffin-embedded tissue. No MSI was detected at the BAT 25 or BAT 26 loci. An additional five microsatellite markers, known to be informative for HNPCC, were examined for MSI in these patients. Apparently-normal profiles were achieved. A tabulated survey of 306 microsatellite markers used to detect MSI in breast cancer revealed that only 35.5% of markers detected MSI at an average rate of 2.9%. The MSI detection rate at the specific HNPCC markers varied from 0% to 10% in breast cancer, with D175250 and TP53 being the HNPCC markers most suitable for analysis of breast cancer. The size of the microsatellite marker's repeat unit did not impact on MSI detection rates. Introdução: Os distúrbios hemostáticos são pouco estudados na Síndrome dos Ovários Policísticos (SOP). Mulheres com SOP manifestam frequentemente fatores de risco tromboembólico como obesidade, hiperandrogenismo e resistência insulínica, representando evidências indiretas que sugerem maior probabilidade de alterações na coagulação. Objetivo: Avaliar alguns marcadores do sistema hemostático e sua associação com parâmetros clínicos e laboratoriais em mulheres com SOP. Sujeitos e Método: Estudo transversal para avaliação de 45 mulheres com SOP atendidas no Ambulatório de Ginecologia Endócrina do Departamento de Tocoginecologia da Faculdade de Ciências Médicas, UNICAMP, e de 45 mulheres com função gonadal normal, pareadas por idade em anos completos (± 2 anos) e IMC (± 2kg/m2). Foram avaliados parâmetros clínicos [circunferência da cintura (CC), circunferência do quadril (CQ), relação cintura-quadril (C/Q), índice Ferriman-Gallwey (IFG)] e laboratoriais [glicemia de jejum, insulina de jejum, testosterona total (TT) e livre (TL)], e dosados os marcadores de hemostasia: inibidor do ativador do plasminogênio do tipo 1 (PAI-1), inibidor da fibrinólise ativado pela trombina (TAFI), D-dímero e teste de geração de trombina (TGT). The establishment of potential age markers of Madeira wine is of paramount significance as it may contribute to detect frauds and to ensure the authenticity of wine. Considering the chemical groups of furans, lactones, volatile phenols, and acetals, 103 volatile compounds were tentatively identified; among these, 71 have been reported for the first time in Madeira wines. The chemical groups that could be used as potential age markers were predominantly acetals, namely, diethoxymethane, 1,1-diethoxyethane, 1,1-diethoxy-2-methyl-propane, 1-(1-ethoxyethoxy)-pentane, trans-dioxane and 2-propyl-1,3-dioxolane, and from the other chemical groups, 5-methylfurfural and cis-oak-lactone, independently of the variety and the type of wine. GC × GC-ToFMS system offers a more useful approach to identify these compounds compared to previous studies using GC−qMS, due to the orthogonal systems, that reduce coelution, increase peak capacity and mass selectivity, contributing to the establishment of new potential Madeira wine age markers.
