Adolescent pregnancy appears today as an intricate tapestry where different dimensions interact. In our study we examined the associations between individual, sociodemographic, familial, and relational variables and their impact in the occurrence of pregnancy and adolescents’ adjustment to it. Participants were Portuguese pregnant and non-pregnant adolescents (N = 833). Ecological contexts were characterized, and individual and relational adjustment (depressive symptoms and quality of life; perceived quality of relationship with significant others – parents, romantic partner and friends) were evaluated. Differences between the ecologies of adolescents in both groups were identified. Familial and relational variables were significantly associated with both the risk of pregnancy and more difficulties in adjustment. Implications for preventive intervention are discussed.

Em inúmeros países, a gravidez na adolescência constitui problema emergente. Este artigo descreve as razões que levam à sua ocorrência e seus reflexos sobre a família, segundo o olhar de seus próprios membros. A pesquisa, de abordagem qualitativa, desenvolveu o método da análise de narrativa para entrevistar 19 pessoas que viveram a experiência da gravidez na adolescência no contexto da família. Das narrativas emergiram duas categorias descritivas: a) As razões da gravidez na adolescência e b) Os reflexos da gravidez sobre a família e a vida das adolescentes. A gravidez na adolescência provocou impacto na dinâmica familiar, e suas características dependeram das crenças e valores prevalentes em cada família. Dados sistematizados a este respeito devem ser obtidos para prover uma assistência apropriada às demandas das adolescentes e dos membros de sua família.; Adolescent pregnancy is a growing public health problem that affects several countries. The purpose of this study was to present a spatial analysis of the social vulnerability of teenage pregnancy by geoprocessing data on births and deaths present on the Brazilian Ministry of Health databases in order to support intersectoral management actions and strategies based on spatial analysis in neighborhood areas. The thematic maps of the educational, occupational, birth and marital status of mothers, from all births and deaths in the city, presented a spatial correlation with teenage pregnancy. These maps were superimposed to produce social vulnerability map of adolescent pregnancy and women in general. This process presents itself as a powerful tool for the study of social vulnerability.

O objetivo desta pesquisa foi estudar um grupo de 106 gestantes adolescentes multigestas, comparando-as com 510 adolescentes primigestas quanto a variáveis demográficas, obstétricas e psicossociais. As pacientes foram recrutadas no ambulatório de obstetrícia no setor de gravidez na adolescência da clínica obstétrica do HC-FMUSP entre janeiro de 2000 a janeiro de 2006. As pacientes tinham até 18 anos e tiveram acompanhamento multiprofissinal. Os dados foram coletados de prontuário eletrônico (Acess-microsoft 98), apresentados de forma descritiva e analisados por teste do χ2, exato de Fisher, t de Student, Mann-Whytney e índice de Kappa, de acordo com a indicação do uso de cada um. O presente trabalho apresenta uma revisão seletiva e não sistemática da literatura a respeito do fenômeno da gestação na adolescência. Três eixos temáticos orientam a organização do trabalho: (a) Riscos e problemas associados à gestação na adolescência; (b) Fatores precursores relacionados à gravidez na adolescência; e (c) Fatores sócio-culturais associados ao desejo de ser mãe na adolescência. Entre as conclusões, destaca-se que a gravidez na adolescência é uma experiência que pode ter consequências tanto negativas quanto positivas para os adolescentes. A maternidade adolescente impõe à jovem novas demandas familiares, educacionais e laborais, além daquelas próprias da adolescência. O objetivo deste trabalho foi revisar estudos sobre a gravidez e a maternidade adolescente, com base em cinco temas: fatores associados à gravidez adolescente, impacto para a jovem, vivência da maternidade, interação mãe-bebê e apoio familiar. Em cada tema, os estudos foram analisados e comparados quanto aos aspectos metodológicos e seus principais achados, o que revelou diversos aspectos contraditórios. Grande parte dos estudos é descritiva, com delineamento transversal e quantitativo, comparando adolescentes e adultas, revelando uma vivência predominantemente negativa da gravidez e da maternidade. Já os estudos qualitativos indicam alguns aspectos positivos. Relatos negativos foram particularmente comuns na gestação, ocorrendo uma adaptação da jovem à maternidade após o nascimento do bebê. A pregnant adolescent’s vulnerability increases when she is a victim of intrafamilial violence and drug addiction, which cause physical and biopsychosocial damage to the mother and her baby. Objective Present and analyze the case of an adolescent who is addicted to drugs, pregnant and the victim of lifelong intrafamilial violence. Method A case study based on a semi-structured interview conducted in the Obstetrics Emergency Unit at the Teaching Hospital of the University of São Paulo. The data were interpreted and analyzed using Content Analysis. Results intrafamilial violence experienced at the beginning of the adolescent’s early relationships seriously affected her emotional maturity, triggering the development of psychopathologies and leaving her more susceptible to the use and abuse of alcohol and other drugs. The adolescent is repeating her history with her daughter, reproducing the cycle of violence. Conclusion Adolescent pregnancy combined with intrafamilial violence and drug addiction and multiplies the adolescent’s psychosocial vulnerability increased the adolescent’s vulnerability.

This study evaluated the association between adolescent pregnancy and the completion of basic education, mediated by macrosocial indicators. A cross-sectional household survey was conducted with individuals between the ages of 18 and 24 in three Brazilian cities. For the purposes of this study, individuals between the ages of 20 and 24 were selected from this sample survey that included 4,634 people. A total of 29.6% of the girls declared that they had become pregnant prior to reaching the age of 20, while 21.4% of the boys stated that they had made a girl pregnant in adolescence. Girls from households with a per capita family income of US$70 or less and who became pregnant at least once during adolescence were more likely to have not completed basic education; whereas from households with a per capita family income of US$70 or less, with parents who separated before the adolescent reached the age of 20 and that had made a partner pregnant prior reaching the age of 20 were more likely to have not completed basic education. It is vital that the school system provides girls and boys with guidance on sexuality and contraception and encourages them to remain in education.

Sexual activity during adolescence can lead to unwanted pregnancy, which in turn can result in serious maternal and fetal complications. The present study aimed to evaluate the complications related to adolescent pregnancy, through a systematic review using the Medical Subject Headings: “pregnancy complication” AND “adolescent” OR “pregnancy in adolescence”. Only full original articles in English or Portuguese with a clearly described methodology, were included. No qualitative studies, reviews or meta-analyses, editorials, case series, or case reports were included. The sample consisted of 15 articles; in that 10 were cross-sectional and 5 were cohort studies. The overall prevalence of adolescent pregnancy was 10%, and among the Brazilian studies, the adolescent pregnancy rate was 26%. The cesarean delivery rate was lower than that reported in the general population. The main maternal and neonatal complications were hypertensive disorders of pregnancy, prematurity and low birth weight, respectively. Adolescent pregnancy is related to increased frequency of neonatal and maternal complications and lower prevalence of cesarean delivery.

Every year, 16 million women aged 15 to 19 years give birth globally. Adolescent births account for 11% of all births globally and 23% of the overall burden of disability and diseases due to pregnancy and childbirth. In the United States, 750,000 adolescents (15-19 years) become pregnant each year, making the United States the developed country with the highest rates of adolescent pregnancy. The economic burden of adolescent pregnancy in the U. S. is $7-15 billion per year. Adolescent pregnancy brings risks associated with pregnancy induced hypertension, preterm infants, maternal and neonatal mortality. Social factors include poverty, low educational levels, alcohol, and drug use. Between 30-50% of adolescent mothers who have a first birth before age 18 years will have a second child within 12 to 24 months. Subsequent adolescent pregnancies compound fetal and maternal risks. Many vulnerable adolescent mothers succumb to external pressures and have a repeat adolescent pregnancy while others are able to overcome the challenges of an adolescent pregnancy and prevent a repeat adolescent pregnancy. Sub-Saharan Africa suffers high rates of maternal and neonatal mortality and morbidity. Adolescent mothers (age ≤ 19 years at time of delivery) are often considered to be particularly prone to adverse pregnancy outcomes, such as low birth weight (LBW, birth weight < 2,500 grams) and prematurity. Various risk factors have been identified so far to contribute to adverse pregnancy outcome, but available data remains insufficient for low income countries. The considerably high rate of adolescent mothers in many regions of sub-Saharan Africa reflects the need for assessing the prevalence of adolescent pregnancies in this area as well as their influence on obstetric outcomes and the necessity of analyzing further associated risk factors. The present study assesses the prevalence of adolescent pregnancies in a semi-urban and rural region in the Central African country Gabon. Furthermore, it analyzes maternal adolescence among other parameters as a potential risk factor for adverse pregnancy outcome, focusing on LBW. A retrospective cohort study was performed. Data from birth registers from three different health centers in Gabon were entered in a purpose built database. OBJECTIVE: The objective of this study was to evaluate whether adolescent pregnancy is a risk factor for low birth weight (LBW) babies. METHODS: This was a cross-sectional study of mothers and their newborns from a birth cohort in Aracaju, Northeastern Brazil. Data were collected consecutively from March to July 2005. Information collected included socioeconomic, biological and reproductive aspects of the mothers, using a standardized questionnaire. The impact of early pregnancy on birth weight was evaluated by multiple logistic regression. RESULTS: We studied 4,746 pairs of mothers and their babies. Of these, 20.6% were adolescents (< 20 years of age). Adolescent mothers had worse socioeconomic and reproductive conditions and perinatal outcomes when compared to other age groups. Having no prenatal care and smoking during pregnancy were the risk factors associated with low birth weight. Adolescent pregnancy, when linked to marital status "without partner", was associated with an increased proportion of low birth weight babies. CONCLUSIONS: Adolescence was a risk factor for LBW only for mothers without partners. Smoking during pregnancy and lack of prenatal care were considered to be independent risk factors for LBW.

Adolescent pregnancy is a current problem which raises concern due to its individual, familiar and collective consequences. Fifteen million adolescents give birth each year in the world. Abortion is the preferred option used in unwanted pregnancies. Adolescent pregnancy is frequent in Nocaima, Cundinamarca and is a community concern in this small town initiating its process of becoming a healthy municipality. As such, the community has highlighted this problem to be studied and submitted to intervention to promote a free and responsible sexuality decreasing unwanted adolescent pregnancies. Objective: To find data on contraception, pregnancy and related factors in selected adolescents therefore, improving current incomplete information. Methods: Descriptive observational study with survey application on 226 female 14 to 19 year old students from three high school facilities in Nocaima including 8th to 11th graders. Results: 88.9% of the participants were between 14 and 17 years of age. 66.8% of the adolescents claim to use correctly contraceptive methods and 28.8% have had sexual intercourse with an average initiation at age 15. 11.1% have been pregnant once in their lives and of these 57.1 % ended in induced abortion and 66.8% were school dropouts. OBJETIVO: Identificar as principais características socioeconômicas, demográficas, antropométricas e comportamentais, bem como os resultados perinatais da gravidez na adolescência. Conhecer os tipos de serviços de saúde utilizados pelas gestantes adolescentes. MÉTODOS: A amostra de 2.429 partos de mulheres residentes em São Luís, MA, abrangeu 94% dos nascimentos hospitalares. As mulheres foram separadas em seis grupos de idade para melhor avaliação do comportamento das variáveis entre os dois grupos de adolescentes (abaixo de 18 anos; 18 a 19 anos) e entre as adolescentes e as demais mulheres. O teste do qui-quadrado foi utilizado para a comparação de proporções e a razão de prevalências foi empregada como medida de efeito. RESULTADOS: Das 2.429 mulheres, 714 eram adolescentes (29,4%). Seu coeficiente específico de fecundidade, 72,2 por mil, foi mais elevado que em outras regiões do País. As adolescentes apresentaram piores condições socioeconômicas e reprodutivas que as demais mulheres, maior proporção de pré-natal inadequado (39,2%) e muitas não tinham companheiro (34,5%). Estudo qualitativo, que objetivou apreender o significado da gravidez da adolescente para seus familiares. Empregou-se a entrevista semi-estruturada e discurso do sujeito coletivo. A gravidez da adolescente é representada como problema a ser enfrentado com o suporte familiar. As famílias preocupam-se e se mobilizam para resolver as adversidades. Além do choque pela notícia, impotência quanto à prevenção da gravidez, conformismo, alegria e melhora no relacionamento familiar pela chegada do bebê, evidenciou-se a frustração devido à interrupção/mudança no projeto de vida familiar em relação à adolescente sem um relacionamento estável com o pai da criança. This study evaluated the association between adolescent pregnancy and the completion of basic education, mediated by macrosocial indicators. A cross-sectional household survey was conducted with individuals between the ages of 18 and 24 in three Brazilian cities. For the purposes of this study, individuals between the ages of 20 and 24 were selected from this sample survey that included 4,634 people. A total of 29.6% of the girls declared that they had become pregnant prior to reaching the age of 20, while 21.4% of the boys stated that they had made a girl pregnant in adolescence. Girls from households with a per capita family income of US$70 or less and who became pregnant at least once during adolescence were more likely to have not completed basic education; whereas from households with a per capita family income of US$70 or less, with parents who separated before the adolescent reached the age of 20 and that had made a partner pregnant prior reaching the age of 20 were more likely to have not completed basic education. It is vital that the school system provides girls and boys with guidance on sexuality and contraception and encourages them to remain in education.

OBJECTIVE: The objective of this study was to evaluate whether adolescent pregnancy is a risk factor for low birth weight (LBW) babies. METHODS: This was a cross-sectional study of mothers and their newborns from a birth cohort in Aracaju, Northeastern Brazil. Data were collected consecutively from March to July 2005. Information collected included socioeconomic, biological and reproductive aspects of the mothers, using a standardized questionnaire. The impact of early pregnancy on birth weight was evaluated by multiple logistic regression. RESULTS: We studied 4,746 pairs of mothers and their babies. Of these, 20.6% were adolescents (< 20 years of age). Adolescent mothers had worse socioeconomic and reproductive conditions and perinatal outcomes when compared to other age groups. Having no prenatal care and smoking during pregnancy were the risk factors associated with low birth weight. Adolescent pregnancy, when linked to marital status "without partner", was associated with an increased proportion of low birth weight babies. CONCLUSIONS: Adolescence was a risk factor for LBW only for mothers without partners. Smoking during pregnancy and lack of prenatal care were considered to be independent risk factors for LBW.

OBJECTIVE: To identify risk factors for adolescent pregnancy among female students in Bogotá, Colombia METHODS: This was a retrospective study of cases and controls matched by age, identified by means of a survey on the sexual behavior of adolescent students in Bogotá (Encuesta sobre el Comportamiento Sexual de los Adolescentes Escolarizados en Bogotá) conducted in the first semester of 2010. All 272 cases and 544 randomly-selected controls were taken from 39 044 total records. Variables considered were sociodemographics, household structure, and family environment; sexual relationships and pregnancy; and knowledge of sexual and reproductive health. Matching and conditional logistic regression were used to adjust for possible confounding factors RESULTS: The factors associated with increased risk of adolescent pregnancy based on multivariate analyses were: attending public school (odds ratio [OR]=2.25; 95% confidence interval [95% CI]: 1.45-3.51); history of siblings with adolescent pregnancy (OR =1.98; 95% CI: 1.55-2.76); early first sexual intercourse (12 years of age or less) (OR =2.34; 95% CI: 1.01-5.40); having a self-reported low- or average-level of contraceptive knowledge (OR =3.92; 95% CI: 1.96-7.83); previous pregnancy (OR =14.09; 95% CI: 8.74- 22.70); and not living with both parents (OR 3.58; 95% CI: 2.10-6.16 CONCLUSIONS: Factors related to individual...
