Background Suicide is a leading cause of death worldwide; however, little information is available about the treatment of suicidal people, or about barriers to treatment. Aims To examine the receipt of mental health treatment and barriers to care among suicidal people around the world. Method Twenty-one nationally representative samples worldwide (n=55 302; age 18 years and over) from the World Health Organization`s World Mental Health Surveys were interviewed regarding past-year suicidal behaviour and past-year healthcare use. Suicidal respondents who had not used services in the past year were asked why they had not sought care. Results Two-fifths of the suicidal respondents had received treatment (from 17% in low-income countries to 56% in high-income countries), mostly from a general medical practitioner (22%), psychiatrist (15%) or non-psychiatrist (15%). Those who had actually attempted suicide were more likely to receive care. Low perceived need was the most important reason for not seeking help (58%), followed by attitudinal barriers such as the wish to handle the problem alone (40%) and structural barriers such as financial concerns (15%). Background: Candidemia affects patient populations from neonates to the elderly. Despite this, little information is available about the epidemiology of candidemia in elderly patients. Methods: We performed a retrospective analysis of 987 episodes of candidemia in adults (>14 years of age) from the databases of three laboratory-based surveys of candidemia performed at 14 tertiary care hospitals. Patients aged >= 60 years were considered elderly (group 1, n = 455, 46%) and were compared to younger patients (group 2, n = 532, 54%) regarding demographics, underlying diseases, comorbidities, exposure to medical procedures, species, treatment, and outcome. Results: The median APACHE II score was significantly higher in the elderly patients (19 vs. 15, p = 0.03). Variables that were observed significantly more frequently in elderly patients included admission to an intensive care unit, diabetes mellitus, renal failure, cardiac disease, lung disease, receipt of antibiotics or H2 blockers, insertion of a central venous catheter, mechanical ventilation, and candidemia due to Candida tropicalis. The 30-day mortality of elderly patients was significantly higher than that of younger patients (70% vs. 45%, p < 0.001). Factors associated with higher mortality by multivariate analysis included APACHE II score and being in group 1 (elderly). Factors associated with mortality in elderly patients were lung disease and the receipt of mechanical ventilation. Conclusions: Elderly patients account for a substantial proportion of patients with candidemia and have a higher mortality compared to younger patients. Abstract Background The Zaprionus genus shares evolutionary features with the melanogaster subgroup, such as space and time of origin. Although little information about the transposable element content in the Zaprionus genus had been accumulated, some of their elements appear to be more closely related with those of the melanogaster subgroup, indicating that these two groups of species were involved in horizontal transfer events during their evolution. Among these elements, the Gypsy and the Micropia retroelements were chosen for screening in seven species of the two Zaprionus subgenera, Anaprionus and Zaprionus. Results Screening allowed the identification of diverse Gypsy and Micropia retroelements only in species of the Zaprionus subgenus, showing that they are transcriptionally active in the sampled species. The sequences of each retroelement were closely related to those of the melanogaster species subgroup, and the most parsimonious hypothesis would be that 15 horizontal transfer events shaped their evolution. The Gypsy retroelement of the melanogaster subgroup probably invaded the Zaprionus genomes about 11 MYA. In contrast, the Micropia retroelement may have been introduced into the Zaprionus subgenus and the melanogaster subgroup from an unknown donor more recently (~3 MYA). Abstract Background Some organisms can survive extreme desiccation by entering a state of suspended animation known as anhydrobiosis. The free-living mycophagous nematode Aphelenchus avenae can be induced to enter anhydrobiosis by pre-exposure to moderate reductions in relative humidity (RH) prior to extreme desiccation. This preconditioning phase is thought to allow modification of the transcriptome by activation of genes required for desiccation tolerance. Results To identify such genes, a panel of expressed sequence tags (ESTs) enriched for sequences upregulated in A. avenae during preconditioning was created. A subset of 30 genes with significant matches in databases, together with a number of apparently novel sequences, were chosen for further study. Several of the recognisable genes are associated with water stress, encoding, for example, two new hydrophilic proteins related to the late embryogenesis abundant (LEA) protein family. Expression studies confirmed EST panel members to be upregulated by evaporative water loss, and the majority of genes was also induced by osmotic stress and cold, but rather fewer by heat. O presente trabalho discute o recebimento de lixiviado de aterros sanitários em estações de tratamento do sistema público de esgotos para tratamento conjunto. Descreve os processos de geração do lixiviado nos aterros e faz uma avaliação desse recebimento em quatro estações de tratamento em operação no Estado de São Paulo que empregam processos biológicos de tratamento por lodos ativados convencional e por batelada, lagoas de estabilização (anaeróbia seguida de facultativa) e lagoas aeradas. Também apresenta a variação da produção de lixiviado em aterros sanitários em função dos períodos de chuva e seca, estabelecidos pela precipitação observada no período avaliado. Conclui que o tratamento conjunto de lixiviado de aterro é uma alternativa a ser considerada no planejamento dos sistemas de resíduos sólidos e de esgotos sanitários. The Portuguese Decreto-Lei 7/2004, dated January 7, which implements the EU Directive 2000/31/CE, dated June 8, that provided a legal framework for certain aspects of electronic commerce, stipulates a contract formation scheme which is different from the one traditionally accepted. In fact, according to the aforementioned legal instruments, there are four stages to the contracting process of an e-contract: an online offer, an order, an acknowledgement of receipt and a confirmation. Confronted with a potentially significant alteration of the general rules of Contract Law, this study aims precisely at clarifying the moment at which the contract is concluded. To that effect, we begin by referring to the various legal regimes applicable to these matters. In the course of this work, the norms and principles of the Portuguese Código Civil are acknowledged as holding a priority. We proceed with a thorough analysis of the legal nature of each of the stages of this novel method of contract formation. Special emphasis is given to the qualification of the online offer as an enforceable offer or as a mere invitation to treat, since the determination of the moment of agreement depends on the conclusion thereby reached. This paper describes a new architecture for supporting the anonymous submission of votes in the REVS voting system. Furthermore, it enables voters to verify during the election that the electoral system processed correctly their anonymously submitted vote. This verification is a weak proof that the vote was effectively considered in the final tally. But since malicious electoral servers have no means to choose which votes should not be considered in the tally, the weak proof is strong enough for ensuring voters that the outcome of the election was not manipulated by the servers towards a particular outcome. In spite have been conceived for the REVS system, our anonymizing architecture can be used in other voting systems as well. The only requirement is that it should be possible to submit the same vote several times and to detect and drop vote replicas at the tallying phase, which is valid for other blind signature voting protocols.

The history of the Estrada de Ferro Sorocabana is highly idiosyncratic. Owned and run successively by Brazilian businessmen, the federal government, and the government of the State of São Paulo, it would be rented out to a foreign company over the period 1907-1919. When it began operating, coffee did not represent a significant share of its overall freight receipt, and even later it would not achieve the relevance it did in Mogiana and Paulista, considered the typical "coffee railways." Unlike these two profitable companies, Sorocabana remained in dire financial straits almost throughout the period 1872-1919. This paper focuses on what lay behind its financial distress. The literature on pioneering railway companies in Brazil emphasises that profitability hinged on the type of ownership as well as on whether coffee was the main source of freight receipt. Besides this, the following hypotheses are evaluated: oversanguine expectations about future cash flows; poor management; shortage of governance engendered by its financing structure; bad luck; and bad governmental policy.

Gene flow can have an especially strong impact on the evolution of small populations. However, empirical studies on the actual rates and patterns of gene flow into small populations are few. Thus, we sought to measure gene flow into small populations of wild radish. Raphanus sativus. We found significant differences in gene flow receipt among experimental populations and within those populations over a season. A maximum-likelihood estimate revealed that almost all of the gene flow into these synthetic populations had its origin in relatively distant (>650 m), large natural populations rather than the proximal (255-400 m), small synthetic populations. We also estimated rates of interpopulation mating from simple paternity analysis of progeny produced by seven small (ca. 50 plants) natural populations. Again, we found significant heterogeneity in gene flow receipt. Although these populations varied 10-fold in their range of isolation distances (100-1000 m), gene flow rates did not vary with distance. The magnitude of gene flow rates estimated in all but one population was great enough for gene flow to play an important role in the evolution of these small populations.

OBJECTIVES—To determine whether, in the context of a face to face interview, socioeconomic groups differ in their propensity to provide details about the amount of their personal income, and to discuss the likely consequences of any differences for studies that use income based measures of socioeconomic position.
DESIGN AND SETTING—The study used data from the 1995 Australian Health Survey. The sample was selected using a stratified multi-stage area design that covered urban and rural areas across all States and Territories and included non-institutionalised residents of private and non-private dwellings. The response rate was 91.5% for selected dwellings and 97.0% for persons within dwellings. Data were collected using face to face interviews. Income response, the dependent measure, was binary coded (0 if income was reported and 1 for refusals, "don't knows" and insufficient information). Socioeconomic position was measured using employment status, occupation, education and main income source. The socioeconomic characteristics of income non-reporters were initially examined using sex specific age adjusted proportions with 95% confidence intervals. Multivariate analysis was performed using logistic regression.

Standard treatment for glioblastoma multiforme (GBM) changed in 2005 when addition of temozolomide (TMZ) to maximal surgical resection followed by radiation therapy (RT) was shown to prolong survival in a clinical trial. In this study, we assessed treatment patterns and survival of patients with GBM in community settings in the United States. Patients with newly diagnosed GBM who were aged ≥20 years in 2006 (n = 1202) were identified as part of the National Cancer Institute 's Patterns of Care Studies. We assessed treatment patterns, and in the subset of patients who received total or partial surgical resection, we used multivariable regression analysis to assess patient, clinical, and health system factors associated with receipt of adjuvant chemotherapy and RT and survival through 2008. Approximately 65% of patients with GBM received total or partial surgical resection, and approximately 70% of these patients received adjuvant TMZ and RT. Receipt of adjuvant therapy was associated with patient age, marital status, health insurance, and tumor location. Median survival in all patients was 10 months (95% confidence interval [CI], 9–11 months). El trabajo se orienta hacia el estudio de la influencia del Aspecto en la eficiencia del diseño gráfico aplicado a un sector en alza como es el envase y embalaje. Para ello, se ha aplicado una metodología de análisis instrumental donde la primera técnica de investigación utilizada es una técnica cualitativa (Focus Group) utilizada para indagar y determinar las variables de estudio (Contraste, Color, Forma, Textura y Orientación) que justifiquen la preferencia del receptor por un Aspecto determinado. Self-report compared to electronic medical record across eight adult vaccines: Do results vary by demographic factors?

Immunizations are crucial to the prevention of disease, thus, having an accurate measure of vaccination status for a population is an important guide in targeting prevention efforts. In order to comprehensively assess the validity of self-reported adult vaccination status for the eight most common adult vaccines we conducted a survey of vaccination receipt and compared it to the electronic medical record (EMR), which was used as the criterion standard, in a population of community-dwelling patients in a large healthcare system. In addition, we assessed whether validity varied by demographic factors. The vaccines included: pneumococcal (PPSV), influenza (Flu), tetanus diphtheria (Td), tetanus diphtheria pertussis (Tdap), Human PapillomaVirus (HPV), hepatitis A (HepA), hepatitis B (HepB) and herpes zoster (shingles). Telephone surveys were conducted with 11,760 individuals, ≥ 18, half with documented receipt of vaccination and half without. We measured sensitivity, specificity, positive and negative predictive value, net bias and over- and under-reporting of vaccination. RESUMONeste artigo, analisa-se como os mercados diferenciados (status), o conselho de administração e as características organizacionais condicionam a reputação das empresas de capital aberto brasileiras. Com base em 1.396 observações coletadas entre os anos de 2002 e 2007 na BM&F Bovespa, utiliza-se um modelo de seleção em dois estágios para evidenciar que empresas que fazem parte do Novo Mercado, como do mercado norte-americano por meio de American Depositary Receipt (ADR), têm maiores chances de ser mais admiradas. Todavia, isso não influencia a diferenciação da reputação. Também se verifica que o capital social dos conselhos, por meio dos laços fracos, está situado em empresas com reputação destacada. Além disso, os resultados apontam que a rentabilidade e o tamanho da empresa também estão positivamente associados com a reputação. Diferentemente do que se hipotetizou, o coeficiente de agrupamento afeta de forma negativa a reputação, remetendo ao chamado lado obscuro das redes. The perceived organizational support (POS) construct has received a significant degree of attention within the literature, helping scholars and practitioners alike to better understand and interpret the relational dynamic between the employee and their employer. However, this thesis contends that there are a number of assumptions, gaps and confounds that limit the extent to which POS can offer greater construct validity. As such, this thesis presents a collection of three stand-alone scholarly papers that aim to further develop and extend the POS construct as well as organizational support theory (OST), both theoretically and empirically. The first paper explores the theoretical assumption that an individual’s POS is increased by both the direct (i.e. idiosyncratic) receipt of supportive organizational treatment, as well as the observation of coworker (i.e. the group/collective) receipt of such treatment. This presents a potential confound in that OST also holds that POS is systemic of notions that the individual is treated fairly; thus hypothetically, an individual’s appraisal that, in comparison, other coworkers have received more supportive organizational treatment, could lead to notions of unfair treatment due to relative under-benefit.
